Abstract

Abstract: Hospitals are required to minimize errors made by medical and non-medical staff to build patient trust in the services provided. Data show that patient visits at Jakarta Islamic Hospital Pondok Kopi from January to August 2023 reached 36,188 with an average of 4,523 visits per month, with the lowest number recorded in April 2023. This study aims to describe the influence of outpatient trust in doctors services on their interest in repeat visits. The research employed a quantitative approach with a cross-sectional design, involving 100 non-BPJS outpatients at the Outpatient Unit, with data collected through questionnaires. Data were analyzed using a Simple Logistic Regression test. The findings indicate that patients trust doctors services and are interested in returning for subsequent visits. Bivariate analysis revealed that outpatient trust in doctors services simultaneously had a significant effect on repeat visit interest, as shown by the wald test result (f count 11.170 > f table 1.984467) with a significance level of 0.001 < 0.05. Thus, patient trust plays a crucial role in fostering loyalty through repeat visits at Jakarta Islamic Hospital Pondok Kopi.Keyword: Patient Trust, Doctor Services, Repeat Visits, Hospital, OutpatientAbstrak: Rumah sakit dituntut untuk meminimalkan kesalahan tenaga medis maupun nonmedis agar tercipta kepercayaan pasien terhadap pelayanan yang diberikan. Data menunjukkan kunjungan pasien di Rumah Sakit Islam Jakarta Pondok Kopi pada JanuariAgustus 2023 sebanyak 36.188 orang dengan rata-rata 4.523 kunjungan per bulan, di mana angka terendah terjadi pada April 2023. Penelitian ini bertujuan menggambarkan pengaruh kepercayaan pasien rawat jalan terhadap pelayanan dokter dalam meningkatkan minat kunjungan ulang. Penelitian menggunakan pendekatan kuantitatif dengan desain cross sectional, melibatkan 100 responden pasien non-BPJS di Unit Rawat Jalan, dan data dikumpulkan melalui kuesioner. Analisis dilakukan menggunakan uji Regresi Logistik Sederhana. Hasil penelitian menunjukkan bahwa pasien menaruh kepercayaan pada pelayanan dokter dan berminat melakukan kunjungan ulang. Analisis bivariat mengungkapkan kepercayaan pasien secara simultan berpengaruh signifikan terhadap minat kunjungan ulang, dibuktikan dengan hasil uji wald (f hitung 11.170 > f tabel 1.984467) pada tingkat signifikansi 0.001 < 0.05. Dengan demikian, kepercayaan pasien menjadi faktor penting dalam membangun loyalitas melalui kunjungan ulang di Rumah Sakit Islam Jakarta Pondok Kopi.Kata kunci: Kepercayaan Pasien, Pelayanan Dokter, Minat Kunjungan Ulang, Rumah Sakit, Rawat Jalan

