Abstract

ABSTRAK Stunting menjadi hal yang krusial di dunia terutama di Indonesia. Di Indonesia, pernikahan dini pada remaja menjadi salah satu hal yang berpengaruh dalam terbentuknya generasi baru yang mengalami stunting. Dampak stunting dapat berimbas pada penurunan sumber daya manusia, sehingga tujuan penelitian ini adalah untuk mengetahui bagaimana kondisi masyarakat dalam pencegahan stunting di Desa Bojongloa, Kecamatan Rancaekek. Pengabdian masyarakat ini difokuskan pada kegiatan sosialisasi melalui kampanye melalui seminar dengan mengangkat judul “Start From Now! Muda, Sehat, dan Keren”, dan untuk kegiatan seminar yang dilaksanakan di SMP PGRI Rancaekek diikuti oleh sebanyak 105 siswa-siswi kelas 9 dan 11 guru, kepala sekolah, serta wakil kepala sekolah. Selanjutnya, untuk kegiatan seminar yang kami laksanakan memberikan wawasan baru terhadap siswa-siswi SMP PGRI mengenai stunting itu sendiri, peningkatan orientasi Pendidikan lanjut pada jenjang yang lebih tinggi SMA atau SMK, serta pencegahan pernikahan dini dan seks bebas masa remaja. Evaluasi kegiatan diberikan pre test dan post test terkait stunting, gaya hidup sehat dan pencegahan sex bebas. Hasil menunjukan bahwa dari 105 siswa yang mengikuti sosialisasi memperlihatkan pengetahuan sebelum dan sesudah kegiatan sosialisasi meningkat menjadi lebih baik terutama aspek bahaya seks bebas dan ciri-ciri stunting. Pengetahuan bahaya seks bebas akan mencegah pernikahan dini. Desa Bojongloa sendiri salah satu desa yang masih terdapat kasus stunting. Sosialisasi mengenani penikahan diri dan seks bebas pada remaja perlu dilakukan agar mencegah terbentuknya generasi yang mengalami stunting. Dari terlaksananya kegiatan tersebut diharapkan terdapat tindak lanjut baik dari pemerintah, sekolah, dan para orangtua untuk bisa bersinergi dan berkolaborasi dalam mencegah terjadinya stunting di Desa Bojongloa. Kata kunci: Seks aman, Peer group, Remaja, Stunting.    ABSTRACT Stunting is crucial in the world, especially in Indonesia. In Indonesia, early marriage among teenagers is one of the things that influences the formation of a new generation that experiences stunting. The impact of stunting can result in a decrease in human resources, so this research aims to find out the condition of the community in preventing stunting in Bojongloa Village, Rancaekek District. This community service was focused on socialization activities through campaigns through seminars with the title "Start From Now! Young, Healthy and Cool", and the seminar activities held at PGRI Rancaekek Middle School were attended by 105 students from grades 9 and 11, teachers, principals, and deputy principals. Furthermore, the seminar activities that we carried out provide new insights for PGRI Middle School students regarding stunting itself, increasing orientation to further education at higher levels of high school or vocational school, as well as preventing early marriage and promiscuous sex during adolescence. Evaluation of activities given a pre-test and post-test related to stunting, healthy lifestyle, and prevention of free sex. The results show that of the 105 students who took part in the socialization, knowledge before and after socialization activities increased for the better, especially aspects of the dangers of free sex and the benefits of protein for teenagers. Knowledge of the dangers of free sex will prevent early marriage. Bojongloa Village itself is one of the villages where there are still cases of stunting. Socialization regarding self-marriage and free sex among teenagers needs to be carried out to prevent the formation of a generation experiencing stunting. It is hoped that there will be follow-up from the government, schools, and parents to be able to synergize and collaborate in preventing stunting in Bojongloa Village. Keywords: Adolescents, Bojongloa Village, Peer group, Stunting.

