Abstract

The prevalence of high blood pressure in the population aged 18 years and above increased from 25.8% to 34.1%. The significant increase in cases of non-communicable diseases is expected to increase the burden on society and government, because handling it requires large costs and requires high technology. The purpose of this study was to determine the relationship between lifestyle, family support and the role of health workers and compliance with taking medication with controlled blood pressure at the Tembilahan City Health Center in 2023. The research method uses quantitative methods with a Cross Sectionl design approach. The number of samples in this study amounted to 160 hypertension prolanis patients. The sample technique used was Proposional stratified Random Sampling. Data collection through primary data by distributing questionnaires with independent variables (diet, smoking habits, physical activity, stress conditions, family support, the role of health workers and compliance with drug consumption) and the dependent variable is controlled blood pressure. The results of this study indicate that there is a significant relationship between adherence to taking anti-hypertensive drugs (p value = 0.000 and OR 14.744), smoking habits (p value = 0.000 and OR 7.082), diet (p value = 0.000 and OR 4.899), family support (p value = 0.012 and OR 2.977), with controlled blood pressure in hypertension prolanis patients. The variable of adherence to taking anti-hypertensive drugs is the dominant factor that most influences controlled blood pressure in hypertension prolanis patients with an OR value of 14.744.

Abstract

Pharmaceutical service is a direct and responsible service to patients related to pharmaceutical preparations with the intention of achieving definite results to improve the quality of life of patients. The purpose of this study was to obtain in-depth information on drug management in the Pharmacy Unit of RSUD Tengku Sulung. The research method used descriptive qualitative with purposive sampling with the number of informants 7 people. Data collection through in-depth interviews, direct observation and document review using content analysis. The results, on the input component obtained information that the lack of human resources, facilities and infrastructure are inadequate and there is no depot and SIM RS. Policies have been made in the form of Drug Management Guidelines in the Pharmacy Unit. In the process component, information is obtained that selection is based on the hospital formulary and national formulary, planning using the combination method, procurement using e-catalog and direct purchase, receipt is carried out by checking and adjusting the order letter, order invoice, and the drug received, storage based on dosage form, classification of types of drugs, medical devices, B3 and arranged alphabetically with the FIFO and FEFO systems, distribution using individual prescribing and Floor Stock systems, deletion is carried out by involving third parties, control is carried out by controlling drug inventory and quality, administration based on recording, reporting and financial administration. The output component obtained by the availability of drugs is still not effective and efficient. In conclusion, the pharmaceutical supply management system in the Pharmacy unit of RSUD Tengku Sulung is not in accordance with the established standards. Suggestions, the need for additional human resources, the fulfillment of infrastructure such as depot procurement and SIMRS, monitoring and evaluation, improved communication during drug selection and budget planning in the fulfillment of facilities and infrastructure.

