Abstract

Salah satu penyakit degeneratif pada lansia yang sering dialami yaitu osteoartritis, yang ditandai dengan adanya nyeri pada ekstremitas bawah dan prevalensinya semakin meningkat dengan bertambahnya usia. Penatalaksanaan non farmakologi merupakan komponen yang sangat penting dalam mengatasi nyeri, salah satu bentuknya adalah latihan Range of Motion. Tujuan: Penelitian ini bertujuan untuk mengetahui pengaruh Range of Motion untuk menurunkan nyeri sendi pada lansia dengan osteoartritis di wilayah kerja Puskesmas Godean I Sleman Yogyakarta. Metode :Penelitian ini adalah study intervensi berupa penelitian kuantitatifdengan rancangan Quasi Eksperiment Design: Pretest-Posttest Control Group Design. Penelitian dilakukan di dua dusun di Kabupaten Sleman yaitu di dusun Mertosutan dan dusun Ngabangan.Sampel pada penelitian ini sebanyak 36 orang lansia dengan masing-masing 18 lansia sebagai kelompok intervensi dan 18 lansia sebagai kelompok kontrol.Pengambilan sampel menggunakan teknik purposive sampling.Analisis data yang digunakan adalah Wilcoxon Test dan Mann Whitney Test. Hasil : Setelah melakukan latihan Range of Motionselama 4 minggu, didapatkan hasil bahwa terdapat pengaruh Range of Motion terhadap skala nyeri sendi pada lansia dengan osteoartritis dengan p value 0,000 (α < 0,05). Kesimpulan :Range of Motion berpengaruh secara signifikan terhadap penurunan tingkat skala nyeri sendi pada lansia dengan osteoartritis

Abstract

Beban kerja perawat yang tinggi menjadi faktor penurunan kinerja unit keperawatan. Penelitian ini menggunakan metode kualitatif dengan tinjauan pustaka berdasarkan Arksey dan O’Malley. Tujuannya adalah menganalisis faktor yang memengaruhi beban kerja perawat di ruang rawat inap. Data diperoleh dari 1404 dokumen literatur Scopus (2020-2024), menggunakan kata kunci “beban kerja perawat,” dan dianalisis dengan VOSViewer. Hasil menunjukkan adanya faktor internal dan eksternal. Faktor internal mencakup jumlah pasien yang meningkat, jam kerja panjang, kerja shift, infrastruktur tidak memadai, serta kekurangan staf, yang terkait dengan manajemen rumah sakit dan sumber daya manusia. Faktor eksternal meliputi kendala anggaran dan kebijakan kesehatan nasional yang berdampak pada rumah sakit. Beban kerja fisik terlihat dari ketidakseimbangan rasio perawat-pasien dan tugas tambahan. Beban kerja mental muncul dari perbedaan volume pekerjaan antar shift, dengan shift pagi lebih berat dibanding shift lainnya. Beban kerja sosial berkaitan dengan konflik rekan kerja di satu ruangan. Penggunaan teknologi menjadi kunci untuk mengurangi beban kerja perawat. Implikasi penelitian ini menyoroti pentingnya pendekatan strategis untuk meningkatkan kualitas pelayanan, mengoptimalkan kinerja, dan memastikan kepuasan pasien terhadap asuhan keperawatan.

Abstract

In Indonesia, there has been a rise in the number of suspected medical malpractice cases, causing tangible harm to patients and undermining trust in the healthcare system. To address this growing concern in the digital era, this study aims to identify effective hospital strategies for handling complaints arising from alleged malpractice incidents. A systematic literature review was conducted, utilizing international and national databases to select and search for pertinent information on hospital complaint-handling processes and strategies in the digital era. The study underscores the significance of establishing a comprehensive complaint-handling system in hospitals to address increasing complaints. Key strategies identified include preparing professional resources dedicated to handling complaints, enhancing awareness regarding patient safety, and ensuring strict adherence to existing Standard Operating Procedures (SOPs). By adopting the identified strategies, hospitals can mitigate the adverse effects of medical accidents, improve patient safety, and rebuild trust between patients and medical personnel in the digital era.

Abstract

Latar Belakang: Masalah nyeri merupakan masalah yang sering dialami oleh pasien post operasi BPH. Salah satu penatalaksanaan non farmakologi adalah pemberian afirmasi positif yang bertujuan untuk mempengaruhi pikiran bawah sadar sehingga rangsangan nyeri teralihkan.Tujuan penelitian:Untuk mengetahui pengaruh afirmasi positif terhadap penurunan nyeri pada pasien post operasi BPH. Metode Penelitian:Jenispenelitian yang digunakan adalah quasi experiment dengan analisis data menggunakan uji time series, instrument nyeri menggunakan VAS dan instrument afirmasi positif menggunakan buku kerja. Populasi dalam penelitian ini adalah 30 pasien post operasi BPH yang terdiri dari 15 kelompok intervensi dan 15 kelompok kontrol dengan tehnik total sampling.Hasil penelitian: Hasil uji time series nilai mean absolute persentage error (MAPE)padapenurunannyeri pre harike 1 sebelumafirmasi 5,582, post harike 1 sesudahafirmasi 10,867, post harike 2 sesudahafirmasi 15,461, post harike 3 sesudahafirmasi 20,461, post harike 4 sesudahafirmasi 30,384 mendekati 1 yang artinya nilai model peramalan terdapat penyesuaian yang besar untuk setiap kesalahan yang terjadi pada nilai model ramalan sebelumnya sehingga hasil perlakuan afirmasi positif mempengaruhi secara signifikan terhadap penurunan nyeri. Kesimpulan: Ada pengaruh afirmasi positif terhadap penurunan nyeri pada pasien post operasi BPH

