Abstract

Pemeriksaan kualitas air minum PDAM Kota Pekalongan bulan Desember 2023, total Coliform  118 CFU/100 ml sampel, Warna 10 TCU,  Besi (Fe) 0,23 mg/l dan Mangan (Mn) 1,14 mg/l melebihi baku mutu Permenkes No 2 Tahun 2023. Adanya keluhan masyarakat yang menyatakan air minum yang kotor serta gangguan kesehatan (gatal-gatal) yang dilaporkan oleh salah satu Lembaga Swadaya Masyarakat (LSM), selain itu terdapat kematian 2 balita diduga karena diare serta masalah stunting naik dari 20,6% menjadi 28,2% di tahun 2023. Kebaruan penelitian ini adalah implementasi rencana pengamanan air minum. Tujuan penelitian ini untuk mengetahui resiko keamanan air minum pada aspek kualitas serta mengevaluasi implementasi rencana pengamanan air minum Kota Pekalongan. Jenis penelitian ini adalah penelitian deskriptif dengan mix method kuantitatif dan kualitatif. Sampel penelitian ini adalah 5 orang operator di lima subsistem PDAM Kota Pekalongan dan 30 titik sampel air yang diambil menggunakan metode total sampling. Penelitian ini diperkuat dengan indepth interview pada informan penelitian dan metadata hasil kegiatan surveilans kualitas air minum rumah tangga (SKAMRT) Kota Pekalongan Tahun 2024. Hasil penelitian ini ditemukan 99 resiko kejadian bahaya dengan tingkatan 4 kejadian kategori sangat tinggi, 10 kejadian kategori tinggi, 6 kejadian kategori sedang dan 79 kejadian kategori rendah. Kualitas air minum Kota Pekalongan belum dapat disebut aman menurut Permenkes No 2 Tahun 2023. Kesimpulannya yaitu Kota Pekalongan secara konsep dan upaya sudah menerapkan RPAM, tetapi belum secara dokumen, sehingga bisa dikatakan ada pada RPAM tingkat dasar.

Abstract

Tuberculosis is the second infectious disease that causes the most deaths in the world according to WHO. Indonesia is a high TB burden country which currently ranks third in the world, with an incidence of 845.000 or 320/100.000 population with a mortality rate of 98.000 or 40/100.000 population and 3,6/100.000 TB-HIV population. The risk factor that can cause tuberculosis disease is malnutrition. The aim of the study was to determine the relationship between nutritional status and the incidence of tuberculosis transmission in the family. The purpose of this study was to determine the relationship between nutritional status and the incidence of tuberculosis. The research design used cross sectional study. The research was conducted in December 2023 in Batusumur Village, Manonjaya District, Tasikmalaya Regency. The research sample was 39 respondents of tuberculosis patients. Inclusion criteria: patients actively communicate and are easy to talk to, patients live with family members and are still in the initial phase of treatment in the first 2 months. The independent variable was nutritional status. The dependent variable was the incidence of pulmonary tuberculosis transmission in family members. The instrument was an observation sheet. Data were analyzed by chi square test using SPSS. This research applies the principles of confidentiality, useful information and scientific development. The results showed that there was a relationship between nutritional status (p=0,026) and the incidence of tuberculosis transmission in the family. The conclusion is that there is a relationship between nutritional status and the incidence of tuberculosis. Tuberculosis patients and their families can make preventive efforts to prevent tuberculosis transmission.

