Abstract

Malaria adalah salah satu penyakit menular di masyarakat yang biasanya ada didaerah beriklim tropis. Penyakit Malaria ditularkan melalui perantara  vektor gigitan nyamuk Anopheles, dimana daerah yang beriklim tropis menjadi salah satu tempat perkembangbiakan dan habitat yang baik bagi nyamuk Anopheles. Infeksi parasite Plasmodium yang ada dalam tubuh nyamuk Anopheles kepada Manusia (Host) akan ditandai dengan gejala demam, sakit kepala, mengigil terkadang ada rasa mual dan anemia. Kebaruan penelitian yaitu mengetahui faktor host dan vector dengan kejadian Malaria berdasarkan penelitian yang sudah ada. Tujuan penelitian yaitu untuk menggambarkan hubungan fakor vector dan host  dengan kesakitan atau kasus malaria. Metode penelitian yang di gunakan dalam Penelitian ini dengan cara sistematis melakukan Tinjauan Pustaka merangkum hasil penelitian dalam kurun waktu 5 tahun terakhir. Sumber data berasal dari Jurnal Kesehatan Lingkungan Indonesia dan Google Scholar dengan kata kunci “Vektor, Host dan Malaria”. Artikel yang dipilih adalah hasil penelitian terkait faktor vector dan host yang menjadi penyebab kejadian malaria. Hasil penelitian bahwa terdapat 8 artikel yang digunakan sebagai referensi saat pembuatan jurnal literature review, dan artikel menyebutkan bahwa antara faktor vector dan host sangat berhubungan dengan kasus kesakitan malaria. Kesimpulan bahwa hubungan antara vector, host dengan kasus atau kesakitan malaria diantaranya lingkungan yang mendukung untuk perkembangbiakan vektor serta perilaku manusia seperti kebiasaan keluar malam, menutup ventilasi rumah dengan kawat kassa dan kebiasaan untuk tidur menggunakan kelambu sangat menentukan penularan kesakitan malaria.

Abstract

Pemeriksaan kualitas air minum PDAM Kota Pekalongan bulan Desember 2023, total Coliform  118 CFU/100 ml sampel, Warna 10 TCU,  Besi (Fe) 0,23 mg/l dan Mangan (Mn) 1,14 mg/l melebihi baku mutu Permenkes No 2 Tahun 2023. Adanya keluhan masyarakat yang menyatakan air minum yang kotor serta gangguan kesehatan (gatal-gatal) yang dilaporkan oleh salah satu Lembaga Swadaya Masyarakat (LSM), selain itu terdapat kematian 2 balita diduga karena diare serta masalah stunting naik dari 20,6% menjadi 28,2% di tahun 2023. Kebaruan penelitian ini adalah implementasi rencana pengamanan air minum. Tujuan penelitian ini untuk mengetahui resiko keamanan air minum pada aspek kualitas serta mengevaluasi implementasi rencana pengamanan air minum Kota Pekalongan. Jenis penelitian ini adalah penelitian deskriptif dengan mix method kuantitatif dan kualitatif. Sampel penelitian ini adalah 5 orang operator di lima subsistem PDAM Kota Pekalongan dan 30 titik sampel air yang diambil menggunakan metode total sampling. Penelitian ini diperkuat dengan indepth interview pada informan penelitian dan metadata hasil kegiatan surveilans kualitas air minum rumah tangga (SKAMRT) Kota Pekalongan Tahun 2024. Hasil penelitian ini ditemukan 99 resiko kejadian bahaya dengan tingkatan 4 kejadian kategori sangat tinggi, 10 kejadian kategori tinggi, 6 kejadian kategori sedang dan 79 kejadian kategori rendah. Kualitas air minum Kota Pekalongan belum dapat disebut aman menurut Permenkes No 2 Tahun 2023. Kesimpulannya yaitu Kota Pekalongan secara konsep dan upaya sudah menerapkan RPAM, tetapi belum secara dokumen, sehingga bisa dikatakan ada pada RPAM tingkat dasar.

Abstract

Tuberculosis is the second infectious disease that causes the most deaths in the world according to WHO. Indonesia is a high TB burden country which currently ranks third in the world, with an incidence of 845.000 or 320/100.000 population with a mortality rate of 98.000 or 40/100.000 population and 3,6/100.000 TB-HIV population. The risk factor that can cause tuberculosis disease is malnutrition. The aim of the study was to determine the relationship between nutritional status and the incidence of tuberculosis transmission in the family. The purpose of this study was to determine the relationship between nutritional status and the incidence of tuberculosis. The research design used cross sectional study. The research was conducted in December 2023 in Batusumur Village, Manonjaya District, Tasikmalaya Regency. The research sample was 39 respondents of tuberculosis patients. Inclusion criteria: patients actively communicate and are easy to talk to, patients live with family members and are still in the initial phase of treatment in the first 2 months. The independent variable was nutritional status. The dependent variable was the incidence of pulmonary tuberculosis transmission in family members. The instrument was an observation sheet. Data were analyzed by chi square test using SPSS. This research applies the principles of confidentiality, useful information and scientific development. The results showed that there was a relationship between nutritional status (p=0,026) and the incidence of tuberculosis transmission in the family. The conclusion is that there is a relationship between nutritional status and the incidence of tuberculosis. Tuberculosis patients and their families can make preventive efforts to prevent tuberculosis transmission.

