Abstract

Purpose: This research aims to analyze medical waste management practices and factors influencing medical waste management in Jember Regency hospitals. Methods: This study employed a qualitative research design with a case study approach. Research data was obtained from the results of in-depth interviews and observations. The determination of research informants was conducted using purposive sampling, involving 16 informants. Data analysis used causal loop diagram (CLD) analysis and the importance-performance analysis (IPA) method. Results: Medical waste management activities in Jember Lung Hospital and Jember Klinik Hospital were generally good. However, some activities could have been more suitable, including sorting, storage, in-situ transportation, and temporary storage. The two hospitals already had incinerators, but the incinerators were damaged due to a lack of maintenance. Therefore, the final medical waste management activities of the two hospitals were carried out with a licensed third party, PT. Sagraha, PT. Wastec International and PT. Universal Eco Pacific. The causal loop diagram (CLD) analysis reveals that the budget availability factor plays a crucial role in supporting effective medical waste management activities, particularly in terms of facility and infrastructure availability, human resource availability, and SOP implementation. Additionally, the quality of human resources and hospital accreditation has a positive influence on suitable medical waste management activities. The importance-performance analysis (IPA) method identified the main priority scale that requires improvement, specifically the hospital staff's discipline. Conclusion: Medical waste management activities in the Jember Lung Hospital and Jember Klinik Hospital, in general, have followed the RI Minister of Health Decree Number 1204/Menkes/SK/X/2004 and PerMenLHK No. 56 of 2015. However, segregation activities were still not optimal, and medical waste containers were not equipped with an infectious symbol. Therefore, efforts to increase periodic monitoring and evaluation of officer performance can improve officer discipline through SOP.

Abstract

Purpose: This study describes the utilization of healthcare facilities before and during the COVID-19 pandemic and the expenditure of national health insurance at primary and secondary healthcare facilities. Methods: This is a descriptive study with a cross-sectional design. We utilized claim data from the National Health Insurance from January 2018 to June 2021. The population included in this study were all Indonesian citizens who contracted COVID-19. Both the Ministry of Health and National Health Insurance expenditure for COVID-19 services were captured. The data were analyzed in frequency and proportions. Results: There was a consistent utilization trend across all types of healthcare facilities (primary vs. secondary; public vs. private), with an upward trend before the Covid-19 pandemic, a downward trend in 2020, and a slight increase before June 2021. Primary healthcare utilization jumped by 19% during the 2018-2019 period and dropped by 34% from March to April 2020. A similar trend was observed in the secondary healthcare facilities, with the utilization cost being lower in the 2020 and 2021 periods than in previous years. Hospital visits also showed a decreasing trend during the start of the pandemic (both public and private-owned), which was linear to the expenditure of inpatient and outpatient visits. The total claim for COVID-19 health care increased to IDR 77.8 trillion by August 2021 (inpatient and outpatient), with the highest cost per patient for inpatients with comorbidities. Conclusion: Healthcare utilization decreased significantly during the COVID-19 pandemic period. On the other hand, since COVID-19 increased expenditure, particularly due to inpatient services and comorbidities, National Health Insurance needs to prepare a health financing strategy to anticipate the shift of financing in time of the endemic.

Abstract

Purpose: Food handlers still need to routinely wash their hands with hand soap or wear aprons/masks, as there are dirty outdoor areas and many flies in canteens at Kendari port. Previous studies found that education level, knowledge, attitude, training, and availability of facilities affect the implementation of food sanitation hygiene. However, there has yet to be research on the combination of these factors in influencing the implementation of food sanitation hygiene, and research on the application of food sanitation hygiene in port areas in Indonesia is still rare. The study aimed to identify one factor that must be present for the implementation of food sanitation hygiene that meets the requirements and does not meet the requirements and identify a combination of factors that influence the implementation of food sanitation hygiene. Methods: The study design was cross-sectional, and 16 canteens and 16 food handlers participated using total sampling techniques. Education level, knowledge, attitude, training, and availability of facilities were analyzed using Qualitative Comparative Analysis (QCA). Results: No single factor was mandatory for implementing food sanitation hygiene that meets or does not meet the requirements. QCA identified three pathways for implementing eligible and not-eligible food sanitation hygiene. Conclusion: Frequent or regular training and adequate facilities are essential to implement food sanitation hygiene that meets the requirements. Meanwhile, poor attitudes, never or rarely-received training, and inadequate facilities were essential factors that led to implementing food sanitation hygiene, which was needed to meet the requirements.

Abstract

Purpose: To explore the nutritional parenting practices and their determinant factors among beneficiaries of the Family Hope Program (PKH) in Ruteng Sub-District. Methods: This study is qualitative research with a case study design using a snowball sampling technique. The primary informants of the study are parents of toddlers who are beneficiaries of the Family Hope Program. Data is collected through focus group discussions, in-depth interviews, observations, and document studies. Results: Incorrect timing of complementary feeding is still common, and lack of dietary variety and insufficient protein consumption are prevalent issues in the community. The behaviour of proper waste disposal remains very low; the attendance rate of parents at posyandu and receiving immunizations still needs to improve, at 71% or below the standard. Regarding the determinant factors of parenting practices, it is known that most mothers are over 35 years old, parents have a low educational level, and parents' occupations consume their time, large families, and husbands' limited involvement in child care, cultural and habitual factors are also known to influence the parenting practices. Conclusion: The nutritional parenting practices for toddlers among PKH beneficiaries in the Ruteng Sub-District still need to be improved, particularly in providing complementary feeding, feeding practices, hygiene and sanitation, and child healthcare maintenance. Several determinants influence these nutritional parenting practices, such as the mother's age, low educational level, parents' occupations, large family size, cultural factors, and low support from husbands in the child-rearing process.

