Abstract

Introduction: Glucose homeostasis greatly affects CKD patients, who are at high risk for hyperglycemia and hypoglycemia. The risk of hypoglycemia is increased due to several factors, namely renal gluconeogenesis failure, impaired renal insulin breakdown, imperfect insulin degradation due to uremia, increased erythrocyte glucose uptake during hemodialysis, impaired counterregulatory hormone responses, nutritional deficiencies, and variability in exposure to antihyperglycemic agents and the effects of exogenous insulin. This study aims to identify the effect of hypoglycemia management in patients with chronic kidney disease who experience decreased blood glucose. Method: This study uses a case report method with a nursing care approach with interventions for unstable blood glucose levels, namely hypoglycemia management to overcome hypoglycemia. Result: Hypoglycemia management is evaluated by the results of the GDS examination and increased meal portions. After the intervention, there was an improvement in the GDS value on the second day of treatment, the increase in meal portions increased on the third day of treatment and signs of hypoglycemia were gone. Conclusion: In this case study indicate that hypoglycemia management intervention in the form of collaboration of 40% dextrose fluid administration in CKD patients experiencing hypoglycemia is an effective option to prevent further complications due to hypoglycemia.

Abstract

ABSTRACT Occupational therapy for stroke patients is an important phenomenon in rehabilitation, which emphasizes the restoration of the patient's functional and social abilities through a structured and evidence-based therapeutic approach. Occupational therapy for stroke patients aims to restore their abilities in daily activities and improve their quality of life through holistic therapeutic interventions. The aim of this research is to map the results of literature findings regarding the effectiveness of occupational therapy interventions for stroke patients in improving independence in daily activities and improving their overall quality of life. The search databases for this study were PubMed, ScienceDirect, and Google Scholar. Article identification was carried out based on the criteria of year of publication (2014-2024), Randomized Control Trials (RCTs), Multicenter, Prospective, Qualitative study, and Longitodinal study, English and Indonesian, and full text. Using the PRISMA Flow Chart diagram, data extraction was carried out by including keywords (in English): "Post-Stroke OR Stroke AND Occupational Therapy AND Combined" and keywords in Indonesian: "Post-Stroke OR Stroke AND Occupational Therapy AND Combined"". The study results found that of the 8 articles that met the criteria, the main findings showed that the use of occupational therapy with other interventions could provide significant benefits in increasing patient independence and quality of life. This study found that occupational therapy with various types of additional interventions, such as co-careldopa treatment, physical therapy, Prism adaptation training, and Mirror Therapy, can improve the independence and quality of life of post-stroke patients. The importance of client-centred activities in daily living (CADL) based approaches and interventions such as Task-Based Mirror Therapy (TBMT) are also highlighted as effective methods in motor rehabilitation. This study concludes that occupational therapy in combination with various other interventions shows the potential to improve post-stroke rehabilitation outcomes, including independence in daily activities and motor function. The importance of integrating a client-centric approach and therapies such as Mirror Therapy emphasizes the need for a holistic approach in the care of stroke patients. Keywords: Pasca Stroke, Occupational Therapy, CombinedABSTRAK Occupational therapy pada pasien stroke merupakan fenomena penting dalam rehabilitasi, yang menekankan pemulihan kemampuan fungsional dan sosial pasien melalui pendekatan terapeutik yang terstruktur dan berbasis bukti. Occupational therapy pada pasien stroke bertujuan memulihkan kemampuan mereka dalam aktivitas sehari-hari serta meningkatkan kualitas hidup mereka melalui intervensi terapeutik yang holistik. Tujuan dari penelitian ini adalah untuk memetakan hasil temuan literature tentang efektivitas intervensi occupational therapy terhadap pasien stroke dalam memperbaiki kemandirian dalam aktivitas sehari-hari dan meningkatkan kualitas hidup mereka secara keseluruhan. Database pencarian untuk studi ini adalah PubMed, ScienceDirect, dan Google Scholar. Identifikasi artikel dilakukan berdasarkan kriteria tahun publikasi (2014-2024), Randomized Control Trials (RCTs), Multy center, Prospective, Qualitative study, and Longitodinal study, bahasa Inggris dan bahasa Indonesia, serta full text. Menggunakan diagram PRISMA Flow Chart, ekstraksi data dilakukan dengan mencakup kata kunci (dalam bahasa Inggris): "Pasca Stroke OR Stroke AND Therapy Occupation AND Combined" dan kata kunci dalam bahasa Indonesia: " Pasca Stroke OR Stroke AND Terapi Pekerjaan AND Gabungannya"". Hasil studi menemukan bahwa dari 8 artikel yang memenuhi kriteria, temuan utamanya menunjukkan bahwa penggunaan terapi okupasi dengan intervensi lain dapat memberikan manfaat signifikan dalam meningkatkan kemandirian dan kualitas hidup pasien. Studi ini menemukan bahwa terapi okupasi dengan berbagai jenis intervensi tambahan, seperti co-careldopa treatment, Physical Therapy, Prism adaptation training, dan Mirror Therapy, dapat meningkatkan kemandirian dan kualitas hidup pasien pasca stroke. Pentingnya pendekatan berbasis client-centred activities in daily living (CADL) dan intervensi seperti Task-Based Mirror Therapy (TBMT) juga disorot sebagai metode yang efektif dalam rehabilitasi motorik. Studi ini menyimpulkan bahwa terapi okupasi dalam kombinasi dengan berbagai intervensi lainnya menunjukkan potensi untuk meningkatkan hasil rehabilitasi pasca stroke, termasuk kemandirian dalam aktivitas sehari-hari dan fungsi motorik. Abstract

