Abstract

Background: Type 2 Diabetes Mellitus (T2DM) is a global health challenge with a rising prevalence, particularly among the elderly. Its management requires a holistic approach, including medical nutrition therapy. Red dragon fruit (Hylocereus polyrhizus) is rich in fibre and antioxidants, which may help regulate glucose metabolism. This study aimed to evaluate the efficacy of red dragon fruit juice on reducing random blood glucose levels in elderly patients with T2DM. Methods: A quasi-experimental study with a pretest-posttest control group design was employed. Thirty-two elderly T2DM patients within the working area of Karo District General Hospital were selected via purposive sampling and allocated into an intervention group (n=16) and a control group (n=16). The intervention group consumed 250 ml of red dragon fruit juice once daily for seven consecutive days, while the control group received routine care only. Random blood glucose levels were measured using a glucometer before and after the intervention. Data were analysed using descriptive statistics and the Wilcoxon Signed Ranks Test. Results: At baseline, the mean random blood glucose was 231.88 ± 99.37 mg/dL in the intervention group and 250.31 ± 63.32 mg/dL in the control group. Post-intervention, the intervention group exhibited a significant decrease to 226.25 ± 96.99 mg/dL (p=0.017). Conversely, the control group showed a non-significant increase to 252.75 ± 63.66 mg/dL (p=0.027). Rank analysis indicated that 75% of subjects in the intervention group experienced a decrease in blood glucose, whereas 81.25% of subjects in the control group experienced an increase. Conclusion: The administration of 250 ml of red dragon fruit juice daily for seven days was effective as an adjuvant therapy in reducing random blood glucose levels in elderly patients with Type 2 Diabetes Mellitus. Red dragon fruit may be considered a safe and affordable complementary nutritional intervention for T2DM management in the elderly population.

Abstract

Hypertension is a non-communicable disease with high prevalence that constitutes a global health burden. Firefighters are a high-risk group due to heavy workloads, rotating shift systems, and unhealthy lifestyle behaviors. This study aimed to analyze the relationship between coffee consumption, poor sleep quality, and smoking habits with hypertension among firefighters at the Medan City Fire and Rescue Department. A quantitative cross-sectional design was used with 60 firefighters selected by total sampling. Data were collected using a coffee consumption questionnaire, Pittsburgh Sleep Quality Index (PSQI), Fagerström Test for Nicotine Dependence (FTND), and sphygmomanometer blood pressure measurements. Bivariate analysis used the Chi-Square test (α=0.05). Results showed 35.0% had hypertension and 40.0% were prehypertensive. No significant relationship was found between coffee consumption (p=0.881), poor sleep quality (p=0.257), and smoking habits (p=0.200) with hypertension. However, the prevalence of above-normal blood pressure reached 75%, far exceeding the national rate of 34.11%, indicating an urgent need for routine occupational health screening and health promotion programs.

Abstract

Hypertension is a non-communicable disease that remains a major public health problem due to its increasing prevalence. Data on patient visits in the working area of Namorambe Public Health Center, Deli Serdang Regency, showed an increase in hypertension cases from 7,643 visits in 2024 to 8,100 visits in 2025. This study aimed to analyze the risk factors associated with hypertension among individuals aged 35–65 years in the working area of Namorambe Public Health Center. This study employed a quantitative design with a cross-sectional approach. The sample consisted of 93 respondents selected using simple random sampling. Data were collected through questionnaires and blood pressure measurements and analyzed using the Chi-Square test and multiple logistic regression. The results showed that most respondents experienced hypertension (67.7%). The majority of respondents had obesity nutritional status (45.2%), family history of hypertension (61.3%), unhealthy lifestyles (62.4%), moderate stress levels (50.5%), low physical activity (51.6%), and poor sleep quality (73.1%). Bivariate analysis indicated that nutritional status, lifestyle, physical activity, and sleep quality were significantly associated with hypertension, while family history and stress level were not significantly related. Multivariate analysis revealed that sleep quality was the most dominant factor associated with hypertension (OR=4.365).

Abstract

Background: Hypertension is a major public health problem with increasing prevalence and is a primary risk factor for various cardiovascular diseases. Workers exposed to work stress and work fatigue are at higher risk. This study aimed to determine the relationship between work stress and work fatigue with the incidence of hypertension among employees at PT. Milano Kebun Sei Daun.  Method: This quantitative study used a cross-sectional design. The population comprised all 52 employees, and total sampling was used to include all as respondents. Data were collected using questionnaires, observation sheets, and blood pressure measurements. Univariate analysis described frequency distributions, and bivariate analysis used the chi-square test (α=0.05). Results: The study found that 14 respondents (26.9%) had hypertension. Among those with high work stress, 2 had hypertension; among those with high work fatigue, 3 had hypertension. Chi-square tests showed significant associations between work stress and hypertension (p=0.044) and between work fatigue and hypertension (p<0.001). Conclusion: There are significant relationships between work stress and work fatigue with the incidence of hypertension among employees at PT. Milano Kebun Sei Daun. Workplace interventions to reduce stress and fatigue are recommended.

Abstract

Background: Hypertension is a non-communicable disease with increasing prevalence, including among productive age adults. Coffee consumption habits and unhealthy lifestyles are suspected to increase the risk of hypertension. This study aimed to determine the relationship between coffee consumption and lifestyle with the incidence of hypertension among productive age adults in Tanjung Gusta Village, Medan City.  Method: This quantitative study used a cross-sectional design. The sample comprised 98 respondents selected using accidental sampling. Data were collected through structured questionnaires and blood pressure measurement using a sphygmomanometer. Data were analyzed using the chi-square test with a significance level of 0.05. Results: Among 98 respondents, 18 (18.4%) had hypertension. Coffee consumption was significantly associated with hypertension (p=0.033), and lifestyle was also significantly associated with hypertension (p=0.041). Respondents who frequently consumed coffee and had unhealthy lifestyles showed higher proportions of elevated blood pressure. Conclusion: There are significant relationships between coffee consumption and lifestyle with the incidence of hypertension among productive age adults in Tanjung Gusta Village, Medan City. Limiting coffee intake and adopting a healthy lifestyle are recommended for hypertension prevention.

Abstract

Background: Hypertension remains a major public health challenge, with approximately 90% of cases lacking a clearly identified cause. Psychological factors, particularly stress, are believed to trigger elevated blood pressure through sympathetic nervous system activation. The Health Belief Model (HBM) offers a framework to explain how individual perceptions of risk and benefit influence preventive health behavior. Objective: This study aimed to analyze the relationship between stress level and the incidence of hypertension based on the Health Belief Model approach, encompassing Perceived Susceptibility, perceived severity, perceived barriers, and self-efficacy. Methods: A quantitative cross-sectional design was employed involving 84 hypertensive patients at Klinik Dandy Dinda, Tembung, selected through Slovin’s formula from a population of 107 patients. Data were collected using the Perceived Stress Scale (PSS-10) and HBM-based questionnaires, then analyzed using univariate and bivariate (chi-square) statistical tests with a 95% confidence level. Results: Most respondents experienced a moderate stress level (57.1%) and stage 1 hypertension (45.2%). Chi-square analysis revealed significant associations between hypertension and stress level (p=0.010), Perceived Susceptibility (p=0.047), and perceived severity (p=0.046), whereas perceived barriers (p=0.149) and self-efficacy (p=0.108) showed no significant association. Conclusion: Stress level, Perceived Susceptibility, and perceived severity were significantly associated with hypertension incidence, while perceived barriers and self-efficacy were not. These findings highlight the importance of stress management and health perception strengthening in hypertension prevention programs.
