Abstract

Implementasi Rekam Medis Elektronik (RME) melalui aplikasi e-Puskesmas di Kota Banjarmasin telah mencapai tingkat pengisian sekitar 95%, namun tingginya angka tersebut belum mencerminkan optimalnya kinerja petugas rekam medis. Permasalahan seperti rendahnya akurasi kode diagnosis, ketidaklengkapan catatan klinis, keterbatasan sarana pendukung, dan minimnya pelatihan teknis masih menjadi kendala nyata di lapangan. Menganalisis hubungan usia, pendidikan, pelatihan, ketersediaan sarana dan prasarana, serta kualitas teknologi dengan kinerja petugas rekam medis dalam penggunaan RME di Puskesmas Kota Banjarmasin. Penelitian kuantitatif dengan desain analitik observasional cross-sectional. Populasi seluruh petugas rekam medis di 28 Puskesmas Kota Banjarmasin dengan sampel 63 responden menggunakan total sampling. Data dikumpulkan melalui kuesioner terstruktur, lembar observasi checklist, dan ekstraksi data sekunder laporan kinerja Puskesmas. Analisis data meliputi univariat, bivariat menggunakan uji Chi-Square dan Fisher’s Exact Test, serta multivariat menggunakan regresi logistik biner dengan tingkat kepercayaan 95%. Kerangka teori mengintegrasikan Teori Kinerja Gibson et al. (1996) dengan Model Human-Organization-Technology Fit (HOT-Fit) Yusof et al. (2006). Sebanyak 58,7% responden memiliki kinerja tidak baik. Analisis bivariat menunjukkan hanya pendidikan yang berhubungan signifikan dengan kinerja (p=0,026; OR=3,223; 95% CI: 1,128–9,210), sedangkan usia (p=0,637), pelatihan (p=0,707), sarana dan prasarana (p=0,502), serta kualitas teknologi (p=0,098) tidak signifikan. Analisis multivariat menghasilkan model final M₂ (AIC=83,448) dengan penambahan variabel kesesuaian pendidikan secara signifikan meningkatkan fit model (Δχ²=4,947; p=0,026). Kesesuaian pendidikan merupakan faktor paling dominan (Exp(β)=3,411; p=0,027; 95% CI: 1,153–10,088), diikuti kualitas teknologi yang menunjukkan kecenderungan positif namun belum signifikan (Exp(β)=2,740; p=0,103). Kinerja petugas rekam medis dalam penggunaan RME dipengaruhi secara simultan oleh faktor individu, organisasi, dan teknologi. Kesesuaian pendidikan formal bidang Rekam Medis dan Informasi Kesehatan (RMIK) merupakan faktor paling dominan, di mana petugas berlatar belakang RMIK memiliki peluang kinerja baik 3,411 kali lebih tinggi dibandingkan petugas non-RMIK setelah dikontrol oleh variabel kualitas teknologi dalam model.

Abstract

Occupational Health and Safety (OHS) is a fundamental aspect of the industrial world, playing a crucial role in protecting workers and ensuring operational continuity. Various reports indicate that the majority of workplace accidents in industrial sectors, such as mining, construction, and manufacturing, are still dominated by human error. Therefore, a comprehensive evaluation of the probability of human error is necessary to reduce the number of workplace accidents and improve safety performance. Objective to analyze and integrate research findings related to the implementation of the Systematic Human Error Reduction and Prediction Approach (SHERPA) and Human Error Assessment and Reduction Technique (HEART) methods in identifying, measuring, and reducing the risk of human error in various industrial sectors. This study is a systematic literature review compiled according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Articles were searched through Google Scholar and PubMed databases using the keywords "Human Error," "Occupational Health and Safety (OHS)," "HEART," "Human Reliability Analysis," and "occupational accident." Inclusion criteria included articles published between 2021 and 2025, with full text, and discussing human error analysis using the HEART method in the industrial sector. Of the 313 identified articles, nine met the criteria and were analyzed qualitatively.  The HEART method produced varying Human Error Probability values, with the highest values for manual work in underground mining (0.89), manual valve operation in oil tank facilities (0.95), and cutting machine operation in the furniture industry (0.888). The dominant factors triggering errors included work fatigue, unclear instructions, time pressure, and lack of training. The HEART method is effective in providing a quantitative and holistic picture of human error risk.

Abstract

Mental health issues in the workplace are a fundamental priority in healthcare systems, particularly in high-risk environments characterized by complex operational demands and acute time pressures. Increased cognitive load, when exceeding individual thresholds, has consistently been identified as a major catalyst for work-related stress, which subsequently impairs the clinical performance of healthcare practitioner. This research is designed to rigorously evaluate the correlation between mental workload quantified through the NASA Task Load Index (NASA-TLX) instrument and job stress manifestations, anchored in the conceptual framework established by the National Institute for Occupational Safety and Health (NIOSH). The investigation focuses on high-risk public healthcare sectors to map the underlying psychosocial dynamics affecting the workforce. The synthesis of findings demonstrates a significant and positive correlation between mental workload and professional stress. Specifically, the dimensions of mental demand, temporal demand, and effort emerged as the predominant predictors of psychological strain. Furthermore, the analysis indicates that organizational variables, notably social support and job control, function as critical moderating factors that either attenuate or exacerbate the intensity of the relationship between cognitive demands and perceived stress. The study concludes that mental workload is a pivotal determinant of job stress among healthcare professionals within high-risk public service domains. Consequently, there is an imperative for interventions rooted in cognitive ergonomics and the reinforcement of organizational support systems. Such proactive measures are vital to mitigate psychosocial risks and sustain the overarching quality of healthcare delivery.
