Abstract

The calculation of leukocyte types can be used to assess the level of infection or inflammation, particularly through the neutrophil-lymphocyte ratio (NLR). The NLR has been established as a useful biomarker for predicting bacteremia. Although differential leukocyte count and the neutrophil–lymphocyte ratio (NLR) have been widely proposed as indicators of infection and inflammation, their clinical interpretation remains unclear across disease states, particularly in tuberculosis. Current evidence is limited regarding how leukocyte profiles and NLR change during standard anti tuberculosis treatment (ATT), especially in the early and intermediate phases of therapy. Furthermore, it is not well established whether changes in leukocyte subtypes reflect treatment response or disease recovery in tuberculosis patients. Therefore, there is a need to quantify changes in leukocytes and NLR during TB treatment and to determine their potential role as readily accessible biomarkers for monitoring therapy outcomes. The goal of this study was to determine the effect of treatment duration on the number of polymorphonuclear cells, mononuclear cells, and the neutrophil-lymphocyte ratio as indicators of recovery in TB patients. This study employed a cross-sectional design with an analytical observational approach, involving 26 TB patients undergoing treatment for 1 to 6 months. The results showed the following average cell counts (per microliter) over the treatment period: basophils at 0.04, eosinophils at 0.30, neutrophils at 4.10, lymphocytes at 2.36, monocytes at 1.84, and a neutrophil-lymphocyte ratio of 0.53. One-way ANOVA analysis revealed a positive correlation between basophils, neutrophils, lymphocytes, monocytes, and the neutrophil-lymphocyte ratio with TB recovery. In conclusion, treatment of TB patients during the fourth to sixth month, or in the advanced phase of treatment showed differences in the number of  polymorphonuclear cells, mononuclear cells and  the NLR ratio compared to intensive phase treatment in TB patients at the Karang Taliwang and akranegara Health Centers.

Abstract

Background : Hyperglycemia creates a urinary environment conducive to bacterial growth, especially when accompanied by decreased immune function and impaired bladder emptying. The presence of glucose in the urine (glucosuria) promotes bacterial proliferation, increasing the risk of urinary tract infections (UTIs). Although diabetes mellitus (DM) cases are relatively high at Kediri Public Health Center, research examining the relationship between blood glucose levels and UTI risk remains limited. Therefore, it is important to assess whether elevated blood glucose levels influence the risk of UTIs in DM patients. Objective: To determine the effect of blood glucose levels on the risk of urinary tract infections in diabetic patients. Method: This study employed an analytical observational design with a cross-sectional approach. A total of 30 diabetic patients with blood glucose levels >200 mg/dL were selected as respondents. Their urine samples, which met the inclusion and exclusion criteria, were examined using purposive sampling technique. Urine analysis was conducted using the urine sediment method to detect the presence of bacteria as an indicator of urinary tract infection (UTI). The data were analyzed using the Chi-Square test. Result: Out of 30 respondents, 23 respondents tested positive for urinary tract infection (UTI) based on bacterial findings in the urine sediment. The Chi-Square test results showed a p-value of 0.002 (p < 0.05), indicating a statistically significant relationship between blood glucose levels and the incidence of UTI. Conclusion: Blood glucose levels have an effect on the incidence of urinary tract infections (UTIs) in diabetic patients.

Abstract

Glucose is one of the products of carbohydrate metabolism in the body that functions as the main source of energy for the body, controlled by insulin. This increase in blood sugar is caused by the inefficient work of insulin, which is caused by the presence of injectable contraceptives. The progesterone hormone contained in injectable contraceptives has anti-insulin properties that cause cells to become less sensitive. Reduced insulin sensitivity can lead to insulin resistance in the body, which can affect glucose metabolism and result in elevated blood sugar levels. The objective of this study is to determine the difference in blood glucose levels among participants in the 1-month injection contraceptive program and the 3-month injection contraceptive program in the Masbagik Health Center service area. The sample in this study consisted of participants in the 1-month and 3-month injection contraceptive programs, with a total of 26 respondents (13 for each group). The data collected were glucose levels measured using the POCT (point-of-care testing) method. The data were analyzed using the Mann-Whitney statistical test. The results showed that the mean glucose level in 1-month contraceptive injection program participants was 108.8 mg/dL, while in 3-month program participants it was 142.9 mg/dL. The Mann-Whitney test results showed no significant difference between the blood glucose levels of 1-month contraceptive injection users and 3-month contraceptive injection users, with a value of 0.068 (p > 0.05). The conclusion of this study is that there is no significant difference in blood glucose levels between 1-month and 3-month contraceptive injection users. However, mathematically, there is an increase in the average values.
