Abstract

Objectives: This case report aims to identify fractures that occur through panoramic radiographs and evaluated by CBCT after being given treatment. Case Report: A 12-year-old male patient came with his older brother to the emergency room (ER) RSKGM/Bandung Dental Hospital due to the trauma of a fall by an iron football goalpost while playing soccer which caused the patient to fall onto the grass field. The patient was experiencing continuous bleeding. Systemic and allergic history was denied. Extraoral examination showed bruising with no facial asymmetry. Intraoral examination revealed a vertical gap between teeth 42 and 43, the jaw within the site was mobile, and active bleeding especially from lingual lacerations. After consulting with Dental Surgery Specialist, the emergency room doctor gave interdental wiring. After 2 months post wiring, a CBCT photo evaluation was carried out. Conclusion: The mandibular fracture in this case report was well identified by clinical examination and supported by panoramic radiography. After that the patient was given treatment and evaluated through CBCT. CBCT provides a clearer and better picture.

Abstract

ABSTRAK Pendahuluan: Salah satu komplikasi diabetes melitus adalah terganggunya kesehatan tulang. Banyak analisis yang dapat digunakan untuk menilai kualitas tulang pada penderita DM tipe 2, seperti menghitung jumlah partikel dan densitas tulang. Tujuan penelitian ini adalah melihat perbedaan nilai densitas dan jumlah partikel pada tulang mandibula kiri dan kanan pasien penderita dan bukan penderita DM tipe 2. Metode: Penelitian ini dilakukan dengan metode deskripsi observasional dengan pendekatan cross-sectional. Populasi merupakan semua radiograf panoramik dari pasien penderita dan bukan penderita DM tipe 2, sedangkan sampel ditentukan dengan memilih data yang sesuai kriteria. Data keseluruhan yang terkumpul berjumlah 34 radiograf dari pasien penderita DM tipe 2 dan 34 radiograf pada pasien bukan penderita DM tipe 2. Setiap radiograf panoramik, baik mandibula kiri maupun kanan, dianalisis nilai densitas dan jumlah partikelnya menggunakan software ImageJ. Data kemudian diolah dengan uji statistik Mann-Whitney U dengan p-value >0,05 dianggap signifikan. Hasil: Nilai rerata densitas kanan dan kiri pasien penderita DM tipe 2 adalah 92,998 dan pasien bukan penderita DM tipe 2 adalah 102,582. Perbedaan densitas bernilai signifikan dengan p-value 0,01 pada sisi kanan dan 0,009 pada sisi kiri. Nilai rata-rata partikel tulang pasien penderita DM tipe 2 adalah 24,087 dan pasien bukan penderita DM tipe 2 adalah 25,205, namun tidak berbeda signifikan, dengan p-value 0,466 untuk sisi kanan dan 0,051 untuk sisi kiri. Simpulan: Ditemukan perbedaan pada nilai densitas pasien penderita DM tipe 2 dibandingkan dengan pasien bukan penderita DM tipe 2. Hal ini menyebabkan tulang mandibula menjadi lebih rapuh sekalipun jumlah partikel tulang pada pasien penderita DM tipe 2 tidak mengalami perbedaan dibandingkan pasien bukan penderita DM tipe 2. Differences in density and particles of the left and right in patients with type 2 diabetes mellitus and non-diabetes mellitus: a descriptive studyIntroduction: One of the complications of diabetes melitus  is the impairment of bone health. Various analyses can be used to assess bone quality in patients with type 2 DM, including calculating the number of particles and bone density. This study aims to examine the differences in the density and particles count values of the left and right mandibular bones in patients with and without type 2 DM. Methods: This research utilized an observational descriptive method with a cross-sectional study design. The population comprised all panoramic radiographs from patients with and without T2DM, while the sample was determined by selecting data that met specific criteria. A total of 34 radiographs with T2DM and 34 radiographs without T2DM were included in the study. ImageJ software was used to analyze the density and the number of bone particles of each panoramic radiograph, both on the left and right side. The data were processed with statistical analysis, specifically the Mann-Whitney U test with significance determined at a p-value >0.05. Results: The average density values for the right and left sides in patients with T2DM were 92.998, while in patients without T2DM, it was 102.582. The difference in mandibular density was significant, with p-values 0.01 for the right side and 0.009 for the left side.The average number of bone particles in patients with T2DM is 24.087, compared to 25.205 in patients without T2DM, and not significantly different, with p-values 0.466 for the right side and 0.051 for the left side. Conclusions: Differences in mandibular density were observed between patients with and without T2DM, leading to increased fragility of the mandibular bone in T2DM patients. However, no significant differences were found in the number of bone particles between patients with and without T2DM.

