Abstract

Schizophrenia is a syndrome characterized by decreased or inability to communicate, impaired reality, cognitive impairment, and difficulty performing daily activities. Hypertension is a persistent condition of systolic blood pressure ≥ 140 mmHg and/or diastolic ≥ 90 mmHg. According to Riskesdas 2018 there are 7.0 per 1,000 Indonesian population suffering from schizophrenia. Meanwhile, the prevalence of hypertension at the age of ≥ 18 years was 34.1%. This case report describes a 73-year-old woman with complaints of difficulty sleeping at night because she still hears whispering voices and accompanied by headaches. Mental status examination revealed perceptual disturbances in the form of auditory hallucinations and thought content disturbances in the form of suspicious delusions. The management provided is in the form of medical and non-medical therapy as a holistic intervention with a family medicine approach. This study is aim to identifying risk factors, clinical problems, and management with a patient-centered and family approach. This study is a case report. Primary data were obtained through anamnesis, physical examination and home visits. Secondary data was obtained from the patient's medical record. The assessment is based on a holistic diagnosis from the beginning, process, and end of the study qualitatively and quantitatively. The results of this study is patient was diagnosed with schizophrenia and hypertension. Limited knowledge of patients and families about the patient's disease is a factor that causes complaints to occur. In addition, dietary patterns in the family that are not suitable are also another factor that causes health problems suffered by patients. Holistic and comprehensive management of patient problems has been carried out by providing counseling to increase the knowledge of the patient's family.

Abstract

Carpal tunnel syndrome (CTS) is a syndrome that arises due to neuropathic pressure on the median nerve in the carpal tunnel at the wrist which is precisely under the flexor retinaculum. Risk factors for CTS include obesity, sedentary wrist activity, pregnancy, genetic heredity, and rheumatic inflammation. So that proper treatment is needed and carried out holistically with a family medicine approach to achieve therapeutic success. Application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient-centered and family problem-solving framework. Primary data were obtained through anamnesis, physical examination, supporting examinations and home visits to assess the physical environment. Patient Mrs. S, 49 years old, came with complaints that the thumb, index finger and middle finger of the right hand felt numb and stiff since one month ago. The patient is worried that the complaint will worsen and interfere with activities. The patient was clinically diagnosed with CTS (ICD-10 G56.0). Furthermore, management, intervention and evaluation are carried out. On evaluation, there were reduced complaints, increased patient knowledge about the disease and changes in behavior in the position of doing daily homework. After carrying out holistic and comprehensive management, patients experience an increase in knowledge about their disease. Complaints of numbness and stiffness in the patient's right hand are reduced, the patient and his family do homework in the right way and position.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) and Hypertension is a non-communicable disease which become a health problem globally and in Indonesia. COPD and Hypertension can be controlled through a healthy lifestyle which support from family and community is needed, therefore it is important to carry out holistic management through a family approach to patients. This study is a case report. Primary data were obtained through anamnesis from the patient's family, physical examination, and home visits to assess the physical environment. The assessment is based on a holistic diagnosis from the beginning, process, and end of the study. The results, Mr. E is 64 years old, with COPD and history of uncontrolled hypertension in the past 10 years. Internal risk factors include a history of smoking in the past 30 years and lack of knowledge about the disease. External risk factors are the lack of knowledge of the patient's family about the patient's disease, smoking behavior in the home by other family members and the patient's workplace with a lot of exposure to smoke. Interventions were carried out in the form of education. After the intervention, changes in patient knowledge, blood pressure, CAT Score and lifestyle were obtained. The diagnosis in this patient complies with the national guidelines. The management given is in accordance with the guidelines, there is a change in knowledge of the patient and the patient's family after the intervention is carried out by providing counseling based on evidence-based medicine which is patient centered and a family approach.

