Abstract

Beta thalassemia occurs a decreasing of beta chain synthesis. In individuals with Beta Thalassemia, there can be a total loss of the β globin chain (Thalassemia Major) or a partial loss of the β globin chain (Thalassemia Minor). In 2019, there are more than 10,531 Thalassemia patients in Indonesia, and estimated 2,500 newborns with Thalassemia every year in Indonesia. The study was case report, Patient An. N, 8 years old, has been diagnosed with Thalassemia and has fears of growing complaints and does not improve because of late blood transfusion. Limited knowledge about the patient's health condition such as risk factors of thalassemia, complications of thalassemia, prevention of thalassemia, food that need to be avoided, also lack of motivation and encouragement for families to bring their children for blood transfusion routinely are factors that cause problems that occur in patients and families. Holistic and comprehensive management of An. N’s problems has been done by providing counseling to improve the patient's family knowledge.

Abstract

Type 2 diabetes mellitus (DM) is a non-communicable disease known as a silent killer with a prevalence of 9.3% of the totalpopulation in the world and 10.7% of the total population in Indonesia according to the International Diabetes Federation(IDF). ) in 2019. The research objective is to implement evidence-based medicine-based family doctor services by identifyingrisk factors, clinical problems and patient management based on a patient-centered and family-centered problem-solvingframework. This study is a case report. Assessment is formulated based on the initial holistic diagnosis, process, and the endof the study qualitatively and quantitatively. The problems found in this case are related to the wrong perceptions of patientsand their families, inappropriate lifestyles, eating patterns and physical activity as well as the lack of support and motivationof family members. After holistic and comprehensive management was carried out, there was an increase in patient andfamily knowledge points about DM from 40 to 90, and a temporary decrease in blood sugar levels from 540 mg/dL to 271mg/dL.

Abstract

Otitis externa is an inflammation of the outer part of the ear which includes auricle, external audiotory canal, or both. The inflammation can be acute and chronic that ussually caused by bacteria, viral, and fungal infections. The objective of this study was to determine the association between risk factor with otitis externa occurrence in otorhinolaryngology head and neck surgery clinic of RSUD Abdul Moeloek Provinsi Lampung in 2019. This study was an observational analytic study with a case control design. The sample divided into 56 respondents who had otitis externa and 56 respondents who did not have otitis extena. The sample taken using consecutive sampling technique. Data analysis using chi square test (α = 0,05). Respondents who had otitis externa were highest among respondents with history of ear cleaning (62,5%), without history of activity in water (62,5%), and without history of diabetes mellitus (91,1%). The results of chi square analysis indicate an association between history of ear cleaning (p value = 0,001) and activity in water (p value = 0,036) with otitis externa occurrence. While history of diabetes mellitus (p value = 0,500) did not affect otitis externa occurrence. There was an association between history of ear cleaning and activity in water with otitis externa occurrence. While history diabetes mellitus did not affect the otitis externa occurrence.

Abstract

One of the activities routinely carried out in antenatal care is the measurement of Fundus Uterine Height (FUH) to determine Estimated Fetal Weight (EFW). Fetal weight growth can be indirectly affected by the basic nutritional knowledge of  pregnant  women  about the  importance of  fulfillinf nutrition during pregnancy to  support the  health  and development  of the  fetus. The aim of the study was to determine the relation between the level of nutritional knowledge of pregnant women and the estimated fetal weight at Kedaton Primary Health Center Bandar Lampung City. The research design used was analytic observational with cross sectional approach using primary data by measuring the knowledge of pregnant  women  using  a  questionnaire  consisting of 28 question  items  and  secondary  data  in  the   form  of uterine fundal height in the KIA book. Sampling was conducted at the Kedaton Primary Health Center in 2020. The sample was taken using purposive sampling with a total sample of 46 samples that met the inclusion and exclusion criteria. Data analysis used Chi Square with α = 0.05 and CI = 95%. The results  showed  that  there  were  8.7%  mothers with good nutritional knowledge, 71.7% mothers with moderate nutritional knowledge and 19.6% mothers with poor nutrition knowledge. Then pregnant women with small EWF during pregnancy and large EFW during pregnancy consisted of 10.9% and 78.3% respectively of mothers with EFW according to gestational age. Bivariate analysis showed that there was a relationship between the level of nutritional knowledge in pregnant women and the estimated fetal weight at Kedaton Health Center with a value of p = 0.003. There is a correlation between the level of nutritional knowledge of pregnant women with the estimated fetal weight.

