Abstract

Latar Belakang: Pasien keganasan darah yang mendapatkan transfusi packed red cell (PRC) berulang akan menyebabkan pembentukan aloantibodi yang dapat terdeteksi pada pemeriksaan crossmatch dan mempersulit proses transfusi selanjutnya. Objektif: Penelitian ini bertujuan untuk mengetahui hubungan antara usia, jenis kelamin, jenis keganasan, dan jumlah unit transfusi dengan kejadian inkompatibilitas crossmatch. Metode: Penelitian ini merupakan penelitian analitik observasional dengan desain cross- sectional. Terdapat 50 sampel yang didapatkan dari rekam medis dan laboratorium. Penelitian ini menggunakan analisis univariat dan analisis bivariat. Hasil: Hasil penelitian ini menunjukan prevalensi inkompatibilitas crossmatch sebesar 16% dengan keseluruhan inkompatibilitas minor. Kejadian terbanyak didapatkan pada usia <60 tahun, perempuan, LMA dan transfusi > 10. Hasil uji statistik menunjukan tidak adanya hubungan yang signifikan antara faktor usia (p= 0,322), jenis kelamin (p= 0,050), jenis keganasan darah (p= 0,662), dan jumlah unit transfusi (p= 0,702) dengan kejadian hasil pemeriksaan crossmatch. Kesimpulan: Faktor risiko seperti usia, jenis kelamin, jenis keganasan, dan jumlah unit transfusi tidak mempunyai hubungan signifikan dengan kejadian inkompatibilitas crossmatch pada pasien keganasan darah yang mendapatkan transfusi PRC berulang

Abstract

Latar Belakang: leukemia mieloblastik akut merupakan salah satu keganasan darah yang terjadi akibat gangguan proses pertumbuhan sel mieloid, yang menyebabkan terjadinya penumpukan sel blast di sumsum tulang, dan menekan pembentukan sel darah lain yang menyebabkan anemia. Pengobatan yang diberikan pada penderita LMA adalah kemoterapi. Kondisi anemia dapat menyebabkan hipoksia di jaringan, yang dapat mengakibatkan berkurang nya efektivitas kemoterapi. Objektif: Tujuan dari penelitian ini adalah untuk menganalisis hubungan antara derajat anemia dengan tercapainya remisi komplit pada pasien LMA yang di kemoterapi induksi. Metode: Penelitian ini dilakukan dengan menggunakan desain cross sectional dengan pendekatan observasional analitik retrospektif. Sampel pada penelitian ini berjumlah 15 sampel yang diambil dari data rekam medik. Analisis univariate dilakukan untuk mendeskripsi frekuensi variable dan analisis bivariate dengan uji statistic fisher exact test. Hasil: Hasil yang didapat dari penelitian ini adalah pasien LMA banyak yang berusia >40 tahun, berjenis kelamin perempuan, dengan klasifikasi LMA M4, keluhan utama lemah letih, derajat anemia ringan dan berhasil remisi komplit yang lebih banyak. Hasil uji statistik memperlihatkan tidak adanya hubungan derajat anemia dengan tercapainya remisi komplit pada pasien. Hasil analisi LMA dengan nilai p=1,000. Kesimpulan: kesimpulan yang didapatkan adalah tidak adanya hubungan antara derajat anemia dengan tercapainya remisi komplit pada pasien leukemia mieloblastik akut dewasa yang menjalani kemoterapi induksi di RSUP Dr. M Djamil Padang.

