Abstract

The goals of decision making process about Do Not Resuscitate orders (DNR) are supporting the autonomy of patient or family and preventing futile medical interventions. Families can experience challenges and difficulties when making decision about DNR. The exploration of the decision-making process involving family is therefore crucial. This study aimed to explore in-depth the decision making of DNR among End of Life (EoL) patients within inpatient area from the perspective of family caregiver. A qualitative descriptive phenomenology study was conducted that recruited 6 family caregivers, within purposive sampling. Data collection used in-depth interviews with face to face and video calls, and data analysis followed the Colaizzi’s method. Twelve major themes emerged from interview finding of family caregiver: decision making made through family counselling, inadequate information communication, consideration of patient health status, family’s beliefs and values, family accompaniment, time allocation, knowledge, trust in health workers, financial consideration, information provision, and needs of second opinions. The findings of this study highlighted providing communication and sharing information were ineffective and inadequate. Several barriers factors were also found related to culture and religious beliefs and values, lack of knowledge and formal training, time constrains, caring experience, and financial consideration. Conclusion thus suggest need to inprove their ability to provide adequate information, to sharing and to perform an effective communication. Assessing appropriate and providing standards of operational procedure is essential to be done in order to improve communication styles and facilitate a shared decision making, partculary in DNR decision making for EoL patients.

Abstract

ABSTRACT The Covid-19 pandemic in Indonesia has led to restrictions on activities outside the home, the closure of public facilities, and the application of online learning which creates challenges for students to stay physically active. This study aimed to describe the impact of Covid-19 pandemic on physical activity in students of the Faculty of Nursing, Universitas Padjadjaran (Fkep Unpad) The purpose of this study was to describe the impact of the Covid-19 pandemic on physical activity in students of the Faculty of Nursing, Universitas Padjadjaran. This research is a quantitative descriptive study on 216 active students of Fkep Unpad class 2018 and 2019 who are taking online learning. Physical activity data were collected using the online International Physical Activity Questionnaire version of the Short Form (IPAQ-SF) which had been tested for validity and reliability. Physical activity was analyzed by looking at the distribution of physical activity before and during the Covid-19 pandemic on heavy, moderate, and walking physical activity. The category of physical activity was calculated by calculating the total Metabolic equivalents (METs) per week. The results showed that before the Covid-19 pandemic, the total weekly energy expenditure of students was 1844 METs-minutes/week with 78 students (36.1%) in the high category, 117 students (54.2%) in the medium category, and 21 students (9.7%) in the low category. Meanwhile, during the Covid-19 pandemic, students' total weekly energy expenditure was 1185 METs-minutes/week with 49 students (22.7%) in the high category, 101 students (46.8%) in the medium category, and 66 students (30.6%) ) in the low category There was a decrease in physical activity among Fkep Unpad students during the Covid-19 pandemic. However, during the Covid-19 pandemic, some samples managed to maintain a level of physical activity in the moderate category which is recommended during the Covid-19 pandemic. Students must do at least 30 minutes of physical activity every day. Keywords: Covid-19 Pandemic, Physical activity, University Students  ABSTRAK Pandemi Covid-19 di Indonesia menyebabkan pembatasan aktivitas di luar rumah, penutupan fasilitas umum, serta penerapan pembelajaran secara daring yang menyebabkan tantangan bagi mahasiswa untuk tetap beraktivitas fisik. Penelitian ini bertujuan untuk mengetahui gambaran dampak pandemi Covid-19 terhadap aktivitas fisik pada mahasiswa Fakultas Keperawatan Universitas Padjadjaran (Fkep Unpad). Tujuan dari penelitian ini adalah untuk menggambarkan dampak pandemi Covid-19 terhadap aktivitas fisik pada mahasiswa Fakultas Keperawatan Universitas Padjadjaran. Penelitian ini merupakan penelitian deskriptif kuantitatif pada 216 mahasiswa aktif Fkep Unpad angkatan 2018 dan 2019 yang sedang mengikuti pembelajaran daring. Data aktivitas fisik diambil menggunakan International Physical Activity Questionnaire versi Short Form (IPAQ-SF) secara online yang telah teruji validitas dan reliabilitasnya. Aktivitas fisik dianalisis dengan melihat sebaran aktivitas fisik sebelum dan selama pandemi Covid-19 pada aktivitas fisik berat, sedang, dan berjalan. Pengkategorian aktivitas fisik dilakukan dengan menghitung total Metabolic equivalents (METs) per minggu. Hasil penelitian menunjukkan sebelum pandemi Covid-19 total pengeluaran energi mingguan mahasiswa sebesar 1844 METs-menit/minggu dengan 78 mahasiswa (36,1%) berkategori tinggi, 117 mahasiswa (54,2%) berkategori sedang, dan 21 mahasiswa (9,7%) berkategori rendah. Sedangkan, selama pandemi Covid-19 total pengeluaran energi mingguan mahasiswa sebesar 1185 METs-menit/minggu dengan 49 mahasiswa (22,7%) berkategori tinggi, 101 mahasiswa (46,8%) berkategori sedang, dan 66 mahasiswa (30,6%) berkategori rendah. Terjadi penurunan aktivitas fisik pada mahasiswa Fkep Unpad selama pandemi Covid-19. Meskipun begitu, selama pandemi Covid-19 sebagian sampel berhasil mempertahankan tingkat aktivitas fisik pada kategori sedang yang dianjurkan pada masa pandemi Covid-19. Mahasiswa harus melakukan aktivitas fisik setidaknya 30 menit setiap hari.. Kata Kunci: Aktivitas Fisik, Mahasiswa, Pandemi Covid -19

