Mapping cognitive function screening instruments for patients with heart failure: A scoping review Arseda, Astuti; Pahria, Tuti; Kurniawan, Titis

Abstract

Background: Patients with heart failure (HF) often experience cognitive impairment, which negatively affects their quality of life. An effective screening tool is essential for nurses and healthcare professionals to assess cognitive function as part of HF management. Although many instruments exist, none are specifically designed for patients with HF. Objective: This study aimed to map the instruments for screening cognitive function in patients with HF. Design: A scoping review. Data Sources: Articles published between 2019 and 2023 were searched in PubMed, ScienceDirect, and Google Scholar, with the last search conducted on 27 January 2024. Review Methods: The review followed the scoping review framework by Arksey and O’Malley and adhered to PRISMA guidelines for scoping reviews. Results: Of the 21 articles meeting inclusion criteria, six cognitive function screening instruments were used across various cognitive domains, effectively identifying cognitive impairment in both inpatient and outpatient HF settings. The Montreal Cognitive Assessment (MoCA) was the most frequently used tool, covering a broad range of cognitive domains. MoCA showed high efficacy with a kappa coefficient of 0.82, Cronbach’s alpha reliability of 0.75, sensitivity of 90%, and specificity of 87%. Conclusion: Instruments like MoCA, Mini-Cog, and TICS-m show promise for assessing cognitive function in patients with HF, each with specific strengths and limitations. MoCA is notable for its comprehensive coverage despite being time-consuming and having language barriers. Further research is needed to revalidate and improve the existing instruments. It is crucial for nurses and healthcare professionals to integrate these tools into regular patient management, highlighting the need for continued research in their application.

Abstract

Background: Impaired cognitive function is associated with poor outcomes, frequent hospitalization, and high mortality in HF. Purpose: To determine the prevalence of cognitive function in HF and what factors contribute. Method: This study was an observational analysis with cross-sectional. Study subjects: HF patients at the Cardiac Polyclinic of Dr. M. Haulussy Hospital, Ambon, Indonesia from August to September 2023, with Inclusion criteria: aged 40-65 years; able to communicate well; not having a stroke or other neurological disorders (Parkinson's disease, Alzeimer's disease, Multiple Sclerosis) or other cognitive function disorders (such as due to injury or substance abuse); can read and write. Exclusion criteria: worsening of physical condition such as loss of consciousness and worsening of symptoms such as severe shortness of breath, inability to communicate, inability to complete the questionnaire, and worsening of symptoms that do not improve after being rested when data collection is carried out. Results: HF patients were male (56.2%), had ≥ 12 years of education (72.4%), were not actively working (56.9%), suffered from HF with NYHA class II functional status (49.1%), had Coronary Artery Disease (CAD) comorbid only (49.2%), had experienced hospitalization (64.6%), and were obese (51.54%). Respondents had an average age of 57.08 ± 6.78 years, a duration of HF of 3.96 ± 4.35 years, an average body mass index of 25.65 ± 4.5 Kg/m2, normal blood pressure with an average systole of 122.73 ± 17.21 mmHg, and an average diastole of 77.44 ± 10.11 mmHg. HF patients who experienced impaired cognitive function were 87.69% with mild cognitive impairment 73.1%, moderate cognitive impairment 13.1%, and severe cognitive impairment 1.5%. The maximum score of MoCA sub-domains of executive function was 46.2%; visuospatial 35.4%; attention 37.7%, naming and language 20%; abstraction 33.8%, delayed memory 3.8% and orientation 86.9%. There was a significant correlation between cognitive function and age (ρ = 0.000; r = -0.324), education level (ρ = 0.000; r = 0.327), and New York Heart Association functional status (ρ = 0.021; r = -202). Conclusion: There is a high prevalence of impaired cognitive function in chronic HF patients accompanied by a global decline in cognitive function subdomains.  Factors that may contribute to HF cognitive function include age, education level, not actively working, New York Heart Association functional status, comorbidities, and obesity.

