Abstract

The implementation of the Regional Public Service Agency Financial Management Model (PPK-BLUD) is a form of public sector financial management reform that aims to improve the flexibility, efficiency, and accountability of government hospital financial performance. This study aims to examine the impact of BLUD implementation on the financial performance of government hospitals in Indonesia. This study uses a literature review approach to empirical articles that discuss hospital financial performance before and after the implementation of BLUD in Indonesia. The selection process was carried out through stages of identification, screening, and feasibility until nine relevant full-text articles were obtained. The results of the study show that, in general, the implementation of BLUD has a positive impact on increasing revenue and certain financial ratios, especially profitability and liquidity. However, several studies found that this increase was not evenly distributed across all indicators, particularly profitability and service performance. These findings indicate that the success of BLUD implementation is greatly influenced by the quality of financial management, human resource competencies, and the optimization of flexibility provided by regulations.

Abstract

The transformation of the healthcare system toward Value-Based Healthcare requires efficient cost management without compromising patient quality and safety. This study aims to analyze the impact of cost accounting implementation on healthcare quality management through a systematic literature review approach. The research method follows the PRISMA 2020 guidelines, with literature searches conducted through Google Scholar, PubMed, and SINTA databases. Article selection was carried out using the PICOS framework to ensure relevance to the research objectives. The screening process resulted in nine eligible articles that were analyzed through qualitative synthesis. The findings indicate that the implementation of modern cost accounting methods, such as Activity-Based Costing (ABC), Time-Driven Activity-Based Costing (TDABC), and Itemized Point Cost (IPC), significantly contributes to improving operational efficiency, enhancing cost transparency, reducing the risk of medical errors, and supporting healthcare accreditation standards. However, excessive emphasis on strict cost control may influence patients’ perceptions of service quality. Therefore, the integration of financial and non-financial performance indicators is essential to ensure that economic efficiency aligns with continuous improvement in healthcare service quality.

Abstract

Drug logistics planning in primary healthcare centers remains a major challenge in ensuring medicine availability and continuity of health services. Inadequate planning often leads to stockouts, overstock, and mismatches between drug supply and patient needs. This study aimed to analyze the implementation of drug logistics planning management at Pakuan Baru Public Health Center, Jambi City. This research used a qualitative observational design with a case study approach. The population consisted of all personnel involved in drug logistics planning, while seven informants were selected using purposive sampling. Data were collected through semi-structured interviews, observation checklists, and document review of LPLPO, stock cards, and planning reports. Data analysis employed the Miles and Huberman model through data reduction, data display, and conclusion drawing with triangulation techniques. The results showed that drug planning was conducted through stages of drug selection, consumption compilation, calculation of drug needs, projection, and procurement adjustment based on LPLPO data, National Formulary, and consumption methods. However, several obstacles were identified, including distribution delays, mismatches between proposed and received drugs, limited budgets, and recurring stock shortages. In conclusion, drug logistics planning management at the health center has generally followed national guidelines, but improvements in data based planning, distribution systems, and monitoring are still required to optimize drug availability and service continuity.

Abstract

The use of digital health services through the Mobile JKN application has become an important part of the transformation of health services in the digital era. However, the use of the Mobile JKN application in the community is still not optimal. This study aims to analyze the relationship between perceived usefulness, perceived ease of use, attitudes toward use, and intention to use with the use of the Mobile JKN application. This study used a quantitative design with a cross-sectional approach. The study population was JKN participants in the Rupit Community Health Center working area with a sample of 96 respondents selected using an accidental sampling technique. The research instrument used a structured questionnaire, while data analysis was carried out using the chi-square test and the calculation of the Prevalence Ratio (PR). The results showed a significant relationship between perceived usefulness (p = 0.045; PR = 2.02), perceived ease of use (p = 0.000; PR = 3.36), attitudes toward use (p = 0.017; PR = 3.31), and intention to use (p = 0.027; PR = 2.14) with the use of the Mobile JKN application. Perceived ease of use was the factor with the greatest risk value for not using the application. The conclusion of this study is that factors in the Technology Acceptance Model are related to the use of the Mobile JKN application, so that increasing ease, benefits, positive attitudes, and usage intentions need to be strengthened to increase sustainable use of the application.