Abstract

Abstract: Electronic medical records (EMR) provide numerous benefits, particularly in terms of patient information access. This study was conducted to evaluate the benefits of EMR implementation at RSPON Prof. Dr. dr. Mahar Mardjono Jakarta, focusing on the aspect of patient information access. This quantitative study was conducted in January-February 2025, involving 10 physicians experienced with manual and electronic medical records, selected using a purposive sampling method. Data analysis in this study used the Wilcoxon test with an alpha value of 5%. The results showed that EMR implementation significantly improved ease of access, completeness, and accuracy of information, as well as efficiency of search time and time to fill out medical records. Specifically, the ease of access score increased significantly (p = 0.005) <0.05, with the average score increasing from 4.40 to 9.00 after EMR implementation. The Wilcoxon test also showed significant improvements in information completeness (p = 0.005) < 0.05 and accuracy (p = 0.007) < 0.05, with most respondents reporting higher scores after the implementation of RME. Furthermore, the duration of information searches and medical record completion decreased significantly (p = 0.008) < 0.05. However, issues such as unstable networks and complex interfaces need to be addressed. This study concluded that RME provides significant clinical benefits in patient information access; however, improvements in infrastructure and interfaces are needed for optimization. Keyword: Electronic Medical Records, Information Access, Benefit Realization Abstrak: Rekam medis elektronik (RME) memberikan banyak manfaat, khususnya dalam hal akses informasi pasien. Penelitian ini dilakukan untuk mengevaluasi manfaat dari penerapan RME di RSPON Prof. Dr. dr. Mahar Mardjono Jakarta, dengan fokus pada aspek akses informasi pasien. Penelitian kuantitatif ini dilakukan pada Januari-Februari 2025, melibatkan 10 dokter yang berpengalaman dengan rekam medis manual dan elektronik, dipilih dengan metode purposive sampling. Analisis data dalam penelitian ini menggunakan uji Wilcoxon dengan nilai alfa sebesar 5%. Hasilnya menunjukkan bahwa penerapan RME secara signifikan meningkatkan kemudahan akses, kelengkapan, dan akurasi informasi, serta efisiensi waktu pencarian dan waktu pengisian rekam medis. Secara spesifik, skor kemudahan akses meningkat signifikan (p = 0,005) < 0,05, dengan skor rata-rata naik dari 4,40 menjadi 9,00 setelah penerapan RME. Uji Wilcoxon juga menunjukkan peningkatan signifikan dalam kelengkapan informasi (p = 0,005) < 0,05 dan akurasi (p = 0,007) < 0,05, dengan sebagian besar responden melaporkan skor yang lebih tinggi setelah penerapan RME. Selain itu, durasi pencarian informasi dan pengisian rekam medis menurun secara signifikan (p = 0,008) < 0,05. Meskipun demikian, masalah seperti jaringan yang tidak stabil dan antarmuka yang rumit perlu diatasi. Penelitian ini menyimpulkan bahwa RME memberikan manfaat klinis signifikan dalam akses informasi pasien; namun, perbaikan infrastruktur dan antarmuka diperlukan untuk optimalisasi. Kata Kunci: Rekam Medis Elektronik, Akses Informasi, Benefit Realization

Abstract

The advancement of information technology in the healthcare sector encourages hospitals to implement reliable and secure digital systems, one of which is the Periksa.id application. This application is used by doctors to record inpatient examination results and is now equipped with a fingerprint authentication feature. This study aims to evaluate the quality of the Periksa.id application using the Software Testing Life Cycle (STLC) method, which consists of six stages: requirement analysis, test planning, test case design, test environment setup, test execution, and test closure. The research was conducted at Soeharto Heerdjan Hospital over a period of three months with a total of 200 virtual users involved in the reliability testing. Testing was carried out on both functional and non-functional aspects, including response time, reliability, security, portability, as well as positive and negative testing, using tools such as JMeter, WAPT, and ZAP. The results showed that the application met over 85% of the software quality criteria, although several minor bugs and 12 security vulnerabilities (2 high, 10 medium) were identified and require improvement. This study contributes to the evaluation of web-based hospital application systems to enhance the efficiency of medical services and ensure secure patient data management.

Abstract

Problems related to work discipline among Medical and Health Information Recorders (PMIK) observed during fieldwork practice in hospitals include non-compliance with punctuality, delays in completing tasks, and ineffective use of break time. These conditions indicate that the level of work discipline among PMIK personnel is suboptimal, which may affect productivity and service quality. Job satisfaction is one of the factors presumed to influence work discipline. This study aimed to examine the effect of job satisfaction on work discipline among PMIK members within the DPD PORMIKI DKI Jakarta area. A quantitative approach with a cross-sectional design was employed. Using a quota sampling technique, 94 respondents were selected from a total population of 1,490. The results showed that job satisfaction had a positive and significant effect on work discipline (p-value = 0.000). The regression equation obtained was Y = 5.288 + 0.606X. Job satisfaction accounted for 75.1% of the variance in work discipline, while the remaining 24.9% was influenced by other factors not examined in this study. Therefore, efforts to enhance job satisfaction are essential to improve work discipline among PMIK professionals.

