Abstract

Objectives: Concrescence is a rare dental developmental anomaly where two fully formed teeth are fused by cementum. It often occurs in the maxillary posterior region and may lead to tooth impaction. This case report presents an incidental radiographic finding of a concrescence related to maxillary third molar (M3) impaction. Case Report: A 72-year-old woman presented with a panoramic radiograph referral for mandibular denture placement with a clinical diagnosis of missing teeth in the right and left mandibular regions. On the panoramic radiograph, the impacted root of tooth 28 was seen, fused with the root of tooth 27. Conclusion: Concrescence is a rare case of anomaly and can occur in an impacted M3 tooth, and Concrescence is suspected as the main cause of the tooth not being erupted/impacted. Keywords: Concrescence, maxillary molar tooth impaction, panoramic radiography

Abstract

Objectives: Rickets, osteomalacia, renal osteodystrophy, and hypophosphatemic rickets are systemic diseases that have the same characteristics, namely the condition of bone softening or weak bone conditions. This condition is caused by low levels of calcium and phosphorus in the bones. As a result, the bones become of poor quality, which can be seen histologically in the form of a picture of bone osteoid that is less calcified. However, the etiology of the causes of abnormalities in these four diseases is different, and in particular, the picture in the radiograph certainly has its own character. Review: The purpose of writing this article is to learn more about the special characteristics of these four systemic diseases. The writing method used is a narrative literature review. This article will discuss the four types of systemic diseases, starting from the definition, clinical appearance, appearance in radiographs, and the characteristics of each type. The results of the characterization that has been carried out indicate that, in general, radiography and other imaging methods are indeed difficult to distinguish because they have the same symptoms, but at the beginning of the case, the picture can be distinguished. Conclusion: The conclusion that can be drawn from this article is that between rickets, osteomalacia, renal osteodystrophy, and hypophosphatemic rickets, there are specific characteristics that are the main key in recognizing each abnormality, especially radiographically, that can be recognized.

Abstract

Objectives: This study aimed to analyze the mandible's length, width, and height in patients with hypertension and T2DM through panoramic radiography. Materials and Methods: This study used the observational analytical description method, which has been used in this study with a cross-sectional approach. The sample consisted of 48 panoramic radiographs from 24 patients with hypertension and T2DM, divided into two age groups: those under 60 and those 60 years or older. The radiographs were analyzed using ImageJ to measure the mandible's length, height, and width, and the results were averaged. Results: The study found no significant differences in the length, width, or height of the mandible between the disease groups or the age groups. Conclusion: The mandible's length, width, and height values in hypertension and T2DM do not show statistically significant differences. This explains that the bone quality between patients with hypertension and T2DM is not much different.

Abstract

Objectives: This case report aims to provide information about post-traumatic condylar hyperplasia and temporomandibular joint ankylosis, and its features on CBCT imaging. Case Report: A 16-year-old female patient visited the Radiology Installation of RSGM Universitas Padjadjaran with a referral for CBCT TMJ scan, prompted by a chief complaint of restricted mouth opening. The patient had a history of a traffic accident two years prior, resulting in multiple facial fractures that were treated through several surgical interventions. CBCT images showed morphological irregularities and enlargement of both right and left condyles, glenoid fossae, and articular eminences. Moreover, the development of fibrous tissue was observed within the joint space, accompanied by bone fusion. Conclusion: Condylar hyperplasia and temporomandibular joint ankylosis are pathological conditions that can arise as a consequence of trauma to the facial region. This case highlights the importance of radiological evaluation in identifying morphological alterations of the temporomandibular joint complex, such as condylar enlargement, joint space obliteration, and fibrous or bony fusion. These conditions significantly impact the function of the stomatognathic system, emphasizing the necessity for prompt intervention. Early recognition of these radiographic features is essential to support accurate diagnosis, guide appropriate clinical management, and prevent further deterioration of mandibular function.

Abstract

Objectives: This case report aims to analyze the specific characteristics of Oral Squamous Cell Carcinoma (OSCC) and its rare invasion of the palate. Case Report: A 49-year-old male patient came to the Dental Radiology Installation of RSGM FKG UNPAD with a referral letter for a Cone Beam Computed Tomography (CBCT) examination. Based on the patient's anamnesis, he complained of swelling on the roof of the mouth that had lasted for 3 months, accompanied by pain, trismus, and persistent bad breath. Oral hygiene was poor; there was plaque, stains, and pale gums. The patient is an active smoker, smoking 1-2 packs per day; there is no history of similar diseases in the family. On extraoral examination, facial asymmetry was found due to swelling on the right cheek. On intraoral examination, gingival hyperplasia was found, accompanied by ulceration and reddish lesions on the roof of the mouth. The patient previously underwent a biopsy and excision of the roof of the mouth. Previous panoramic X-ray results showed lesions with unclear boundaries. On CBCT radiography, bone damage with irregular bone invasion and signs of malignancy indicate the characteristics of Squamous Cell Carcinoma (OSCC). 3D CBCT examination can be performed to accurately analyze and determine further diagnosis. Conclusion: 3D CBCT radiography can be used as a supporting examination to analyze specific characteristics in OSCC cases. The results of CBCT analysis can be a guideline in planning pre-operative follow-up to be carried out.

Abstract

Objectives: The purpose of this case study is to report a case of quite painful facial swelling observed on a panoramic radiograph and diagnosed as Bilateral Mumps.   Case Report: A 34-year-old woman came to the Radiology Department with complaints of throbbing pain accompanied by swelling in the right and left mandible. The swelling has been present for 5 days, and the pain worsens when eating. In addition, the patient complains of having a fever and being unable to sleep. Physical examination revealed a body temperature of 38.6°C; the patient appeared lethargic and weak. Extraoral examination shows the lower part of the face is swollen, symmetrical between the left and right sides, with increased surface temperature of the lesion, and upon palpation, it feels soft and painful when touched. During the intraoral examination, no swelling was observed. The results of the panoramic radiograph examination show a diffuse radiolucent image spread from the angle to the ramus of the left and right mandible. There are no changes in the structure of the cortical bone of the mandible. From the panoramic examination, a suspect radiodiagnosis of bilateral mumps on the right and left mandible was found. Conclusion: Mumps is an infection of the salivary glands caused by a virus. On panoramic radiography, the image is more indicative of the formation of osteolytic lesions in the form of radiolucent foci with varying diameters and ill-defined borders. Pockets spread in the inflamed area, and in this case, panoramic radiography can recognize the condition.
