Abstract

Diabetes mellitus (DM) has reached epidemic proportions worldwide, Indonesia is the sixth ranked country in the world. Diabetes with complications is the third highest cause of death in Indonesia. This study applies a holistic and comprehensive family doctor service by identifying risk factors, clinical problems and patient management based on Evidence Based Medicine with a patient centered approach and a family approach in cases of diabetes with complications of diabetic foot ulcers. Primary data were obtained through history taking (autoanamnesis from the patient and alloanamnesis from the patient's family), physical examination and home visits. Patient Mrs. K, 56 years old, was diagnosed with Diabetic Foot Ulcers and Type 2 Diabetes Mellitus. The patient has limited support and knowledge of the family about the patient's disease and its complications as well as the pattern of curative treatment. Holistic and comprehensive management of Mrs. K has been carried out to increase the knowledge of patients and families about DM with complications.

Abstract

Introduction: Diarrhea in 2019 became the second leading cause of death in children under five. The causes of diarrhoea include infection, malabsorption, and food and are influenced by several factors such as behavioural, nutritional, environmental, and socioeconomic factors. This study aimed to analyze the effect of variables on nutritional status and environmental sanitation on the incidence of diarrhoea in children under five in the South Lampung Regency. Method: This study is survey research with a cross-sectional design and uses binary logistic regression analysis. The samples taken were 380 toddlers aged 6-59 months. Result: 10 of the 21 predictor variables had a significant effect on the 5% significant level of the incidence of diarrhoea, where there were nine predictor variables, including poor/poor nutritional status, history of exclusive breastfeeding, clean water sources from BOR wells, clean water sources from PDAM. mountain spring drinking water sources, latrine facilities, septic tank facilities, managed waste management, and the floor of a ceramic house can reduce the incidence of diarrhea with odds ratio and [p=] values of 0.06[0.000], 0.33[0.017], 0.01[0.0000], 0.02[0.000], 0.02[0.004], 0.04[0.000], 0.01[0.025], 0.02[0.001], and 0.10[0.000]. Toddlers living in noncoastal areas have a 1.08 times higher chance of experiencing diarrhoea than coastal areas. In contrast, one variable, namely the age of toddlers, can increase the susceptibility to diarrhoea in toddlers with the odds ratio and [p=] values of 1.04 [0.007, respectively. Conclusion: This study shows that the influence of nutritional status variables and environmental sanitation has a great possibility to reduce the incidence of diarrhoea in children under five, namely in the group of nutritional status variables, including poor/poor nutritional status, age of toddlers, and history of exclusive breastfeeding.

Abstract

Hipertensi atau yang sering disebut darah tinggi merupakan salah satu penyakit tidak menular yang merupakan masalah di Indonesia. Riskesdas 2018 menunjukkan prevalensi hasil pengukuran tekanan darah, hipertensi naik dari 25,8% menjadi 34,1%. Penanganan hipertensi dapat dilakukan dengan modifikasi faktor risiko berupa gaya hidup termasuk diet, kebiasaan olahraga, stres, dan kebiasaan merokok. Banyak penderita hipertensi yang masih minim pengetahuan mengenai hipertensi. Tujuan dari penelitian ini unutk mengidentifikasi faktor resiko, masalah klinis, melakukan penatalaksanaan pasien secara holistik dengan menerapkan dokter keluarga sesuai evidence-based medicine melalui pendekatan patient centred dan family approach. Pasien perempuan usia 55 tahun dengan sakit kepala yang dirasakan hilang timbul dan mengalami gannguan tidur sejak 2 hari yang lalu. Selain itu, pasien juga ingin mengontrol tekanan darahnya. Pasien memiliki riwayat hipertensi sejak 4 tahun yang lalu. Kurangnya perhatian dan dukungan keluarga tentang penyakit, faktor risiko dan pencegahan timbulnya masalah yang terjadi pada pasien dan keluarga. Penegakan diagnosis dan penatalaksanaan pada pasien ini telah dilakukan secara holistik, patient centered, family approach serta sesuai panduan nasional dan berdasarkan. Perubahan pengetahuan serta perilaku pasien dan keluarga terjadi setelah dilakukan intervensi yang bersifat patient centred dan family approach.