Abstract

Highlights: • A patient with grade IV osteoarthritis underwent an arthroplasty and developed a surgical site infection caused by Staphylococcus aureus. • The treatment for the surgical site infection included antibiotic medications and revision surgery for the previous total knee arthroplasty. Abstract: This article presents a case report of a knee arthroplasty surgical site infection caused by non-hemolytic Staphylococcus aureus. A 56-year-old woman came to the Orthopedic Outpatient Clinic, with the chief complaint of pain in the left knee for the last three years. After being diagnosed with grade IV osteoarthritis, the patient underwent a total knee arthroplasty. The patient had routine post-operative follow-ups at the Orthopedic Outpatient Clinic. However, the patient complained of swelling, pain, and discharge at the surgical site after three months. The patient underwent a second surgery for debridement, implant removal, and interspacer placement. A broad-spectrum antibiotic (gentamicin) was administered while waiting for the culture and antibiotic sensitivity test results. The culture results showed non-hemolytic Staphylococcus aureus presence. The antibiotics were then changed and administered for two weeks according to the culture and antibiotic sensitivity test results. As the results were good, the patient was scheduled for revision surgery for her previous total knee arthroplasty.

Abstract

Healthcare-Associated Infections (HAIs) remain a significant challenge in healthcare settings due to their impact on patient morbidity, mortality, and treatment costs. This study aimed to describe the trends and risk factors of HAIs using retrospective surveillance data from 2019–2023 at a public hospital in East Java, Indonesia. A total of 18,115 patients with invasive medical devices and 1,674 HAI cases were analyzed. Descriptive statistics, chi-square tests, and epidemiological indicators—point prevalence, incidence density, and device-associated infection rates—were used. The highest point prevalence occurred in 2021, reaching 112.02 per 1,000 patients (11.2%), with an incidence density of 3.11 per 1,000 patient-days and a device-associated infection rate of 7.94 per 1,000 device-days. These figures declined in 2023 to 77.18 per 1,000 patients (7.7%), 2.30 per 1,000 patient-days, and 6.42 per 1,000 device-days, respectively. Major risk factors included prolonged hospitalization, use of invasive devices such as catheters and ventilators, and comorbidities like diabetes mellitus and chronic diseases. The findings highlight the need for continuous monitoring, staff training, and optimized infection prevention strategies to reduce HAIs and enhance patient safety.

Abstract

Hemodialysis patients are at risk of preventable adverse outcomes as a result of the ongoing medical treatments required throughout their life. Minimizing risk is crucial for ensuring patient safety in healthcare environments. Healthcare Failure Mode and Effect Analysis (HFMEA) is a proactive risk assessment method designed to identify potential failures in healthcare processes and improve the quality and safety of patient care. This qualitative descriptive study aimed to identify potential failure modes in hemodialysis services in Nitipuran Hemodialysis Clinic by implementing HFMEA. A multidisciplinary team was involved as the unit of analysis to identify processes and subprocesses for in-center hemodialysis treatment. The study employed purposive sampling, selecting 10 team members who were directly involved in providing hemodialysis services. Data collected were analyzed using the HFMEA worksheet. Over five weeks, the team convened six times to identify Failure Modes (FMs) and Failure Mode Causes (FMCs), conduct a Hazard Analysis, and determine necessary actions to address the FMCs. Five processes, 23 subprocesses, 74 Failure Modes (FMs), 39 Failure Mode Causes (FMCs) were identified. Based on the Hazard Analysis results, 27 FMCs required corrective actions and thirteen actions were proposed to address the FMCs and improve patient safety based on the findings of this study. Further research is needed to evaluate the effectiveness of the implementation of these corrective actions in improving patient safety.

Abstract

A drug formulary free of haram substances is one of the standards in medicine service. The purpose of this study is to analyze the implementation of the halal medicine selection process in Nur Hidayah Hospital Yogyakarta. It is qualitative research using a case study method. Data were collected using interviews, observation, and documentation techniques. The study reveals four themes in the selection process for halal medicine implementation at Nur Hidayah Hospital. The themes are sharia-based medicine screening, halal label limitations, haram medicine use based on doctor-patient agreement using sharia informed consent, and halal statements from drug manufacturers. The research concluded that sharia principles had been implemented in the medicine selection process at Nur Hidayah Hospital, but the number of halal-labeled medications remains limited. The use of non-halal substances in medicine through a doctor-patient agreement based on sharia informed consent. Halal statements from pharmaceutical companies support implementing the halal lifestyle in hospitals.
