Abstract

Breast milk is the single best food for babies aged 0 6 months. For this reason, exclusive breastfeeding for infants aged 0-6 months is the right step to maximize growth and development in infants aged 0-6 months and growth and development at later ages, because babies get proper nutrition and reduce the risk of infectious diseases that can affect nutritional conditions and growing. This study aims to determine the relationship of exclusive breastfeeding to growth and development in infants aged 6-12 months. This type of research is descriptive analytic with a cross-sectional study approach. The population in this study were all infants aged 6-12 months, namely 1538 infants. Sampling using consecutive sampling technique with a sample size of 56 respondents. The results showed that there was no significant relationship between exclusive breastfeeding and growth based on body weight (p = 1,000) and growth based on head circumference (p = 0.743). There was a significant relationship between exclusive breastfeeding and growth based on body length (p = 0.027) and the development of infants aged 6-12 months (p = 0.013). It can be concluded that exclusive breastfeeding has a significant relationship with growth based on body length and development of infants aged 6-12 months. Therefore, it is expected that breastfeeding mothers always give exclusive breastfeeding to infants aged 0-6 months in order to maximize the baby's growth and development and growth and development at a later age.Breast milk is the single best food for babies aged 0 6 months. Abstract

Introduction : Anxiety is one of the most non-motorized symptoms in patients with Parkinson's which greatly affects the quality of life, but in clinical practice it is often neglected. Anxiety in patients with Parkinson's can accelerate motor deterioration / disability and also increase mortality. M ethods: This cross-sectional study was conducted at the Neurology polyclinic Dr. M Djamil Padang from July to December 2020 in patients who had been diagnosed with Parkinson's Disease (PD) by excluding secondary Parkinson's and a history of stroke. Anxiety was measured using the Hamilton Anxiety Rating Scale (HARS). The research subjects were 60 people. Univariate analysis to present baseline characteristics and bivariate tests assessed factors associated with anxiety and the relationship between variables. A p value <0.05 was considered statistically significant. Results: Most of the subjects were male (55%) with a mean age of 58.05 ± 9.7 years and disease duration of 6.35 ± 5.29 years. By examining the Hamilton Anxiety Rating Scale (HARS), it was obtained 38.3% of Parkinson's sufferers with anxiety. There is a significant relationship between marital status, duration of illness and degree of disease with the incidence of anxiety (p <0.05) and there is no significant relationship between age and sex with the incidence of anxiety in patients with Parkinson's. Conclusion: There is a significant relationship between marital status, duration of illness and degree of disease with the incidence of anxiety in patients with Parkinson's and there is no relationship between age and sex with the incidence of anxiety in patients with Parkinson's.

Abstract

Parkinson's disease (PD) is the second most common neurodegenerative disease after Alzheimer's. It is chronically progressive with the main symptoms of resting tremor, rigidity, bradykinesia, and postural disturbances. Lewy's body and Lewy's neurite are the main findings in brain biopsies of patients with PD. The main component is alpha-synuclein, a misfolding protein that plays an essential role in the pathogenesis of PD. This study aims to determine the association between serum alpha-synuclein levels during the PD stage and compare the levels between PD patients and healthy populations of the same age. A case-control study was conducted on 62 people with PD and 20 normal subjects as controls in the outpatient Department of Neurology of Dr. M. Djamil General Hospital and Ibnu Sina Islamic Hospital, Padang, from March to September 2020. The ELISA method examined serum alpha-synuclein examination, and the PD stage was assessed according to Hoehn and Yahr stages. The differences in alpha-synuclein levels between cases and controls and between stages of PD were analyzed by the Mann-Whitney test. Alpha-synuclein levels in PD patients were higher than in controls, and this difference was statistically significant (p<0.05). On the other hand, alpha-synuclein levels were higher in the severe stage than in the mild stage but not statistically significant (p>0.05). In conclusion, there was no association between alpha-synuclein levels and the stage of Parkinson's disease. Still, serum alpha-synuclein levels in PD patients were significantly higher than in the healthy population.

Background: Vitamin D acts as a neuroprotector and modulator in the central nervous system. Hypovitaminosis is a risk factor for cognitive dysfunction. Hypovitaminosis D and impaired cognitive function are common in patients with chronic kidney disease. Impaired cognitive function is comorbid that can increase the morbidity and mortality of patients with chronic kidney disease. This study aims to assess the relationship between serum vitamin D levels and cognitive function in patients with chronic kidney disease. Methods: This research is an observational study with a cross-sectional approach. A total of 60 research subjects participated in this study. Sociodemographic data, cognitive function, and vitamin D levels were analyzed in this study. Data analysis was performed with SPSS 25 to perform univariate and bivariate tests. Results: The impaired cognitive function was found in 56.7% of CKD patients. The median serum vitamin D level of patients with chronic kidney disease with impaired cognitive function was 30.80 ng/mL and without impaired cognitive function 42.98 ng/mL. There was a significant relationship between serum vitamin D levels and impaired cognitive function (OR=4.125, p=0.035). The cut-off point of serum vitamin D levels associated with impaired cognitive function in CKD patients was 34.8 ng/mL (sensitivity 64.7% and specificity 69.2%). Conclusion:There is a significant relationship between serum vitamin D levels and the incidence of cognitive dysfunction in patients with chronic kidney disease. The cut-off point for serum vitamin D levels associated with impaired cognitive function in chronic kidney disease is 34.8 ng/mL.