Abstract

The implementation of Electronic Medical Record (EMR) faces challenges regarding patient information privacy, making it essential to assess the maturity level of its security. This study aimed to conduct an EMR security assessment to evaluate current conditions against those expected to meet the ISO 27002:2022 standard. This qualitative research employed a case study design. Data were analyzed using the System Security Engineering–Capability Maturity Model (SSE-CMM) method and gap analysis. The assessment revealed that the current EMR security level is at the initial/ad hoc stage (level 1), with an average score of 1.06 and a gap of 1.94 from the target defined process level (level 3). Thus, EMR security remains in its early stages, necessitating improvements in formally documented policies and security procedures, which have yet to be implemented.

Abstract

Code precision has received significant attention due to the increased utilization of encoded procedural data. Coding errors have been documented in multiple research investigations. This study aims to assess the variables that affect coding quality. The prevalence of diabetes has increased substantially in the past two decades and is a significant cause of morbidity and mortality. Method: This study was conducted in 2 hospitals in East Java, Indonesia, that were selected through simple random sampling from a population of hospitals meeting the predefined inclusion criteria. The bed capacities of these hospitals are 211, with details of 62 and 149, respectively, for the specialised ones. The sample in this study was 60 medical record files taken randomly in 2022 in the case of diabetes mellitus. The result showed coding quality testing uses six elements: reliability, accuracy, relevancy, timeliness, completeness, and legibility. Data analysis was carried out analytically using the Fisher Exact test. The results of the study from 60 samples showed that four elements were significant out of a total of 6 aspects of coding quality elements. The four essential elements consisted of Accuracy (p=0.001), Reliability (p=0.001), Completeness (p=0.046), and Legibility (p=0.046). Reliability elements also impact coding accuracy or vice versa (p=0.001); Completeness also affects Legibility and vice versa (p=0.046). The odds ratio value of each component shows that Reliability and Accuracy are 8.782, which means that Reliability can increase Accuracy 8 times and vice versa. Meanwhile, completeness and legibility are at 3.818, which means completeness also increases legibility by three times and vice versa. The Hospitals should consider four significant coding quality elements, including completeness, accuracy, reliability, and legibility, for use in coding audits. Timeliness and Relevance were insignificant.

Abstract

To ensure the quality of Electronic Medical Records (EMR), indicators are needed that function as measuring tools and performance evaluations. These indicators are formulated in the form of an indicator dictionary, which describes operational information to measure performance systematically. Currently, the National Brain Center Hospital (RSPON) does not have an EMR indicator dictionary. Therefore, this study aims to design and test an EMR indicator dictionary, especially in outpatient service units. The study used the Research and Development (R&D) method with the RAND/UCLA Appropriateness Method approach, implemented from November 2024 to January 2025. Based on the method above, the research stages include designing an EMR indicator dictionary using data collection methods through interviews with heads of medical record work units, testing the feasibility of the EMR indicator dictionary using the Delphi technique through a questionnaire involving four experts, and testing EMR indicators through document review of 64 EMR samples. Data analysis used content and descriptive analysis. The results show that the designed indicator dictionary contains operational definitions, data sources, data collection methods, and achievement targets of 100% which are made in accordance with applicable standards. Four experts in the Delphi method agreed that the draft RME indicator dictionary was feasible to use. From the results of the review of 64 RMEs, 50 RMEs (78.13%) were declared complete and on time, while the timeliness indicator separately reached 96.88%. In conclusion, the dictionary of completeness and timeliness indicators is feasible to use as an evaluation tool for filling RMEs in RSPON.