Stroke incidence increases with age, and the combination of a growing population and aging demographics is expected to lead to a significant rise in global mortality and disability in the future. Stroke survivors experience substantial health, emotional, social, and economic impacts, which are felt not only by individuals but also by families and communities. This study aimed to identify the relationship between depression level and stroke severity with family support in post-stroke patients at Sumedang Regency Hospital. This study used quantitative methods with a cross-sectional design. A total of 123 post-stroke patients participated in this study. Data were collected using the Beck Depression Inventory-II (BDI-II) questionnaire, National Institutes of Health Stroke Scale (NIHSS), and Perceived Social Support-Family (PSS-Fa), and analyzed using the Chi-Square test. Statistical tests showed a significant relationship between stroke severity and family support in post-stroke patients with a p value of 0.017 (p value <0.05), as well as between depression level and family support in post-stroke patients with a p value of 0.001 (p value <0.05). There is a significant relationship between stroke severity and depression level with family support in post-stroke patients. To improve health outcomes,   it is important for healthcare providers to develop and implement strategies that strengthen family involvement in post-stroke patients care.

Abstract

Background: Breast cancer is the most common cancer in women, with a global incidence of 2.3 million cases and the fourth highest mortality rate. In Indonesia, 66,271 new cases were recorded in 2022, while West Java Province saw an increase from 594 cases in 2019 to 973 referrals in 2023. Chemotherapy, while effective in destroying cancer cells systemically, often reduces patients' quality of life due to physical, psychological, and social side effects. Supportive care can alleviate this burden, but providing and meeting patient needs in facilities remains suboptimal. Purpose: To identify supportive care needs in breast cancer patients undergoing chemotherapy. Method: This quantitative descriptive study was conducted on patients undergoing chemotherapy in January 2025 at Al-Ihsan Regional General Hospital, Bandung. Sampling was conducted consecutively to select 105 breast cancer patients. The instrument used was the Supportive Care Needs-Short Form 34 (SCNS-SF34), and univariate analysis was performed. Results: Most respondents (52.3%) required supportive care. The highest level of need was in the health systems and information domain (58%), followed by the physical and daily activities domain (56.1%), the psychological domain (53.3%), and the support and care domain (52.3%). Most respondents (61%) did not require supportive care in the sexuality domain. Conclusion: Breast cancer patients undergoing chemotherapy at Al-Ihsan Regional Hospital, Bandung, require supportive interventions, particularly in the form of health information, physical and psychological support, with nurses playing a key role in improving quality of life. Suggestion: Expand outpatient consultations and utilize print and digital media for therapy information, implement self-education at home, provide structured psychological counseling, develop a protocol for interventions related to sexual side effects, and train healthcare workers in empathy and sensitive communication.   Keywords: Breast Cancer; Chemotherapy; Supportive Care.   Pendahuluan: Kanker payudara menjadi kanker paling umum pada wanita dengan insiden global mencapai 2.3 juta kasus dan menempati peringkat ke-4 kematian tertinggi. Di Indonesia, tercatat 66.271 kasus baru pada 2022, sementara Provinsi Jawa Barat mencatat peningkatan dari 594 kasus pada 2019 menjadi 973 rujukan pada 2023. Kemoterapi, meski efektif secara sistemik dalam menghancurkan sel kanker, sering menurunkan kualitas hidup pasien karena efek samping fisik, psikologis, dan sosial. Perawatan suportif dapat meringankan beban ini, tetapi identifikasi dan pemenuhan kebutuhan pasien di fasilitas masih belum optimal. Tujuan: Untuk mengidentifikasi kebutuhan perawatan suportif pada pasien kanker payudara yang menjalani kemoterapi. Metode: Penelitian deskriptif kuantitatif yang dilakukan kepada pasien yang menjalani kemoterapi pada bulan Januari 2025 di RSUD Al-Ihsan Bandung. Pengumpulan sampel menggunakan teknik consecutive sampling dan mendapatkan 105 pasien kanker payudara untuk menjadi responden. Instrumen yang digunakan adalah Supportive Care Needs-Short Form 34 (SCNS-SF34) dan dianalisis menggunakan univariat. Hasil: Sebagian besar responden (52.3%) membutuhkan perawatan suportif. Tingkat kebutuhan tertinggi berada pada domain sistem dan informasi kesehatan (58%), dilanjut domain fisik dan aktivitas sehari-hari (56.1%), domain psikologis (53.3%), dan domain dukungan dan perawatan (52.3%). Sebagian besar responden (61%) tidak membutuhkan perawatan suportif pada domain seksualitas. Simpulan: Pasien kanker payudara yang menjalani kemoterapi memerlukan intervensi suportif, terutama informasi kesehatan, dukungan fisik dan psikologis dengan perawat sebagai kunci peningkatan kualitas hidup. Saran: Perluas konsultasi rawat jalan dan dapat memanfaatkan media cetak/digital untuk informasi terapi. Selain itu, dapat dilakukan edukasi mandiri di rumah, adanya ketersediaan konseling psikologis terstruktur, mengembangkan protokol intervensi efek samping seksual, serta melatih empati dan komunikasi sensitif bagi tenaga kesehatan.   Kata Kunci: Kanker Payudara; Kemoterapi; Perawatan Suportif.