Abstract

Penelitian ini dilakukan dengan tujuan guna mengkaji hubungan hubungan antara infeksi Blastocystis sp dengan profil leukosit darah pada pasien yang menderita kanker kolorektal. Jumlah sampel yang digunakan sebanyak 22 sampel, yang dilaksanakan di RSUP Dr. M. Djamil Padang. Hasil Penelitian ini menyatakan bahwa dalam 22 pasien penyakit kanker kolorektal paling banyak ditemui pada lelaki dibandingkan wanita, dengan rata-rata umur 50,6 tahun. Sebesar 27,3% ditemukan pada pasien kanker kolorektal yang terkena infeksi Blastocystis sp. Rata-rata nilai leukosit pada pasien yang terkena infeksi Blastocystis sp pada penyakit kanker kolorektal sebanyak 9.792/mm3 dengan p value 0,9. Rata-rata jenis basofil / eosinophil / neutrofil / limfosit / monosit pada pasien yang terkena infeksi Blastocystis sp pada penyakit kanker kolorektal sebanyak 0/3/0/72/21/8, sedangkan pada pasien yang tidak terkena infeksi Blastocystis sp sebanyak 0/2/0/66/14/8 dengan p value lebih dari 0,05. Dengan demikian, hasil akhir dalam penelitian ini menyatakan bahwa tidak adanya hubungan antara infeksi Blastocystis sp dengan profil leukosit darah pada pasien yang menderita kanker kolorektal.

Abstract

Background: Chronic hepatitis B (CHB) is a persistent hepatitis B virus (HBV) infection and can cause progressive liver damage. This damage can be measured through a decrease in albumin levels and an increase in liver enzymes such as aspartate aminotransferase (AST/SGOT), alanine aminotransferase (ALT/SGPT), and de Ritis ratio (AST/ALT). This study aims to analyze the relationship between albumin levels and SGOT, SGPT, and de Ritis ratio in CHB patients. Methods: This cross-sectional retrospective analytical study involved 50 CHB patients diagnosed at Dr. M. Djamil General Hospital Padang between June 2022 to June 2023. Data on albumin levels, SGOT, SGPT, and de Ritis ratio were obtained from medical records. The de Ritis ratio, SGOT, and SGPT were grouped into normal and increased. Statistical analysis used an independent t-test with a significance of p < 0.05. Results: The research subjects consisted of 26 men (52%) and 24 women (48%) with an average age of 42.46 ± 13.39 years. The mean albumin level in the increased SGOT group (3.69 ± 0.78 g/dL) was significantly lower than the normal SGOT group (4.39 ± 0.61 g/dL) (p = 0.003). The mean albumin level in the increased de Ritis ratio group (3.90 ± 0.80 g/dL) was also significantly lower than the normal de Ritis ratio group (4.46 ± 0.70 g/dL) (p = 0.006). There was no significant difference between albumin levels in the normal and increased SGPT groups (p = 0.548). Conclusion: There is a significant negative relationship between albumin levels and SGOT and de Ritis ratio in CHB patients. Decreased albumin levels may be an indicator of more severe liver damage in these patients.

Red blood cell (RBC) alloimmunization, the development of antibodies against foreign red blood cell antigens, is a critical concern in transfusion medicine. Alloantibodies can lead to adverse transfusion reactions, including hemolytic disease of the fetus and newborn (HDFN) and delayed hemolytic transfusion reactions (DHTR). This comprehensive literature review explores the intricacies of RBC alloimmunization, focusing on the screening and identification of erythrocyte antibodies. We delve into the prevalence and clinical significance of various alloantibodies, the underlying immunological mechanisms, and the evolution of laboratory techniques for their detection. Additionally, we discuss the challenges and future directions in managing alloimmunization, emphasizing the importance of personalized medicine and innovative approaches to ensure safe and effective blood transfusions.