Abstract

RSU Fastabiq Sehat PKU Muhammadiyah Pati telah mengimplementasikan RME sejak tahun 2017 dengan SIMRS Khanza, namun penggunaannya belum optimal. Meskipun sistem ini telah memiliki fitur yang tergolong lengkap dan memudahkan rumah sakit untuk penggunaan RME. Beberapa fitur RME dalam SIMRS Khanza belum sesuai dengan kebutuhan pengguna. Setiap update modifikasi sistem harus dilakukan secara mandiri dan memerlukan pengecekan menyeluruh terhadap seluruh perangkat yang digunakan. Dalam rangka mengupdate RME diperlukan asesmen kesiapan. Penelitian ini bertujuan untuk menilai kesiapan RME dalam 4 aspek meliputi aspek alur kerja, aspek infrastruktur teknologi (IT), aspek sumber daya manusia (SDM), aspek organisasi. Penelitian ini dilakukan pada bulan September 2024 – Januari 2025. Jenis penelitian Kualitatif menggunakan Metode Qualitative Content Analysis (QCA). Data dikumpulkan melalui wawancara mendalam (in-depth interview) dan observasi dengan 4 informan yang terdiri dari kepala instalasi rawat jalan, kepala tim IT dan staf IT, kepala SDI yang relevan dan keterlibatan langsung dalam proses implementasi RME di instalasi rawat jalan. Analisis data menggunakan MAXQDA Software. Hasil penelitian dari 4 aspek tersebut menunjukkan bahwa pada aspek alur kerja, implementasi RME belum sepenuhnya siap karena belum tersedianya SOP yang sesuai dengan sistem RME, dimana kebijakan yang digunakan masih mengacu pada pedoman rekam medis manual. Pada aspek infrastruktur teknologi (IT), bahwa hardware masih diperlukan adanya UPS (Uninterruptible Power Supply), software sudah memadai namun fitur RME seperti asesmen awal rehabilitasi medik dan asesmen fisioterapi belum sesuai dengan kebutuhan pengguna. Selain itu fitur pendataan hasil echo jantung dan format laporan TB juga belum tersedia. Pada aspek SDM, terdapat 59 pengguna RME yang secara kuantitas dan kompetensi sudah memadai. Penguatan dilakukan melalui pelatihan terstruktur, kolaborasi, dan pendampingan intensif, serta evaluasi berkala oleh bagian SDI. Selain pengguna, terdapat tim pengelola SIMRS yang terdiri dari 19 orang tim adhoc dan 22 orang tim pengembangan. Pada aspek SDM, terdapat 59 pengguna RME dengan kompetensi dan jumlah yang memadai. Penguatan dilakukan melalui pelatihan, kolaborasi, pendampingan intensif, serta evaluasi berkala oleh bagian SDI. Tim pengelola SIMRS terdiri dari 19 orang tim adhoc dan 22 orang tim pengembangan, sementara tim IT berjumlah tiga orang dan masih membutuhkan dua programmer tambahan. Pada aspek organisasi, manajemen menunjukkan komitmen melalui pembentukan kedua tim tersebut yang bertugas mengkoordinasi kendala, menyediakan solusi, dan mengembangkan sistem guna memastikan implementasi RME berjalan optimal.

Abstract

Abstract: The mandatory of Electronic Medical Records (EMR) in Indonesia encourages Health Information Recorders (HIMs) to find the best solution to maintain the quality of EMR. Although EMR has been implemented, the electronic system in hospitals does not have enough features for effective quality assurance, resulting in difficulties for HIMs in quantitative and qualitative analysis. Based on KMK HK.01.07/Menkes/1128/2022 on Hospital Accreditation Standards, if the electronic system does not support service quality assurance operations, system development is required. This study aims to design a User Interface (UI) for digitizing the completeness analysis of Medical Records (MR). The method used is design thinking with five UI principles (proximity, similarity, continuity, figure and background and common fate). Design testing using heuristic evaluation by five evaluators. The result was a UI with features of MR completeness analysis, notification, MR completeness analysis report and activity log. The three highest heuristic principles were Help and Documentation score 2.80 (major), Recognition Rather Than Recall score 2.20 (minor) and Help Users Recognize, Diagnose and Recover from Errors score 1.60 (minor). These principles have been improved according to the evaluator's recommendations. It is recommended to improve the UI to achieve a score of 0 (no problem). Keyword: Heuristic Evaluation, Medical Record Completeness Analysis, User Interface Abstrak: Kewajiban Rekam Medis Elektronik (RME) di Indonesia mendorong para Perekam Medis dan Informasi Kesehatan (PMIK) mencari solusi terbaik untuk menjaga kualitas RME. Meskipun RME telah diterapkan, sistem elektronik di Rumah Sakit (RS) belum memiliki fitur yang cukup untuk penjaminan mutu yang efektif, mengakibatkan PMIK kesulitan dalam analisis kuantitatif dan kualitatif. Berdasarkan KMK HK.01.07/Menkes/1128/2022 tentang Standar Akreditasi RS, bila sistem elektronik tidak mendukung operasional penjaminan mutu pelayanan, pengembangan sistem diperlukan. Penelitian ini bertujuan merancang User Interface (UI) digitalisasi analisis kelengkapan Rekam Medis (RM). Metode yang digunakan adalah design thinking dengan lima prinsip UI (proximity, similarity, continuity, figure and background dan common fate). Pengujian desain menggunakan evaluasi heuristik oleh lima evaluator. Hasil penelitian adalah UI dengan fitur analisis kelengkapan RM, notification, report analisis kelengkapan RM dan log aktivitas. Diperoleh tiga prinsip heuristik tertinggi yakni Help and Documentation skor 2,80 (major), Recognition Rather Than Recall skor 2,20 (minor) dan Help Users Recognize, Diagnose and Recover from Errors skor 1,60 (minor). Prinsip-prinsip tersebut telah diperbaiki sesuai rekomendasi evaluator. Abstract