Abstract

Readmission is a condition where a patient needs to be re-hospitalized shortly after their initial discharge. One crucial component in preventing readmission is the quality of discharge education. Objective: This study aims to describe the quality of discharge education among patients with chronic diseases Method : A quantitative descriptive study design was used. A total of 121 in patients with chronic disease from a regional general hospital in West Java were selected through consecutive sampling. Demographic and characteristic questionnaires and the Quality of Discharge Teaching Scale (QDTS) were used as instruments. It consisting of 6 paired questions with a validity value 0.98 and reliability 0.92. Descriptive statistics were applied for data analysis. Results: Chronic patients' score of content needed and content received were high (46.7 ± 11.9). This means that patients have high information needs and the information they received from nurses answer their needs. However, there are some points where it is considered that the needed information has not been fulfilled by the information received. Conclusions: Even though the quality of the provided discharge education is high, the improvement particularly in terms of information related to who and when should be contacted and information about emotion are needed.

Abstract

Families often act as informal caregivers for family members who suffer from chronic diseases. Intensive family involvement improved patients’ adherence in performing post-discharge treatment program, hospital readmission, clinical outcomes, and quality of life. To effectively help the patients managing their diseases, the family caregivers required not only understand but also must be ready for helping patients manage their conditions. This study aims to identify the readiness for hospital discharge among family caregivers of patients with chronic diseases. Data were collected from 121 family caregivers recruited consecutively from adult inpatients wards using the respondents’ characteristics questionnaire and Readiness for Hospital Discharge Scale for Family with Adult Patients (RHDS Fam) questionnaire (inter-item correlation = 0.90, and Cronbach alpha 0.89). Descriptive analysis was applied. The results showed that most of family were female (77.7%%), unemployed (69.4%), graduated from senior high school (57.9%), and had no chronic diseases (86.8%). Generally, family caregivers reported high readiness for hospital discharge (7.75 Out of 10). While the coping abilities identified as the readiness domain with highest average score (9.07±1.15), the personal domain identified as the lowest domain (7.07 ±0.68). Higher readiness was showed in the age group of 20-40 years, female, graduated from senior high school, employed, and are the siblings of the patients. It can be concluded that the family caregivers of chronic disease patients are ready to return home and help patients continue their post-discharge treatment program. However, interventions are needed to improve their readiness related to patient’s personal status, particularly those who are male, lower educational background, unemployed, has a relationship as parents of the patients, and has a chronic illness.

Abstract

Gastroparesis is a complication experienced by 20% - 50% of chronic diabetes mellitus patients with poor glycemic control. Hyperglycemia will slow down gastric emptying thereby reducing oral intake and mismatch of nutrient absorption which causes impaired nutritional status. Objective: to determine the nutritional status and symptoms of diabetic gastroparesis in type 2 diabetes mellitus patients with uncontrolled blood glucose levels. Method: Quantitative descriptive study on type 2 DM patients at RSAU Dr. M. Salamun. The sampling technique uses consecutive sampling with a sample size of 82 subjects. Data were obtained through gastrointestinal symptom score (GSS) questionnaires, height and weight checks, and patient medical records. Furthermore, the data results will be processed using univariate analysis. The questionnaire is considered reliable with the results of the reliability test of the crombach alpha value >0.50. Result: 71.9% of respondents experienced symptoms of diabetic gastroparesis with normal nutritional status (37.1%) and obesity (37.1%). The majority were female, ≥60 years old, and 1-5 years of DM. Conclusion: Health education and intervention in modifying lifestyle better so that it can control blood glycemia and make a diet that is suitable for patients with gastroparesis so that it can minimise the symptoms of gastroparesis experienced.

