Abstract

Background: Dental caries is a preventable disease. This prevention covers all aspects of dentistry carried out by dentists, individuals, and society that affect the health of the oral cavity. Prevention services are focused on the early stages, before the onset of disease or pre-pathogenesis. At the onset stage, the disease is a primary preventive service or a service to prevent disease. This is marked by efforts to improve health (Health Promotion) and provide special protection (specific protection).Objective: To explore the differences in dental caries prevention behavior: puzzles and storytelling as a medium for health education.Methods: Quasi-experimental design research with non-equivalent control group design.Results: Prevention of dental caries in the group that was given health education by means of a puzzle game showed an average of adequate prevention behavior (20.9); with storytelling games, it shows the average of good preventive behavior (26.5); The prevention of dental caries in the control group showed an average of adequate prevention behavior (12.8), and differences in behavior between groups given puzzle games and storytelling in the prevention of dental caries in children at Pacarkembang Kindergarten Surabaya with the result ρ = 0.000.Conclusion: There is an influence between health education with puzzle games and storytelling in the prevention of dental caries in children.

Abstract

Background: Chronic suppurative otitis media (CSOM) is still a significant public health problem, especially in developing countries. Eustachian tube dysfunction is one of the factors that influence the occurrence of CSOM. The pathogenesis of acute and chronic otitis media is initiated by negative pressure in the middle ear. One theory of the event of CSOM is the continuum theory. The eustachian tube as one of the factors that influence the occurrence of CSOM has been proven from various studies comparing CSOM patients and the average population and CSOM with and without cholesteatoma. With the advanced technology, CT scan helps diagnose pathological conditions in the ear as a diagnostic tool and evaluation modality. CT scan imaging with a multiplanar reconstruction technique can provide a clear image of the anatomical shape of the Eustachian tube. Objective: This study aims to determine the relationship between the angle and length of the Eustachian tube in the ear with CSOM and the contralateral ear's angle and length. Method: An analytic study with a cross-sectional design involving 30 samples of CSOM patients with the contralateral ear. The measurement of the angle and length of the Eustachian tube using the multiplanar HRCT temporal reconstruction technique. Result: Statistical analysis with paired t-test showed that the Eustachian tube angle was significantly more horizontal in the CSOM ear than the contralateral ear (p<0.05). Eustachian tube length was shorter in CSOM than contralateral (p<0.05). Conclusion: The angle of the eustachian tube in the CSOM ear is smaller (more horizontal) and shorter than the contralateral ear.

Abstract

Instagram, which ranks fourth globally in usage in Indonesia, has become an effective and cost-efficient marketing tool, especially in the healthcare sector. Hospitals, such as RSUD Dr. Saiful Anwar Malang, utilize Instagram to promote services and engage patients. However, despite a growing following and content, fluctuating patient visits suggest that the hospital's Instagram marketing strategy may need further optimization. The purpose of this study was to identify social media marketing activities (SMMA) and customer experience on Instagram regarding the relationship quality of outpatient patients at RSUD Dr. Saiful Anwar Malang. This study used a quantitative approach with a cross-sectional design and accidental sampling of outpatients during February-March 2024. Data were collected through surveys and analyzed statistically to examine the impact of SMMA and customer experience on Instagram on the relationship quality of outpatients.  The results of the multiple linear regression analysis show that SMMA has a significant effect on relationship quality (regression coefficient = 0.34, p=0.039), while customer experience has a significant positive effect (regression coefficient = 1.003, p=0.000), indicating that improving customer experience has a stronger impact on relationship quality compared to SMMA. The study concludes that SMMA has a partial effect on relationship quality, meaning that better SMMA increases relationship quality. This suggests that future media services should prioritize building connections, satisfaction, and patient retention.