Abstract

Space occupying lesion merupakan desakan ruang yang diakibatkan peningkatan volume di dalam ruang intrakranial. Desakan ruang di intrakranial dapat mengakibatkan jaringan otak mengalami nekrosis sehingga dapat menyebabkan gangguan neurologik progresif. Pasien SOL dengan HIV dan Toxoplasmosis Cerebri menunjukkan hampir 80-90% ditemukan memiliki kelainan neurologik. Tujuan : menganalisis karakteristik pasien dan menganalisis status neurologi. Metode : penelitian dekriptif dengan pendekatan studi kasus observasi. Tehnik pengambilan sampel yang digunakan adalah consecutive sampling. Alat pengumpulan data menggunakan lembaran ceklis yang merupakan kriteria status neurologis berdasarkan Nanda, lembar observasi dan MMSE. Hasil : Durasi mulai sakit yang di alami kedua pasien lebih dari 3 bulan dengan lama hari perawatan lebih dari 7 hari. Kedua pasien mempunyai riwayat penyakit penyerta yang sama dan baru mendapatkan terapi Atiretroviral setelah dirawat di rumah sakit. Gangguan status neurologis yang paling dominan tampak pada pasien Space Occupying Lesion dengan HIV dan Toxoplasmosis Cerebri diantaranya : Keluhan sakit kepala, gangguan kognitif dan gangguan berbicara serta kelemahan otot. Saran : Monitoring status neurologi secara komprehensif merupakan bagian penting terutama pada pasien Space Occupying Lesion dengan HIV dan Toxoplasmosis Cerebri, agar pelayanan yang diberikan akan lebih optimal dan berkualitas. Sehingga dapat mengurangi angka morbiditas dan mortalitas yang diakibatkan oleh gangguan fungsi neurologi. Case Study : Neurology Status of Patients Space Occupying Lesion with HIV and Cerebri Toxoplasmosis. Space occupying lesion is the insistence of space caused by an increase in volume in the intracranial space. Pressure in the intracranial space can brain tissue to experience necrosis so that it can cause progressive neurologic disorders. SOL patients with HIV and Cerebri Toxoplasmosis show nearly 80-90% are found to have neurological abnormalities. Aims : To analyze patient characteristics and analyze neurological status. Method : Descriptive research with an observational case study approach. The sampling technique used was consecutive sampling. Data collection tools use checklist sheets which are neurological status criteria based on Nanda, observation sheets and MMSE. Results : Duration of pain started in both patients was more than 3 months with a length of treatment more than 7 days. Both patients had a history of the same comorbidities and had only received Atiretroviral therapy after being hospitalized. The most dominant neurological status disorders seen in Space Occupying Lesion patients with HIV and Cerebri Toxoplasmosis include: Complaints of headaches, cognitive disorders and speech disorders and muscle weakness. Suggestion: Monitoring Neurology Status comprehensively is an important part especially in the patient's occupying lesion with HIV, so that the service provided will be more optimal and quality. Thus, it can reduce the morbidity and mortality rates caused by impaired neurological functions.

