Abstract

Background: Optimal blood glucose control is the main key to preventing complications of type 2 Diabetes Mellitus (DM). However, many patients have difficulty maintaining glycemic control due to limited knowledge, ongoing education and lack of therapeutic interaction. The FEBI Education Model (Focused Interaction-Based Education) is designed to increase patient engagement through an interactive educational approach based on digital applications. Objective: This study was to test the effectiveness of the FEBI Education Model in helping control blood glucose in patients with type 2 DM. Method: The study used a quasi-experimental design with a one-group Pretest-Posttest design approach. With a sample of 30 patients with type 2 DM in the working area of Andalas Health Center and Ambacang Kuranji Health Center, with a purposive sampling technique and data analysis using Paired T-test samples. The FEBI model integrates self-education, glucose monitoring, medication reminders, and online consultative interaction features. The main parameters measured were changes in knowledge and attitudes and glycated albumin levels during one month of intervention. Results: The results of the study showed that there was an increase in knowledge and attitudes with a p-value of 0.00 and a decrease in glycated albumin levels with a -value of 0.025, statistically it can be concluded that there is an influence of the FEBI education model with an increase in knowledge and attitudes and a decrease in glycated albumin levels. Conclusion: The application-based FEBI Education Model is effective in improving blood glucose control in patients with type 2 DM. This interactive approach has the potential to be an innovative strategy in supporting technology-based diabetes management. Abstract

Background: Acute leukemia is a significant global health concern, with increasing prevalence worldwide and in Indonesia. Accurate diagnosis and classification of acute leukemia subtypes, primarily acute lymphoblastic leukemia (ALL) and acute myeloblastic leukemia (AML), are crucial for effective treatment. Immunophenotyping and bone marrow aspiration (BMA) are commonly used diagnostic methods, each with its strengths and limitations. This study aimed to analyze the concordance between immunophenotyping and BMA in diagnosing and classifying acute leukemia in children. Methods: A retrospective cross-sectional study was conducted on 46 children diagnosed with acute leukemia at Dr. M. Djamil General Hospital Padang from January 2022 to July 2023. Data were collected from medical records, including patient demographics, immunophenotyping results, and BMA findings. Concordance between the two diagnostic methods was analyzed using Fisher's exact test. Results: The study population consisted of 30 (65.2%) males and 16 (34.8%) females, with a median age of 4 years. Immunophenotyping identified 24 (52.2%) cases as ALL and 22 (47.8%) as AML. BMA classified 26 (56.5%) cases as ALL and 20 (43.5%) as AML. There was a high concordance between the two methods, with only 2 (4.3%) cases showing discordant results. These two cases were classified as AML by immunophenotyping but as ALL by BMA. Conclusion: Immunophenotyping and BMA demonstrate a high level of concordance in diagnosing and classifying acute leukemia in children. The few discordant cases highlight the importance of considering both methods, especially in challenging cases, to ensure accurate diagnosis and appropriate treatment.

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a growing global health threat, where chronic hyperglycemia and insulin resistance can affect coagulation profiles.Objective: This study aims to examine coagulation factor profiles and platelet counts in T2DM patients. Methods: A descriptive cross-sectional observational study using data from prothrombin time (PT), activated partial thromboplastin time (APTT), international normalized ratio (INR), and platelet counts of T2DM patients admitted to the Internal Medicine Ward of Dr. M. Djamil General Hospital, Padang, in October 2023. Data analysis was performed descriptively using SPSS v26 software. Results: Of the 39 patients analyzed, the distribution of characteristics showed a male predominance of 64.1%, with the largest age group being 45–59 years (48.7%). The most common complication associated with T2DM was chronic kidney disease, identified in 82.1% of the study population. Evaluation of hemostasis parameters showed that APTT was prolonged in 56.4% of patients. Conversely, PT (87.2%), INR (87.2%), and platelet count (66.7%) were within reference ranges. Conclusion: This study found most coagulation parameters normal, but a relatively high proportion had prolonged APTT, underscoring the need for individualized coagulation monitoring in T2DM, especially with renal complications