Background: Parathyroid hormone is known as one of the main uremic toxicants, which, if the amount increases in plasma, will affect motor conduction in patients with chronic kidney disease. This study aims to determine the relationship between Parathyroid hormone levels with uremic neuropathy in patients with chronic kidney disease. Methods: This study was an analytic observational study with a cross-sectional approach, where as many as 42 subjects participated in this study. Data analysis was performed using SPSS using univariate and bivariate, with p<0.05. Results: There was a difference in the average thyroid hormone levels in neuropathy and without neuropathy, but statistically, it was not different p>0.05. Conclusion: There is no relationship between parathyroid hormone and uremic neuropathy in patients with chronic kidney disease.

Abstract

A new stroke-prevention service (ERDANELA model) has been developed at nursing homes by empowering elderly caregivers to become service-implementing cadres. The aim of this study is to investigate the effect of empowerment on the knowledge, attitude, skill, and behavior of elderly caregivers about stroke-prevention services in nursing homes. A quasi-experimental study was conducted on 52 elderly caregivers who were working at 3 nursing homes in West Sumatra Province, Indonesia. The questionnaire design consists of knowledge, attitudes, skills, and behavior variables. Intervention in the form of training was carried out for three days, using pre-test and post-test methods. The results of this research are the majority of caregivers are 40-49 years old (34.60%), male (51.9%), have middle school education (48.10%), have monthly income bigger than the RMW (75.00%), worked in nursing home>5 years (67.30%), and never attended training while working at nursing-home (73,10%). Caregivers knowledge, attitude, skill, and behavior mean was 18.23±1.89, 7.65±1.22, 6.75±1.37 and 6.00±0,91 respectively before training, and significantly it has increased to 25.48±3.38, 10.15±.09, 9.10±0.96, and 7.06±1.43 respectively after training with sig=0.000<0.05. Empowerment has succeeded in increasing the knowledge, attitudes, skills, and behaviors of elderly caregivers to become service-implementing cadres of the new stroke-prevention service at nursing homes

Abstract

Background: Cystatin-C acts as a neuroprotector in the central nervous system at normal levels, but high serum cystatin-C levels are associated with impaired cognitive function. Serum cystatin-C levels increase in impaired renal function, and cognitive impairment are comorbidities that can increase the morbidity and mortality of chronic kidney disease patients. This study aimed to assess the relationship between serum cystatin-C levels and cognitive function in chronic kidney disease patients. Methods: This study used a cross-sectional design with 73 samples of non-hemodialysis chronic kidney disease patients. Cognitive function was assessed using the MoCa-Ina. Serum cystatin-C levels of all samples were measured by the ELISA method. Data analysis was carried out using SPSS. Results: Impaired cognitive function in CKD patients was found to be 76.7%. The median serum cystatin-C level of CKD patients with impaired cognitive function (n=56) was 1.015 mg/dL, and without cognitive impairment (n=17) was 0.929 mg/dL. There was no significant relationship between serum cystatin-C levels (cut-off point 0.98 mg/dL) and impaired cognitive function (OR : 2.05, 95% CI : 0.680-6.175, p= 0.198). Conclusion: There is no relationship between serum cystatin-C levels and cognitive function impairment in non-hemodialysis chronic kidney disease patients.

Abstract

Background: Dickkopf-related protein – 1 (DKK-1) is an antagonist protein to the Wingless (Wnt)-β catenin signal. Dysregulation of Wnt-β catenin signaling by DKK-1 causes disruption of neuronal synapses, which can result in impaired cognitive function. Impaired cognitive function in people with HIV or HIV-associated neurocognitive disorder (HAND) is a spectrum of cognitive disorders related to HIV neuroinvasion and neuroinflammation, which significantly results in impaired cognitive function and daily activity. This study aimed to assess the relationship between plasma DKK-1 levels and impaired cognitive function in HIV patients. Methods: This study used a cross-sectional design, consisting of 84 HIV sufferers who went to the voluntary counseling and testing (VCT) polyclinic at Dr. M. Djamil General Hospital, Padang, Indonesia, in the period December 2022 - March 2023, who met the inclusion and exclusion criteria. Cognitive function was assessed using the Montreal Cognitive Assessment Indonesian version (MoCa-Ina), and daily activity function using the instrumental activity of daily living (IADL) examination. Plasma DKK-1 levels were measured using the enzyme-linked immunosorbent assay (ELISA) method. Results: The mean plasma DKK-1 level of HIV sufferers was 358.4 (+ 157.6) pg/mL. ANI type disorder was found in 78.6%, MND in 21.4%, and no HAD type. There was a significant relationship between plasma DKK-1 levels and impaired cognitive function in HIV patients (OR = 2.82, CI95% = 1.128-7.043, p = 0.025), but there was no significant difference in plasma DKK-1 levels between types of cognitive impairment ANI and MND (p = 0.858). Conclusion: Plasma DKK-1 levels are significantly associated with impaired cognitive function in HIV patients.