Abstract

This study aims to identify factors that influence the increase in Interdialytic Weight Gain (IDWG) in hemodialysis patients. This research method uses a narrative literature review method by entering keywords into five databases: PubMed, CINAHL, Science Direct, Wiley, and ProQuest. The results showed that the search results obtained 521 articles and ten relevant and qualified articles, then extracted using the DSVIA method. The intrinsic and extrinsic factors that contribute to the increase in Interdialytic Weight Gain (IDWG) in hemodialysis patients. Intrinsic factors include age, gender, knowledge, body mass index, psychological condition, thirst, xerostomia, and self-efficacy. In contrast, external factors (extrinsic) that influence the increase in IDWG are the duration and frequency of hemodialysis, and serum dialysate, including serum sodium and serum creatinine. In conclusion, the rise in Interdialytic Weight Gain (IDWG) can severely impact the patient's condition, so it is necessary to identify the factors that contribute to the increase in interdialytic weight gain plan appropriate nursing actions. Abstract

Anxiety in burn patients is a serious problem that can worsen the patient's condition and slow the healing process. Effective management of anxiety can improve the patient's quality of life and support their recovery. Although various non-pharmacological interventions have been proposed, there is still uncertainty regarding the effectiveness and application of each technique in the context of burn care. Objective: To explore various non-pharmacological interventions used to reduce anxiety in burn patients. The method used is scoping review. Articles were selected from four major databases: CINAHL, PubMed, and Scopus, using keywords namely “burn patients”, “anxiety”, “intervention”, and “burn injury”. Inclusion criteria included studies involving adult patients with burns, original research with RCT or quasi-experimental design, using experimental or quasi-experimental design, and articles published in the last 10 years (2014-2023). Data were extracted using manual tables and analyzed descriptively qualitatively. Based on initial research, we found 453 articles. After selected by authors, Nine articles met the inclusion criteria. The interventions discussed included aromatherapy, Benson relaxation technique, music therapy, Swedish and Shiatsu massage, and transcranial direct current stimulation (tDCS). Findings showed that aromatherapy and Benson relaxation technique were effective in reducing anxiety and improving sleep quality, music therapy reduced anxiety during and after wound care, Swedish and Shiatsu massage increased relaxation, while tDCS significantly reduced pain and anxiety. This study recommends the integration of these techniques into nursing practice to improve the quality of care and patient satisfaction. Further studies are needed to confirm these findings and explore the mechanisms of action of each intervention in more depth.

Abstract

Selulitis merupakan infeksi bakteri yang mengenai dermis dan jaringan subkutan yang biasanya muncul dibagian yang mengalami kerusakan kulit. Selulitis akan cepat menyebar dan menyebabkan berbagai komplikasi jika tidak ditangani dengan segera. Maka dari itu, pasien dengan selulitis harus mendapat penanganan yang tepat untuk mencegah penyebaran infeksi. Perawatan luka pada selulitis menjadi intervensi yang dapat dilakukan untuk mengendalikan infeksi bakteri serta mengembalikan fungsi kulit. Respon inflamasi akibat infeksi menyebabkan nyeri yang jika dibiarkan berkepanjangan akan menunda penyembuhan dan mengurangi kualitas hidup pasien sehingga manajemen nyeri sangat diperlukan untuk menunjang kesembuhan pasien. Tujuan: Untuk mendeskripsikan penerapan pengetahuan dan praktik keperawatan pada pasien selulitis dengan masalah keperawatan gangguan integritas kulit dan nyeri akut. Metode: Laporan kasus ini menggunakan single case design melalui pemberian asuhan keperawatan. Manajemen nyeri dievaluasi dengan NRS dan perawatan luka selulitis dievaluasi menggunakan instrumen Bates-Jensen Wound Assessment Tool (BWAT). Hasil: Setelah diberikan intervensi ditemukan jika pasien mengalami penurunan nyeri NRS 4 (0-10)  menjadi NRS 2 (0-10) dan penurunan skor BWAT dari 50 ke 49. Kesimpulan: Hasil studi kasus ini menunjukan bahwa relaksasi nafas dalam efektif dalam menurunkan nyeri dan pemberian perawatan luka infeksi dengan NaCl 0,9% + gentamicin dapat membantu penyembuhan luka.

