Abstract

In accordance to Act No 24 Year 2011 on The Social Security Administrating Body, the Indonesian National Social Security program is managed by  two national organizations, namely: Social Security Administering Body for Health (Badan Penyelenggara Jaminan Sosial Kesehatan = BPJS Kesehatan) and Social Security Administering Body for Labor (Badan Penyelenggara Jaminan Sosial Ketenagakerjaan = BPJS Ketenagakerjaan). The former is responsible for providing health coverage for all Indonesians through the National Social Health Insurance Scheme known as the Jaminan Kesehatan Nasional/JKN. The latter is  responsible for  providing the worker's social security consisting of Provident Fund Benefit, Accident Benefit, Pension Benefit, and Death Benefit.1The Indonesian government has been continuously improving the health service program towards better national universal health coverage and has set the 2019 functional achievement target of 95% of the population enrolled in the program. To ensure that this target of  the program will be achieved,  the government pays the premium of BPJS Kesehatan of the poor and near poor.2

Abstract

Up to the present, there has been a controversy on the relationship between Parkinson’s disease and the welding job. The aim of this study was to obtain evidence-based information regarding the relationship between Parkinson’s disease and the welding job through an evidence-based case report derived from a literature review. The review was conducted through a method of search and selection of articles in the Pubmed, Cochrane Library and JSTOR databases aimed at answering the study question. The process of searching articles used the keywords “Welding” OR “Welder” AND “Parkinson”. Article selection was performed using the defined inclusion and exclusion criteria. At the initial search, 117 articles were retrieved from the three databases. Following the selection process, three articles remained, which consisted of one systematic review and two observational studies. Comparing the selected articles, the systematic review of Mortimer et al. is more relevant and appropriate for answering the clinical question. Mortimer et al. used a meta-analytical method, applied strict inclusion and exclusion criteria and excluded studies that potentially led to bias effects, lack of validity or inadequate statistical methods. Based on the selected evidence-based resources, Parkinson’s disease is not related to the welding job. The welding job, however, may produce clinical symptoms and signs resembling Parkinson’s disease, known as manganism.

Abstract

BACKGROUNDThe application of Standard Precautions (SP) has become a significant challenge for healthcare workers, especially in developing countries, endangering their safety and increasing their exposure to blood-related pathogens. This study was aimed at exploring the factors related to nurses’ compliance with the practice of SP.METHODSA cross-sectional study was conducted on 120 nurses working at a hospital in Palembang. Health Belief Model components of the subjects were recorded through questionnaires. A 12-point observation form assessed the nurses’ SP compliance. Multiple logistic regression models were used to explore factors associated with nurses’ compliance.RESULTSThe results showed that 56.7% of participants had good compliance, although the five moments of hand hygiene still needed to be improved. The study revealed that the proportion of the operating room and emergency room nurses who complied with the SP was larger than the proportion of those who worked at the other wards (OR=2.57, 95% CI 1.51-4.36). The nurses who had received training also showed a larger proportion of compliance with SP than those who had not been trained (OR=2.70, 95% CI 1.07-6.79).CONCLUSIONNurses’ behavior to SP was significantly associated with the adequacy of the training and work unit. It is suggested that the practice of SP was also influenced by enabling factors and subjective norms. Adequate training of nurses, provision of infection prevention equipment, and assessment of occupational exposures need to be introduced.

