Abstract

Type 2 diabetes mellitus (DM) is a non-communicable disease known as a silent killer with a prevalence of 9.3% of the totalpopulation in the world and 10.7% of the total population in Indonesia according to the International Diabetes Federation(IDF). ) in 2019. The research objective is to implement evidence-based medicine-based family doctor services by identifyingrisk factors, clinical problems and patient management based on a patient-centered and family-centered problem-solvingframework. This study is a case report. Assessment is formulated based on the initial holistic diagnosis, process, and the endof the study qualitatively and quantitatively. The problems found in this case are related to the wrong perceptions of patientsand their families, inappropriate lifestyles, eating patterns and physical activity as well as the lack of support and motivationof family members. After holistic and comprehensive management was carried out, there was an increase in patient andfamily knowledge points about DM from 40 to 90, and a temporary decrease in blood sugar levels from 540 mg/dL to 271mg/dL.

Abstract

Otitis externa is an inflammation of the outer part of the ear which includes auricle, external audiotory canal, or both. The inflammation can be acute and chronic that ussually caused by bacteria, viral, and fungal infections. The objective of this study was to determine the association between risk factor with otitis externa occurrence in otorhinolaryngology head and neck surgery clinic of RSUD Abdul Moeloek Provinsi Lampung in 2019. This study was an observational analytic study with a case control design. The sample divided into 56 respondents who had otitis externa and 56 respondents who did not have otitis extena. The sample taken using consecutive sampling technique. Data analysis using chi square test (α = 0,05). Respondents who had otitis externa were highest among respondents with history of ear cleaning (62,5%), without history of activity in water (62,5%), and without history of diabetes mellitus (91,1%). The results of chi square analysis indicate an association between history of ear cleaning (p value = 0,001) and activity in water (p value = 0,036) with otitis externa occurrence. While history of diabetes mellitus (p value = 0,500) did not affect otitis externa occurrence. There was an association between history of ear cleaning and activity in water with otitis externa occurrence. While history diabetes mellitus did not affect the otitis externa occurrence.

Abstract

One of the activities routinely carried out in antenatal care is the measurement of Fundus Uterine Height (FUH) to determine Estimated Fetal Weight (EFW). Fetal weight growth can be indirectly affected by the basic nutritional knowledge of  pregnant  women  about the  importance of  fulfillinf nutrition during pregnancy to  support the  health  and development  of the  fetus. The aim of the study was to determine the relation between the level of nutritional knowledge of pregnant women and the estimated fetal weight at Kedaton Primary Health Center Bandar Lampung City. The research design used was analytic observational with cross sectional approach using primary data by measuring the knowledge of pregnant  women  using  a  questionnaire  consisting of 28 question  items  and  secondary  data  in  the   form  of uterine fundal height in the KIA book. Sampling was conducted at the Kedaton Primary Health Center in 2020. The sample was taken using purposive sampling with a total sample of 46 samples that met the inclusion and exclusion criteria. Data analysis used Chi Square with α = 0.05 and CI = 95%. The results  showed  that  there  were  8.7%  mothers with good nutritional knowledge, 71.7% mothers with moderate nutritional knowledge and 19.6% mothers with poor nutrition knowledge. Then pregnant women with small EWF during pregnancy and large EFW during pregnancy consisted of 10.9% and 78.3% respectively of mothers with EFW according to gestational age. Bivariate analysis showed that there was a relationship between the level of nutritional knowledge in pregnant women and the estimated fetal weight at Kedaton Health Center with a value of p = 0.003. There is a correlation between the level of nutritional knowledge of pregnant women with the estimated fetal weight.