Abstract

Objectives: This review article aims to explain how the regulation of vascular endothelial growth factor (VEGF) and angiogenesis on alveolar abscess healing process evaluation using radiograph. Review: The databases used in this review are Google Scholar, PubMed, and Science Direct. A total of 1280 search results appeared based on keywords. The search results were selected by title and abstract according to their relevance to the review topic. A total of 24 literatures were reviewed. The alveolar bone destruction is one of the signs of an inflammatory lesion in the alveolar bone. Bone damage that occurs in cases of the abscess will reduce the absorption of x-rays thereby giving a radiolucent appearance on radiographic examination. A radiographic examination is a supporting examination that can be used to develop the healing process. The processes of angiogenesis and osteogenesis of bone homeostasis will complement each other for the bone healing process, while VEGF is a growth factor that can increase the expression of BMPs and osteoblast differentiation so that the bone healing process can take place properly. Conclusion: VEGF plays a significant role in both bone healing and regulation of vascular development and angiogenesis. However, excessive VEGF can also be harmful to the process of bone repair because it can stimulate the recruitment of osteoclasts. Therefore, VEGF regulation has an important role in apical abscess healing, and radiographic images that are quantitatively analyzed can be used to quantify this healing process.

Abstract

Objectives: This case aims to report the finding of a unique lesion in the maxilla in a young female patient. Case Report: The patient, a 21-year-old female, presented with a painless swelling on the left palate for the past 3 months, causing discomfort during eating. On intra-oral examination, there was visible swelling in the left hard palate area, extending from teeth 23 to 26 and extending to the middle of the palate. The patient was referred for a CBCT examination. The aim of writing this article is to assess the findings of a unique case of benign tumor of the palate. The results of the CBCT examination showed radiolucent lesions in the palatal mucosal area without bone resorption. The density of the lesion was higher than that of the surrounding mucosa. The lesion does not damage the tissue of the surrounding area. This is unique because swelling of this size is usually accompanied by extensive bone resorption. Based on its nature, this lesion was diagnosed as a benign soft tissue tumor with differential diagnoses of pyogenic granuloma, pleomorphic adenoma, leiomyoma, and desmoplastic fibroma. Conclusion: The lesion found was a soft tissue tumor lesion at the time; it was found to have a non-aggressive and non-expansive nature, making it difficult to determine a specific radiodiagnosis. The differential diagnosis of this case has been established as follows: Pleomorphic adenoma, pyogenic granuloma and leiomyoma, and pyogenic granuloma, were the options for establishing a provisional radiodiagnosis.

Abstract

Objectives: The aim of this case report is to describe radiographically the specific features of maxillary sinusitis on CBCT radiograph. Case Report: A 20-year-old female patient came to RSGM UNPAD with a consul letter from Oral Surgery specialist for a CBCT radiography examination to see impacted teeth. The results showed radiointermediate images in the maxillary sinus which showed thickening of the sinus mucosa and an inverted impacted teeth on the right maxillary. Conclusion: Maxillary sinusitis could be assessed using extra oral radiography and CBCT. CBCT examination was used in determining the source of the lesion, the extent of the lesion, and the thickness of the maxillary sinus mucosa.

Abstract

Objectives: This case report was created to provide further insight into the use of CBCT in detecting sialolith lesions. Case Report: A 27-year-old female patient was referred to the radiology department of Padjadjaran University Dental Hospital for CBCT radiograph examination related to complaints of swelling in the right lingual area of the mandible and pain. The results of the radiograph examination analysis showed irregular, well-defined radiopaque, in the lingual area of regio 44-45, measuring 34.31 mm2, and the lesion was not associated with the mandible. Intra-oral examination revealed irregular swelling and the same color as the lingual mucosa of regio 44-45. The analysis showed that the lesion was located in the salivary duct of the sublingual area and the patient was diagnosed with sublingual sialolithiasis dextra. Conclusion: CBCT analysis can be used to accurately identify the position, quantity, and morphology of sialoliths and interpret their three-dimensional positioning in relation to adjacent structures.

Abstract

Objectives: The purpose of this case study was to report the compound odontoma’s founding in a panoramic radiograph. Case Report: An 11-year-old female patient came with her parents to the pedodontics of Dental Hospital Padjadjaran Bandung with the chief complaint of a palatal bite with an unerupted left maxillary posterior tooth. Then the patient took the panoramic and cephalometry radiographs to confirm the diagnosis. The result from the panoramic radiograph was that there were multiple radioopaques with a tooth-like structure contained, surrounded by a radiolucent band, and the lesion had a corticated, well-defined border. Radiodiagnosis has been established as a compound odontoma associated with an unerupted maxillary second premolar. Conclusion: Compound odontoma is a benign tumor that often occurs at the age of tooth development and often causes delayed tooth eruption. Panoramic radiographs as radiographic aids play a very important role in identifying these tumors.