Abstract

Penelitian ini bertujuan untuk mengevaluasi faktor risiko penularan demam berdarah dengue (DBD) di daerah endemis Kabupaten Katingan, Kalimantan Tengah, pada tahun 2023, yang mengalami lonjakan kasus signifikan. Berdasarkan data 2023, incidence rate di Kalimantan Tengah mencapai 113,86 per 100.000 penduduk, dengan Case Fatality Rate (CFR) 0,38% dan delapan kematian. Penelitian observasional dengan desain cross-sectional ini dilakukan pada April 2024 di tiga kecamatan endemis DBD, yakni Katingan Hilir, Katingan Tengah, dan Sanaman Mantikei. Hasil penelitian menunjukkan bahwa Breeding Risk Index (BRI) di daerah tersebut berada pada kategori sedang, dengan 46% rumah berisiko sedang. Hygiene Risk Index (HRI) juga berada pada kategori sedang, dengan 48,7% rumah berisiko sedang. Lebih lanjut, sebanyak 23,9% rumah di daerah endemis DBD berada pada kategori sedang (BRI 2/HRI 2). Selain itu, 45,1% rumah memiliki Maya Index yang tinggi, menunjukkan potensi besar untuk berkembangnya vektor nyamuk. Kesimpulannya, wilayah ini memiliki risiko sedang untuk penularan DBD, dengan faktor lingkungan yang mendukung penyebaran nyamuk Aedes aegypti. Implikasi penelitian ini menunjukkan perlunya peningkatan program pengendalian vektor melalui pendekatan berbasis komunitas, seperti perbaikan sanitasi dan peningkatan kesadaran masyarakat dalam mengurangi tempat perindukan nyamuk. Abstract

Dengue Hemorrhagic Fever (DHF) is a public health problem in Indonesia and still often causes extraordinary events with a high mortality rate. All regions in Indonesia are at risk of contracting dengue disease, because the virus that causes and mosquitoes has spread widely in residential and public places in Indonesia. Penjaringan District North Jakarta City has 70% densely populated residential areas, so it has a risk for dengue disease transmission. The sample in this study amounted to 80 people, with a division of 40 case groups and 40 control groups. This research uses observational analytical design with case control method. The variables studied were Mosquuito Nest Eradication practice and the presence of larvae. Data on Mosquuito Nest Eradication Behavior-DHF variables were obtained through questionnaires and variable data on the presence of larvae were obtained through direct observation of the environment around the respondent's house. Based on the results of Chi Square analysis showed that the practice of Mosquuito Nest Eradication (p = 0.007) and the presence of larvae (p = 0.047) had a significant relationship. Respondents who are lacking in Mosquuito Nest Eradication practice are at 5,952 times greater risk of developing DHF compared to people who are good in Mosquuito Nest Eradication practice. There is a relationship between the practice of Mos and the existence of larvae with dengue incidence in densely populated settlements in Penjaringan District, North Jakarta. The community should pay attention to the eradication of their nest by conducting the 3M Plus program regularly

Abstract

There are several locations in DKI Jakarta which have the potential become breeding places for Anopheles spp larvae. The aim of this study was to analyze environmental vulnerability to the presence of Anopheles spp. larvae with malaria cases using a geographic information system approach. This study was an observational study with a cross sectional design. This study was carried out in March – June 2023 in North Jakarta City, DKI Jakarta. The sample consists of data from all malaria patients from 2020-2023 in North Jakarta. The global positioning system coordinates were taken at the addresses of malaria patients. In addition, data on the presence of Anopheles spp. obtained from larval surveys conducted in Cilincing and Penjaringan Sub-Districts and the coordinates for the habitat of Anopheles spp. larvae. The GPS points obtained were then used for spatial analysis. The distribution of malaria cases was random, while the distribution of Anopheles larvae was dispersed. Buffering zone >1000 m from the breeding site indicated that it does not have the potential in malaria transmission due to malaria patients were at a long distance from the breeding site of Anopheles spp. and flight distance of Anopheles spp. not exceeding 1000 m