Abstract

Background: Nasopharyngeal carcinoma (NPC) is a squamous cell carcinoma originating from mucosal epithelium of the nasopharynx with complex disease progression. About 95% are caused by Epstein-Barr virus (EBV) infection which is characterized by the detection of viral gene product protein of Latent Membrane Protein-1 (LMP-1). Tumor growth and metastasis depend on the mechanism of angiogenesis. Interleukin-8 (IL-8) is a potent angiogenic factor and involved in the angiogenesis mechanism. Kirsten Rat Sarcoma (KRAS) is one of the proto-oncogenes that has an increased expression of more than 60% in NPC. The KRAS activation played a role in the modulation of IL-8 expression by triggering several important signaling pathways, which triggered neovascularization in the process of angiogenesis. Purpose: To determine the correlation between expression of LMP-1 with KRAS and IL-8, in mechanism of angiogenesis in NPC WHO type III. Method: Analytical observational study with a cross-sectional approach involving 30 paraffin blocks of biopsy tissue from NPC patients who had not received radiotherapy or chemotherapy. Expressions of LMP-1, KRAS, and IL-8 were examined with immunohistochemistry (IHC) staining method, and calculated using manual counting by Anatomic Pathologists. Result: Statistical analysis of LMP-1 expression with KRAS showed an insignificant positive correlation (p=0.546), with a correlation coefficient (ρ=0.115). The KRAS expression with IL-8 showed an insignificant positive correlation (p=0.851), with a correlation coefficient (ρ=0.036). The LMP-1 expression with IL-8 showed a significant positive correlation (p=0.042), with a correlation coefficient (ρ=0.321). Conclusion: The increase in the expression of LMP-1 was followed with the increase in the IL-8 expression. Keywords: NPC, LMP-1, KRAS, IL-8, angiogenesis   ABSTRAK Latar belakang: Karsinoma nasofaring (KNF) adalah karsinoma sel skuamosa yang berasal dari epitel mukosa nasofaring. Perkembangannya melibatkan hubungan yang kompleks. Sekitar 95% disebabkan oleh infeksi virus Epstein-Barr (VEB) yang ditandai dengan terdeteksinya protein produk gen virus, salah satunya yaitu Latent Membrane Protein-1 (LMP-1). Pertumbuhan dan metastasis tumor tergantung pada mekanisme angiogenesis. Interleukin-8 (IL-8) adalah faktor angiogenik yang kuat, dan terlibat dalam mekanisme angiogenesis. Kirsten Rat Sarcoma (KRAS) merupakan salah satu proto- onkogen yang mengalami peningkatan ekspresi lebih dari 60% pada KNF. Aktivasi KRAS memainkan peran dalam modulasi ekspresi IL-8 dengan memicu beberapa jalur sinyal penting, dan hal ini dapat memicu neovaskularisasi pada proses angiogenesis. Tujuan: Mengetahui korelasi antara ekspresi LMP-1 dengan ekspresi KRAS dan IL-8 dalam mekanisme angiogenesis KNF WHO tipe III. Metode: Penelitian observasional analitik dengan pendekatan potong lintang yang melibatkan 30 blok parafin jaringan biopsi penderita KNF yang belum mendapat pengobatan radioterapi maupun kemoterapi. Pemeriksaan ekspresi LMP-1, KRAS, dan IL-8 menggunakan pewarnaan imunohistokimia, dan hasilnya dihitung secara manual oleh ahli Patologi Anatomi. Hasil: Analisis statistik ekspresi LMP-1 dengan KRAS menunjukkan korelasi positif yang tidak signifikan (p =0,546), dengan koefisien korelasi ρ=0,115. Ekspresi KRAS dengan IL-8 menunjukkan korelasi positif yang tidak signifikan (p=0,851), dengan koefisien korelasi ρ=0,036. Ekspresi LMP-1 dengan IL-8 menunjukkan korelasi positif yang signifikan (p=0,042), dengan koefisien korelasi ρ=0,321. Kesimpulan: Semakin tinggi ekspresi LMP-1, maka diikuti oleh tingginya ekspresi IL-8. Kata kunci: KNF, LMP-1, KRAS, IL-8, angiogenesis

