Abstract

Postherpetic neuralgia (PHN) is a chronic neuropathic pain complication following herpes zoster infection. PHN often leads to persistent pain and facial muscle dysfunction, affecting patients' quality of life. This study aimed to evaluate the physiotherapy management using electrical stimulation and facial exercises in a case of facial PHN. A case report design was used to observe a 36-year-old male patient presenting with facial pain and muscle weakness after herpes zoster infection. The physiotherapy intervention, consisting of electrical stimulation and facial exercises, was conducted over three sessions within two weeks. Assessment tools included NPRS, MMT, and Ugo Fisch Scale. There was a significant reduction in pain from NPRS 5/10 to 0, and improvement in frontal muscle strength from MMT 0 to 3. The Ugo Fisch score increased from 62 to 66, indicating functional improvement of facial muscles, though not yet optimal. Electrical stimulation and facial exercises were effective in reducing pain and improving facial muscle function in a PHN patient. Further research with larger samples is needed to validate these findings.

Abstract

Introduction: Cerebral palsy (CP) is a non-progressive neuromotor disorder that causes increased muscle tone and impaired motor control, thus affecting functional ability and participation in adaptive sports such as Boccia. One of the main challenges in athletes with spastic cerebral palsy is the slow recovery of muscles due to spasms and fatigue. Cryotherapy is used in rehabilitation to lower spasticity, but scientific evidence on Boccia athletes is still limited. Objective:  This study was to analyze the effectiveness of cryotherapy on the physical recovery process in Boccia athletes with spastic cerebral palsy. Methods: Pre–post experimental research without a control group in 18 Boccia athletes aged 18–35 years. Local cryotherapy is administered to the extremities for four weeks with a frequency of three times per week. Measurements were made using the Modified Ashworth Scale (MAS), hand-held dynamometer, and Borg Rating of Perceived Exertion (RPE). Results and Discussion: Results showed a 26% decrease in muscle tone, a 14% increase in muscle strength, and a 15% decrease in fatigue perception after cryotherapy. Physiologically, this effect is related to decreased stretch reflex activity and increased neuromuscular efficiency due to local vasoconstriction and slowing down of nerve conduction. Conclusion: Cryotherapy effectively accelerates the physical recovery of Boccia athletes with spastic CP through improvements in muscle tone, strength, and fatigue.

Abstract

Abstract

Background: The main important goal in rehabilitation of chronic stroke patients is improvement walking speed. EMG biofeedback training is promising treatment for improving that outcome. It offers an exceptional approach (self-control training) that does not exist in conventional physiotherapy. Based on a continuous biofeedback of EMG signal recorded in selective muscles, with the goal of modifying it. Objectives: Efficiency EMG biofeedback training in combination conventional physiotherapy approaches and the implementation into clinical settings to improve walking speed in individuals‘ chronic stroke. Methods: We searched English-language of randomised controlled studies (RCT) and words related EMG biofeedback (electromyography, biofeedback, visual biofeedback and auditory biofeedback) and related walking speed (gait speed, walking velocity, and gait velocity) as well as chronic stroke between 1980-2019 using MEDLINE, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Cochrane Library, ScienceDirect, and Physiotherapy Evidence Database (PEDro), Grey Literature Report, The British Library. Results: Eleven studies including 110 participants met the inclusion criteria but only nine checked sufficient outcome data for gait speed. For this meta-analysis, the mean effect size was statistically significant (p = 0.03) with mean 2.15 (95% CI, 0.16 [0.01, 0.30]), Z test. However, the result did not exceed Minimal Clinically Important Difference (MICD) score of walking speed improvement in chronic stroke patients. Conclusions: Statistically, the result indicate that EMG biofeedback training is superior to conventional physiotherapy alone for enhancing walking speed in individuals with chronic stroke. However, the meta-analysis verified lack of clinical significance for implementing EMG biofeedback training in clinical settings. Applications/Originality/Value: This study contributes to solving nutrition problem in adolescent female athlete and as a basis for intervention in the problem of female athlete triad.