Abstract

Latar Belakang: Transfusi trombosit pasien immune thrombocytopenic purpura (ITP) diindikasikan pada kasus ITP dengan perdarahan. Transfusi trombosit pada pasien ITP tidak diikuti dengan peningkatan jumlah trombosit. Corrected count increment (CCI) merupakan penghitungan untuk menilai efektivitas transfusi trombosit. Efektif bila CCI 1 jam pasca transfusi >7,5 x 109/L dan CCI 24 jam >4,5 x 109/L. Tujuan: Mendapatkan gambaran efektivitas transfusi trombosit concentrate (TC) berdasarkan nilai CCI. Metode: Penelitian dilakukan terhadap 31 pasien ITP yang mendapatkan transfusi TC Desember 2022- April 2023. Efektivitas transfusi dinilai berdasarkan nilai CCI 1 jam dan 24 jam pasca transfusi. Hasil: Median jumlah trombosit sebelum transfusi adalah 5,0 x 103/L. Median CCI (Min-Maks) 1 jam 1,88 (0,26-14,5) dan medianCCI 24 jam adalah 2,39 (0,26-15,89). Sebanyak 16,1% sampel efektif berdasarkan nilai CCI 1 jam dan 83,9% tidak efektif. Nilai CCI 24 jam pasca transfusi, 35,5% efektif dan 64,5% tidak efektif. Diskusi: Transfusi TC pada pasien ITP tidak efektif berdasarkan nilai CCI 1 jam dan 24 jam karena pendeknya masa hidup trombosit yang ditransfusikan akibat didestruksi oleh sistem imun. Simpulan: Efektivitas transfusi TC pasien ITP berdasarkan nilai CCI 1 jam pasca transfusi adalah 16,1% pasien dan CCI 24 jam adalah 35,5%. Transfusi trombosit pada ITP perlu indikasi tepat

Abstract

Background: Beta thalassemia major is a genetic blood disorder inherited by parents thalassemia traits (carrier) characterized by abnormal hemoglobin formation. Abnormalities during the process of hemoglobin formation cause gene changes or mutations that cause symptoms of severe anemia which has a long-term clinical impact on patients. High performance liquid chromatography can be used to determine the description of abnormal hemoglobin analysis, thus suspected diagnosis of thalassemia β major can be established by the clinician as a definitive diagnosis to enhance a more comprehensive diagnosis. Primary theraphy of beta thalassemia major is blood transfusion to support the quality of life regularly throughout the patient's life, transfusion of patients requires measurements such as pre-transfusion hemoglobin levels.  Objective: This study aims to determine the description of the results of the analysis of hemoglobin patients with suspected thalassemia beta major at RSUP Dr. M. Djamil Padang. Methods: This study used a cross sectional design.  The sampling technique uses total sampling with a total sample of 19 people from 1-35 years old. Patient data was from the suspected diagnosis of medical records established by clinicians at Dr. M. Djamil Hospital Padang. This data was analyzed univariately. Results: The results of this study indicated is mostly female with an average age of 13,74 years (SD ±12.1). This study resulted in a low pretransfusion hemoglobin average of 6.20 g/dL. The results of hemoglobin analysis for beta thalassemia major with average HbA (48,1%), HbA2 (27,3%) and HbF (24,5%). Conclusion: The results of the examination hemoglobin analysis on the HPLC method had results that did not compatible predictions of the suspected diagnosis of beta thalassemia major from the clinician

Abstract

Anemia is a global health problem characterized by low hemoglobin and ferritin levels due to iron deficiency, which affects growth, cognition, and disease risk, particularly in children and women of reproductive age. Local food–based nutritional interventions such as skipjack tuna (heme iron), spinach (non-heme iron), and dadih (probiotics) have the potential to provide synergistic effects in improving iron status. This study aimed to determine the effect of cookies made from a combination of skipjack tuna flour and spinach with the addition of dadih probiotics on serum ferritin levels, as well as the effectiveness of Sodium Nitrite (NaNO₂) in decreasing ferritin and hemoglobin levels as indicators of anemia. The study used a Post-Test Only Control Group design with 36 female Wistar rats induced with anemia using NaNO₂ at a dose of 25 mg/200 g BW for 18 days, then divided into six treatment groups for 28 days. Ferritin levels were measured using the ELISA method. The results showed that administering cookies made from the combination of skipjack tuna flour and spinach with dadih probiotics significantly increased ferritin levels (P=0.000) compared to the control and single-ingredient groups. NaNO₂ was proven effective as an anemia-inducing agent. This combined cookie formulation has the potential to improve serum ferritin levels.