Abstract

Stunting is a chronic nutritional problem with long-term impacts on child development and future productivity. The Family Assistance Team (FAT) plays a crucial role in community-based interventions by providing education and mentoring to at-risk families. This study aims to explore the role of FAT in supporting behavioral changes to prevent stunting in maternal and child nutrition. A qualitative case study approach was used. The primary informants, including FAT members and supporting informants, were the Head of the Community Health Center, nutritionists, the Head of Population Control, field coordinators for the Family Planning Program, and mothers of children under five years old. Purposive sampling was employed to select informants. Data were gathered through in-depth interviews, Focus Group discussions, and document review. Data validity was ensured through source and method triangulation. Thematic analysis was used to identify key findings. Five major themes emerged: (1) FAT’s role in health education and growth monitoring; (2) collaboration across programs; (3) risk factors for stunting; (4) challenges in mentoring; and (5) behavioral changes in mothers and children. FAT reported changes in mothers' behavior, particularly in toddler feeding patterns, which were supported through regular monitoring using WhatsApp messaging. However, program implementation still faces various obstacles, requiring capacity building for cadres, policy support, and cross-sector collaboration to ensure the sustainability of the stunting reduction program. Abstrak. Stunting adalah masalah gizi kronis yang berdampak jangka panjang terhadap perkembangan anak dan produktivitas di masa depan. Tim Pendampingan Keluarga (TPK) memainkan peran penting dalam intervensi berbasis masyarakat melalui pendidikan dan pendampingan keluarga berisiko. Penelitian ini bertujuan untuk mengeksplorasi peran TPK dalam mendukung perubahan perilaku terkait gizi ibu dan anak untuk mencegah stunting. Pendekatan studi kasus kualitatif digunakan dalam penelitian ini. Informan utama yaitu TPK dan informan pendukung terdiri dari Kepala Puskesmas, ahli gizi, Kepala Pengendalian Penduduk, koordinator lapangan Program Keluarga Berencana, dan ibu balita. Teknik pengambilan sampel menggunakan purposive sampling. Data dikumpulkan melalui wawancara mendalam, Fokus Grup Diskusi, dan tinjauan dokumen. Validitas data melalui triangulasi sumber dan metode. Analisis tematik digunakan untuk mengidentifikasi temuan kunci. Lima tema utama yaitu: (1) peran TPK dalam pendidikan kesehatan dan pemantauan pertumbuhan; (2) kolaborasi lintas program; (3) faktor risiko stunting; (4) tantangan dalam pendampingan; dan (5) perubahan perilaku ibu dan anak.   TPK melaporkan adanya perubahan perilaku ibu, khususnya dalam pola makan balita, yang didukung melalui pemantauan rutin menggunakan media perpesanan WhatsApp. Namun demikian, implementasi program masih menghadapi berbagai hambatan, sehingga diperlukan penguatan kapasitas kader, dukungan kebijakan, serta kolaborasi lintas sektor guna menjamin keberlanjutan program penurunan stunting.

Abstract

Stunting is a chronic nutrition problem that affects human resource quality and remains a national priority. One specific nutrition intervention implemented is the provision of Supplementary Feeding using Foods for Special Medical Purposes (PMPK). The success of this program depends on proper planning, implementation, and monitoring–evaluation systems. To analyze the implementation of PMPK activities for stunted children in the working area of the Kampar District Health Office in 2025. This residency used a qualitative descriptive design with a field study approach. Informants included nutrition program managers, health workers, and related stakeholders. Data were collected through in-depth interviews, observations, and document reviews. Thematic analysis was applied through data reduction, data display, and conclusion drawing. PMPK implementation in Kampar District generally follows national guidelines, covering planning, organizing, implementation, and monitoring–evaluation stages. However, several obstacles were identified, including limited human resources, varied understanding of PMPK among staff, and suboptimal recording and reporting systems. Fishbone analysis indicated that major problems originated from human, method, and system factors. PMPK implementation for stunted children in Kampar District has been conducted, but strengthening staff capacity, standardizing procedures, and improving monitoring and evaluation systems are necessary to enhance program effectiveness in reducing stunting

Abstract

Iron deficiency anemia among adolescent girls remains a significant public health problem with long-term implications for reproductive health and future productivity. One of the national strategies to prevent anemia is the provision of iron–folic acid supplementation, known as iron supplementation tablets; however, its effectiveness largely depends on program coverage and adherence among the target population. This residency study aimed to describe the coverage and adherence of iron supplementation tablet consumption among adolescent girls in Pekanbaru City and to identify factors influencing program implementation. A descriptive qualitative approach was applied through document review of the iron supplementation program reports from 2023–2024, in-depth interviews with nutrition and school health program officers, and direct observation at the Pekanbaru City Health Office. Data were analyzed using a fishbone approach to identify root causes and the Urgency, Seriousness, Growth (USG) method to determine priority problems. The findings indicate that both coverage and adherence to iron supplementation tablet consumption among adolescent girls remain suboptimal, reflected by inadequate follow-up of anemia screening results, weaknesses in recording and reporting systems, limited human resources, and the absence of specific standard operating procedures for adolescent anemia management. In conclusion, strengthening program governance through the development of clear SOPs, capacity building of health personnel, optimization of health information systems, and reinforcement of school and family involvement is essential to improve coverage and adherence to iron supplementation tablet consumption among adolescent girls in Pekanbaru City.