Abstract

Stunting is an irreversible growth disorder that can be influenced by many factors, one of which is influenced by inadequate nutritional intake. The prevalence of stunting in Indonesia is still quite high, so various efforts have been made by the government to tackle the stunting problem. One of the efforts made is to socialize the use of yard land as an effort to prevent and deal with stunting. This article is a literature review compiled to determine the effect of nutritional intake from the home garden on the prevalence of stunted and non-stunted toddlers. The literature sources used in this article use national and international scientific articles published in 2013-2022. Nutritional intake which is the main cause is macronutrient intake, namely carbohydrate, protein and fat intake. Intake of macronutrients provides a large energy contribution to the level of daily energy adequacy. Energy deficit that occurs in toddlers aged 1-5 years is the biggest cause of stunting. Toddlers with stunting will experience chronic growth failure, if not treated immediately it will have a long impact until puberty. Utilization of the yard will affect the amount and type of variation in nutritional intake in toddlers, the diversity of commodities from the home yard will increase the nutritional intake of toddlers so as to avoid chronic malnutrition and stunting. Abstract

Type 2 diabetes mellitus (DMT2) is a global health problem with a significant impact on life. The prevalence of DM in the world reached 537 million people in 2021. DMT2 is a metabolic disorder caused by increased blood glucose levels. One of the risk factors DMT2 is dietary intake. Dietary intake is the main cause of the rapid increase in the incidence of DM in developing countries. Blood glucose levels are mainly influenced by dietary carbohydrates, high intake of fast food, carbonated drinks, intake of red meat, sweet foods and excessive intake of white rice are also thought to contribute to an increased risk of insulin resistance in DMT2. Based on this, the author are interested in conducting a literature study about the relationship between dietary intake and type 2 diabetes mellitus. Based on the literature study, it can be concluded that there is a significant relationship between dietary intake and DMT2 so that food intake can affect the incidence of DMT2 in individuals. Various types of dietary components can affect the risk of developing DMT2, therefore healthy dietary intake and regular diet patterns can be done to reduce the risk of various diseases.

Abstract

Kematian bayi dapat dicegah melalui pemberian Air Susu Ibu. Banyak faktor yang menyebabkan seseorang tidak dapat menyusui bayi. Salah satunya ialah air susu tidak keluar. Studi ini menggunakan artikel jurnal nasional dan internasional dari tahun 2012 hingga tahun 2022 sebanyak 17 artikel, dan 10 artikel yang memenuhi syarat. Database yang digunakan adalah Pubmed, NCBI dan Google Scholar dengan kata kunci “kolostrum” dan “pengeluaran”. Peneliti membuat rangkunan dan analisis serta interpretasi. Rata-rata waktu keluarnya kolostrum pertama pada ibu post sectio caesaria adalah lebih lambat dibandingkan rata-rata waktu keluarnya kolostrum pertama pada ibu post partum spontan. Hal ini mengindikasikan bahwa terdapat perbedaan lama waktu keluarnya kolostrum pertama pada ibu post partum spontan dengan post sectio caesaria. Faktor yang mempengaruhi pengeluaran kolostrum antara lain adalah pijat okstosin, daya hisap bayi dan inisiasi menyusu dini.

Abstract

In Indonesia, dermatophytosis is 52% of all dermatomycosis and Tinea Cruris is the most common dermatophytosis. This disease is important to be managed comprehensively so that treatment goals can be achieved. Family doctor practice are evidence based medicine by identifying risk factors, clinical problems, and patient management based on patient problems with a patient centered and family approach. This study is a case report. Primary data were obtained through history taking, physical examination, labs exam and home visits to assess the physical environment. Assessment based on a holistic diagnosis from the beginning to the end. Before intervention, the patient's knowledge about the disease and personal hygiene was lacking. The family's knowledge of Tinea Cruris is still low. After the intervention, there was an improvement in the knowledge score of patients and their families which increased by 50 points. A holistic management has been carried out with the approach of a family doctor, Mr. I was 52 years old with Tinea Cruris based on initial holistic diagnostics. The interventions carried out have increased patient knowledge and changed some of the behavior of patients and their families, as indicated by improvements in the final holistic diagnostic

