Abstract

Background: Exclusive breastfeeding coverage was 52.30% in 2015, which was below the national target of 80%. For the past few years Klaten local government has launched the Exclusive Breastfeeding Advocacy Program with the objective to increase exclusive breastfeeding coverage. This study aimed to determine the effectiveness of Exclusive Breastfeeding Advocacy Program using Theory of Planned Behaviour framework.Subjects and Method: This was an analytic observational study with retrospective cohort design. This study was conducted from 2 August to 16 September 2016 in Klaten and Magelang, Central Java, Indonesia. A total of 200 lactating mothers of infants aged 6 to 12 months was selected for this study by multi-stage random sampling. The dependent variables were exclusive breastfeeding and intention. The independent variables were exclusive breastfeeding advocacy program, attitude, subjective norm, and perceived behavior control. The data were collected  using a set of questionnaire, and analyzed by path analysis.Results: The path model showed Goodness of Fit indices as follows: CMIN=4.24, p=0.374, GFI=0.99, NFI=0.99, CFI=1.00, RMSEA=0.017. Strong intention (b=0.15; p <0.001) had direct effect on exclusive breastfeeding. Positive atittude (b = 0.27; p<0.001), positive subjective norm (b=0.26; p <0.001), strong perception of behavior control  (b=0.25; p<0.001), and exclusive breastfeeding advocacy program (b=0.36; p<0.001), had positive effects on intention.Conclusion: Intention has direct effect on exclusive breastfeeding. Atittude toward exclusive breastfeeding, subjective norm, and perception of behavior control, have positive effects on intention. Abstract

Background: The Indonesian Ministry of Health has launched a community-based non-com­mu­nicable disease (PTM) control program through the non-communicable disease Integrated Deve­lopment Post (Posbindu PTM). One of the non-communicable disease which is a very serious health problem is hypertension which is called the silent killer. The purpose of this study was to analyze the contextual effect of integrated development posts on the tertiary prevention behavior of patients with hypertension using Theory of Planned Behavior.Subjects and Method: A cross sectional study was conducted at 25 integrated health post in Surakarta, from September to October 2019. A sample of 200 hipertensive patients was selected by stratified random sampling. The dependent variable was tertiary prevention behavior. The inde­pendent variables were education, attitude, subjective norm, perceived behavior control, intention, cadre support, and peer support. The data were collected by questionnaire and analyzed by multi­ple logistic regression run on Stata 13.Results: Tertiary prevention behavior in patients with hypertention increased with education level ≥Senior high school (b= 1.22; 95% CI= 0.37 to 2.08; p= 0.005), positive attitude (b= 1.51; 95% CI= 0.54 to 2.48; p= 0.002), supportive subjective norm (b= 1.38; 95% CI= 0.46 to 2.29; p= 0.003), strong perceived behavioral control (b= 1.10; 95% CI= 0.17 to 2.03; p= 0.020), strong intention (b= 0.97; 95% CI= 0.13 to 1.82; p= 0.023), strong integrated development post cadre support (b= 1.57; 95% CI= 0.69 to 2.46; p<0.001), and strong peer support (b= 1.46; CI 95% = 0.57 to 2.35; p = 0.001). Integrated development post had a contextual effect on tertiary prevention behavior of hypertension with intra-class correlation (ICC)= 16.03%Conclusion: Education, attitude, subjective norm, perceived behavior control, intention, cadre support, and peer support increased tertiary prevention behavior of hypertension.Keywords: Tertiary prevention of hypertension, Theory of Planned BehaviorCorrespondence: Angga Ferdianto, Institute of Health Science Ngudia Husada Madura, Jl. Abstract

Background: The performance of an orga­nization is largely determined by the human resources in it. If the human resources are highly motivated, creative and innovative and are led by leaders who are responsible and have good leadership skills, their performance will be good. This study aims to analyze the influ­ence of the leadership style of the head of the puskesmas and the work motivation of the em­ployees on the performance of the employees of the Puskesmas Ciamis District.Subjects and Method: This study was con­ducted using a cross sectional research design. The study was conducted in Ciamis Regency in November – December 2020. Sampling was carried out using random sampling with a total of 200 puskesmas employees in Ciamis Regen­cy. The dependent variable is employee per­form­ance. The independent variables include leadership style, work motivation, job satisfac­tion, education, treatment status, employee status, length of work. Data were collected by questionnaire and analyzed using a multi­level multiple logistic regression.Results: Good leadership style (b=3.58; 95% CI= 1.11 to 3.80; p= 0.000), high work moti­va­tion (b=3.12; 95% CI= 0.83 to 3.65; p= 0.002), have job satisfaction (b= 2.89; 95% CI= 0.67 to 3.53; p= 0.004), higher education (b= 2.89; 95% CI= 0.60 to 3.13; p= 0.004), inpatient care status (b= 2.66; 95% CI= 0.47 to 3.12; p= 0.008), health personnel status (b=3.68; 95% CI= 1.46 to 4.80; p= 0.000), and length of ser­vice 3 years (b=2.07; 95% CI= 0.78 to 2.76; p= 0.038) increase the possibility of the perform­ance of puskesmas employees. Variations at the puskesmas level show that there is a contextual effect on employee performance (ICC= 11.75%).  Conclusion: There is a positive and statis­tically significant effect on leadership style, work motivation, job satisfaction, educa­tion, treatment status, employee status, length of work on the performance of puskesmas employees. Abstract

