Abstract

Bangakalan Sub-district occupies with the most patients infected by COVID-19 in Bangkalan Regency. Meanwhile, Bangkalan Regency itself has a significant number of COVID-19 patients. Various efforts are continuously have been done to control the rate of COVID-19 transmission, and one of the efforts is mask usage. Mask usage compliance can be influenced by several factors. This study aimed to find out which factors can significantly influence mask usage among the Bangkalan Sub-district community. This study used an analytical observation with a cross-sectional approach and obtained 100 samples. The number of respondent who compliant were 32 people (32%) (male 8 (8%), female 24 (24%), moderately compliant 33 people (9% male (9%), female 24 (24%)) and 35 people less compliance (male 13 (13%), female 22 (22%)). Based on the crosstab analysis Chi square test, only enabling and reinforcing factor that correlated with mask usage compliance. In enabling factor, there are two elements that are significantly related to mask usage compliance : providing spare masks whenever leaving the house (p-value=0,027) and good mask maintenance (p-value=0,000). In reinforcing factor, there are two elements that are significantly related to mask usage compliance : receiving warnings on mask usage from co-workers/schoolmates/college friends (p-value=0,002) and receiving warnings on mask usage from community leaders (p-value=0,040). The ordinal regression analysis showed  factors that did not significantly  influence mask usage compliance were predisposing factor (p-value = 0,452) and enabling factor (p-value = 0,527). Meanwhile, reinforcing factor (p-value = 0,001) significantly influenced mask usage compliance. When these three factors were described, the results of the ordinal regression showed ease of getting mask (p-value = 0,000), good mask maintenance (p-value = 0,037), receive warnings through social media (p-value = 0,043), and recieving warnings from the community leaders (p-value = 0,046) significantly influenced mask usage compliance among Bangkalan Sub-district community during COVID-19 pandemic.

Abstract

The World Health Organization (WHO) estimates a continuous increase in the number of hypertension cases alongside the expanding population. It is projected that approximately 29%, or around 1.6 billion people worldwide, will be afflicted with hypertension by 2025. The findings of the 2018 Riskesdas survey reveal a prevalence of hypertension in the population aged > 18 years based on national measurements, standing at 34.11%. This represents a 9% surge from the 2013 Riskesdas data, which recorded a prevalence of 25.8%. East Java Province ranks seventh with a prevalence rate of 36.32%, surpassing the national average. In the pursuit of hypertension control, it has been integrated into the Preventive Efforts of the Healthy Indonesia Family Approach Programs (PIS-PK) under point 7, mandating continuous monitoring of hypertension status for family members with a history of hypertension.  This study aims to identify determinant factors associated with enhanced services and visits for hypertension patients at Universitas Brawijaya Hospital in Malang amidst the pandemic. This study employs an observational analytic design, aiming to analyze the relationship between education level and medication adherence among hypertension patients during the COVID-19 pandemic. Temporally, this research adopts a cross-sectional approach, where both dependent and independent variable data are collected simultaneously. The study was conducted at Brawijaya University Malang, utilizing data from hypertensive patients who visited the hospital between March 2020 and March 2022. Logistic regression analysis with α = 0.05 will be employed for analytical purposes. The variable most strongly associated with hypertension patient visits is insurance ownership, exhibiting a Chi-Square value of 26.716. Additionally, the age variable demonstrates a correlation with patient visits, with a Chi-Square value of 4.312. The corresponding P-values for insurance ownership (p = 0.000) and age (p = 0.038) indicate a significant relationship between insurance ownership, age, and patients' motivation to seek healthcare. The determinant factors that positively influence hypertensive patient visits during the pandemic at Brawijaya University Hospital are insurance ownership and age. On the other hand, gender, regional affordability, and occupation exert a lesser impact on patient visits.