Abstract

Introduction. Non-communicable diseases as a cause of death in Indonesia are increasing. Hyperglycemia, hyperlipidemia, and obesity are some of the factors that contribute to an increased risk of non-communicable diseases. Education workers are a group of office workers who are at risk of obesity and non-communicable diseases because they have low physical activity. The study aimed to determine a correlation between body mass index with fasting blood glucose levels and total cholesterol levels in the health screening of educational staff faculty of medicine of Andalas University.  Methods. This research was an analytical study with a cross-sectional design using secondary data for the faculty of medicine of Andalas University health screening that was held on September 08, 2021. The tools used in the study were a calibrated glucometer to measure fasting blood glucose and a calibrated auto-check for total cholesterol examination. 37 data met the inclusion and exclusion criteria. Data were processed and analyzed using univariate and bivariate analysis using the Pearson correlation test.  Results. The result showed that the educational staff of the medical faculty of Andalas University are mostly in the late adult category, most are female and most of them are obese. The mean of body mass index is 25.3 kg/m2, the mean of fasting blood glucose level is 126.57 mg/dL and the mean of total cholesterol level is 205, 14 mg/dL. The bivariate analysis results showed no correlation between body mass index and fasting glucose levels (r = 0.316; p = 0.057) and no correlation between body mass index and total cholesterol levels (r = 0.091; p = 0.591). Conclusions. This study concludes that there is no correlation between body mass index with fasting blood glucose levels and total cholesterol levels in the health screening of educational staff faculty of medicine of Andalas University.

Abstract

Diabetic Foot Ulcers are one of the most common, serious, and costly complications of type 2 diabetes mellitus. Diabetic Foot Ulcers have become more common in tandem with the rising prevalence of type 2 diabetes mellitus. Changes in many hematological parameters, such as Prothrombine Time and Activated Partial Thromboplastine Time, are influenced by the mechanism of diabetic ulcer occurrence. Understanding the various aspects of hematological parameters will help in the prevention and treatment of diabetic foot ulcers. This study aims to determine the comparison in the hematological profile in type 2 DM patients with and without diabetic foot ulcers at Dr. M. Djamil Padang hospital in 2019.  This type of research is an observational analytic study with a retrospective design using medical record data of patients at Dr. M. Jamil Padang hospital from August 2020 to March 2021. The sample consisted of the medical records of patients with type 2 diabetes mellitus with foot ulcers and without foot ulcers, respectively, as many as 60 subjects. Bivariate analysis using independent t-test or Mann Whitney test.  The results showed that the mean PT level of type 2 DM patients with diabetic foot ulcers was 12.8417 ± 2.36939, without diabetic foot ulcers 11.997 ± 1.8325 (p <0.001). The mean aPTT of patients with diabetic foot ulcers was 41.2733 ± 35.1686, without diabetic foot ulcers 32.912 ± 13.1263 (p <0.001).  This study concludes that there are significant in between type 2 DM patient with foot ulcer and without foot ulcers.