Abstract

Thalassemia major necessitates routine treatment that affects the physical, psychological, and social capacities of both the sufferers and their parents. In these circumstances, the way parents perceive their child's illness significantly influences their quality of life, making this a crucial aspect to consider.Objective: This study aims to explore the relationship between illness perception and quality of life in parents of adolescents with thalassemia major at RSUD dr. Slamet Garut. Method: The study employed a cross-sectional approach, including 64 parents of adolescents with thalassemia major (aged 10-18 years) selected through consecutive sampling. The Brief Illness Perception Questionnaire (BIPQ) was used to measure illness perception, and the World Health Organization Quality of Life Brief Version (WHOQoL-BREF) was used to assess quality of life. Both instruments are validated and reliable, with BIPQ having reliability coefficients of r=0.40-0.78 (α=0.74) and WHOQoL-BREF having r=0.39-0.79 (α=0.94). The Spearman correlation test was utilized for bivariate analysis. Results: The findings indicated that 55 respondents (85.9%) had a positive illness perception of thalassemia, and 61 respondents (95.3%) reported a high quality of life. The correlation analysis revealed a significant relationship between illness perception and the quality of life of parents, with a correlation coefficient of 0.548 (p<0.05). Conclusions: This concludes that there is a significant relationship between illness perception and quality of life in parents of adolescents with thalassemia major at RSUD dr. Slamet Garut.

Abstract

Anxiety in burn patients is a serious problem that can worsen the patient's condition and slow the healing process. Effective management of anxiety can improve the patient's quality of life and support their recovery. Although various non-pharmacological interventions have been proposed, there is still uncertainty regarding the effectiveness and application of each technique in the context of burn care. Objective: To explore various non-pharmacological interventions used to reduce anxiety in burn patients. The method used is scoping review. Articles were selected from four major databases: CINAHL, PubMed, and Scopus, using keywords namely “burn patients”, “anxiety”, “intervention”, and “burn injury”. Inclusion criteria included studies involving adult patients with burns, original research with RCT or quasi-experimental design, using experimental or quasi-experimental design, and articles published in the last 10 years (2014-2023). Data were extracted using manual tables and analyzed descriptively qualitatively. Based on initial research, we found 453 articles. After selected by authors, Nine articles met the inclusion criteria. The interventions discussed included aromatherapy, Benson relaxation technique, music therapy, Swedish and Shiatsu massage, and transcranial direct current stimulation (tDCS). Findings showed that aromatherapy and Benson relaxation technique were effective in reducing anxiety and improving sleep quality, music therapy reduced anxiety during and after wound care, Swedish and Shiatsu massage increased relaxation, while tDCS significantly reduced pain and anxiety. This study recommends the integration of these techniques into nursing practice to improve the quality of care and patient satisfaction. Further studies are needed to confirm these findings and explore the mechanisms of action of each intervention in more depth.

Abstract

Vaksinasi merupakan salah satu upaya preventif pemerintah Indonesia untuk mengurangi transmisi COVID 19. Berbagai penelitian menunjukkan bahwa terdapat orang tua yang tidak yakin menerima vaksinasi COVID 19 untuk anak serta rendahnya penerimaan vaksinasi COVID 19 anak dibandingkan orang dewasa. Tujuan penelitian ini adalah untuk mengetahui gambaran sikap orang tua terhadap penerimaan vaksinasi COVID 19 anak usia 6-11 tahun. Rancangan penelitian menggunakan metode penelitian deskriptif kuantitatif. Instrumen penelitian menggunakan kuesioner Parents Attitudes about Childhood Vaccine (PACV). Populasi penelitian adalah orang tua yang memiliki anak usia 6-11 tahun di Kecamatan Telukjambe Timur, Kabupaten Karawang dengan jumlah sampel 384. Data diperoleh melalui pengisian kuesioner kemudian ditampilkan dalam distribusi frekuensi dan presentase. Hasil penelitian ini yaitu 62,2% orang tua tidak ragu-ragu dan 37,8% orang tua ragu-ragu menerima vaksinasi COVID 19 untuk anak. Sebagian besar orang tua mengkhawatirkan efek samping (61,7%) dan keamanan vaksinasi (62,2%), serta hampir setengahnya mengkhawatirkan efektivitas vaksinasi (47,1%). Hal tersebut dapat mempengaruhi sikap orang tua terhadap terhadap penerimaan vaksinasi COVID 19. Dapat disimpulkan bahwa sikap orang tua dalam penerimaan vaksinasi COVID 19 cukup baik, namun masih diikuti dengan beberapa kekhawatiran lain. Maka dari itu, sosialisasi dan pemberian informasi dari tenaga kesehatan sangat diperlukan sehingga herd immunity dapat tercapai
