Abstract

The successful implementation of an Occupational Safety and Health Management System (OSHMS) in high-risk industries, such as sugar manufacturing, is determined not only by the completeness of formal systems but also by human factors at the operational level. This study aims to analyze the influence of competence, workload, and job stress on the implementation of OSHMS among production employees at the Ngadirejo Sugar Factory in Kediri. A quantitative research design with a cross-sectional approach was employed. A total of 109 employees were selected from a population of 150 using simple random sampling. Data were collected through structured questionnaires and analyzed using multiple linear regression. The results indicate that job stress has a positive and significant effect on OSHMS implementation (β = 0.329; p = 0.004), whereas competence (β = −0.062; p = 0.643) and workload (β = 0.119; p = 0.275) do not show significant effects. Simultaneously, the three variables explain 10.1% of the variance in OSHMS implementation (R² = 0.101), suggesting that other factors outside the model play a more dominant role. The findings suggest that job stress may function as a driver of alertness that enhances compliance with safety procedures in the operational context of Ngadirejo Sugar Factory. Managerial implications emphasize the importance of adaptive stress management, not to eliminate stress entirely, but to maintain it at an optimal level that supports safe work behavior while preventing long-term fatigue and burnout.

Abstract

Background: The prevalence of ACS in Indonesia is high, at least 2 million people in Indonesia are diagnosed with ACS. It is one of the main causes of death among adults in many countries around the world, including Indonesia with prevalence of heart disease in Indonesia is estimated at 2 million ACS cases. One of the causes of the high ACS mortality rate is a prehospital delay. Decision time delay refers to the length of time the patient takes for early detection or to make a decision to seek help. This study will focus on the influence of social demographic factors on the ability of early detection of ACS. Purpose: This study aims to explain the relationship between treatment-seeking behavior, transportation, and socio-demographic factors (age, gender, socio-economic, educational status, health insurance). Methods: A prospective cross-sectional study was conducted in this study. The samples will be obtained in Blitar regency with 22 public health center sub-districts with sample 126 respondents. The correlation among variables was analyzed using chi-squared (?2), and for determining the dominant factors, multiple logistic regression with the enter method was used. A p value <0.05 was considered significant. Results: The study found that the age, health insurance status, education level, and employment status of the patients were significant factors for early detection. Delay to early detection increased with the increase in age of the patients, although it was not significant by logistic regression. Conclusions:  This study reveals that several sociodemographic factors that can affect early detection abilities are education, employment status, and health insurance.

Abstract

Background: The lack of knowledge and skills among family members in caring for post-stroke patients can make prolong recovery time and increase the risk of complications. Health education is essential to enhance understanding and optimize caregiver skills. Purpose: This study aims to examine the effect of health education on family knowledge and skills in post stroke care at Poncokusumo Community health center in Malang regency. Methods: This research employs pra experimental with one group pre-post test design with a total population of 35  family with the post stroke patients in Poncokusumo Community Health Center , from which 32 participants were selected using purposive sampling. The research instrument consists of a questionnaire, and data were analyzed using the Wilcoxon test. Results: The results show that before the health education intervention, half of the participants had a moderate level of knowledge (16 people, 50.0%). After the intervention, nearly all participants demonstrated good knowledge (28 people, 87.5%). In terms of skills, before the intervention, the majority of participants had poor caregiving skills (19 people, 59.4%). After the intervention, most participants improved to a moderate skill level (19 people, 59.4%). There was a significant effect of health education on family knowledge regarding post-stroke care at Poncokusumo Community Health Center, with a p-value of 0.000 (<0.05). Conclusion: Health education is effective in improving family knowledge and skills in caring for post-stroke patients. With structured education, families understand how to care for patients, from mobilization, prevention of complications, to psychosocial support. Direct training and educational media help them apply skills with more confidence. This increased understanding contributes to better quality of care, accelerates patient recovery, and reduces the risk of complications.

Abstract

Praktik penunjang disertasi ini dilaksanakan di Dinas Kesehatan Provinsi Nusa Tenggara Barat sebagai lembaga pembina dan pengendali mutu pelayanan kesehatan primer, dengan fokus pada Puskesmas di Kabupaten Lombok Tengah. Tujuan praktik ini adalah mengembangkan model peningkatan kepuasan pasien melalui persepsi terhadap keamanan layanan dan empati tenaga kesehatan dengan kepercayaan pasien sebagai variabel mediasi. Metode yang digunakan meliputi pengkajian sistem pembinaan, analisis Fishbone, penentuan prioritas masalah menggunakan USG, serta analisis SWOT sebagai dasar perumusan intervensi kebijakan. Hasil pengkajian menunjukkan bahwa kepuasan pasien di Puskesmas Lombok Tengah sangat dipengaruhi oleh persepsi terhadap keamanan layanan dan empati tenaga kesehatan. Namun, pengaruh kedua faktor tersebut terhadap kepuasan pasien bekerja secara tidak langsung melalui tingkat kepercayaan pasien. Ketika pasien merasa aman dan diperlakukan dengan empati, kepercayaan terhadap Puskesmas meningkat dan mendorong kepuasan yang lebih tinggi. Sebagai solusi dikembangkan Provincial Trust-Based Primary Care Quality System (PTBPCS–NTB), yaitu model pembinaan mutu pelayanan primer berbasis penguatan keamanan layanan, empati tenaga kesehatan, dan kepercayaan pasien. Model ini menempatkan Dinas Kesehatan Provinsi NTB sebagai pengendali standar mutu, keselamatan pasien, dan pembinaan perilaku pelayanan di Puskesmas. Implementasi awal PTBPCS–NTB menunjukkan peningkatan rasa aman, tumbuhnya kepercayaan pasien, dan perbaikan kepuasan pasien di Puskesmas Kabupaten Lombok Tengah. Praktik ini menegaskan bahwa penguatan kepercayaan pasien melalui mutu dan empati pada level regulator merupakan kunci dalam membangun pelayanan kesehatan primer yang berorientasi pada pasien.