Abstract

Pneumonia is leading cause of death worldwide, with a prevalence of 44%, and is second leading cause of death among under-fives in Indonesia, at 15.5%. Pneumonia is caused by viruses and bacteria that are associated with physical conditions, air quality, and smoking habits in house. This study aimed to determine association between physical house conditions and smoking habits with incidence of pneumonia in children under five years in Cengkareng Health Center working area. This study was a case-control design analytic observational study. Population involved chindren under five aged 12-59 months with a sample of 43 cases and 43 controls calculated using Lemeshow’s formula, taken using purposive sampling technique. Independent variables included floor and wall types, ventilation, occupancy density, temperature, humidity, light intensity, PM10 and PM2.5 levels, and smoking habits. Dependent varariable are incident of pneumonia. Data were collected through observation, sanitarian kits, and questionnaires. Data were analyzed using chi-square and logistic regression with 95% CI and alpha 0.05. The results showed that PM2.5 levels (p=0.010, OR=3.290, 95% CI=1.323-8.181) and indoor smoking (p=0.044, OR=2.758, 95% CI=1.027-7.410) were most significant factors associated with pneumonia. This study recommends optimizing educational programs regarding PM2.5 exposure and smoking habits to improve healthy living practices and reduce risk of pneumonia

Abstract

Diarrhea is a common symptom of gastrointestinal infection caused by various bacterial, viral, and parasitic organisms and is characterized by loose stools and increased frequency when infected. Sanitation factors are closely related to environmental health and can affect the level of public health. The absence of a spatial picture of areas that are vulnerable to an increase in cases of children under five years old diarrhea complicates important case finding efforts to prevent outbreaks. The purpose of the study was to spatially analyze the effect of drinking water quality and sanitation on the incidence of diarrhea in children under five years old in the Administrative City of West Jakarta in 2023. This study used a quantitative method with a Cross Sectional research design and an ecological study with a spatial approach to determine the correlation of risk factors with cases of diarrhea in children under five years old geographically. The results of the study showed that the quality of environmental sanitation between the diarrhea group in the last 3 months and the no diarrhea group in the last 3 months had a value (p value = 0.373), water quality (p value = 0.036) with PR 1.840, CTPS habits (p value = 0.100) with PR 1.509, and type of drinking water source (p value = 1.151) with PR 1.442. And the water quality variable is the most dominant factor related to the incidence of diarrhea in children under five years old  in the West Jakarta Administrative City in 2024. The results of the multivariate analysis of logistic regression with the ward conditional backward method p value = 0.32 with PR = 4.110 CI = Low 1.129 and upper 14.96 which shows that children under five years old with drinking water quality that does not meet the requirements have a 4.110 greater chance than toddlers whose drinking water quality meets the requirements in the ODF sub-district in the West Jakarta Administrative City

Abstract

Tuberculosis (TB) remains a major health problem in Indonesia, including in Tembalang Sub-district, Semarang City, which is listed as one of the areas with high cases. Indonesia ranks among the top three countries in the world with the highest number of TB cases exceeding one million cases. This research is an observational analytic study with a case-control study design. The purpose of this study was to assess the risk factors of household physical environment that affect the incidence of Tuberculosis in Tembalang Sub-district. The results showed that most of the houses with tuberculosis patients had unhealthy environmental conditions, such as poor air circulation (37.5%), high occupancy density (35%), improper flooring (57.5%), insufficient ventilation (67.5%), low lighting (85%), and high humidity (87.5%). These factors were found to have a significant association with an increased risk of TB. However, ceiling conditions and room temperature did not show a significant association with increased TB risk. These findings emphasize the importance of improving the living environment through education and community involvement in improving housing quality as a preventive measure to reduce TB transmission.

Abstract

The Troso weaving industry in Jepara, Indonesia, is a traditional craft sector that relies on synthetic dyes and manual production, thereby exposing workers to chromium (Cr) and particulate matter (PM2.5 and PM10) with potential respiratory and neurological risks. This study aimed to analyze their association with respiratory and neurological disorders among workers. A cross-sectional design was applied involving 101 weaving workers from four production sites. Environmental sampling measured Cr concentrations in wastewater and PM2.5/PM10 in the ambient air, while worker characteristics and health complaints were assessed using the ECSC-87 and Q18 questionnaire. Data were analyzed using chi-square tests and odds ratios. Cr concentrations in wastewater ranged from <0.010 to 0.104 mg/L, remaining below the effluent quality standard of 1.0 mg/L but confirming the continuous use of chromium-based dyes discharged without treatment. Average PM2.5 and PM10 levels were 108.75 µg/m³ and 117.75 µg/m³, both exceeding national air quality standards (55 and 75 µg/m³). Bivariate analysis showed that respiratory disorders were significantly associated with exposure duration ≥40 hours/week (OR=7.333; p=0.0001) and working period >5 years (OR=20.935; p=0.0001), whereas dye type, dyeing frequency, and PPE use were not significant. Neurological disorders were significantly associated with exposure duration (p=0.001), working period (p=0.001), type of dye (p=0.0001), frequency of dyeing (p=0.018), and PPE use (p=0.0001). Environmental monitoring confirmed elevated PM levels and chromium-based dye use, while statistical analysis showed that prolonged exposure and long working periods associated with respiratory disorders, and neurological disorders were linked to multiple occupational factors.