Abstract

Myasthenia gravis (MG) merupakan penyakit autoimun kronis yang dimediasi oleh antibodi terhadap acetylcholin receptor (AChR)  pada membran postsynaptic dari tautan otot saraf. Hilangnya situs AchR mengakibatkan kelemahan pada otot rangka yang berhubungan dengan pernafasan serta pergerakan ekstrimitas. Sebanyak 15 % – 20 % pasien dengan MG setidaknya mengalami satu kali myasthenic crisis. Myasthenic crisis  merupakan keadaan darurat medis yang terjadi akibat kelemahan otot-otot pernafasan sehingga pasien mengalami penurunan status pernafasan. Tujuan : untuk mengetahui gambaran karakteristik dan menganalisis status pernafasan pasien MG. Metode : penelitian dekriptif dengan pendekatan observasi studi kasus. Teknik pengambilan sampel menggunakan  consecutive sampling. Pengumpulan data dan pengkajian menggunakan form pengkajian asuhan keperawatan RSHS dan lembar observasi status pernafasan nursing intervention clasification. Hasil : karakteristik  pasien dalam studi ini adalah pasien MG dengan riwayat gagal nafas,  jenis kelamin perempuan, dengan klasifikasi klinis MG IIb dan IIIb. Hasil Kedua pasien mengalami keluhan kesulitan bernafass namun saat diobservasi pasien kedua mengalami dua kali gagal nafas karena melakukan aktivitas seperti berbicara lama, mengedan, dan tertawa berlebih yang mengakibatkan kelemahan pada otot-otot pernafasan sehingga terjadi peningkatan frekuensi pernafasan dan penurunan saturasi oksigen. Simpulan : edukasi yang tepat mengenai aktivitas serta observasi status pernafasan secara berkala dibutuhkan pasien MG agar dapat mengontrol dan mencegah terjadinya gagal nafas yang dapat menyebabkan kematian. Case Study: Respiratory Status of Patients Myasthenia Gravis at Azalea Room Hasan Sadikin Bandung Hospital. Myasthenia gravis (MG) is a chronic autoimmune disease that is mediated by antibodies to the acetylcholine receptor (AChR) in the post-synapses membrane of the neural muscle tissues.  Loss of the AchR site results in weakness in skeletal muscle associated with breathing and limb movements. A total of 15%-20% of patients with MG have suffered a one-time crisis. The Myasthenic crisis is a medical emergency that occurs due to the weakness of the respiratory muscles so that the patient experiences decrease in respiratory status. Objective: to determine the characteristics and analyze the respiratory status of MG patients. Method: Descriptive research with an observation approach to case studies. The sampling technique uses consecutive sampling. Data collection and assessment used the RSHS nursing care assessment form and an observation sheet about the classification status of nursing interventions. Results: The characteristics of the patients in this study were MG patients with a history of respiratory failure, female sex, with clinical classification of MG IIb and IIIb. Both patients had a history of respiratory failure but when observed the second patient experienced two symptoms of respiratory failure due to activities such as prolonged talking, straining, and excessive laughter which resulted in weakness in the respiratory muscles which resulted in an increase in respiratory frequency and decreased oxygen saturation.  Conclusion: proper education about the activity and observation of respiratory status regularly is needed by MG patients to be able to control and prevent respiratory failure which can cause death.

Abstract

Dekubitus merupakan cedera lokal pada kulit atau jaringan biasanya disebabkan oleh tekanan pada tonjolan tulang. Biaya pengobatan dekubitus tergolong mahal sehingga dekubitus sebaiknya dapat dicegah pada pasien-pasien yang menjalani perawatan yang lama di Rumah Sakit. Maka akan menjadi penting untuk mencari literature dalam pencegahan dekubitus, untuk studi literature ini akan mencari artikel tentang pengaruh penggunaan virgin coconut oil (VCO) terhadap pencegahan dekubitus. Metode pencarian dengan electronic database seperti proquest, ebsco, google scholar, pubmed, science direct, CINAHL dan perpustakaan nasional yang dipublikasikan full text pada rentang tahun 2011-2022 yang menggunakan metode penelitian randomize controlled trial dan quasi experiment dan ditulis dalam bahasa indonesia dan ingris. Setelah itu artikel dipilih dengan cara critical appraisal untuk menentukan artikel yang relevan dengan tujuan penelitian. Desain dalam penelitian ini menggunakan systematic reviews dan pedoman (PRISMA). Hasil dari analisis 11 artikel yang didapat dan memenuhi kriteria. Ada 8 artikel menyimpulkan bahwa melakukan massage dengan VCO efektif dalam pencegahan dekubitus maupun meminimalisir terjadinya infeksi dan dapat menurunkan derajat luka dekubitus, hanya ada satu artikel yang menunjukkan hasil berbeda, satu artikel membahas tentang cara mendeteksi eritema blanchable yang tepat untuk mencegah luka dekubitus dan satu artikel lagi menyimpulkan bahwa reposisi rutin pada pasien dapat mengurangi risiko terkena luka dekubitus. Dapat disimpulkan dari hasil systematic review bahwa penggunaan VCO efektif untuk pencegahan dekubitus maupun meminimalisir terjadinya infeksi dan menurunkan derajat luka dekubitus. Sehingga disarankan agar tindakan reposisi dan ditambahkan dengan terapi topikal VCO dapat diaplikasikan untuk mencegah terjadinya dekubitus.