Abstract

Hypertensive heart disease (HHD) is a term applied to refer to heart disease as a whole, starting from left ventricular hypertrophy (LVH), cardiac arrhythmias, coronary heart disease, and chronic heart disease, which are caused by increased blood pressure, either directly or indirectly. The number of patients with Hypertensive Heart Disease (HHD) is not known with certainty, but in several studies it is stated that people with hypertension will develop heart disease. Often patients fail in treatment because of their inability to modify their lifestyle, so doctors must treat patients holistically from various aspects. Application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient-centered and family problem-solving framework. The study conducted was a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from patient medical records. Patient Mrs. S, 65 years old, came with complaints that discomfort when breathing and frequent palpitations. Known to suffer from hypertension since 18 years ago and a history of hospitalization due to a previous heart disease. The patient had been taking antihypertensive medication but stopped it because he felt he was fine. The patient is worried that the complaint will worsen and interfere with activities. The patient was clinically diagnosed with HHD. Furthermore, management, intervention and evaluation are carried out. The diagnosis and management of these patients has been carried out holistically, patient centered, family approach and community oriented based on several recent theories and research. In the process of behavior change, the patient has reached the trial stage.

Abstract

Water is a very important nutrient and is the most constituent component of the human body. Most of the water required by the body comes from drinking. Each person's fluid needs vary depending on physical activity, age, weight, ambient temperature and humidity. Fluid water is recommended over other types of beverages. Water needs about two liters or eight glasses a day for adolescents and adults who do light activities in the office and home under daily temperature conditions. Hydration knowledge can be a factor that affects the fulfillment of a person's water intake. Good hydration knowledge will make a person choose fluid consumption based on quality and quantity so that their fluid needs are met. Unfulfilled fluid intake can lead to dehydration. Dehydration is defined as a state of the body when fluid expenditure is greater than fluid intake. Dehydration can be a risk factor for overweight. Overweight is a condition when there is an accumulation of excess fat due to an imbalance between energy intake and energy used for a long time. Dehydration can cause a decrease in cognitive function and mood that causes weakness and laziness in physical activity; dehydration also causes fluid and electrolyte imbalances that increase appetite; and dehydration can increase glycerol and free fatty acids resulting in increased triglyceride formation. Adequate water intake can lead to weight loss by slowing gastric emptying and increasing metabolism.

Abstract

Asthma is one of the health problems that the world is fighting against, according to WHO in 2019 the global prevalence of asthma sufferers reached 262 million people with high mortality, causing 455,000 deaths. Apart from the high mortality rate, asthma also has a high morbidity rate. One of the reasons for the high morbidity rate in asthma is the high recurrence rate of asthma, which affects the productivity of asthma sufferers. In 2020, the asthma recurrence rate in adults was 40.7% and in children the asthma recurrence rate was divided into two groups based on age, namely in the under 18 age group it was 42.7% and in the under 5 year age group it was 52. 9%. There are many factors that influence the rate of asthma recurrence, namely allergic and non-allergic factors. One non-allergic factor that can trigger an asthma recurrence is an infection in the sinuses or sinusitis. The aim of this study was to determine the relationship between chronic rhinosinusitis and the rate of asthma recurrence in children. The method used was a review article by searching for articles using the keywords rhinosinusitis, asthma, asthma recurrence rate via Google Scholar and Pubmed, then the articles were reviewed and analyzed. The results of this review article are that there is a relationship between chronic rhinosinusitis and the rate of asthma recurrence in children, especially influencing the degree of asthma attacks in children.

Abstract

Diarrhea is the second cause of death in children under five years in developing countries. Diarrhea indicates an infection in the digestive tract. Applying the principles of family medicine and implementing holistic management based on the patient-centered, family-focused, and community-oriented approach grounded in Evidence Based Medicine. This study is a case report utilizing primary data obtained through anamnesis, physical examination, and home visits to get family, psychosocial, and enviromental data. Secondary data were obtained from the patient’s medical records. A 2-years-old female patient came with watery stools from 2 days ago. Patient didn’t care about self-hygiene with washing her hands. Patient also often buy snack near her house which is not healthy. The patient was diagnosed with acute diarrhea without dehydration. Holistic management was performed with a family physician-designed approach for A 2-years-old female patient, with acute diarrhea without dehydration based on the initial holistic diagnosis. The intervention has improved the patient’s knowledge and changed some behaviours of the patient and her family.