Abstract

The issue of overcrowding in the Emergency Department (ED) has not been resolved in Indonesia, which can be caused by prolonged Length of Stay (LOS) of patients in the ED, resulting in delayed service time and increased mortality rates in the ED. Methods: This research is quantitative research using primary data. The research design used observational analytics with a prospective cohort approach with a sample size of 62 patients. Research Findings: Triage, length of diagnostic testing, the number of patients in the ED at the time the patient arrives, and the nurse-to-patient ratio do not have a significant correlation with ED LOS. There is a significant correlation between Boarding Time and ED LOS (p<0.001) with strong correlation strength (0.77). Conclusions: There is a strong positive linear correlation between Boarding Time and the length of time patients stay in the ED (p<0.001) with strong correlation strength (0.77) and is a dominant factor with a capacity of 58.6% to explain patient’s ED LOS.

Abstract

Background: Women are susceptible to unwanted pregnancies. This makes them susceptible to risk factors such as maternal mortality, abortion, and stress. This study aims to explore health workers' perspectives on contraceptive selection among couples of childbearing in East Lombok Regency. Methods: Qualitative research design was employed with an interpretive phenomenological approach. This study was conducted in 4 Public health centers (PHCs) in East Lombok Regency, namely the Montong Betok PHC, Wanasaba PHC, Selong PHC, and Denggen PHC between May and June 2024. There were 12 participants consisting of 2 doctors, 8 midwives, and 2 nurses, recruited by purposive sampling. Data was collected using a recording device through in-depth interviews and field notes. Data were analyzed using a process based on four stages: transcribing, organizing, recognizing, and coding. Results: Eight themes emerged through data analysis, including 1) the role of health workers in contraceptive selection, 2) provider factors, 3) contraceptive availability, 4) access to contraceptive services, 5) cost of contraceptive use, 6) provision of contraceptive information by health workers, 7) social influences, and 8) fears and myths about contraception. Conclusion: Many factors influence contraceptive choice, but social influence is the most critical factor that determines whether couples of childbearing age choose and use contraception

Abstract

Background: Stroke is a serious condition can cause sudden loss of neurological function. Sleep disorders, such as insomnia, hypersomnia, sleep apnea, and restless leg syndrome, often occur in stroke patients and can worsen the post-stroke rehabilitation process, especially who have a history of diabetes mellitus (DM), hypertension, and heavy smoker. Effective management of post-stroke patients requires identifying and treating sleep quality disorders and associated risk factors to improve quality of life. Objective: This study aims to explore the association between DM, hypertension, and smoking in post-stroke patients and sleep quality. Methods: A cross-sectional study was conducted on post-stroke control patients at the Neurology Clinic at Dr. Saiful Anwar Hospital, East Java, from December 2023 to February 2024. Sleep quality disorder were measured using the Pittsburgh Sleep Quality Index (PSQI) within one month after outpatient treatment. Additional data were analyzed using the chi-square test and binary logistic regression. Results: Among the 74 participants, 25 patients (25.7%) had a history of DM, 68 (91.9%) with hypertension, and 20 (27%) were heavy smokers. There were 34 patients (45.9%) who experienced sleep quality disorders. There was a significant association between DM and sleep disorders (p=0.005), while hypertension and smoking did not show a significant correlation. History of DM was a significant predictor of sleep disorders (p=0.09 | OR 4.782, 95% CI 1.483–15.421). Conclusion: This study shows a significant association between history of DM in post-stroke patients and sleep quality based on PSQI.