Abstract

This study aims to analyze the readiness of health facilities in implementing the Free Health Check Program (CKG) at Kebun Kopi Public Health Center using the Organizational Readiness for Change approach. The results indicate that the overall level of organizational readiness is moderate with a high level of vulnerability. Change commitment is very high due to compliance with government policies, but it is not matched by adequate change efficacy due to operational constraints. Human resource readiness reflects strong commitment but is challenged by workload overload and burnout risk. Infrastructure is sufficient in quantity but constrained by damaged equipment, limited reagents, and unstable digital systems. Budget limitations increase reliance on internal BLUD funds. Policies and SOPs are strong but lack flexibility. Socialization is extensive but hindered by low public trust in data security.

Abstract

This study aims to analyze the implementation of Mobile JKN services at dr. Bratanata Hospital, Jambi City in 2025. This study uses a qualitative approach with a case study design to explore real conditions in the field. Data were collected through in-depth interviews, observation, and documentation involving key informants, main informants, and supporting informants. The findings indicate that the implementation of Mobile JKN has not been fully optimal. Technical barriers such as system errors, unstable internet connections, and data synchronization issues are still frequently encountered. In addition, non-technical barriers such as low digital literacy among patients and limited human resources also affect service effectiveness. Weak coordination between institutions further complicates the implementation process. These conditions result in increased waiting times, higher patient complaints, and a return to manual service systems. Strengthening infrastructure, improving human resource capacity, and optimizing inter-system integration are necessary to enhance digital health service performance.

Abstract

Maternal Mortality Rate (MMR) remains a major public health concern in Indonesia. Optimal utilization of Antenatal Care (ANC) is essential for early detection of pregnancy-related risks and prevention of maternal complications. Maternal knowledge, family support, and the quality of healthcare services may influence ANC utilization. This case-control study analyzed the association of maternal knowledge, family support, and healthcare service quality with ANC utilization among 60 respondents (30 cases and 30 controls) at Putri Ayu Public Health Center, Jambi City. Data were collected using structured questionnaires and analyzed with the Exact McNemar test at a 95% confidence level. Results: Maternal knowledge was significantly associated with ANC utilization (p = 0.039). However, family support (p = 0.727) and healthcare service quality (p = 0.250) showed no significant association. Abstract

The waste problem in Batang Hari Regency continues to increase in line with population growth, with a daily volume of around 31 tons and 87% of temporary waste storage sites being illegal. This condition has an impact on public health, aesthetics, and environmental pollution. Waste management analysis is needed to improve management effectiveness, but such analysis has not been conducted comprehensively with involvement of stakeholders. The research aims to explore and provide an in-depth understanding of the social phenomenon being observed, focusing on detailed descriptions of field conditions. This research uses qualitative case study design. Data was collected through in-depth interviews, observations, and document reviews involving 10 informants. Data analysis was conducted using content analysis techniques, and data validity was ensured through triangulation of sources. The results show that waste management planning has been developed but has not been optimally implemented due to limitations in budget, fleet, and manpower. The implementation of collection and transportation is constrained by facilities and low public awareness of waste sorting. Evaluations are conducted but are not consistent, especially regarding illegal temporary waste storage sites. Follow-up actions in the form of socialization, additional facilities, and technical improvements have been carried out, but they have not completely resolved the problem. The implementation of management plays a crucial role in identifying weaknesses and promoting continuous improvement. Strengthening the budget, adding facilities, increasing the capacity of personnel, and active community participation are urgently needed to make waste management more optimal, sustainable, and positively impact health and the environment.