Abstract

Penyebab anemia remaja putri secara umum adalah kekurangan gizi besi. Bahan pangan sehari-hari yang tinggi mengandung zat besi adalah hati ayam. Tujuan penelitian ini adalah untuk mendapatkan formula terbaik sosis hati ayam dalam sosis ayam untuk memenuhi kecukupan zat besi remaja putri. Penelitian ini merupakan penelitian eksperimental murni dengan desain rancang acak lengkap yang dilakukan dalam 3 tahap yaitu: proses pembuatan sosis hati ayam; uji organoleptik meliputi warna, aroma, rasa, tekstur dari sosis; dan pengujian kandungan kadar air, kadar zat besi dan kadar protein dalam sosis. Sosis hati ayam dibuat dalam 6 formula dan 1 formula kontrol. Didapatkan tingkat kesukaan terhadap warna, aroma, rasa dan tekstur paling tinggi berturut-turut terdapat pada sosis formula F4 (3,77), F2 (3,93), F4 (3,75), F4 (3,68), namun tidak terdapat perbedaan nyata warna, aroma, rasa dan tekstur antar formula (p>0,000). Kadar air dan kadar protein semua formula sosis sudah sesuai dengan SNI 2015. Kadar zat besi tertinggi terdapat pada formula F6. Disimpulkan formula terbaik yang sesuai tujuan penelitian yaitu formula F6 karena mengandung paling tinggi kandungan zat besi yaitu 7,27 mg/100 g atau setara 16,0% AKG zat besi untuk remaja putri.

Abstract

Menstruasi merupakan proses fisiologis pada remaja putri, dengan siklus normal minimal 21 hari dan maksimal 35 hari. Terdapat beberapa faktor risiko yang dapat mempengaruhi siklus menstruasi seperti Indeks Massa Tubuh (IMT) dan persentase lemak tubuh. Penelitian ini bertujuan untuk menyelidiki hubungan antara indeks massa tubuh (IMT), persentase lemak tubuh, dan siklus menstruasi pada remaja putri di SMAN 10 Bandar Lampung. Penelitian ini menggunakan pendekatan cross sectional, dengan teknik pengambilan sampel cluster random sampling. Data variabel IMT didapatkan dengan perhitungan rumus IMT, variabel persentase lemak tubuh diukur dengan Bioelectrical Impedance Analyze (BIA), dan variable siklus menstruasi diperoleh dengan teknik wawancara responden. Data yang terkumpul dianalisis secara univariat dan bivariat Chi-Square. Berdasarkan hasil univariat didapatkan gambaran IMT  tidak normal sebesar 85,1% dan IMT normal sebesar 14,9%. Pada persentase lemak tubuh, didapatkan 76,6% tidak normal dan 23,4% normal. Pada siklus menstruasi diperoleh responden dengan siklus tidak normal sebesar 26,4%, dan normal sebesar 73,6%. Berdasarkan analisis bivariat antara IMT dengan siklus menstruasi menunjukkan p-value = <0,001, dan sedangkan pada persentase lemak tubuh dengan siklus menstruasi menunjukkan p-value = 0,482. Kesimpulan penelitian ini menunjukan bahwa terdapat hubungan antara IMT dengan siklus menstruasi pada remaja putri di SMAN 10 Bandar Lampung. Tidak terdapat hubungan antara persentase lemak tubuh dengan siklus menstruasi pada remaja putri di SMAN 10 Bandar Lampung.

Abstract

Patients with end-stage renal disease, especially those receiving HD treatment, often show characteristics in the body's protein and energy storage, called protein energy wasting (PEW). HD is the mainstay of treatment for end-stage renal disease and the number of patients undergoing HD is increasing every year. Appropriate nutritional therapy is needed to improve nutritional status and improve the patient's quality of life. Malnutrition is a common condition in patients undergoing haemodialysis (HD) and is associated with increased morbidity and mortality. The main aim of this study was to evaluate the relationship between energy and macronutrient intake and nutritional status in HD patients at at Haemodialysis Unit Dr. H. Abdul Moeloek General Hospital Bandar Lampung. This research is an observational analytical study with a cross sectional design with 30 respondent with chronic kidney failure who underwent HD at at Haemodialysis Unit Dr. H. Abdul Moeloek General Hospital Bandar Lampung from April to May 2024, sampling technique using consecutive sampling technique. Information regarding eating patterns was collected using the 1 x 24 hour food recall method, mini mental status examination (MMSE) used to evaluate dementia in elderly respondents and the nutritional status index was based on an assessment using the mid-upper arm circumference percentile, the patient's height was obtained by measuring the patient's knee height which was then calculated using the Chumlea equation to obtain ideal body weight. The nutritional status index uses mid-upper arm circumference to categorize patients with good and poor nutritional status. Data were analyzed using Fisher's test. The results showed that the majority of respondents had inadequate energy intake as many as 20 patients (66.7%), inadequate protein intake as many as 24 patients (80%), inadequate fat intake as many as 26 patients (86.7%) and inadequate carbohydrate intake as many as 23 patients (76.7%). So it can be concluded that there is no relationship between energy intake (p = 0.633), protein (p = 0.656), fat (p = 0.557) and carbohydrates (p = 0.567) with nutritional status in HD patients at at Haemodialysis Unit Dr. H. Abdul Moeloek General Hospital Bandar Lampung. Further research needs to be carried out to determine the intake of other nutrients such as fluids, sodium and potassium as well as the duration of HD which is a common problem in HD patients.