Abstract

Objective: To validate the Indonesian version of the urinary stent related health (UriSteRH) questionnaire. Material & Methods: The Indonesian version of the UriSteRH was developed with multistep process by urologist, two nursing lecturers and two independent translators. A total 13 patients with indwelling DJ Stent completed the Indonesian UriSteRH questionnaires on the 30th day after Stenting. The reliability of the Indonesian version was evaluated using Cronbach’s alpha test. Domain structures were examined by using Pearson correlation coefficient (R). Results: Urinary pain, body pain, physical activity, social life, mental health, sexual activity and quality of life showed good internal consistency. There were strong correlations of physical activity (r=0.851) and social life (r=0.815) with the quality of life (r=0.736). In addition, there were moderate correlation of mental health general (r=0.676) related to urinary pain (r=0.612) and body pain (r=0.567). However, sexual activity had a lower correlation (r=0.456). Conclusion: The Indonesian version of UriSteRH is a reliable and valid instrument to evaluate health and quality of life in patients with indwelling DJ Stent. Abstract

This research aims to examine further how families are involved in family-based self-management education for hypertension sufferers. The method used is a scoping review with PCC (Population, Context & Concept) components. Population is "hypertension", Context is "self-management" Concept is "family-based oriented". This scoping review is aimed at answering the question "what and what forms of family-based self-management education for hypertension sufferers, compliance with treatment plans and providing care support for patients." The results of this research obtained 369 articles through searches using several databases consisting of PubMed, Science Direct and CINAHL, resulting in 5 articles analyzed in the research. The conclusion of this research is that mapping self-management interventions for hypertensive patients involving families plays an important role in improving self-management in hypertensive sufferers. Abstract

This study aims to identify the negative impact of using the Double J Stent (DJ stent) in patients with urinary tract stones. The method used is a Scoping Review using the Arksey & O'Malley (2005) framework through the Science Direct, Pubmed, and Scopus databases in the last five years (2017-2022). The research results showed ten articles identified the negative impact of DJ stent installation after ureteroscopy (URS), including pain, Lower Urinary Tract Symptoms (LUTS), urinary tract infections, sexual dysfunction, and decreased physical activity. In conclusion, the negative impact of DJ stent installation causes morbidity and reduces quality of life. Keywords: Ureteral stones, Impact, DJ Stent, Ureteroscopy

Abstract

This study aims to map the influence of mobile applications as self-management support in management of diabetic feet. This research uses a scoping review method by entering keywords into the CINAHL, Pubmed, and Scopus databases. The research results show that mobile applications are effectived in increasing knowledge, self-care behavior, self-efficacy, compliance with self-care foot care, eating behavior, reducing wound/ulcer recurrence rates, and cost-effectiveness. The effectiveness of mobile applications can be used as a strategy in providing interventions to support self-management in the management of diabetic feet. The use of mobile applications has an impact on improving independent foot care management as well as increasing knowledge of diabetic foot care so that it has benefits in controlling glycemia, increasing adherence to diet, and increasing physical exercise. In conclusion, Mobile application-based based programs had the potential to improve self-management in diabetic foot patients. Keywords: Diabetic Foot, Mobile Applications, Self-Management

Abstract

This research aims to determine the level of culture shock in nursing students in the professional nurse program at STIKes Dharma Husada Bandung. The research method used is a quantitative descriptive method. The research showed that 60 nursing students in the STIKes Dharma Husada Bandung professional nurse program experienced moderate culture shock (100%). In conclusion, the level of culture shock experienced by nursing students in the professional nurse program at STIKes Dharma Husada Bandung is in the medium category.