KLASTER SIMTOM PADA KANKER PARU Yuniko Febby Husnul Fauzia1, Tuti Pahria2, Atlastieka Praptiwi2 Dosen Keperawatan, Universitas Muhammadiyah Cirebon1, Dosen Fakultas Keperawatan, Universitas Padjadjaran2 ABSTRAK Perkembangan penyakit kanker paru di Indonesia termasuk dalam kategori buruk, dengan angka kematian yang tergolong tinggi. Hal ini disebabkan oleh proses penyakit yang membutuhkan waktu panjang untuk menimbulkan gejala atau keluhan, serta pengalaman gejala yang dirasakan pasien kanker paru pada umumnya tidaklah khas seperti gejala pada penyakit lain. Penulisan literature review ini bertujuan untuk menjelaskan berbagai teori yang menunjang self management pasien stoma usus.Metode yang digunakan dalam penulisan literature review ini adalah dengan penelusuran yang bersumber dari electronic data base mencakup Google Scholar, Pubmed, dan Science Direct dengan kata kunci symptom cluster, lung cancer. Hasil yang didapatkan bahwa symptom cluster atau klaster simtom pada kanker paru adalah suatu kelompok gejala yang terdiri dari dua atau lebih gejala yang berhubungan satu sama lain yang muncul secara bersama-sama pada penyakit kanker paru. Pada umumnya klaster simtom yang ada saat ini masih belum konsisten dan tidak sama satu sama lain, karena perbedaan metode analisis statistik yang digunakan saat melakukan penelitian mengenai klaster simtom. Abstract

Adrenal crisis is an endocrine emergency condition, with one of its clinical manifestations being hypoglycemia. To date, its management focuses on the administration of corticosteroid therapy primarily hydrocortisone as the first-line treatment alongside glucose administration. This study aims to observe and report blood glucose levels, hemodynamic status, and multidisciplinary collaboration in the administration of corticosteroids, as a collaborative intervention in stabilizing blood glucose. This case study was conducted on a 70 years old geriatric patient who experienced an adrenal crisis due to the sudden discontinuation of long-term steroid therapy without a doctor's prescription. Data were collected over five days through patient interviews, family reports, and medical records. The patient experienced severe hypoglycemia despite receiving parenteral glucose. Following the administration of dexamethasone, the patient's blood glucose levels improved and began to stabilize. Dexamethasone proved effective as an alternative initial therapy for adrenal crisis when hydrocortisone was unavailable or the diagnosis is not yet confirmed. Nurses played a vital role in the early recognition of adrenal crisis signs, monitoring of blood glucose levels and hemodynamic parameters. Education for the family is essential, considering the dependency of geriatric patients on caregivers in managing daily health needs. Multidisciplinary collaborative management and educational support are key to successful care in geriatric patients with adrenal crisis.

Abstract

Patients with peritoneal abscesses who are immobilized are at risk of decubitus ulcer. Various complications in patients such as malnutrition and anaemia in patients cause a decrease in body immunity, causing a high risk of decubitus ulcers and difficulty in wound healing. Massage using Virgin Coconut Oil (VCO) and repositioning are combined interventions to reduce the risk and healing of decubitus ulcers. The study aims to determine the results of massage using VCO and repositioning patients with decubitus ulcers in healing and efforts to prevent decubitus ulcer effectively. The study is a case report of the application of massage using VCO and repositioning in a patient with decubitus ulcer with intraperitoneal abscess, pneumonia, malnutrition, and anaemia at Sumedang Regional Hospital. The patient is a 33-year-old woman with grade 2 decubitus ulcers in the sacrum area which appeared red, serum, skin peeling, and shallow holes. The patient was selected for treatment due to the decubitus ulcers, high risk of decubitus ulcers, and the absence of any interventions. Massage with VCO and repositioning were performed twice daily for 6 days. The interventions were carried out consistently by researchers, nurses, patients, and their families. There were a decrease in degree 2 to degree 1 in the patient’s sacrum decubitus ulcers after massage using VCO and repositioning.

Abstract

ABSTRACT Intertrochanteric fractures, characterized by breaks in the proximal femur outside the joint capsule, present significant rehabilitation challenges, especially in older adult with comorbidities. These fractures disrupt the attachment points for major muscles and can lead to complications if not promptly and effectively treated. This case report explores the complex clinical scenario of a 58-year-old female patient with a history of Type 2 diabetes mellitus (T2DM) and tuberculosis, who suffered a closed intertrochanteric fracture dextra. The complications were exacerbated by neurogenic bowel disorder (NBD) due to prolonged immobility. The patient presented to the emergency department with severe hip pain following a fall at home, where she had been bedridden for two weeks. During this time, she developed a grade 2 decubitus ulcer. Despite her painful condition, initial treatment was delayed, leading to severe complications. A subsequent chest X-ray revealed infiltration along with atelectasis of the left superior lobe and left pleural effusion indicative of exacerbated pulmonary tuberculosis. Diagnostic imaging also identified an obstructive ileus. Immediate surgical intervention was indicated to manage the fracture. In addition to surgical care, the patient required specialized nursing care to address her acute and chronic medical needs, including blood glucose monitoring and management of her pulmonary condition. Prior to her surgery, Swadesh massage therapy was employed, resulting in minor relief indicated by the passage of a small amount of hard, mucous-like feces. Effective management of closed intertrochanteric fractures in patients with significant comorbidities requires a multidisciplinary approach. Early surgical intervention, combined with tailored nursing care and continuous monitoring of associated complications, is crucial for restoring functionality and preventing further health deterioration. Keywords: Intertrochanteric Fracture, Neurogenic Bowel Disorder, Type 2 Diabetes Mellitus, Pulmonary Tuberculosis, Multidisciplinary Care.