Abstract

Pelayanan kesehatan balita merupakan salah satu indikator Standar Pelayanan Minimal (SPM) yang wajib dipenuhi oleh Puskesmas. Namun, capaian pelayanan kesehatan balita di Puskesmas Simpang Sungai Duren belum mencapai target SPM, sehingga diperlukan evaluasi untuk mengetahui penyebab rendahnya capaian tersebut. Penelitian ini bertujuan untuk mengevaluasi capaian indikator kinerja SPM kesehatan balita di Puskesmas Simpang Sungai Duren berdasarkan aspek input, activities, dan output. Penelitian ini menggunakan jenis penelitian kualitatif deskriptif dengan pendekatan studi kasus. Informan dalam penelitian ini ditentukan menggunakan teknik purposive sampling sebanyak 10 orang, terdiri dari informan kunci, informan utama, dan informan pendukung. Pengumpulan data dilakukan melalui wawancara mendalam, observasi dan dokumentasi. Hasil penelitian menunjukkan bahwa pada aspek input, sumber daya manusia, sarana prasarana, dana dan kebijakan secara umum telah mendukung pelaksanaan pelayanan meskipun masih terdapat beberapa keterbatasan. Pada aspek activities, pelayanan kesehatan balita telah dilaksanakan sesuai standar, namun beberapa kegiatan belum berjalan optimal. Pada aspek output, capaian indikator SPM kesehatan balita belum mencapai target. Rendahnya capaian tersebut lebih dominan dipengaruhi oleh faktor eksternal, terutama rendahnya partisipasi masyarakat dalam memanfaatkan pelayanan kesehatan balita secara rutin. Oleh karena itu, dapat disimpulkan bahwa meskipun pelayanan telah berjalan sesuai standar, masih diperlukan optimalisasi pelaksanaan program dan peningkatan partisipasi masyarakat perlu menjadi prioritas dalam upaya meningkatkan capaian SPM kesehatan balita.

Abstract

Pengelolaan sampah di Kabupaten Tanjung Jabung Timur menghadapi berbagai kendala ditandai meningkatnya timbulan sampah, adanya praktik pembakaran dan pembuangan sampah kesungai, berdampak terhadap lingkungan dan kesehatan masyarakat, sehingga diperlukan manajemen pengelolaan sampah yang efektif. Penelitian ini bertujuan untuk menganalisis manajemen pengelolaan sampah di Kabupaten Tanjung Jabung Timur menggunakan pendekatan PDCA. Penelitian ini metode kualitatif deskriptif dengan pendekatan studi kasus. Informan ditentukan menggunakan teknik purposive sampling yang terdiri dari 10 orang. Data dikumpulkan melalui wawancara mendalam, observasi dan dokumentasi kemudiaan dianalisis menggunakan analisis tematik. Hasil penelitian menunjukkan bahwa perencanaan pengelolaan sampah telah mengacu pada peraturan dan dokumen Jakstrada Namun belum tersosialisasi ke masyarakat terkait perencanaan tersebut. Pelaksanaan pengelolaan sampah belum berjalan maksimal, ditandai dengan belum optimalnya kegiatan pengurangan sampah berbasis 3R, pemilahan sampah dari sumber, serta cakupan pelayanan yang belum menjangkau seluruh wilayah. Keterbatasan sumber daya manusia, anggaran, sarana prasarana, dan rendahnya partisipasi masyarakat menjadi kendala utama. Tahap pengecekan telah dilakukan melalui monitoring rutin, tetapi belum disertai pengukuran capaian target secara spesifik, sedangkan tindak lanjut masih berfokus pada penyelesaian masalah operasional dan belum menghasilkan perbaikan sistem yang berkelanjutan. Dapat disimpulkan bahwa manajemen pengelolaan sampah di Kabupaten Tanjung Jabung Timur telah menerapkan siklus PDCA, namun implementasinya belum optimal sehingga diperlukan penguatan partisipasi masyarakat, penyediaan sumber daya, serta sistem evaluasi yang lebih terukur untuk meningkatkan efektivitas pengelolaan sampah.