Abstract

Latar Belakang: Coronavirus disease 2019 (COVID-19) adalah penyakit menular baru yang disebabkan SARS-CoV-2. Indeks Fibrosis-4 (FIB-4) saat ini digunakan sebagai prediktor awal menilai derajat berat dan perburukan COVID-19. Tujuan: Mengetahui perbedaan rerata indeks FIB-4 antara manifestasi klinis berat dan kritis pada pasien COVID-19. Metode: Penelitian analitik dengan rancangan potong lintang dilakukan terhadap 100 pasien COVID-19 dengan manifestasi klinis berat dan kritis di   RSUP Dr. M. Djamil Padang sejak Juli 2022 hingga Februari 2023. Indeks FIB-4  dihitung dengan rumus umur × SGOT dibagi jumlah trombosit × √SGPT. Data dianalisis dengan uji T tidak berpasangan, bermakna jika p <0,05. Hasil: Subjek penelitian sebagian besar laki-laki (62%). Rerata umur subjek penelitian 57 tahun, rentang 38-80 tahun. Rerata indeks FIB-4  seluruh subjek penelitian 4,24 (1,72-8,66). Rerata indeks FIB-4 dengan manifestasi klinis berat 2,83 dan manifestasi klinis kritis 5,65 bermakna (p=0,001). Pembahasan: Terdapat peningkatan indeks FIB-4 yang lebih tinggi pada pasien manifestasi klinis kritis dibandingkan dengan manifestasi klinis  berat, sehingga pasien dengan manifestasi klinis kritis sudah terjadi fibrosis hati. Keterbatasan pada penelitian ini adalah tidak dilakukannya fibroscan sebagai konfirmasi fibrosis hati. Simpulan:Terdapat peningkatan indeks FIB-4 yang lebih tinggi pada manifestasi klinis kritis dibandingkan dengan manifestasi klinis berat pada pasien COVID-19.

Abstract

The neutrophil lymphocyte ratio (NLR) and platelet lymphocyte ratio (PLR) tests are simple tests can be done before a COVID-19 patient hospitalized. This examination is useful for determining the diagnosis and prognostic of the patient. The aim of the study was to determine the correlation between NLR and PLR with the severity of COVID-19. This research was an analytical study with a cross-sectional approach using secondary data from 289 medical records of patients with confirmed COVID-19 for the period June-August 2021. The total sampling technique was used and processed with the chi-square test. Most of the research subjects were in the range of 26-45 years (28.4%). More than half of the subjects were female (61.6%). There were 99 patients (34.3%) with severe conditions. Less than half of the patients had abnormal leukocyte and platelet examination results, namely 64 (22.1%) and 56 (19.4%). Patients with abnormal NLR and PLR examinations were 135 (46.7%) and 123 (42.6%). There was a significant correlation between NLR and the severity of COVID-19 inpatients (p<0.001, OR=9.452). There is a significant correlation between PLR and the severity of COVID-19 inpatients (p<0.001, OR=7.268). This study concludes a significant correlation between NLR and PLR with the severity of COVID-19.

Abstract

Tujuan : Sirosis hepatis stadium dekompensata memiliki angka ketahanan hidup yang lebih rendah. Child-Pugh Score secara klinis banyak digunakan untuk menentukan derajat penyakit sirosis hepatis. Relevansi nilai mean platelet volume (MPV) dengan Child-Pugh Score menggambarkan kondisi inflamasi kronis pada sirosis hepatis. Tujuan penelitian ini untuk mengetahui korelasi nilai MPV dengan Child-Pugh Score pada pasien sirosis hepatis stadium dekompensata; Metode : Penelitian analitik dengan pendekatan retrospektif dilakukan terhadap 54 pasien sirosis hepatis stadium dekompensata dengan etiologi infeksi virus hepatitis B dan C di RSUP Dr. M. Djamil Padang sejak Mei hingga Agustus 2022. Nilai MPV dan Child-Pugh Score diperoleh dari observasi data rekam medis. Data dianalisis dengan uji korelasi Spearman, bermakna jika p<0,05; Hasil : Subjek penelitian sebagian besar laki-laki (75,9%) dengan rerata usia 53,3 (10,87) tahun. Rerata nilai MPV didapatkan 10,49 (0,13) fL. Median Child-Pugh Score adalah 11 (7–15). Uji korelasi Spearman menunjukkan terdapat korelasi positif lemah antara nilai MPV dengan Child-Pugh Score (r=0,29) dan bermakna secara statistik (p=0,03); Kesimpulan : Terdapat korelasi positif lemah antara nilai MPV dengan Child-Pugh Score pada pasien sirosis hepatis stadium dekompensata di RSUP Dr. M. Djamil Padang