Abstract

Irritant contact dermatitis (ICD) is an inflammatory skin disease caused by irritant materials or substances and can be found in everyone from all age groups, races, and genders. The definitive management is to find and avoid the cause (irritants). Applying a holistic and comprehensive family doctor approach in detecting risk factors, clinical problems, and patient management based on evidence based medicine through family-approached, patient-centered, and community-oriented. This is a case report study, which the primary data were obtained through anamnesis, physical examination, home visits. The secondary data were obtained through patient’s medical records. Mrs. P, 52 years old, presents redness and itchy spots on both soles of the feet and peeled skin since two weeks ago. The patient is worried that the complaints may interfere her daily activities. The patient is a housewife who has just replaced the bathroom floor cleaning fluid. Patient was diagnosed with ICD based on history and physical examination. The intervention results were patient complaints’ were reduced, knowledge of patients and her family regarding ICD increased, and some behaviours of the patients and family changed.

Abstract

Scabies is a skin disease caused by the mite Sarcoptes scabei parasite, which causes itching that gets worse at night and can be transmitted through prolonged contact with sufferers. Factors that influence the prevalence of scabies are age, gender, poor hygiene, contact with sufferers, and also low knowledge about the disease. So that proper treatment is needed and carried out holistically with a family medicine approach to achieve therapeutic success. The purpose of writing this article is to apply of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient problem solving framework using a patient centered and family approach. This study is a case report. The data obtained are primary data obtained through autoanamnesis, alloanamnesis, physical examination, and home visits and secondary data obtained from the patient's medical record. Patient An. N, 15 years old, came with his mother with complaints of severe itching at night since a month ago, there were reddish nodules the size of pins appearing on the palms of his hands, then spreading to other areas. The patient is worried that it will get worse and interfere with activities. The patient was clinically diagnosed with scabies. Furthermore, management, intervention and evaluation are carried out. On evaluation, there were reduced complaints, patient knowledge about the disease increased, and changes in behavior in maintaining personal and environmental hygiene. After holistic and comprehensive management, patients experience an increase in knowledge about their disease. Complaints of itching and nodules in patients are reduced, patients and their families maintain personal and environmental hygiene.

Abstract

Ada sekitar 64 juta remaja berusia antara 10 hingga 24 tahun, atau 28,64% dari total populasi Indonesia. Pendidikan kesehatan reproduksi bagi remaja merupakan upaya penting untuk menghindari pergaulan bebas, penyebaran penyakit menular seksual dan kehamilan yang tidak diinginkan. Tujuan dari pengabdian masyarakat ini adalah untuk memberikan penyuluhan tentang kesehatan reproduksi kepada remaja guna mewujudkan remaja yang sehat dan berprestasi. Metode pelaksanaan promosi kesehatan yang digunakan adalah Participatory Action Research (PAR) dengan pelatihan tentang masalah kesehatan reproduksi dan pencegahan penyakit menular seksual yang diikuti oleh siswa-siswi Madrasah Aliyah (MA) Dinniyah Putri, Kabupaten Pesawaran, Lampung. Hasil pengabdian ini menunjukkan terdapat perbedaan yang signifikan antara nilai rerata pretest (63,96) dan nilai posttest (92,2) tentang kesehatan reproduksi pada remaja di MA Dinniyah Putri, Pesawaran, Lampung. Kegiatan pengabdian ini terbukti dapat meningkatkan pengetahuan dan pemahaman remaja tentang kesehatan reproduksi sehingga dapat menjadi salah satu upaya dalam mencegah terjadinya penyakit.