Abstract

Abstract Prone positioning in spine surgery improves surgical exposure but is associated with significant ocular, neurologic, and pressure-related risks. Central retinal artery occlusion (CRAO) is a rare yet devastating complication linked to compromised ocular perfusion during prone procedures. Prolonged prone positioning also increases the risk of perioperative pressure injuries due to sustained external pressure and immobility. Perioperative nurses play a crucial role in risk identification, safe positioning, pressure injury prevention, and early recognition of adverse events to minimize irreversible complications. This case report describes a 38-year-old woman who underwent cervical spine surgery in the prone position and developed sudden painless unilateral vision loss accompanied by incomplete cranial nerve palsies postoperatively. Perioperative nursing interventions focused on comprehensive risk stratification, safe prone positioning with appropriate padding and eye protection, prevention of pressure-related tissue injury, close hemodynamic monitoring, and targeted postoperative visual and skin integrity assessments. Despite prompt multidisciplinary management, visual recovery remained limited. This case highlights the importance of evidence-based perioperative nursing practices in reducing prone position–related ocular and pressure injuries, supports the implementation of standardized positioning and pressure injury prevention protocols, and underscores the need for continuous vigilance in perioperative clinical practice. Abstract

Intensive Care Unit (ICU) is an area of ​​care activity and well designed to treat critical conditions, nurses must have cultural competence. Culture participates in health values, beliefs, behavior and situational judgments. If left untreated will result in negative social and clinical consequences, unclear environment, confusion, messages not delivered, patient ignorance, delay in informed consent and low quality of care. The purpose of this study is to describe the experience of nurses related to the implementation of Cultural Competence in the ICU from research that has been done. The method used by searching electronic articles consists of qualitative studies collected from electronic databases such as Medline, Google Scholar, Science Direct, PubMed and Proquest using relevant MeSH keywords: Cultural Competence, Intensive Care Unit, and Nursing. Articles selected according to the SPIDER criteria, published in 2012-2018. Evaluation of articles uses critical appraisal and PRISMA guidelines. From the results of the literature search conducted, as many as 7 (seven) related articles were found and met the criteria. The results of the themes that emerged related to the implementation of cultural competence include the involvement of the family in the care process, using translators, maintaining a multicultural team, communication procedures, and clear diagnosis and recognizing cultural diversity.

Abstract

Background: Patients with heart failure (HF) often experience cognitive impairment, which negatively affects their quality of life. An effective screening tool is essential for nurses and healthcare professionals to assess cognitive function as part of HF management. Although many instruments exist, none are specifically designed for patients with HF. Objective: This study aimed to map the instruments for screening cognitive function in patients with HF. Design: A scoping review. Data Sources: Articles published between 2019 and 2023 were searched in PubMed, ScienceDirect, and Google Scholar, with the last search conducted on 27 January 2024. Review Methods: The review followed the scoping review framework by Arksey and O’Malley and adhered to PRISMA guidelines for scoping reviews. Results: Of the 21 articles meeting inclusion criteria, six cognitive function screening instruments were used across various cognitive domains, effectively identifying cognitive impairment in both inpatient and outpatient HF settings. The Montreal Cognitive Assessment (MoCA) was the most frequently used tool, covering a broad range of cognitive domains. MoCA showed high efficacy with a kappa coefficient of 0.82, Cronbach’s alpha reliability of 0.75, sensitivity of 90%, and specificity of 87%. Conclusion: Instruments like MoCA, Mini-Cog, and TICS-m show promise for assessing cognitive function in patients with HF, each with specific strengths and limitations. MoCA is notable for its comprehensive coverage despite being time-consuming and having language barriers. Further research is needed to revalidate and improve the existing instruments. It is crucial for nurses and healthcare professionals to integrate these tools into regular patient management, highlighting the need for continued research in their application.