Abstract

Kista dentigerous merupakan kista odontogenik yang dihubungkan dengan mahkota gigi yang impaksi, Itidak erupsi atau yang sedang tumbuh. Kasusnya sekitar 14%-24% dari seluruh kista rahang. Biasanya terjadi pada usia dekade pertama dan ketiga serta dapat ditemukan pada anak-anak dan remaja periode pertumbuhan gigi campuran.Kebanyakan kista dentigerous berhubungan dengan gigi molar ketiga mandibula dan jarang melibatkan impaksi gigi supernumerary anterior rahang atas. Laporan kasus ini membahas kista dentigerous pada anak anak usia 8 th atau periode gigi bercampur akibat belum erupsi sempurna pada anterior rahang atas menggunakan CT Scan dan panoramik. Pasien perempuan berusia 8 tahun bersama orang tuanya datang ke Poli Gigi Anak Rumah Sakit Umum Daerah Kota Tangerang, dengan keluhan gigi depan kiri rahang atasnya hilang dan terasa ada benjolan di regio gigi incisivus sampai premolar kiri rahang atas. Kemudian pasien dirujuk ke bagian radiologi untuk dilakukan pemeriksaan panoramik dan CT Scan. Hasil radiograf panoramik dan CT Scan menunjukkan lesi kistik, well-defined, well corticated, benih gigi 22 terdesak ke arah posterior dan superior, sementara lesi meluas dan mendesak benih gigi 23, 24, 25 yang belum erupsi. Diagnosis secara radiografi memberikan gambaran suatu kista dentigerous. Dapat disimpulkan bahwa melalui pemeriksaan radiografi panoramik dan CT Scan dapat digunakan untuk mendeteksi gambaran kista dentigerous.

Abstract

Objectives: This case report aims to present a case of ameloblastic fibroma, an odontogenic tumor, and to describe its characteristic radiographic features as observed on a panoramic radiograph. Case Report: A 28-year-old woman presented to the RSGM FKG Unpad with a referral for evaluation of a jaw swelling. According to the patient’s medical history, the swelling had gradually appeared over the past two years. While it was not painful, it caused discomfort, prompting her to seek medical attention. Upon examination, the lesion was found in the posterior region of the mandible, and further diagnostic imaging was recommended to determine the extent and nature of the lesion. Ameloblastic fibroma of the jaw is a benign, relatively rare, mixed odontogenic tumor whose epithelial and mesenchymal components are neoplastic. This tumor is usually diagnosed in the first and second decades of life (72.4%), when odontogenesis has been completed (80% of cases), and mainly affects the mandible. In this case, the lesion was diagnosed in the second decade of life, and occurred in the posterior region of the mandible. Conclusion: Ameloblastic fibroma is a benign odontogenic mixed tumor, although rarely ameloblastic fibroma can recur and develop into malignancy. The aim of this case report is to analyze the radiographic appearance of the lesion with information from the history and clinical signs to establish a correct radiodiagnosis.

Abstract

Introduction: Osteoporosis is a systemic bone disease characterized by increased bone fragility and susceptibility to fractures, decreased bone mass and changes in the bone’s microscopic structure, leading to increased porosity and decreased strength. This study aimed to evaluate the differences in the mandibular bone density and condylar head morphology between osteoporosis and non-osteoporosis patients. Methods: The study used an observational descriptive design with a cross-sectional approach. The study population using secondary data consisted of all panoramic radiography photos collected over a six-month period from July to December 2019.  A total of 24 radiographs were included, comprising women with osteoporosis (age 40 to 60 years) and without osteoporosis (aged 40 to 50 years). Bone density was measured using a region of interest (ROI) of 3x3mm through histogram analysis, while condyle shape was evaluated visually. The collected data were processed with t-test results. Results: This study found that right condyle osteoporosis mean 137.51 ± 17.3, right condyle non osteoporosis 143.65 ± 21,1. Left condyle osteoporosis 133.46 ± 18,6, and left condyle non osteoporosis 143.64 ± 18. The mandibular bone density in condylar region was lower in osteoporosis patients by 6.14 pixels on the right side and 10.48 pixels on the left side. The result of the statistical analysis in all groups were p value > 0,05. However, the t-test results indicated no statistically significant differences between two groups. In terms of morphology, the oval shape was the most common in both groups. The bird beak shape was observed exclusively in the osteoporosis group, while the flat shape was more prevalent in the non-osteoporosis group. Conclusion: This study suggests that the mandibular bone density in osteoporosis patients is lower than in the non-osteoporosis group. However, this difference was not statistically significant. Additionally, changes in condylar head morphology did not correlate with osteoporosis status or reflect the observed differences in bone density.