Abstract

Penelitian ini bertujuan untuk mengevaluasi faktor risiko penularan demam berdarah dengue (DBD) di daerah endemis Kabupaten Katingan, Kalimantan Tengah, pada tahun 2023, yang mengalami lonjakan kasus signifikan. Berdasarkan data 2023, incidence rate di Kalimantan Tengah mencapai 113,86 per 100.000 penduduk, dengan Case Fatality Rate (CFR) 0,38% dan delapan kematian. Penelitian observasional dengan desain cross-sectional ini dilakukan pada April 2024 di tiga kecamatan endemis DBD, yakni Katingan Hilir, Katingan Tengah, dan Sanaman Mantikei. Hasil penelitian menunjukkan bahwa Breeding Risk Index (BRI) di daerah tersebut berada pada kategori sedang, dengan 46% rumah berisiko sedang. Hygiene Risk Index (HRI) juga berada pada kategori sedang, dengan 48,7% rumah berisiko sedang. Lebih lanjut, sebanyak 23,9% rumah di daerah endemis DBD berada pada kategori sedang (BRI 2/HRI 2). Selain itu, 45,1% rumah memiliki Maya Index yang tinggi, menunjukkan potensi besar untuk berkembangnya vektor nyamuk. Kesimpulannya, wilayah ini memiliki risiko sedang untuk penularan DBD, dengan faktor lingkungan yang mendukung penyebaran nyamuk Aedes aegypti. Implikasi penelitian ini menunjukkan perlunya peningkatan program pengendalian vektor melalui pendekatan berbasis komunitas, seperti perbaikan sanitasi dan peningkatan kesadaran masyarakat dalam mengurangi tempat perindukan nyamuk. Abstract

The Troso weaving industry in Jepara, Indonesia, is a traditional craft sector that relies on synthetic dyes and manual production, thereby exposing workers to chromium (Cr) and particulate matter (PM2.5 and PM10) with potential respiratory and neurological risks. This study aimed to analyze their association with respiratory and neurological disorders among workers. A cross-sectional design was applied involving 101 weaving workers from four production sites. Environmental sampling measured Cr concentrations in wastewater and PM2.5/PM10 in the ambient air, while worker characteristics and health complaints were assessed using the ECSC-87 and Q18 questionnaire. Data were analyzed using chi-square tests and odds ratios. Cr concentrations in wastewater ranged from <0.010 to 0.104 mg/L, remaining below the effluent quality standard of 1.0 mg/L but confirming the continuous use of chromium-based dyes discharged without treatment. Average PM2.5 and PM10 levels were 108.75 µg/m³ and 117.75 µg/m³, both exceeding national air quality standards (55 and 75 µg/m³). Bivariate analysis showed that respiratory disorders were significantly associated with exposure duration ≥40 hours/week (OR=7.333; p=0.0001) and working period >5 years (OR=20.935; p=0.0001), whereas dye type, dyeing frequency, and PPE use were not significant. Neurological disorders were significantly associated with exposure duration (p=0.001), working period (p=0.001), type of dye (p=0.0001), frequency of dyeing (p=0.018), and PPE use (p=0.0001). Environmental monitoring confirmed elevated PM levels and chromium-based dye use, while statistical analysis showed that prolonged exposure and long working periods associated with respiratory disorders, and neurological disorders were linked to multiple occupational factors.

Abstract

Background: Patient safety is a crucial aspect in improving the quality of healthcare and reducing the risk of preventable injuries in hospitals. Incident data suggests that the quality of care still needs to be improved for safe and quality care. Purpose: To examine the implementation of the 2022 national hospital accreditation standards for patient safety goals at RSI PKU Muhammadiyah Pekajangan, evaluate the hospital’s efforts in reducing patient safety incidents, and assess patient satisfaction. Method: Qualitative phenomenological approach with in-depth interview techniques and triangulation. Informants consisted of 3 main informants and 5 triangulation informants. Results: Although the hospital has undertaken various initiatives to reduce risks and enhance patient safety, incidents related to patient safety continue to occur. Nevertheless, the implementation of patient safety measures appears to positively influence patient satisfaction. Survey results indicate an increase in patient satisfaction following the promotion of patient safety programs. Additionally, the hospital achieved a 91.67% score in meeting patient safety goals, reflecting substantial compliance with accreditation standards. The adoption of the Hospital Accreditation Standards in accordance with Ministry of Health Regulation No. 1128 of 2022 has also contributed positively to strengthening patient safety practices. Conclusion: Findings from in-depth interviews across the dimensions of structure, process, and outcomes indicate that, following accreditation, RSI PKU Muhammadiyah Pekajangan has consistently implemented the Hospital Accreditation Standards in accordance with Ministry of Health Regulation No. 1128 of 2022. A notable reduction in Near Miss (NM) incidents was observed, along with a high level of patient satisfaction. The achievement of full accreditation and adherence to the updated standards have had a positive impact on enhancing patient safety within the hospital.