Abstract

Penyebab anemia remaja putri secara umum adalah kekurangan gizi besi. Bahan pangan sehari-hari yang tinggi mengandung zat besi adalah hati ayam. Tujuan penelitian ini adalah untuk mendapatkan formula terbaik sosis hati ayam dalam sosis ayam untuk memenuhi kecukupan zat besi remaja putri. Penelitian ini merupakan penelitian eksperimental murni dengan desain rancang acak lengkap yang dilakukan dalam 3 tahap yaitu: proses pembuatan sosis hati ayam; uji organoleptik meliputi warna, aroma, rasa, tekstur dari sosis; dan pengujian kandungan kadar air, kadar zat besi dan kadar protein dalam sosis. Sosis hati ayam dibuat dalam 6 formula dan 1 formula kontrol. Didapatkan tingkat kesukaan terhadap warna, aroma, rasa dan tekstur paling tinggi berturut-turut terdapat pada sosis formula F4 (3,77), F2 (3,93), F4 (3,75), F4 (3,68), namun tidak terdapat perbedaan nyata warna, aroma, rasa dan tekstur antar formula (p>0,000). Kadar air dan kadar protein semua formula sosis sudah sesuai dengan SNI 2015. Kadar zat besi tertinggi terdapat pada formula F6. Disimpulkan formula terbaik yang sesuai tujuan penelitian yaitu formula F6 karena mengandung paling tinggi kandungan zat besi yaitu 7,27 mg/100 g atau setara 16,0% AKG zat besi untuk remaja putri.

Abstract

Introduction: The prevalence of anemia among teenage girls in senior high schools in Lampung Province is still quite high. A more comprehensive coping model is needed to improve this situation, such as applying the CBA (Anemia Free Club) model, which consists of nutrition education and the supervisory role of mothers and teachers. This study aims to analyze the effect of applying the CBA model and administering blood supplementary tablets on knowledge, attitudes and hemoglobin levels in anemic teenage girls of high school students in South Lampung Regency. Methodology: Quasi-experimental research with a non-randomized control group pretest–posttest design. The research was conducted in February-May 2022 using the consecutive sampling technique by dividing into two groups: the intervention group of senior high school and the control group of senior high school. The Anemia Free-club model was applied for anemic teenage girls, and supplementary tablets were given for six weeks. The analytical test used was the paired t-test using the SPSS program and was considered to have a significant effect if p less than 0,05. Results: After implementing the Anemia Free-club model and giving iron supplements, in the intervention group, there was an increase in the nutrition knowledge (up 45.33 points), the nutritional attitude (up 9.8 points) and the Hb level (up 1,06 mg/dL). Conclusion: It showed a significant effect between the application of the CBA model and the administration of blood supplementary tablets on nutritional knowledge, nutritional attitudes, and Hb levels (p less than 0,05) in the intervention group.

Abstract

Adolescence is a period of a transitional development from children to adult women who are generally 10-21 years old. Adolescents have significant physical, psychosocial, and cognitive changes that can affect adolescent dietary behavior. The purpose of this study was to determine the effect of affecting factors on unhealthy dietary behavior in adolescent girls. This research is an analytic observational study, with a cross-sectional design approach using primary data, which was conducted from April to June 2022. The sample is 135 adolescent girls aged 15-21 years old taken using multistage random sampling who met the inclusion and exclusion criteria. The data was obtained by filling out the questionnaire and analyzed by using the chi-square test. The results showed that 56.3% of respondents had unhealthy dietary behavior. The results of the bivariate analysis showed that nutritional status (p = 0.000; PR = 2.072) and body image (p = 0.000; PR = 0.559) influenced unhealthy dietary behavior. Meanwhile, knowledge (p=0,227), age (p=0,158), pocket money (p=0,452), and peers (p = 0.120) did not influence unhealthy dietary behavior. It can be concluded that there is an influence of nutritional status and body image factors on unhealthy diet behavior. Based on the research results, adolescent girls must have a healthy diet according to balanced nutrition guidelines in achieving normal nutritional status and a good body image.