Abstract

Background: The @telmed.fkub account is a health promotion program of the Medical Faculty of Brawijaya University that provides health information services to the public. This program conducts regular evaluations to determine the effectiveness of services to the community.  Objective: To evaluate this program in determining the user's satisfaction with appearance, usability, service, and willingness to recommend. Then, the level of satisfaction is linked to user demographic data, whether there is a relationship or not.  Methods: This research uses a cross-sectional observational research method where data is collected by a Google Form questionnaire distributed randomly to Instagram account followers @telmed.fkub. The demographic variables were age, gender, education level, and educational background. The satisfaction level was categorized into five subtypes: appearance, service, usability, and willingness to recommend. These satisfaction levels were then measured on a five 5-level Likert scale. The Chi-Square or Fisher's Exact tests analyzed the collected data to determine the p-value.  Results: The total number of respondents was 99. Most respondents are satisfied with the @telmed.fkub akun and program (96.97%). It was also shown that there was a significant relationship between the genders of @telmed.fkub users in telehealth display satisfaction, and there were no significant differences between age groups, education levels, and backgrounds education of @telmed.fkub users in telehealth satisfaction.  Conclusion: There is a significant relationship between gender and appearance satisfaction levels of @telmed.fkub users.

Abstract

Bronchial asthma is a long-term lung disease that makes the airways swell up and changes how air flows. To manage effectively, it is essential to understand the host and environmental factors that aggravate diseases. This case study analyzes the interaction of genetic factors, environmental influences, and psychosocial stressors in a 46-year-old male with a ten-year history of asthma. Mr. I, who is 46 years old, is having a really serious asthma attack that is making him cough and making it hard for him to breathe. The cold air and stress at work often made matters worse for him. His mother and daughter both have asthma, therefore it runs in the family. Recent flare-ups show how hard it is for people with certain environmental and job-related risk factors to keep their asthma under control for a long time, even after being diagnosed and treated. The patient's symptoms show how hard it may be to deal with asthma. Cold air and stress at work are two key things that make it worse. He smokes at work, which makes it more probable that objects in the surroundings may hurt him. Functional Degree 1 means that the patient can take care of himself, but he needs to see a doctor since he is worried about his health and can't sleep well. To enhance his quality of life and avert subsequent flare-ups, it is crucial to implement a family-centered strategy that encompasses educating him on avoiding triggers and complying with both pharmaceutical and non-pharmacological treatments. This instance exemplifies the necessity of identifying and mitigating specific environmental and behavioral triggers in the management of chronic asthma. A personalized treatment strategy and thorough education for patients and their families can considerably improve asthma control and maintain functional independence, especially in cases of chronic illness and severe risk factors.

Abstract

Chronic abdominal pain, a prevalent yet often multifactorial complaint in primary care, frequently extends beyond purely biomedical etiologies to encompass significant psychosocial stressors and family dynamics. This case report highlights the importance of a holistic, family-centered approach in diagnosing and managing such conditions, moving beyond a narrow symptomatic focus. We present the case of a 35-year-old female kindergarten teacher experiencing recurrent right lower abdominal bloating and pain for three days, exacerbated by activity and alleviated by rest. Associated symptoms included nausea, irregular bowel movements (once every three days), and feelings of overwhelm due to work and domestic responsibilities. Despite her concerns about a prior hepatomegaly during her second pregnancy, physical examination and initial assessment pointed towards a more generalized abdominal discomfort influenced by lifestyle and stress. This case illustrates how chronic abdominal discomfort, initially perceived by the patient as a singular biomedical issue, can be intricately linked to a confluence of biological, psychological, and social factors. The patient's irregular eating habits, high coffee consumption, lack of exercise, and significant work-life imbalance contributed to her symptoms. While an initial diagnosis of acute gastritis was considered, the persistent nature and association with stress suggest a broader functional gastrointestinal disorder. The family APGAR score of 9 indicated a highly functional family unit, yet the Family SCREEM identified cultural factors (husband's limited domestic involvement) and the patient's poor stress management as potential contributors. Comprehensive intervention involved patient education on lifestyle modification, stress management techniques, and family involvement to alleviate domestic burdens. This case underscores the necessity of a comprehensive, family-oriented approach in managing chronic abdominal pain. By addressing not only the physical symptoms but also the underlying psychosocial stressors and family dynamics, healthcare providers can facilitate more effective and sustainable patient outcomes. This holistic perspective aligns with the principles of family medicine, advocating for interventions that consider the patient within their unique family and community context.
