Abstract

Wound dehiscence is a serious postoperative complication characterized by the reopening of the surgical incision wound and is reported to occur in about 0.5–3.4% of abdominopelvic surgery cases. These complications generally appear on the 5th to 8th day postoperatively, especially in the proliferation phase of the wound healing process. Various risk factors play a role in the occurrence of wound dehiscence, including surgical wound infection, poor nutritional status, anemia, and comorbid diseases such as uncontrolled diabetes mellitus. The case report discusses a 46-year-old male patient with a history of uncontrolled diabetes mellitus who experienced wound dehiscence after undergoing an exploratory laparotomy. Patients complain of detached wound sutures accompanied by pain and signs of local infection. Clinical and laboratory examinations show the presence of leukocytosis, anemia, and hypoalbuminemia that worsen the wound healing process. Management is carried out through reoperation to close the wound and give broad-spectrum antibiotics to control infection. The results of the treatment showed a gradual improvement in the patient's clinical condition. This case confirms the importance of early detection of risk factors, control of comorbidities, and appropriate surgical and medical treatment in preventing advanced complications such as evisceration and sepsis. A comprehensive approach is indispensable to improve the outcomes of patients with postoperative wound dehiscence.

Abstract

This study aims to reveal efforts to resolve disputes within families through a case study in Sidorejo Village, Wonomulyo Subdistrict, from the perspective of Maqasid al-Syariah (primary maslahat). This is a qualitative study. The research design is field research. The primary data sources for this study consist of the Head of Sidorejo Village as a community leader, Yulianti as a resident of Sidorejo Village, Wati as Yulianti's mother and a resident of Sidorejo Village, the Religious Advisor of the Wonomulyo Subdistrict KUA, and the Imam of Sidorejo Village. The results of this study indicate that: First, the causes of family disputes based on the case study in Sidorejo Village, Wonomulyo Subdistrict, are economic problems, differences in opinions and thoughts, and communication and understanding. Second, efforts to resolve family disputes based on the case study in Sidorejo Village, Wonomulyo Subdistrict, through deliberation and mediation show indicators that are in line with hifz al-Din, hifz al-Nafs, hifz al-'Aql, hifz al-Mal, and Hifz al-Nasl.

Abstract

Background: Rapid initiation of reperfusion therapy in acute myocardial infarction (AMI) is vital for a favourable prognosis and is significantly affected by the duration from symptom onset to hospital arrival. Previous studies have indicated that cardiac autonomic neuropathy (CAN), prevalent in diabetic patients, may impair pain perception, potentially delaying healthcare-seeking behaviour Objective: This study aimed to compare chest pain characteristics and healthcare-seeking delays between patients with diabetes mellitus (DM) and without DM (non-DM) myocardial infarction. Methods: A cross-sectional study was conducted involving 93 subjects (35 DM and 58 non-DM) diagnosed with either ST-elevation myocardial infarction (STEMI, n=78) or non-ST elevation myocardial infarction (NSTEMI, n=15). Data on symptoms, including pain quality and healthcare-seeking timing within 72 hours of admission, were gathered via a standardized questionnaire. Result: Logistic regression analysis highlighted stabbing pain as a strong predictor (OR = 3.296, p-Value = 0.038), indicating that patients with this symptom are more than three times more likely to delay seeking care. Patients with DM are more frequently reported back-radiating stabbing pain (p<0.05). No significant differences between the groups were observed in pain symptomatology and decision-making delay, although DM patients were more likely to delay seeking care for over two hours. Conclusion: While pain characteristics showed no significant differences between DM and non-DM myocardial infarction patients, DM patients were prone to later healthcare engagement. This delay could be attributed to the misinterpretation of cardiac symptoms as diabetic complications, underscoring the need for targeted patient education. Therefore, strengthened patient education and routine screening for cardiovascular symptoms in individuals with diabetes are essential to support early detection and timely management.

Abstract

Nebulization therapy is an essential method of drug administration that enables effective delivery of medicationsto the respiratory tract in aerosol form; however, contaminated nebulizer components may serve as a source of pathogenicmicroorganisms associated with nosocomial pneumonia. This in vitro experimental study aimed to evaluate the effectiveness ofchlorhexidine as a nebulizer disinfectant in eliminating pathogenic bacteria. The study employed a post-test control group designusing six groups of nebulizer chambers, consisting of two control groups and four intervention groups. Two clinically relevantbacteria, Pseudomonas aeruginosa and Acinetobacter baumannii, were inoculated into the nebulizer chambers. The interventiongroups were disinfected using chlorhexidine gluconate at concentrations of 2.5%, 4%, and 5% diluted in 70% alcohol, and 2.5%chlorhexidine diluted in distilled water, while the control groups received sterile water and 70% alcohol, in accordance withexisting guidelines. After a standardized exposure period, bacterial growth was assessed using Colony Forming Unit (CFU) counts. Data were analyzed descriptively and comparatively to evaluate bacterial eradication across groups. The results demonstrated that 5% chlorhexidine diluted in 70% alcohol achieved complete bacterial elimination (0 CFU) for both bacterial strains, whereas lower concentrations showed residual growth. These findings indicate that chlorhexidine, particularly at higher concentrations, demonstrates strong disinfectant activity against common nosocomial pneumonia pathogens in nebulizer chambers. Nevertheless, the results are limited to in vitro conditions; therefore, further studies involving a wider range of microorganisms, standardized exposure times, and assessments of aerosol contamination during clinical nebulization are warranted to support its practical application.