Abstract

The level of dependence of children on smartphones increased during the Covid-19 pandemic to 47.5%. The average smartphone use time for school-age children is 6.85 hours a day. Meanwhile, the ideal screen time for school-age children is no more than 2 hours/day. Smartphone addiction is a growing problem in children with adverse physical, psychological, and social health conse-quences. The purpose of the study was to see the levels of smartphone addiction in school-age children after the Covid-19 pandemic in rural area of Indonesia. This study used a cross-sectional descriptive design. The research subjects were school-aged children at "X" Elementary School (N=114), total sampling. Data collection was carried out using the Indonesian version of the Smartphone Addiction Scale (SAS) instrument which is valid and reliable. Data were analyzed using descriptive statistics. The smartphone addiction levels were 13.2% for low addiction, 77.2% for moderate addiction, and 9.6% for severe addiction. Item 4 SAS, which contains a statement of feeling happy when using a smartphone gets the highest score. The positive anticipation dimension is the most dominant with the highest score. The level of smartphone addiction after the Covid-19 pandemic among school-age children in this study was dominated by moderate smartphone addiction. The role of parents, teachers, health workers, and the government is an important key to reducing smartphone addiction incidents in school-age children in the future.

Abstract

 Lung cancer is one of the leading causes of cancer-related deaths worldwide. In advanced stages, patients often experience pain and shortness of breath, which significantly reduce their quality of life. Non-pharmacological interventions such as foot massage with lavender essential oil and pursed lip breathing (PLB) exercises offer potential supportive therapy, but evidence on their effectiveness remains limited. This case study aimed to describe the application of foot massage and PLB techniques in reducing pain and dyspnea in a 64-year-old male diagnosed with stage IVA squamous cell lung carcinoma. The intervention was conducted over six consecutive days. Foot massage was performed using five standard techniques—effleurage, petrissage, friction, tapotement, and vibration—while PLB was practiced for 10 minutes prior to each session. Pain intensity and oxygen saturation were measured using the Numeric Rating Scale (NRS) and pulse oximetry, respectively. The results showed a decrease in pain score from 7 to 3 and an improvement in oxygen saturation from 92% to 96%. The patient also reported improved comfort and the ability to perform light activities with assistance. These findings suggest that the combination of foot massage and PLB may be effective in managing pain and respiratory symptoms in advanced lung cancer. Further research with larger sample sizes and controlled study designs is recommended to confirm these outcomes and support broader clinical use.Keywords: Foot Massage Therapy, Lavender Oil , Lung Cancer, Pain, Pursed Lip Breathing, Shortness of Breath

Abstract

Introduction: Glucose homeostasis greatly affects CKD patients, who are at high risk for hyperglycemia and hypoglycemia. The risk of hypoglycemia is increased due to several factors, namely renal gluconeogenesis failure, impaired renal insulin breakdown, imperfect insulin degradation due to uremia, increased erythrocyte glucose uptake during hemodialysis, impaired counterregulatory hormone responses, nutritional deficiencies, and variability in exposure to antihyperglycemic agents and the effects of exogenous insulin. This study aims to identify the effect of hypoglycemia management in patients with chronic kidney disease who experience decreased blood glucose. Method: This study uses a case report method with a nursing care approach with interventions for unstable blood glucose levels, namely hypoglycemia management to overcome hypoglycemia. Result: Hypoglycemia management is evaluated by the results of the GDS examination and increased meal portions. After the intervention, there was an improvement in the GDS value on the second day of treatment, the increase in meal portions increased on the third day of treatment and signs of hypoglycemia were gone. Conclusion: In this case study indicate that hypoglycemia management intervention in the form of collaboration of 40% dextrose fluid administration in CKD patients experiencing hypoglycemia is an effective option to prevent further complications due to hypoglycemia.

