Abstract

Drug-resistant tuberculosis (TB) is a public health problem that poses major challenges to patients, health workers, society, and the health care system. This research is a qualitative study to explore the implementation of drug side-effect monitoring and the role of health workers in handling side-effect monitoring medication for tuberculosis patients. We collected data through Focus Group Discussions (FGD) and conducted direct, in-depth interviews. The informants in this study were doctors, pharmacists, and nurses who were directly involved in treating drug-resistant tuberculosis patients. Two drug-resistant TB referral hospitals in the Cirebon area hosted the research. INVIVO 12 was used for qualitative analysis. Both hospitals still need to optimise the mechanism for tracking drug side effects in drug-resistant TB patients in accordance with the technical guidelines. Doctors at both hospitals play a significant role in implementation compared to other health workers. The similar role of doctors in both hospitals in implementation is in initial examination and history taking, determining causality, management, and education regarding drug side effects. Pharmacists have a similar role in managing drug side effects and documenting "unwanted events". The role of nurses in both hospitals is similar, namely providing patients with education on drug side effects. Efforts are needed to increase the implementation of monitoring of drug side effects by conducting assessments of the causality of drug side effects and documentation using specific instruments according to standards, ensuring the ensuring the completeness of manual monitoring forms, and providing continuous drug side effect monitoring training for health workers.

Abstract

Pneumonia is an infection caused by microorganisms. Antibiotics are one of the treatments given to patients with pneumonia caused by bacterial infections. Rational use of antibiotics will prevent resistance. This study aims to determine the relationship between the rationality of antibiotic use and therapeutic outcomes in community-acquired pneumonia patients. We used a cross-sectional study and retrospectively collected data using purposive sampling. The inclusion criteria are non-ICU community pneumonia patients have complete medical record data, while the exclusion criteria are patients who died or had other infections. We evaluated the rationality of antibiotics using the Gyssens method. Based on the results, there are 240 patients, with 210 patients receiving rational therapy and 30 patients receiving irrational antibiotics. Based on the results, 6 patients did not receive antibiotics on time, 4 patients received the wrong dose, 2 patients did not get the correct interval, 16 patients received a dose that was too short, and 2 patients were prescribed alternative antibiotics that were more effective. The results of the analysis showed that there is a significant relationship between the rationality of antibiotic use and the therapeutic outcomes of community-acquired pneumonia patients (p-value < 0.001; OR 14.36).

Abstract

Background: Despite available treatment options, diabetes mellitus (DM) management faces significant barriers affecting patient outcomes. While most research focuses on patient factors, limited attention has been given to healthcare provider perspectives, particularly hospital pharmacists who are key stakeholders in diabetes care. Objective: This study explored treatment barriers and solutions from hospital pharmacists’ perspectives in Indonesia. Methods: This qualitative phenomenological study involved 18 hospital pharmacists at Cilacap Regional General Hospital, Central Java, Indonesia. Participants were selected through purposive sampling and had minimum one-year experience in diabetes patient care. Semi-structured interviews were conducted from January to February 2025, audio-recorded, transcribed verbatim in Bahasa Indonesia, and analyzed using thematic analysis. All interviews were conducted in Bahasa Indonesia and participant quotes were translated to English while preserving original meaning and cultural context. Results: Analysis of three predetermined barrier themes revealed: adverse drug reactions (ADRs) (particularly metformin-related gastrointestinal effects and insulin-associated hypoglycemia), medication non-adherence (influenced by feeling healed, treatment boredom, age-related denial, and therapy burden), and monthly follow-up challenges (geographic barriers, transportation difficulties, physical limitations, and insufficient family support). Pharmacists proposed comprehensive solutions including patient and family education, interprofessional collaboration, technology integration through WhatsApp support, and flexible monitoring approaches. Conclusion: Pharmacist-identified barriers operate as interconnected system components requiring multi-level interventions. Evidence-based solutions emphasizing education, collaboration, family engagement, and technological support can enhance diabetes management outcomes while respecting cultural contexts and resource constraints.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is one of the leading causes of global morbidity and mortality, with a continuously increasing health burden. The complexity of long-term therapy in COPD increases the risk of Drug-Related Problems (DRPs), which can affect patient clinical outcomes. This literature review was conducted descriptively by searching articles in the PubMed, Scopus, Google Scholar, and ScienceDirect databases using keywords related to DRPs and COPD. The selection was made on publications from the last 10 years (2015–2025) relevant to the topic and available in full text. Based on studies by Li et al., (2019) and Apikoglu-Rabus et al., (2016), the classification of DRPs according to PCNE V9.0 mainly included aspects of medication safety (54.2%), inappropriate drug selection (up to 8.7%), excessive dosage (19.9%), excessive duration of therapy (17.7%), and errors in the drug use process (up to 63.3%). Patient factors such as non-compliance (25%) and incorrect inhalation techniques were the main contributors to the occurrence of DRPs. Inappropriate use of medications, drug-herb interactions, and lack of therapy monitoring were also consistently found. The findings suggest that DRPs significantly worsen symptom control and increase the risk of exacerbations. Pharmacists are critical in identifying, preventing, and managing DRPs through patient education, therapy review, and ongoing therapy monitoring. DRPs in COPD patients are a serious challenge that requires a multidisciplinary approach. Ongoing education, monitoring of therapy rationality, and collaboration between healthcare professionals are needed to improve patient safety and the effectiveness of COPD treatment.

Abstract

Non-communicable chronic diseases such as diabetes mellitus (DM) are a significant health burden worldwide. In the treatment of DM patients, long-term drug use is inevitable and increases the risk of Drug-Related Problems (DRPs). The presence of DRPs can affect the effectiveness of therapy and the risk of side effects in hospitalized DM patients. This review aims to evaluate various studies conducted on DRPs in hospitalized DM patients based on the Pharmaceutical Care Network Europe (PCNE) classification. A systematic search of relevant articles in the last 10 years through the SpringerLink, ScienceDirect, Scopus, and PubMed databases. Irrelevant studies, review articles, and no data on DRP classification using PCNE or outpatients will be excluded. The review found four articles discussing DRP classification using PCNE in hospitalized DM patients. The number of DRPs varied, ranging from 253 to 873 cases. The proportion of patients experiencing at least one DRP was also relatively high, ranging from 48.1% to 84.5%. The most common problem was treatment ineffectiveness (P1.2), which accounted for more than half of the DRPs in the three studies reviewed, namely 62.0%, 79.6%, and 52.7%. The most common cause of DRPs came from the patient-related domain (C7.1) at 71.85%. Other domains that caused DRPs were the Drug use domain (C6.1) at 62.0%, other domains (C9) at 40.9%, the drug selection domain (C1.6) at 26%, and the dose selection domain (C3.5) at 25.9%. The occurrence of DRPs is a significant problem in the management of diabetes mellitus, especially in the hospital environment. The ineffectiveness of therapy is the main problem of DRPs. The high number of DRPs from other domains indicates that many causes of DRPs are not classified explicitly in the PCNE category.