Stunting among children remains a global public health issue, with as many as 149 milion children under the age of five affected worldwide. One of the contributing factors to stunting is poor sanitation. This research aims to estimate the extent to which poor sanitation contributes to the risk of stunting in children under five years old using a meta- analysis approach. study employed a systematic review and meta-analysis using the PICO framework (Population, Intervention, Controls/Comparisons, Outcome), defined as follows: Population: children under five years old; Intervention: poor sanitation; Comparison: good sanitation; Outcome: stunting. Articles were collected using PRISMA flow diagrams. A total of nine articles that met the quality standards were then included in the quantitative synthesis using meta-analysis. Articles were analysed using the Review Manager 5.3 application. The findings of this meta-analysis indicate a significant association between sanitation conditions and the risk of stunting in children under five. Children who live in environments with poor sanitation are 3.71 times more likely to experience stunting than those living in areas with good sanitation. Poor sanitation constitutes a major risk factor for stunting, which significantly impacts children's growth and development. Enhancing access to improved sanitation facilities and promoting good hygiene practices are priorities to prevent stunting, particularly in areas with high prevalence. These findings offer strong evidence to support the policymakers and other stakeholders in integrating sanitation- based interventions into public health programs to reduce the prevalance of stunting among children under five years of age.

Abstract

Sejak tahun 2006, Departemen Kesehatan meluncurkan kebijakan program Desa Siaga. Tampaknya, kebijakan tersebut tidak mampu memberdayakan masyarakat dalam mengidentifikasi dan memecahkan masalah kesehatan pada level komunitas (desa). Penelitian ini bertujuan merumuskan model pemberdayaan masyarakat bidang kesehatan pada program Desa Siaga. Sasaran penelitian adalah Forum Kesehatan Desa dan Pos Kesehatan Desa Siaga di 30 desa di Kabupaten Karanganyar, Jawa Tengah. Metode yang digunakan dalam penelitian ini adalah survei dan studi kasus. Hasil penelitian menunjukkan faktor-faktor yang berhubungan dengan kemampuan masyarakat dalam mengidentifikasi masalah kesehatan dan kemampuan masyarakat dalam memecahkan masalah kesehatan. Model pemberdayaan masyarakat bidang kesehatan meliputi kemampuan mengidentifikasi dan memecahkan masalah kesehatan. Faktor-faktor internal dan eksternal komunitas pada level anggota masyarakat, institusi masyarakat, kepemimpinan masyarakat, dan akses informasi kesehatan memiliki peran penting dalam pemberdayaan masyarakat bidang kesehatan. Since 2006, the Health Department had launched the Village Preparedness program policy. But, this policy apparently not capable of empowering the community in identifying and solving the health problem at community (village) level. The objective of research is to formulate the community empowerment model in health in the Village Preparedness program. The targets of research were the Village Health Forum and Village Health Post in 30 Village Preparedness in Karanganyar Regency, Central Java. The method involving survey and case study. The case study showed factors related to community capability of identifying health problem and community capability of solving the health problems. The community empowerment model in health including the capabilities of identifying and of solving the health problems. The community internal and external factors at members of community level, community institution, community leadership, and information access played important role in community empowerment in health.

Abstract

Tuberkulosis merupakan masalah kesehatan masyarakat yang utama di tingkat global, regional, nasional, maupun lokal. World Health Organization menggulirkan strategi directly observed treatment short course (DOTS) dan strategi stop tuberculosis partnership bertujuan untuk menjangkau semua penderita tuberkulosis. Kedua strategi tersebut masih belum mampu mencapai target case detection rate (CDR) secara konsisten. Penelitian ini bertujuan merumuskan model modal sosial dan partisipasi masyarakat dalam crude death rate. Sasaran penelitian adalah petugas tuberkulosis dan kader di 30 desa di Kabupaten Karanganyar, Jawa Tengah. Metode yang digunakan adalah survei dan studi kasus. Hasil penelitian survei menunjukkan, desa dengan modal sosial yang tinggi mempunyai kemungkinan untuk melampaui target CDR ≥ 70%, 9 kali lebih besar daripada desa dengan modal sosial rendah. Desa dengan partisipasi masyarakat tinggi mempunyai kemungkinan 7,5 kali lebih besar daripada desa dengan partisipasi masyarakat rendah. Hasil penelitian studi kasus menunjukkan, faktor-faktor modal sosial yang berhubungan dengan CDR terdiri dari dimensi kognitif meliputi kepercayaan dan merasa mempunyai program tuberkulosis. Dimensi relasional meliputi norma sosial, penanaman jasa pribadi, kerja sama, dan komunikasi. Dimensi struktural meliputi jejaring dan persatuan. Faktor-faktor partisipasi yang berhubungan dengan CDR meliputi identifikasi kebutuhan, menggerakan sumber daya program, dan kepemimpinan. Tuberculosis is an important public health problem of global, regional, national, and local levels. World health organization launched directly observed treatment short course (DOTS) and stop tuberculosis partnership strategies aiming to reach all tuberculosis patients. Both strategies have not been able to reach the case detection rate (CDR) target consistently. This research aimed to formulate a social capital and participation model in crude death rate. The target of research was the officers of tuberculosis programs and cadres in 30 villages in Karanganyar Regency, Central Java. The method used in this research was survey and case study. The result of survey research showed that the village with high social capital had 9 times probability of surpassing CDR target ≥ 70% than the one with low social capital and 7.5 times higher than the one with low public participation. The result of case study showed that the social capital factors relating to CDR consisted of cognitive dimension encompassing trust and sense of belonging to tuberculosis program. Relational dimension encompassed social norm, personal service implantation, cooperation, and communication. Structural dimension involved public network and association. The factors of participation relating to CDR included need identification, activating the program resource, and leadership.