Abstract

One of the common problems in managing emergency logistics is the emergency trolley that conflicts with the standard operating procedures. It causes a mismatch of the emergency trolley contents. In fact, the availability of unstandardized emergency trolleys will impact the quality of emergency services in the emergency department. The emergency medicine shortage and emergency trolley content discrepancy also occurred several times in the ER. This study was conducted to analyze the determinant factor of content discrepancy in the emergency trolley in the emergency room of a private hospital in Malang Regency. The research method used was descriptive quantitative. The methods of identifying the root of the problem were carried out through field observations, literature studies, brainstorming, and Focus Group Discussions (FGD). Statistical tests were done using Bartlett's test of sphericity and Kaiser-Mayer Olkin (KMO) with a value higher or equivalent to 0.5. The study results formed two determinants of content discrepancies in the emergency trolley in the ER. The stock procurement factor is able to explain the problem by 48.02%, and the process documentation factor is able to explain the problem by 17.63%.

Abstract

Pulmonary Tuberculosis (TB) is a global health problem. Of all people infected with Mycobacterium tuberculosis only a small proportion develops into TB. IL 23 is the key cytokine in the pathogenesis of TB infection. This study aims to determine the association of IL-23 R rs 7518660 gene polymorphism with susceptibility and disease severity of Pulmonary TB. A case control study involved 105 people consisting of 31 drug sensitive pulmonary TB patients, 40 patients with drug-resistant pulmonary TB and 34 healthy subjects as a control. IL-23 R rs 7518660 gene polymorphism G allele increases susceptibility to both TB drug-sensitive and drug-resistant. G and A allele, AA and AG genotypes indicates (p value >0.05) in correlation with disease severity based on lesion in chest x-ray and high load of Mycobacterium tuberculosis in sputum. There was a significant relationship between allele A and susceptibility to pulmonary TB with an odds ratio of 0.231. It showed that patients with A alleles (AG and AA genotypes) were at risk of developing TB by 1/0.231 = 4.33 times lower than patients with GG genotypes. Meanwhile, the relationship of the G allele with susceptibility to pulmonary TB obtained (p value <0.05) and an odds ratio value of 0.127 indicating that patients with G alleles (GG and AG genotypes) were at risk of developing TB of 1/0.127 = 7.87 times higher than in patients with the AA genotype. Conclusion: We found significant correlation between IL-23 R rs 7518660 gene polymorphism G allele with susceptibility to pulmonary TB, but the result was not significant with disease severity.

Abstract

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a progressive respiratory disorder characterized by increasing Leukotriene B4 (LTB4) and eosinophil counts. Volatile organic compounds (VOCs) have shown promise as non-invasive biomarkers, reflecting COPD pathophysiology. Identifying specific VOCs associated with increased LTB4 and eosinophil counts could lead to the discovery of potential biomarkers for COPD severity or progression. AIMS: This study aims to investigate the correlation between VOCs and leukotriene B4 (LTB4) levels, as well as eosinophil counts counts in COPD patients. METHOD: Using an observational-analytic method with a case-control approach, 20 COPD patients and 20 controls were enrolled from the respiratory outpatient department of Dr. Saiful Anwar General Hospital, Malang. VOC levels were measured using a breath analyzer, while LTB4 levels were determined through enzyme-linked immunosorbent assays. Spearman’s correlation tests examined associations between VOCs, LTB4, eosinophil counts, and comorbidity, with Mann-Whitney tests comparing results against the control group. Data significance was set at p < 0.05. RESULT: There were 40 COPD patients and 40 controls in this study. There were significant differences between VOCs in the COPD group and the control group (p < 0.05). LTB4 level significantly increased in the COPD group than in the control group (p < 0.001), and there was no difference in the eosinophil level. There was a correlation between LTB4 and VOC level of C2H5OH in COPD patients (p = 0.009; r = 0.410). There was no correlation between eosinophil counts and VOCs (p = 0.939). The level of VOCs was significantly different between patients with only COPD and patients with COPD and comorbid lung cancer (p < 0.05). CONCLUSION: There is a correlation between VOC and LTB4 in COPD patients.
