Abstract

Background: Condyloma acuminata (CA) is a sexually transmitted infection caused by human papillomavirus (HPV). Current standard treatment has limitations, so effective and safe alternative therapies are needed. Garlic (Allium sativum L.) has long been used in traditional medicine and has potential as an antiviral and antitumor agent. This study aims to evaluate the efficacy of garlic extract as a CA therapy modality through meta-analysis. Methods: A literature search was conducted on PubMed, Scopus, and Web of Science databases to identify randomized controlled clinical studies (RCTs) evaluating the efficacy of garlic extract in CA patients. Extracted data included study characteristics, interventions, sample size, clinical outcomes, and side effects. Statistical analysis was performed using Review Manager 5.4 software. Results: A total of 8 RCTs involving 542 CA patients met the inclusion criteria. Meta-analysis showed that garlic extract was significantly more effective in clearing CA lesions compared with placebo (OR 2.87; 95% CI 1.95-4.23; p < 0.00001). Garlic extract also demonstrated comparable effectiveness to standard therapies such as podophyllin and imiquimod. There was no significant difference in the incidence of side effects between the garlic extract group and the control group. Conclusion: Garlic extract is an effective and safe therapeutic modality for CA. Further studies with more robust designs are needed to confirm these findings and explore the mechanism of action of garlic extract on CA.

Abstract

Background: Childhood leprosy is an important marker of the status of the ongoing leprosy control program, as it is an indicator of active disease transmission in the community. Children are believed to be the most vulnerable group to infection with M. leprae due to their immature or nascent immunity and exposure to intrafamilial contacts. Case report: A 8 years ole male patient presented with hypopigmented patch and erythematous patch with not itchy, not pain, and loss of sensation which increasing more since 1 week ago. Initially 2 months ago, white patch that felt numbness appeared on the back, then white patches wider and spread to the chest, both arms and thighs. Dermatologic finding showed erythematous plaques, hypopigmentation macule with infiltration on cheek, back, both of arms, xerosis cutis. Decreased sensory function was found in the median, ulnar and posteriol tibial nerves. Acid-fast bacilli (AFB) staining revealed bacterial index (BI) +5 and morphological index (MI) 80%. Histopathological examination showed atrophic stratified squamous epithelium, grenz zone, many groups of foamy macrophages reached the fat layer. The patient was diagnosed Morbus Hansen borderline lepromatous leprosy type with mild reversal reaction and treated with MDT-MB drug package, vitamin B complex tablet, zinc tablet, paracetamol tablet, and 10% urea cream on dry skin 2 times a day on dry skin after bath during 2 month and the lesion was reduced. Conclusion: The rate of childhood leprosy continues to be a significant problem. There is a clear need to strengthen early detection, treatment, and regular follow up of these cases in both high and low endemic setting is essential in the prevention of deformities.

Abstract

Background: Acne vulgaris is a chronic inflammation of the pilosebaceous unit. Although acne vulgaris is not fatal, it can cause psychological effects. This study aimed to compare the quality of life, anxiety, and depression levels based on the severity of acne vulgaris among medical students at Andalas University. Methods: This was an observational-analytic study with a cross-sectional design. It was conducted from May to June 2023. The population included the third-grade students of the Medical Study Program of the Faculty of Medicine, Andalas University, who had acne vulgaris. The respondents were 70 people, recruited with consecutive sampling techniques. This research used Lehmann classifications to classify acne severity, the Cardiff Acne Disability Index (CADI) to assess quality of life, and the Depression Anxiety Stress Scale (DASS-21) to assess anxiety and depression. The statistical analysis used the Kolmogorov-Smirnov test. Results: Most female respondents, aged 20 years old, experienced mild acne vulgaris (84.3%). Additionally, the respondents predominantly had mild quality of life disorders (80%), normal anxiety level (32.9%), and normal depression level (52.9%). The Kolmogorov-Smirnov test found no relationship between the severity of acne vulgaris with quality of life (p = 0.076), anxiety level (p = 0.623), and depression level (p = 0.751). Conclusion: The results showed that most respondents had mild acne vulgaris, mild impairment in quality of life, normal anxiety level, and normal depression level. Statistical tests revealed no significant association between the severity of acne vulgaris and these variables.

Abstract

Pendahuluan: Penuaan kulit wajah merupakan proses fisiologis kompleks yang dipengaruhi oleh faktor intrinsik seperti usia dan genetik, serta faktor ekstrinsik seperti paparan sinar ultraviolet, polusi, dan gaya hidup. Area periorbital dan perioral termasuk bagian wajah yang paling awal menunjukkan tanda penuaan berupa keriput halus, kehilangan elastisitas, dan perubahan tekstur kulit. Chemical peeling merupakan salah satu modalitas terapi noninvasif yang terbukti efektif dalam memperbaiki kerutan superfisial dan memperbaiki kualitas kulit melalui proses eksfoliasi kimiawi dan stimulasi regenerasi kolagen dermal. Laporan kasus: Seorang pasien perempuan usia 51 tahun datang ke Klinik Griya Satya Estetika dengan keluhan utama garis kerutan pada daerah sekitar mata dan bibir yang semakin tegas sejak 1 tahun yang lalu. Pemeriksaan dermatologis ditemukan fine wrinkles periorbital, finemotion wrinkles sudut mata (crow’s feet), fine wrinkles perioral. Pasien didiagnosis aging skin dengann Glogau Wrinkle Scale derajat III (advanced photoaging). Pasien dilakukan tindakan chemical peeling kombinasi (30% glycolic acid, 14% salicylic acid, 5% phytic acid). Evaluasi pasca tindakan menunjukkan peningkatan kelembapan kulit, penurunan Trans Epidermal Water Loss (TEWL), serta perbaikan tekstur kulit dengan berkurangnya kedalaman kerutan. Kesimpulan: Penggunaan tindakan chemical peeling  dengan kombinasi glycolic acid, salicylic acid dan phytic acid terbukti efektif dan aman sebagai terapi noninvasif untuk memperbaiki kerutan periorbital dan perioral pada pasien dengan penuaan kulit derajat sedang. Terapi ini dapat menjadi alternatif perawatan yang efisien untuk rejuvenasi tanpa memerlukan tindakan invasif.