Abstract

HBsAg examination is important to detect Hepatitis B in pregnant women, so it needs to be done during the first visit with the help of health. The results of the pre-survey at the Way Kandis Health Center in 2020, it was found that only 55.2% of pregnant women performed HbsAg examinations. The Purpose of the study was to determine the factors that influence the behaviour of pregnant women in the HBsAg examination at Way Kandis Health Center, Bandar Lampung City, in 2021. The design of this study was analytic with a cross-sectional approach. The number of samples in this study was 112 pregnant women, with a sampling technique of accidental sampling. Bivariate analysis using Chi-square. The results of the univariate study found that most pregnant women, 50.9% did not perform HBsAg examination, most of the pregnant women have less knowledge of 53.6%, the attitudes of pregnant women were negative 57.1%, the perception of pregnant women was negative 50.9%. Most of the distance travelled to close health services is 57.1%, negative husband support is 50.9%. The results of the bivariate test showed that there was a significant relationship between knowledge of attitudes, perceptions, distance travelled, husband's support with the behaviour of pregnant women with HBsAg examination at Way Kandis Health Center, Bandar Lampung City in 2021.  Abstrak: Pemeriksaan HbsAg merupakan hal penting untuk mendeteksi Hepatitis B pada ibu hamil, sehingga perlu dilakukan pada saat kunjungan pertama di tenaga kesehatan. Hasil prasurvei di Puskesmas Rawat Inap Way Kandis pada tahun 2020, didapatkan hanya 55,2% ibu hamil yang melakukan pemeriksaan HbsAg. Tujuan penelitian ini untuk mengetahui faktor-faktor yang memengaruhi perilaku ibu hamil dalam pemeriksaan HbsAg di Puskesmas Rawat Inap Way Kandis Kota Bandar Lampung Tahun 2021. Desain penelitian ini adalah analitik dengan pendekatan cross sectional. Jumlah sampel pada penelitian ini 112 ibu hamil, dengan tekhnik pengambilan sampel accydental sampling. Analisa bivariat menggunakan Chi-square. Hasil penelitian univariat didapatkan bahwa sebagian besar ibu hamil 50,9% tidak melakukan pemeriksaan HbsAg, sebagian besar pengetahuan ibu hamil kurang 53,6%, sikap ibu hamil negatif 57,1%, persepsi ibu hamil negatif 50,9 %. Sebagaian besar jarak tempuh kepelayanan kesehatan dekat yaitu 57,1%, dukungan suami negatif 50,9%. Hasil uji bivariat didapatkan ada hubungan yang bermakna antara pengetahuan sikap, persepsi, jarak tempuh, dukungan suami dengan perilaku pemeriksaan HbsAg ibu hamil di Puskesmas Rawat Inap Way Kandis Kota Bandar Lampung Tahun 2021.

Abstract

Since the COVID-19 outbreak was first discovered in Wuhan City, China, Indonesia has issued a policy to maintain social distance at the community level, so that the education sector diverts online learning. Online learning is learning using the internet network and handheld cellphones and computers as learning media. There are several models of online learning applications such as classroom, zoom, as well as whatsapp group. In the case of COVID-19 in Indonesia, children aged 5-12 years have an obese nutritional status with a diet often followed by weight gain and an increase in the frequency of sitting. This is caused by children's activities to stay at home and lead to the creation of a new lifestyle for children, namely a sedentary lifestyle. Parents also experience a dilemma in teaching their children during online learning because parents are not yet popular with distance learning using handheld cellphones or computers as learning media. Therefore, the SKB 4 Ministers also stipulate face-to-face learning because the pandemic has become endemic. During face-to-face learning in Indonesia, children aged 5-12 years have good nutritional status due to the activities of children who are able to return to activities outside the home such as returning to school. If food intake is balanced according to needs accompanied by the selection of the right food ingredients will give birth to good nutritional status. Food intake that exceeds the body's needs will cause excess weight and conversely food intake that is less than needed will cause the body to become thin and susceptible to disease.

Abstract

Gout is a progressive disease due to deposition of monosodium urate (MSU) crystals in joints, kidneys, and other connective tissues as a result of chronically elevated serum uric acid levels above normal. The prevalence of hyperuricemia and gout in Indonesia is higher than other Asian countries. Dietary factors with excessive puring consumption are one of the risks factor of gout. Therefore, in an effort to treat disease, the role of the family is needed to achieve good therapeutic goals. The purpose of this study is to identify internal and external risk factors in patients, apply a holistic and comprehensive family doctor approach, and perform management based on Evidence Based Medicine that is familyapproach and patient-centered. A 59-year-old female patient came with complaints of pain in both knees. Initially the pain was only felt in the patient's right knee, a few weeks later the patient began to feel pain in the patient's left knee. Complaints are accompanied by swelling, redness and heat. The patient has been diagnosed with gout since 2 years ago. Patients do not routinely control their illness and only seek treatment if complaints interfere with their activities. Internal risk factors are the patient does not know the target of normal uric acid levels, what types of food can cause uric acid levels, the pattern of curative treatment, and the patient does not like to exercise. The external risk to the patient is that the family does not know how to prepare food according to the patient's diet and appropriate physical activity. Intervention was carried out regarding the type of food and appropriate physical activity and the importance of routinely checking uric acid in 3 home visits. After the evaluation, the knowledge of the patient and family about the disease were increased.