Abstract

Jumlah penderita Diabetes Mellitus (DM) di dunia mencapai 537 juta orang dewasa dengan Indonesia berada di peringkat ke-7 yaitu 10,7 juta. Tujuan penelitian ini untuk menerapkan pelayanan kedokteran keluarga yang dilakukan dengan mengidentifikasi faktor-faktor risiko, masalah klinis, serta penatalaksanaan pasien berdasarkan kerangka penyelesaian masalah pasien dengan pendekatan patient centered dan family approach, sehingga dapat mencegah terjadinya komplikasi. Analisis studi ini merupakan suatu laporan kasus dengan data primer diperoleh melalui autoanamnesis, pemeriksaan fisik, dan kunjungan rumah. Penilaian dilakukan berdasarkan diagnosis holistik dari awal, proses dan akhir studi secara kuantitatif dan kualitatif. Pasien Ny.K, 41 tahun memiliki risiko internal yaitu tidak memiliki kebiasaan berolahraga, sering menghentikan konsumsi obat, Tingkat Kecukupan Gizi (TKG) energi kurang, dan asupan protein serta lemak berlebih, dan kurangnya pengetahuan tentang faktor risiko DM, makanan yang dianjurkan dan dilarang, pentingnya berolahraga, pentingnya keteraturan minum obat, pengarus stress psikis terhadap kadar gula darah, jenis-jenis obat DM, dan keadaan gawat darurat pada DM. Pasien diintervensi dengan materi dalam bentuk power point. peningkatan pengetahuan sebesar 40 poin, penurunan kadar gula darah sebanyak 90 mg/dL dan perubahan perilaku pasien untuk mengkonsumsi makanan sesuai dengan TKG.

Abstract

Penderita hipertensi berusia 30-79 tahun di dunia saat ini diperkirakan berjumlah 1,28 miliar jiwa. Di Indonesia sendiri diperkirakan lebih dari 70 juta orang menderita penyakit ini. Penelitian ini bertujuan untuk menganalisis masalah klinis dan mengidentifikasi faktor risiko pasien dengan menerapkan pelayanan kedokteran keluarga serta menatalaksana pasien secara holistik dan komprehensif sehingga mencegah terjadinya komplikasi. Data primer kasus yang dilaporkan diperoleh melalui autoanamnesis, pemeriksaan fisik, dan kunjungan rumah. Penilaian dilakukan berdasarkan diagnosis holistik pada awal, selama, dan akhir studi secara kuantitatif dan kualitatif. Pasien Ny. P, usia 50 tahun memiliki risiko internal riwayat keluarga dengan hipertensi, kurangnya pengetahuan mengenai penyakit hipertensi, stres psikologis dan kualitas tidur yang kurang, serta pola makan yang kurang energi dan tinggi lemak. Pasien diintervensi dengan edukasi dengan materi berbentuk poster dan power point. Setelah intervensi, terdapat peningkatan pengetahuan pasien sebesar 40%, keluhan pasien dirasakan membaik, serta perubahan perilaku menerapkan pola gizi seimbang dan meningkatkan aktivitas fisik

Abstract

Introduction: The prevalence of anemia among teenage girls in senior high schools in Lampung Province is still quite high. A more comprehensive coping model is needed to improve this situation, such as applying the CBA (Anemia Free Club) model, which consists of nutrition education and the supervisory role of mothers and teachers. This study aims to analyze the effect of applying the CBA model and administering blood supplementary tablets on knowledge, attitudes and hemoglobin levels in anemic teenage girls of high school students in South Lampung Regency. Methodology: Quasi-experimental research with a non-randomized control group pretest–posttest design. The research was conducted in February-May 2022 using the consecutive sampling technique by dividing into two groups: the intervention group of senior high school and the control group of senior high school. The Anemia Free-club model was applied for anemic teenage girls, and supplementary tablets were given for six weeks. The analytical test used was the paired t-test using the SPSS program and was considered to have a significant effect if p less than 0,05. Results: After implementing the Anemia Free-club model and giving iron supplements, in the intervention group, there was an increase in the nutrition knowledge (up 45.33 points), the nutritional attitude (up 9.8 points) and the Hb level (up 1,06 mg/dL). Conclusion: It showed a significant effect between the application of the CBA model and the administration of blood supplementary tablets on nutritional knowledge, nutritional attitudes, and Hb levels (p less than 0,05) in the intervention group.