Abstract

Faktor internal komunitas yang berperan dalam pemberdayaan masyarakat bidang kesehatan adalah kepemimpinan dan modal sosial, sedangkan faktor eksternal komunitas yang berperan adalah akses informasi kesehatan, petugas dan fasilitator kesehatan. Tujuan penelitian adalah mengetahui dan menganalisis peran kepemimpinan, modal sosial, akses informasi kesehatan, petugas, dan fasilitator kesehatan dalam pemberdayaan masyarakat bidang kesehatan. Desain penelitian adalah potong lintang dengan pendekatan penelitian kualitatif melalui metode studi kasus terpancang. Penelitian dilakukan selama tiga bulan di Kabupaten Tulungagung, Jawa Timur dengan mengambil dua desa, yaitu Desa Bulus di Kecamatan Bandung dan Desa Tanggul Kundung di Kecamatan Besuki tahun 2013. Hasil penelitian menunjukkan bahwa peran kepemimpinan dalam pemberdayaan masyarakat bidang kesehatan pada program Desa Siaga adalah menyebarluaskan informasi, memberikan contoh, menyadarkan, memotivasi, membimbing, menggerakkan sasaran dan masyarakat, memfasilitasi dan mengalokasikan sumber daya. Peran modal sosial adalah saling percaya, kekerabatan, pertemanan, pertetanggaan, norma sosial, tolong menolong, kerjasama, dan jaringan masyarakat. Peran akses informasi kesehatan adalah meningkatkan pengetahuan dan keterampilan kesehatan, mengambil keputusan, dan meminta pelayanan kesehatan. Peran petugas kesehatan adalah sosialisasi, memberikan petunjuk, melatih, membina, memfasilitasi, menumbuhkembangkan partisipasi, serta memantau dan mengevaluasi program. Sedangkan peran fasilitator kesehatan adalah sosialisasi, memotivasi, memengaruhi pengambilan keputusan, memediasi masyarakat dan pemerintah, memfasilitasi dan menumbuhkembangkan partisipasi. The internal factors of community contributing to public empowerment in health sector were leadership and social capital, meanwhile the external factors included health information access, health duty and facilitator. This study aimed to determine and analyze the roles of leadership, social capital, information access, and health duty and facilitator within public empowerment in health sector. The study design was cross sectional with qualitative study approach through embeded case study method. The study was conducted in Tulungagung District, East Java by taking two villages namely Bulus Village at Bandung Subdistrict and Tanggul Kundung Village at Besuki Subdistrict in 2013. The result showed the roles of leadership and public empowerment in health sector in Alert Village program were spreading information, giving examples, awakening, motivating, educating, moving targets and the public, facilitating and allocating resources. The roles of social capital were mutual trust, kinship, friendship, neighborhood, social norms, mutual help and public network. The roles of health information access were improving health knowledge and skill, making decision and asking for health services. The roles of health duty were socialization, giving instructions, training, building, facilitating, developing participation as well as monitoring and evaluating the program. Meanwhile, the roles of health facilitators were socialization, motivating, influencing decision making, mediating public and government, facilitating and developing participation.

Abstract

 The aim of the study was to analyse the direct and indirect effects of knowledge, attitudes, and eating behavior on anthropometry status and anemia incidence in female adolescents. This observational study employed case-control design and involved a total with total of female adolescents 117, classified into case groups (n=39) and control groups (n=78). The data were collected using questionnaires on knowledge and attitude about nutrition of anemia and eating behaviors, 2x24 hour food recall, Food Frequency Questionnaire (FFQ), anthropometry, and also Hb level measurement by Cyanmethemoglobin method. The data were analysed using path analysis. The results showed that there was a significant relationship (p<0.05) and direct effects between eating behavior on anthropometry status and incidence of anemia, which accounted for 66% and 22% respectively. There was no relationship (p>0.05) and indirect effects between attitude and incidence of anemia with value 4.1%. Eating behavior had a strong effect on anemia incidence through anthopometry in female adolescents.
