Abstract

Background: The PICO framework is frequently applied to formulate question for conducting literature searching in the evidence-based practices (EBPs) development. However, students experienced challenges in fitting the clinical questions into PICO's format. In Indonesia, the EBP was mostly taught conventionally through lectures and discussions. There was no specific evaluation for assessing the accuracy of the PICO formula. Previous studies indicated that incorporating visual art in learning effectively improves students' observation skills, self-confidence, communication skills, and promotes transformational learning.Purpose: To describe the implementation of the PICO game and visual art as an EBP learning method and evaluate the accuracy of the PICO formula and the visual art created by nursing students, including their perspectives on the applied EBP learning method.Method: This descriptive study was conducted at a secondary hospital in Sumedang District, West Java-Indonesia, in July 2023. Totally 53 nursing profession program (NPP) students of a University in Indonesia were voluntarily recruited as participants. They divided into 9 groups and received a description of how to play the PICO game and visual art for developing the PICO formula question. Each group required to develop PICO questions from the PICO trigger pictures and keywords on the PICO worksheets and to draw at least one visual art illustration from the developed PICO question. At the end of those sessions, they were also required to provide opinions on this EBP learning approach. The PICO question accuracy was assessed using the PICO formula observation sheet, while the visual art compatibility was measured using the visual art observation sheet. Quantitative data were analyzed descriptively and the students’ perspectives on the EBP learning processes were analyzed by identifying the topic mostly described. Results: There were 63 PICO formulas and 18 visual art illustrations. Most of the PICO formulas (93.7%) were identified as accurate. However, more than one-third (41.3%) of the PICO formula did not include the C (comparison) component. More than half of the developed visual art illustrations (61%) were inappropriately describing the developed PICO formula. Participants reported that using PICO Games and visual art was an interesting and fun approach that enhanced their comprehension of the PICO question formula, EBP, and improved their teamwork capacity. Conclusion: The PICO game and visual art learning method facilitates high accuracy of the PICO question formula and is perceived as an effective and interesting EBP learning method. However, as the “C” (comparison) is the PICO component mostly found uncompleted, it is important to elaborate more on this issue in the EBP learning session. It is also important to extend the duration of the EBP learning method to facilitate students drawing the expected visual art appropriately. Further studies are needed to further confirm and follow up on the findings of this study.

Abstract

Introduction Most nursing students in Indonesia require specialized competency training to implement evidence-based practice (EBP) in the clinical setting. Some literature revealed that art and game-based learning methods can improve students' skills and self-efficacy on the topics studied. Therefore, this study was conducted to evaluate the application of visual art plus the PICO game in improving EBP competence in nursing students. Methods: This study used a quantitative experimental design with a pretest-posttest one-group design approach. The study was conducted on 50 students at one of the nursing education institutions in Bandung, West Java, Indonesia, with a purposive sampling technique. The visual art plus PICO game intervention was given for 25 minutes by implementing drawing activities and card games. The EBP questionnaire consisting of eight questions was used as a research instrument to obtain data on students' EBP competence at the pretest and posttest. Demographic data was analyzed by descriptive statistical analysis, and the difference in EBP competence between the pretest and posttest was analyzed by independent t-test.Results: The data analysis results showed a significant difference in students' EBP competence before and after being given the visual art plus PICO game intervention (p<0.00). The mean score of students' EBP competence increased from 38.22 in the pretest to 41.56 in the posttest. Gender, grade point average, and learning style of students also influenced the results obtained in this study. Conclusion: Visual art and PICO game interventions were effective learning methods to improve students' EBP competence. Visual art plus the PICO game can be one of the options for learning methods to improve students' EBP competence.

Abstract

Chronic kidney disease (CKD) is a global health problem that can lead to serious complications. In patients with CKD and diabetes mellitus (DM), there is an increased risk of falls, dependence in daily activities, and stroke. Although these risks are often associated with the elderly population, the phenomenon of multimorbidity is now also increasing trends among individuals of working age. This study aims to describe changes in fall risk levels, dependency levels, and stroke risk knowledge among CKD patients with DM and glaucoma during hospital care. Data collection was conducted through observation, interviews, physical examinations, and medical record reviews. The sample used was a 43 year old woman. The study results showed a significant improvement in reduced fall risk (MFS: 35 to 25), decreased dependency level (CDS: 53 to 66), and increased stroke risk knowledge (SRQ: 28 to 40). This study indicates that comprehensive interventions such as hemodialysis, wound care, pharmacological therapy, and education effectively improve functionality and understanding in productive aged patients with complex multimorbidity.