Abstract

ABSTRACT Occupational therapy for stroke patients is an important phenomenon in rehabilitation, which emphasizes the restoration of the patient's functional and social abilities through a structured and evidence-based therapeutic approach. Occupational therapy for stroke patients aims to restore their abilities in daily activities and improve their quality of life through holistic therapeutic interventions. The aim of this research is to map the results of literature findings regarding the effectiveness of occupational therapy interventions for stroke patients in improving independence in daily activities and improving their overall quality of life. The search databases for this study were PubMed, ScienceDirect, and Google Scholar. Article identification was carried out based on the criteria of year of publication (2014-2024), Randomized Control Trials (RCTs), Multicenter, Prospective, Qualitative study, and Longitodinal study, English and Indonesian, and full text. Using the PRISMA Flow Chart diagram, data extraction was carried out by including keywords (in English): "Post-Stroke OR Stroke AND Occupational Therapy AND Combined" and keywords in Indonesian: "Post-Stroke OR Stroke AND Occupational Therapy AND Combined"". The study results found that of the 8 articles that met the criteria, the main findings showed that the use of occupational therapy with other interventions could provide significant benefits in increasing patient independence and quality of life. This study found that occupational therapy with various types of additional interventions, such as co-careldopa treatment, physical therapy, Prism adaptation training, and Mirror Therapy, can improve the independence and quality of life of post-stroke patients. The importance of client-centred activities in daily living (CADL) based approaches and interventions such as Task-Based Mirror Therapy (TBMT) are also highlighted as effective methods in motor rehabilitation. This study concludes that occupational therapy in combination with various other interventions shows the potential to improve post-stroke rehabilitation outcomes, including independence in daily activities and motor function. The importance of integrating a client-centric approach and therapies such as Mirror Therapy emphasizes the need for a holistic approach in the care of stroke patients. Keywords: Pasca Stroke, Occupational Therapy, CombinedABSTRAK Occupational therapy pada pasien stroke merupakan fenomena penting dalam rehabilitasi, yang menekankan pemulihan kemampuan fungsional dan sosial pasien melalui pendekatan terapeutik yang terstruktur dan berbasis bukti. Identifikasi artikel dilakukan berdasarkan kriteria tahun publikasi (2014-2024), Randomized Control Trials (RCTs), Multy center, Prospective, Qualitative study, and Longitodinal study, bahasa Inggris dan bahasa Indonesia, serta full text. Menggunakan diagram PRISMA Flow Chart, ekstraksi data dilakukan dengan mencakup kata kunci (dalam bahasa Inggris): "Pasca Stroke OR Stroke AND Therapy Occupation AND Combined" dan kata kunci dalam bahasa Indonesia: " Pasca Stroke OR Stroke AND Terapi Pekerjaan AND Gabungannya"". Hasil studi menemukan bahwa dari 8 artikel yang memenuhi kriteria, temuan utamanya menunjukkan bahwa penggunaan terapi okupasi dengan intervensi lain dapat memberikan manfaat signifikan dalam meningkatkan kemandirian dan kualitas hidup pasien. Studi ini menemukan bahwa terapi okupasi dengan berbagai jenis intervensi tambahan, seperti co-careldopa treatment, Physical Therapy, Prism adaptation training, dan Mirror Therapy, dapat meningkatkan kemandirian dan kualitas hidup pasien pasca stroke. Pentingnya pendekatan berbasis client-centred activities in daily living (CADL) dan intervensi seperti Task-Based Mirror Therapy (TBMT) juga disorot sebagai metode yang efektif dalam rehabilitasi motorik. Abstract

Stroke incidence increases with age, and the combination of a growing population and aging demographics is expected to lead to a significant rise in global mortality and disability in the future. Stroke survivors experience substantial health, emotional, social, and economic impacts, which are felt not only by individuals but also by families and communities. This study aimed to identify the relationship between depression level and stroke severity with family support in post-stroke patients at Sumedang Regency Hospital. This study used quantitative methods with a cross-sectional design. A total of 123 post-stroke patients participated in this study. Data were collected using the Beck Depression Inventory-II (BDI-II) questionnaire, National Institutes of Health Stroke Scale (NIHSS), and Perceived Social Support-Family (PSS-Fa), and analyzed using the Chi-Square test. Statistical tests showed a significant relationship between stroke severity and family support in post-stroke patients with a p value of 0.017 (p value <0.05), as well as between depression level and family support in post-stroke patients with a p value of 0.001 (p value <0.05). There is a significant relationship between stroke severity and depression level with family support in post-stroke patients. To improve health outcomes,   it is important for healthcare providers to develop and implement strategies that strengthen family involvement in post-stroke patients care.