Abstract

Background : Katingan Regency has experienced an increase in dengue fever cases in Central Kalimantan Province over the past three years, from 2020 to 2023, with an incidence rate in 2023 of 168.4 per 100,000 population and a Case Fatality Rate (CFR) of 0.66%. This is due to the Maya Index, which is a potential breeding ground for Aedes sp mosquitoes, remaining a problem. Therefore, research will be conducted to detect the Maya Index of Aedes sp mosquitoes in dengue fever endemic areas in Katingan Regency.Methods : This study was conducted using an observational research method with a cross-sectional approach in April 2024. The population in this study consisted of all cases of dengue fever in January–February 2024, with a sample size of 113 samples. The sampling technique used was random sampling. Data were obtained through direct observation and observation of the home environment using the visual larval method.Result : The Breeding Risk Index (BRI) was in the moderate category, with 52 (46%) homes, while the Hygiene Risk Index (HRI) was also in the moderate category, with 55 (48.7%) homes. in dengue endemic areas, it is in the moderate category (BRI 2/HRI 2), which is 27 (23.9%) houses. Most houses in dengue endemic areas in Katingan Regency are in the high Maya Index status, which is 51 (45.1%) houses.Conclusion : Houses with low to moderate BRI will have less potential for having positive larval containers than houses with high BRI. Houses with low to moderate HRI have higher positive larval containers than houses with high HRI.

Abstract

Introduction: The second-largest tuberculosis hotspot in the world is located in Indonesia, which creates a significant threat to global health. The disease causes severe harm to patients' health status because it damages their physical health, mental state, and social relationships. The development of tuberculosis control strategies requires knowledge about elements that impact the maintenance of the health quality of tuberculosis patients. This study aims to explore the determinants of quality of life in tuberculosis patients qualitatively using thematic analysis techniques with Atlas.ti software in Yogyakarta. Methods: This study used a qualitative design and a case study technique. Patients and TBC program holders from Yogyakarta City's primary healthcare institutions participated in the event. Nine participants were selected using a purposive sampling strategy from individuals who had recently completed treatment and were educated about TBC. In-depth interviews were conducted utilizing an interview guide to gather data. Atlas.ti was used for thematic analysis of the data. This project has been granted ethical approval under license number 369/EA/KEPK-FKM/2024. Results: Most TBC patients have a low quality of life due to physical and psychological disorders, as well as the influence of social stigma. At the same time, the environmental health dimension is in good condition. Determining factors include intrapersonal aspects (knowledge, self-efficacy, motivation), interpersonal aspects (family knowledge and support), and environmental aspects (stigma). Therefore, psychosocial education for patients and their families is crucial for improving quality of life. Conclusion: Tuberculosis patients in Yogyakarta City continue to have a bad quality of life in terms of physical, psychological, and social dimensions. As a result, education is required to promote awareness of TBC among patients and their families, encouraging social support to aid in the healing process, and reducing societal stigma.