Abstract

Tuberculosis (TB) is an infectious disease that is still a problem in developing countries. Worldwide. Although the government has implemented many programs for controlling the TB, the role of family physician is very important in treating TB. In dealing with these problems, the need for cooperation from various parties in a comprehensive and holistic manner. Implementing comprehensive and holistic family doctor services by identifying risk factors, clinical problems, and patient management based on evidence based medicine with strategies that are patient centered, family oriented and community oriented.The analysis of this study is a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from aupporting investigation.The assessment is carried out based on a holistic diagnosis of the initial, process, and final study quantitatively and qualitatively. Patient Nn.N, 19 years old, came to the clinic with cough , easy fatigue, weight loss and night sweats since 2 months ago. The patient was diagnosed with clinically confirmed pulmonary tuberculosis. After the intervention, pulmonary TB complaints improved. There is a change in behavior towards the patient's disease where the quantitative assessment found an increase in all aspects assessed, namely knowledge, diet, and physical activity.

Abstract

Vertigo is a term that comes from the Latin, vertere, which means twisting. In general, vertigo is known as an illusion of movement or a hallucination of movement. Vertigo is found in the form of complaints in the form of a sense of spinning or a sense of moving from the surrounding environment (circular vertigo) but sometimes complaints are also found in the form of a feeling of being pushed or pulled away from the vertical plane (vertical linear). Apply a holistic and comprehensive approach to family doctors in detecting risk factors, clinical problems, and patient management based on evidence based medicine and is family- approached and patient-centered. Primary data were obtained through anamnesis, physical examination, supporting examinations and home visits to assess the physical environment. Mrs. S, 34 years old, came to the Campang Raya Community Health Center with complaints of dizziness since two days before coming to the Puskesmas. Spinning dizziness arises suddenly, is felt continuously and gets worse when the patient raises his head to sit or stand after sleeping. Complaints accompanied by nausea, vomiting, and cold sweats. Complaints have been repeated and the first time was felt six years ago. The patient's concern is that the complaint gets heavier and interferes with daily activities. The patient was diagnosed with vertigo based on history and physical examination. A holistic management has been carried out with the approach of a family doctor, through non-medical and medical interventions were carried out on patients and their families with education related to the patient's illness in three home visits. The evaluation results obtained were reduced patient complaints and increased knowledge of patients and their families regarding vertigo.

Abstract

Based on data from the World Health Organization, it is estimated that around 15-20% of the world's population suffers from hypertension and data from the 2018 National Basic Health Research, there are 54.4% of the Indonesian population aged ≥ 15 years with cholesterol levels (≥ 200 mg/dl. Purpose of writing This is to apply the principles of family medicine services and carry out holistic management based on a framework for solving patient problems using a patient-centered, family focused and community oriented approach based on Evidence Based Medicine. This study is a case report. Internal risk factors were found, namely family history, unbalanced nutritional intake, and lack of knowledge of the disease. Ekxternal risk factors include lack of family knowledge about desease prevention and complication, lack of supervision over diet and medication. Patient Mrs. J, 59 years old, was diagnosed as grade I hypertension, dyslipidemia, and grade I obesity. In this case, diagnosis and treatment were carried out in accordance with theory and related journals. After the intervention was carried out, there was a reduction in clinical symptoms and an increase in patient and family knowledge. Holistic management can improve the knowledge, attitudes and behavior of patients and families in maintaining health.

Abstract

Water is a very important nutrient and is the most constituent component of the human body. Most of the water required by the body comes from drinking. Each person's fluid needs vary depending on physical activity, age, weight, ambient temperature and humidity. Fluid water is recommended over other types of beverages. Water needs about two liters or eight glasses a day for adolescents and adults who do light activities in the office and home under daily temperature conditions. Hydration knowledge can be a factor that affects the fulfillment of a person's water intake. Good hydration knowledge will make a person choose fluid consumption based on quality and quantity so that their fluid needs are met. Unfulfilled fluid intake can lead to dehydration. Dehydration is defined as a state of the body when fluid expenditure is greater than fluid intake. Dehydration can be a risk factor for overweight. Overweight is a condition when there is an accumulation of excess fat due to an imbalance between energy intake and energy used for a long time. Dehydration can cause a decrease in cognitive function and mood that causes weakness and laziness in physical activity; dehydration also causes fluid and electrolyte imbalances that increase appetite; and dehydration can increase glycerol and free fatty acids resulting in increased triglyceride formation. Adequate water intake can lead to weight loss by slowing gastric emptying and increasing metabolism.