Abstract

Praktik penunjang disertasi ini dilaksanakan di Dinas Kesehatan Provinsi Nusa Tenggara Barat sebagai lembaga pembina dan pengontrol tata kelola sumber daya manusia rumah sakit daerah, dengan fokus pada perawat RSUD Provinsi NTB. Tujuan praktik ini adalah mengembangkan model peningkatan retensi perawat melalui penguatan lingkungan kerja dan kompetensi dengan komitmen organisasi sebagai variabel mediasi. Metode yang digunakan meliputi pengkajian sistem pelatihan SDM, analisis Fishbone, penentuan prioritas masalah menggunakan USG, serta analisis SWOT untuk merancang strategi intervensi. Hasil pengkajian menunjukkan bahwa rendahnya retensi perawat bukan semata-mata disebabkan oleh faktor individu, tetapi merupakan konsekuensi dari belum optimalnya sistem pelatihan lingkungan kerja dan pengembangan kompetensi yang dikendalikan oleh Dinas Kesehatan Provinsi NTB. Penyebab masalah utama terletak pada ketiadaan sistem terintegrasi yang membentuk komitmen organisasi perawat sebagai dasar kekurangan tenaga konservasi. Sebagai solusi, dikembangkan Sistem Komitmen Kompetensi dan Lingkungan Kerja Provinsi (PWECCS–NTB), yaitu model pelatihan lingkungan kerja dan pengembangan kompetensi perawat berbasis komitmen organisasi yang dikendalikan oleh Dinas Kesehatan Provinsi NTB. Model implementasi ini menunjukkan peningkatan persepsi keadilan lingkungan kerja, peluang pengembangan kompetensi, serta penguatan komitmen organisasi yang berimplikasi positif terhadap retensi perawat RSUD Provinsi NTB. Praktik ini menyimpulkan bahwa penguatan peran Dinas Kesehatan sebagai pembina SDM rumah sakit merupakan kunci dalam membangun retensi perawat yang berkelanjutan dan mendukung mutu pelayanan kesehatan daerah.

Tujuan praktik ini adalah mengembangkan model peningkatan kualitas pelayanan melalui pelatihan, pembinaan, dan keterlibatan karyawan dengan persepsi tenaga kesehatan tentang kebijakan peningkatan mutu pelayanan sebagai variabel mediasi. Metode yang digunakan meliputi pengkajian sistem pembinaan, analisis Fishbone, penentuan prioritas masalah menggunakan USG, serta analisis SWOT sebagai dasar perumusan intervensi kebijakan. Hasil pengkajian menunjukkan bahwa kualitas pelayanan RSUD Patut Patuh Patju Lombok Barat sangat dipengaruhi oleh efektivitas pelatihan, sistem pembinaan, dan tingkat keterlibatan karyawan. Namun, dampak ketiga faktor tersebut terhadap kualitas pelayanan sangat bergantung pada bagaimana tenaga kesehatan memersepsikan kebijakan peningkatan mutu pelayanan yang dikendalikan oleh Dinas Kesehatan Provinsi NTB. Persepsi yang lemah terhadap kebijakan mutu menyebabkan pelatihan dan pembinaan tidak sepenuhnya terinternalisasi dalam praktik pelayanan. Sebagai solusi dikembangkan Provincial Quality Policy Internalization System (PQPIS–NTB), yaitu model pembinaan mutu pelayanan berbasis penguatan pelatihan, pembinaan, dan keterlibatan karyawan yang diarahkan untuk memperkuat persepsi tenaga kesehatan terhadap kebijakan peningkatan mutu. Model ini menempatkan Dinas Kesehatan Provinsi NTB sebagai pengendali utama kebijakan mutu dan pembinaan sumber daya manusia rumah sakit daerah. Implementasi awal PQPIS–NTB menunjukkan peningkatan pemahaman dan penerimaan tenaga kesehatan terhadap kebijakan mutu, meningkatnya keterlibatan karyawan, serta perbaikan kualitas pelayanan di RSUD Patut Patuh Patju Lombok Barat. Praktik ini menegaskan bahwa penguatan internalisasi kebijakan mutu pada level regulator merupakan kunci dalam meningkatkan kualitas pelayanan rumah sakit daerah.