Abstract

Dengue Hemorrhagic Fever (DHF) remains one of the major public health challenges, particularly in tropical countries such as Indonesia. Conventional vector control strategies, such as fogging and mosquito breeding site eradication, have encountered significant obstacles, including insecticide resistance and limited long-term effectiveness. This study aims to examine the effectiveness of various natural attractants as an alternative approach to attracting Aedes mosquitoes through a Systematic Literature Review (SLR) of 24 published articles from 2020 to 2025. The attractants analyzed include fermented rice straw water, coconut husks, fermented sugar solutions, as well as grass-based and plant semiochemical attractants. The review results indicate that fermented rice straw water is the most effective attractant for Aedes mosquitoes, especially for gravid females, due to its organic compounds produced during fermentation, such as ammonia and volatile acids. Sugar solutions, particularly in the form of Attractive Toxic Sugar Baits (ATSB), also show high effectiveness in both attracting and killing mosquitoes. In contrast, coconut husk attractants tend to be less effective and their performance varies depending on environmental conditions. Grass-based attractants such as Megathyrsus maximus also show potential but still require broader field validation. Based on a literature review of 24 studies, it can be concluded that straw-soaked water is the most effective attractant. This is effective with 3-7 days of fermentation at warm temperatures and in dark containers. Environmental health implications include its suitability as a low-cost lure and kill. Integrating natural attractants with attract-and-kill approaches such as ATSB can enhance the effectiveness of dengue control programs in a sustainable manner, especially in resource-limited settings.

Abstract

Introduction: The rising global production of municipal solid waste (MSW) poses significant ecological challenges. In developing countries, inadequate landfill management results in the uncontrolled release of hazardous gases and leachate, posing significant risks to human health. This systematic review investigated the association between environmental factors at these landfills and adverse human health outcomes. Methods: This review was conducted in accordance with the PRISMA guidelines. A comprehensive search was rapidly conducted across ScienceDirect, Scopus, PubMed, and EBSCOhost for articles published between 2020 and 2025. PECO criteria screening was conducted independently by two reviewers, resulting in the selection of 25 articles for critical synthesis. Results: Hazardous landfill pollutants—including heavy metals, volatile organic compounds (VOCs), and particulate matter—are rapidly escalating severe health issues. Residents and landfill workers face acute respiratory and cardiovascular disorders, heightened cancer risk, reproductive issues, and neurological impairments. Leachate contamination of soil and groundwater compounds these urgent systemic risks. Conclusion:The ongoing failure to manage landfills effectively, particularly in developing countries, has triggered a pressing environmental and public health crisis. Immediate action to improve waste management and enforce stricter environmental policies is critical for preventing further harm.

Abstract

Demam Berdarah masih menjadi penyakit endemis di Indonesia serta sering menimbulkan wabah atau kejadian luar biasa (KLB) dengan angka kematian yang tinggi. Demam Berdarah Dengue (DBD) disebabkan oleh virus dengue dan ditularkan oleh nyamuk aedes betina dan saat ini masih menjadi masalah kesehatan yang belum dapat diatasi sepenuhnya. Tujuan penelitian adalah menganalisis faktor yang berhubungan dengan kejadian demam berdarah dengue di Kabupaten Demak. Desain penelitian menggunakan analisis kuantitatif dengan pendekatan cross sectional. Penelitian dilaksanakan di Kabupaten Demak Provinsi Jawa Tengah Indonesia pada bulan Januari-Desember 2022. Populasi penelitian adalah pasien yang melakukan periksa di pelayanan kesehatan Kabupaten Demak. Sampel penelitian adalah pasien yang mendapatkan diagnose demam berdarah sejumlah 100 responden. Tekhnik pengumpulan data yaitu observasional dan menggunakan data sekunder. Variabel bebas penelitian adalah umur, jenis kelamin, riwayat demam, nyeri kepala, dan mual atau muntah sedangkan variable terikat adalah kejadian demam berdarah dengue. Data dianalisis dengan uji chi square menggunakan SPSS 23. Hasil penelitian menunjukkan ada hubungan umur (p=0,046), jenis kelamin (p=0,009), riwayat demam (p=0,004), nyeri kepala (p=0,031), dan mual atau muntah (p=0,000) terhadap kejadian demam berdarah dengue. Simpulan adalah perilaku manusia yang menyebabkan terjadinya dan tersebarnya kasus DBD adalah kebiasaan masyarakat yang kurang menjaga kebersihan lingkungan. Untuk mendukung penegakan diagnose pasti demam berdarah dengue dan kesembuhan pasien, maka perlu dilakukan upaya baik secara pribadi maupun kelompok untuk meningkatkan tanggapan cepat berobat.