Abstract

Tujuan : mengetahui hubungan indeks massa tubuh dengan kadar glukosa darah puasa dan tekanan darah pada tenaga kependidikan Fakultas Kedokteran Universitas Andalas. Metode : Jenis penelitian ini merupakan penelitian analitik dengan desain penelitian cross-sectional dengan subjek penelitian sebanyak 37 orang yang dilaksanakan di Fakultas Kedokteran Universitas Andalas pada tanggal 08 September 2021. Metode pengambilan sampel ini dengan total sampling, analisis data hubungan indeks massa tubuh dengan kadar glukosa darah puasa dan tekanan darah menggunakan uji korelasi Spearman. Hasil: Nilai median indeks massa tubuh sebesar 24,22 Kg/, nilai rerata 24,94 Kg/, nilai minimum sebesar 16,23 Kg/ dan nilai maksimum 40,15 Kg/. Nilai median kadar glukosa darah puasa 127 mg/dL, nilai rerata 143,65 mg/dL, nilai minimum sebesar 93 mg/dL dan nilai maksimum 399 mg/dL. Nilai median dari tekanan darah 126 mmHg,nilai rerata 128,7 mmHg,  nilai minimum 94 mmHg, dan nilai maksimum 213 mmHg. Tidak terdapat korelasi bermakna antara indeks massa tubuh dengan kadar glukosa darah puasa (p=0,203) ataupun pada indeks massa tubuh dengan tekanan darah (p= 0,097) pada subjek penelitian. Kesimpulan: tidak terdapat korelasi bermakna antara indeks massa tubuh dengan kadar glukosa darah puasa dan antara indeks massa tubuh dengan tekanan darah pada tenaga kependidikan pada Fakultas Kedokteran Universitas Andalas.

Abstract

 Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in the lungs causes alveolar cell inflammation and disruption, leading to increased levels of proinflammatory cytokines that stimulate platelet activation and consumption. In response, megakaryocytes will increase the production of large, immature platelets. Large platelets bind more to fibrinogen than small platelets, increasing the potential for thrombus formation. This study aims to analyze the platelet large cell ratio as a prothrombotic biomarker to predict the severity of thrombosis in patients. A prospective cohort study was conducted in May-November 2021 at M. Djamil Hospital involving 206 samples of confirmed coronavirus disease 2019 (COVID-19) patients. The examinations included platelet count, P-LCR, and D-dimer. Large platelet counts were calculated using the Platelet Large Cell Ratio (P-LCR) parameter with an automated hematology analyzer. Clinical manifestations of disease severity were monitored based on WHO criteria, grouped into non-severe and severe disease. The 3 results showed a mean age of 47.41 (SD = 17.82). Platelet count was 263,690 (116,995)/mm , P-LCR was 30.86 (6.63)%, and D-dimer value was 2,215.97 (2,590.86) ng/mL. The P-LCR in the severe group was 35.08 (8.21)%, and the non-severe group was 26.64 (6.81)%, with p <0.001. D-dimer in the severe group was 3,680.36 (3,006.23) ng/mL, and in the non-severe group, 869.12 (977.03) ng/mL, with p <0.001. The relative risk of a high P-LCR causing severe COVID-19 is 2.35 compared to a low P-LCR, with p <0.001. The relative risk of a high D-dimer value causing severe COVID-19 is 6.80 compared to a low D-dimer, with p <0.001. The conclusion is  that a greater increase in large platelet production occurs in severe COVID-19 disease. P-LCR is a crucial biomarker for evaluating platelet activity. A high P-LCR value is a risk factor for predicting the severity of COVID-19. Suggestions for the use of PLCR. Increased risk of thrombotic events in COVID-19 patients can be identified by P-LCR examination upon admission to the hospital, allowing for preventive treatment.