Abstract

Paralytic ileus is a disorder that results in a halt of intestinal flow, causing fluid and gas accumulation. Although paralytic ileus is commonly associated with postoperative conditions, it may also influenced by electrolyte imbalances from long-term antacid use and age-related factors that increase mortality risk. If inadequately unmanaged, the condition can result in significant pain, increased risk of complications, and greater care dependency. The purpose to describe a case of paralytic ileus in an elderly patient with long-term antacid use and evaluate changes in pain intensity and care dependency during treatments. This case report involved a 74-year-old male patient with paralytic ileus and a primary nursing diagnosis of acute pain. Pain intensity and care dependency were assessed using the Numeric Rating Scale and Care Dependency Scale during a four-day hospitalization period. The patient showed clinical improvement, include the return of flatus and defecation, normalization of electrolyte levels, reduced pain intensity (NRS 6 to 3), and increased independence (CDS 46 to 62). Discussion: Nursing care included electrolyte correction and pain management through pharmacological therapy and guided imagery, which contribute to decreased pain an improved care dependency, particularly in continence-related aspects. Recall bias related medication history was identified as a limitation. This case underscores the importance of early recognition of electrolyte imbalance and comprehensive nursing assessment in patients with paralytic ileus. Holistic nursing care supported by multidisciplinary collaboration and non-pharmacological intervention played a critical role in enhancing patient outcomes and promoting functional independence during hospitalization.

Abstract

Meningioma is an intracranial tumor of unknown etiology; however, progesterone is considered a hormonal factor associated with its incidence. Clinical manifestations include psychosis depending on the tumor location, with distinct symptoms such as the emergence of Charles Bonnet Syndrome (CBS). Additionally, Increased Intracranial Pressure (IICP) is a recurrent complication of craniotomy procedures. IICP poses a risk of cerebellar herniation, which can lead to decreased consciousness and respiratory arrest; therefore, intracranial management is crucial. One management strategy is 30° head elevation. This study aims to describe the application of a 30° head elevation intervention for decreased adaptive intracranial capacity in patient post-craniotomy for Space Occupying Lesion (SOL) Meningioma. This study utilized a case report design conducted in May 2023 at RSUD Sumedang. The results indicate that elevating the head above heart level facilitates venous return from the brain to the heart and helps activate the parasympathetic nervous system, which regulates bodily functions during rest. This maximizes patient relaxation, resulting in improved cerebral tissue perfusion, adequate oxygenation status, reduced headache, resolution of nausea and vomiting, and stable blood pressure. The 30° head elevation intervention is recommended as a non-pharmacological approach to address decreased adaptive     1     intracranial capacity, ensuring adequate cerebral blood circulation.

Abstract

Cataract surgery in a day-care unit significantly reduces preoperative education time, posing a challenge for elderly patients with cognitive decline. Traditional lectures rely on nurse availability and verbal recall, necessitating a more standardized and sensory-engaging alternative. This study compared the effectiveness of audio-visual media versus traditional lectures on the Quality of Discharge Teaching (QTDS) and Readiness for Hospital Discharge (RHDS) in phacoemulsification patients. A quasi-experimental, post-test-only study was conducted at a tertiary referral hospital in August 2023. Using sequential sampling, 100 participants were allocated to either the lecture group (n=50) or the audio-visual group (n=50) based on a systematic time-based approach (odd-even surgery dates) to prevent cross-contamination. Data were collected through the validated Indonesian P-QTDS and P-RHDS questionnaires and analyzed using the Mann–Whitney U test. No significant difference was found in teaching quality (p=0.647), with both methods rated as satisfactory. However, the audio-visual group had a higher mean rank for discharge readiness (55.47) than the lecture group (45.53). Although approaching marginal significance (p=0.073), this reflects a clinically meaningful trend favoring the audio-visual intervention. The use of audio-visual media is recommended as the primary educational method to improve discharge readiness and ensure nursing standardization in high-volume day-care units.