Abstract

Background: Eczema, also known as atopic dermatitis (AD), is a chronic inflammatory skin condition that significantly impacts the quality of life for both children and adults, affecting up to 20% of children and 10% of adults globally. While topical corticosteroids (TCS) have been the primary treatment, their long-term use can lead to adverse side effects and corticophobia, underscoring the need for alternative therapies like microbial agents (MA) which have shown promise in improving eczema symptoms and reducing reliance on TCS.      Methods: This systematic review focused on full-text English literature published between 2014 and 2024 using the PRISMA 2020 guidelines. The literature was compiled using PubMed, ScienceDirect, and SagePub, among other online venues. Result: Five publications were found to be directly related to our ongoing systematic examination after a rigorous three-level screening approach. Conclusion: This systematic review suggests that microbial agents (MA) are a promising add-on therapy for moderate to severe eczema in children when used alongside topical corticosteroids (TCS). Mixed-strain probiotics and synbiotics showed the most significant improvement in eczema severity, but the optimal dosage and steroid-sparing effect require further investigation. Future research should explore MA's effectiveness across different age groups and severities of eczema.

Abstract

Abstract: The aim of this research is to understand natural phenomena regarding social problems related to education in Makkaliki Village. The phenomenon of dropping out of school and children's work practices has an impact on the low interest in going to school in Makkaliki Village, which results in the village being left behind. This research was conducted using a phenomenological study so that researchers understand and interpret phenomena through observation, in-depth interviews and documentation. Based on a phenomenological study, it was found that the problem of underdevelopment in Makkaliki village was caused by a lack of awareness of the importance of education for the younger generation. The wrong paradigm about education has had a negative impact on Makkaliki village. Therefore, through this article, role of Christian religious education in increasing educational awareness as an effort to modernize Makkaliki village. These roles is carried out by teaching an educational philosophy based on Christian teachings and motivating the younger generation to have an interest and determination to go to school. Abstrak: Tujuan dari penelitian ini adalah untuk memahami fenomena alamiah mengenai permasalahan sosial yang berkaitan dengan banyaknya remaja putus sekolah di Desa Makkaliki. Fenomena putus sekolah dan praktik kerja anak berdampak pada rendahnya minat bersekolah di Desa Makkaliki yang mengakibatkan desa tersebut tertinggal. Penelitian ini dilakukan dengan menggunakan studi fenomenologi dengan memahami dan menafsirkan fenomena melalui observasi, wawancara mendalam dan dokumentasi. Berdasarkan kajian fenomenologi ditemukan bahwa permasalahan keterbelakangan di Desa Makkaliki disebabkan oleh kurangnya kesadaran akan pentingnya pendidikan pada generasi muda. Paradigma yang salah mengenai pendidikan telah membawa dampak buruk bagi Desa Makkaliki. Oleh karena itu melalui artikel ini ditemukan fakta bahwa pendidikan agama Kristen berperan dalam meningkatkan kesadaran pendidikan sebagai upaya modernisasi desa Makkaliki. Peran tersebut dilakukan dengan mengajarkan filosofi pendidikan berdasarkan ajaran Kristiani dan memotivasi generasi muda agar mempunyai minat dan tekad untuk bersekolah.

Abstract

This study aims to improve Modules for science subject with the Computer-Based Test Assessment in order to train students’ critical thinking on one of science topics which is human’s respiratory system using ADDIE model. This model of research is the development research level 3. The data were collected by involving expert’s validation, observation, test, and questionnaire. The results showed that: the module validity result in terms of module itself was 93.75% with a very valid category. The validity in terms of language and material were 88.77% and 99.54% respectively, while the results by the Computer-Based Test application were 97%. The module’s practical results assessed by questionnaires were 89.41% and it means this module is highly practical to be used. The results of the module’s efficacy in terms of normality, the n-gain of all classes in pre-test and post-test’s sig value > 0.05, therefore the data was distributed normally. The results of paired sample t-test indicate that there is a noticeable difference on the average of pre-test and post-test results in the experimental group. Hence, the science module with Computer-Based Test is effective to be utilized as learning materials that can increase students’ critical thinking skills at schools.

Abstract

The purpose of this study was to determine the analysis of teacher and student needs for teaching modules based on regional potential, module validity, level of practicality, and effectiveness of the module in improving students' literacy skills. This study is a development research. Data were analyzed using qualitative and quantitative methods. Qualitative descriptive analysis was used to analyze data on needs, validity, and practicality of the module, while quantitative analysis was used to test the effectiveness of the module. The results of the needs analysis showed that teachers needed modules that were relevant to the local context, and students needed interesting and easy-to-understand learning media. Expert validation showed that the module had a validity index of 97.73% with a very valid category. The practicality level of the module reached 78% with a practical category. Effectiveness testing using the independent sample t-test produced a sig value. (2-tailed) of 0.000 (<0.05), which shows that the module is effective in improving students' literacy skills. The conclusion of this study is that the regional potential-based teaching module is valid, practical, and effective in improving students' literacy skills in biodiversity material.