Abstract

Introduction: The increasing number of older adults poses growing challenges for primary healthcare services, particularly in achieving the Minimum Service Standards (MSS) for elderly health. In community health centers (Puskesmas), first-line nurse managers play a strategic role in planning, implementing, and monitoring health programs for the elderly. However, achievement of MSS varies across facilities, suggesting that managerial competencies and work-related factors may influence service outcomes.Objective: This study aimed to examine the direct and indirect relationships between first-line nurse managers’ managerial competencies—specifically information management, critical thinking, strategic decision-making, and the achievement of Minimum Service Standards for elderly health, with job satisfaction as a mediating variable.Methods: An analytical quantitative cross-sectional study was conducted among 126 first-line nurse managers from 17 community health centers in Malang City and Malang Regency, Indonesia. Data were collected using the Primary Care Manager’s Managerial Competence Scale (15 items) and the short version of the Minnesota Satisfaction Questionnaire (16 items). Data were analyzed using Partial Least Squares–Structural Equation Modeling (PLS-SEM).Results: Critical thinking–strategic decision-making competence showed no significant direct association with MSS achievement (β = −0.012; T = 0.740; p = 0.460), nor did information management competence (β = −0.016; T = 0.734; p = 0.463). However, mediation analysis revealed significant indirect associations through job satisfaction. Critical thinking–strategic decision-making competence demonstrated a positive indirect association with MSS achievement (β = 0.028; p = 0.021), while information management competence also showed a significant indirect association (β = 0.037; p = 0.024).Conclusion: Managerial competencies of first-line nurse managers were not directly associated with the achievement of Minimum Service Standards for elderly health. However, these competencies showed significant indirect associations with MSS achievement through job satisfaction, highlighting the importance of fostering supportive work environments to enhance elderly health service performance in primary care settings.

Abstract

Background: Nurse credentialing is intended to ensure professional competence, improve performance, and enhance work motivation. However, despite its widespread implementation, evidence regarding its actual impact on nurses’ motivation and performance remains inconsistent and inconclusive. This inconsistency reflects an unresolved gap in understanding whether credentialing directly influences nurse performance or operates indirectly through work motivation. This study aimed to examine the relationship between credentialing, work motivation, and nurse performance, with work motivation tested as a mediating variable. Methods: This quantitative cross-sectional study followed the STROBE reporting guideline. The population comprised all inpatient nurses at Dr. Sam Ratulangi Tondano Regional General Hospital (N = 176). Using purposive sampling, 122 nurses were selected based on inclusion criteria of at least one year of work experience and direct involvement in nursing care; nurses on leave were excluded. Credentialing level (PK0–PK3) was the independent variable, nurse performance the dependent variable, and work motivation the mediating variable. Motivation and performance were measured using validated Likert-scale questionnaires, while credentialing data were obtained from institutional records. Structural Equation Modeling–Partial Least Squares (SEM-PLS) was used to test direct and mediating effects. Results: Credentialing showed a negligible and non-significant effect on nurse performance (β = 0.002; p = 0.979) and work motivation (β = 0.027; p = 0.683). Conversely, nurse performance had a strong and significant effect on work motivation (β = 0.768; p < 0.001). Work motivation did not mediate the credentialing–performance relationship. Conclusion: Credentialing alone does not significantly influence nurse performance or motivation. Integrating credentialing with performance management and continuous professional development systems is essential to enhance its effectiveness

Abstract

Electronic Medical Records (EMR) as a form of healthcare digitalization, aim to improve the effectiveness, efficiency, and quality of healthcare. However, the transition from paper-based systems remains limited in many countries and continues implementation challenges. review aims to identify facilitators and barriers of EMR implementation among healthcare workers in hospitals settings. The study design used was a scoping review. A comprehensive search was conducted using the PCC (Population-Concepts-Context) strategy, using terms related to factors influencing the implementation of EMR by healthcare workers in hospitals, in four databases (ProQuest, ScienceDirect, PubMed, and Google Scholar) for studies from 2020 - 2025. Article selection was performed using the PRISMA-ScR guidelines. A total of twenty-eight studies met the inclusion criteria. The findings indicate that facilitators and barriers of EMR implementation emerge from the interaction between staff-related, organization, and technological factors. Key facilitators include digital literacy, organizational training support, and system quality. Major barriers involve low staff readiness, insufficient organizational support, and technology quality and complexity issues. Most studies conducted in low- and middle-income countries (LMICs) reported staff-related factors as facilitators and barriers of EMR implementation. Thus, effective implementation strategies need to be designed contextually through the simultaneous strengthening of human resource capacity, organizational support, and technological quality.