Abstract: The Health Information Management Study Program at Esa Unggul University, Jakarta, has been around for quite a long time, but public interest in choosing this study program has not yet met the expected target. This study aims to determine the influence of student motivation and perception on the selection of the Health Information Management Study Program at Esa Unggul University, Jakarta. The study was conducted from June to August 2022. The type of research was observational with a quantitative analytical approach using a cross-sectional design. The population was 241 with a sample size of 71 students taken by simple random sampling. Data collection was carried out through a questionnaire with a Likert scale, and analyzed using multiple linear regression. Of the 71 respondents, it was found that 84% were mostly female, 33.8% were aged between 21-23 years with a high school/equivalent education, and 33.8% came from DKI Jakarta. The selection of the Health Information Management Study Program at Esa Unggul University, Jakarta, was influenced by perception (0.442) and motivation (0.354). The results of multiple linear regression showed that there was a significant influence between motivation and perception on the Selection of the Health Information Management Study Program at Esa Unggul University Jakarta (p-value 0.000 <0.05) with the equation Selection of MIK Study Program = 7.167 + 0.354 (Motivation) + 0.442 (Perception). The selection of MIK Study Program was influenced by motivation and perception by 39.9%, the rest was other variables. Keyword: Selection, Motivation, Perception Abstrak: Program Studi Manajemen Informasi Kesehatan di Universitas Esa Unggul Jakarta sudah cukup lama, namun minat masyarakat untuk memilih program studi tersebut belum sesuai dengan target yang diharapkan. Penelitian ini bertujuan mengetahui pengaruh motivasi dan persepsi mahasiswa terhadap Pemilihan Program Studi Manajemen Informasi Kesehatan di Universitas Esa Unggul Jakarta. Penelitian dilakukan pada Juni - Agustus 2022. Jenis penelitian observasional dengan pendekatan kuantitatif analitik menggunakan desain cross sectional.  Populasi sebanyak 241 dengan besar sampel 71 mahasiswa yang diambil secara simple random sampling. Pengumpulan data dilakukan melalui kuesioner dengan skala likert, dan dianalisis menggunakan regresi linier berganda. Dari 71 responden diketahui bahwa 84% Sebagian besar berjenis kelamin perempuan, 33,8% berusia antara 21-23 tahun dengan Pendidikan terakhir SMA/Sederajat serta 33,8 % berasal dari DKI Jakarta. Pemilihan Program Studi Manajemen Informasi Kesehatan di Universitas Esa Unggul Jakarta dipengaruhi oleh persepsi 0,442 dan motivasi 0,354. Hasil regresi linier berganda diperoleh bahwa ada pengaruh signifikan antara motivasi dan persepsi terhadap Pemilihan Program Studi Manajemen Informasi Kesehatan di Universitas Esa Unggul Jakarta (p-value 0,000<0,05) dengan persamaan Pemilihan Prodi MIK =  7,167 + 0,354 (Motivasi) + 0,442 (Persepsi). Pemilihan Prodi MIK dipengaruhi oleh motivasi dan persepsi sebesar 39,9%, sisanya variabel lain. Kata kunci: Pemilihan, Motivasi, Persepsi