Abstract

Objectives: Knee osteoarthritis patient has the risk for ambulation disability as a result of knee pain. Nuhonni-Tulaar rheumatic exercise was designed to prevent complications of rheumatic diseasesincluding knee osteoarthritis. The purpose of this study is to investigate the effectivity of Nuhonni-Tulaar rheumatic exercise to reduce pain of knee osteoarthritis patient.Methods: Twenty two subjects did the rheumatic exercise thrice a week in the gymnasium of medicalrehabilitation department Cipto Mangunkusumo hospital. Twenty other subjects, the control group, didthe standard hamstring stretching exercise at home. Each group performed the exercise for 8 weeks.Pain was measured using Visual Analog Scale (VAS) each week and analyzed before and after theintervention.Results: After 8 weeks VAS was reduced 1.5 in the intervention group while in the control group the reduction was 0.9. VAS was then transformed into no pain, mild pain, moderate pain and severepain. VAS was significantly reduced in the intervention group (p=0.002) but not in the control group (p=0.059). VAS changes between the two groups was significantly different (p=0.004).Conclusions: Rheumatic exercise has greater benefit in reducing pain than standard stretching exercise.Warming up, core exercise II and cooling down is safe and comfortable for knee osteoarthritis patientwith grade I-II Kellgren Lawrence radiographic grading.Keywords: Nuhonni-Tulaar Rheumatic exercise, Knee osteoarthritis, Visual Analog Scale

Abstract

Latar belakang: Implan koklea merupakan pilihan utama untuk habilitasi pendengaran dan berbicara pada anak tuli sensorineural berat bilateral. Pengaturan comfortable dan threshold level berdasarkan nilai evoked compound action potential (ECAP) direkam dengan neural responses imaging (NRI) saat pemetaan. Tujuan: Memperoleh nilai ECAP sebagai acuan pemetaan berdasarkan jarak elektroda intrakoklea ke modiolus, jarak terpanjang elektroda nomor satu dengan elektroda berhadapan, jarak marker dengan lubang kokleostomi dan faktor lainnya. Metode: Anak tuli sensorineural usia 2-10 tahun, menggunakan implan koklea dengan desain contour atau straight terdiri dari 16 elektroda, 120 channel sebagai subjek penelitian. Subjek penelitian sebanyak 46 telinga (39 anak), terpasang implankoklea diperoleh secara konsekutif dengan desain potong lintang. Perekaman ECAP elektroda 3-5, 8-10, 13-15 mewakili daerah apeks, medial dan basal. Hasil tomografi komputer resolusi tinggi koklea dengan program OsiriX dilakukan rekonstruksi 3D untuk menilai posisi dan jarak elektroda. Analisis data diawali dengan univariat dan uji korelasi Spearman ‘s pada bivariat. Kandidat faktor yang berperan disertakan pada regresi ganda untuk mendapatkan faktor determinan ECAP. Comfortable zone untuk populasi diperoleh dari analisis area pada distribusi normal menggunakan comfortable level. Hasil: Diperoleh persamaan yaitu: (rerata ECAP)=-21,19+5,87 rerata jarak elektroda ke modiolus (mm)+1.31, rerata threshold level (cu)+0.48 lama penggunaan implan koklea (bulan). (R square=0.60). Comfortable zone diperoleh dengan ECAP yang berada pada variasi 84-87,5% comfortable level. Kesimpulan: Jarak elektroda ke modiolus, lama penggunaan implan koklea dan t level merupakan faktor determinan ECAP. Nilai ECAP dapat digunakan untuk mengidentifikasi penyimpangan jarak elektroda dan memperoleh comfortable zone.Kata kunci : ECAP, implan koklea, lokasi elektroda, tuli sensorineuralABSTRACT Background: Currently cochlear implant remains a preferred choice in hearing and speechhabilitation in children with bilateral profound SNHL. Comfortable and threshold level setting based on ECAP value is recorded by NRI during mapping. Purpose: To obtain ECAP value as mapping guidance based on the distance between electrode to modiolus, the longest distance between electrode number one with the ones it faces, the distance between marker and cochleostomy and other factors. Methods: Research subject were children with SNHL, between 2-10 years old using CI with 16 electrodes, 120 channels. There were 46 ears (39 children) with CI chosen consecutively by cross sectional design. Using NRI, ECAP was recorded on electrode 3-5, 8-10, 13-15 that represent the apex, medial and basal area. Their cochlears were examined with HRCT then 3D reconstruction with OsiriX programto determine the electrode position and calculate the distance. Data analysis started with univariat 1 and bivariat with Spearman’ correlation. Candidates’ factor were analysed with multiregression test to gain ECAP determinant factor. Comfortable zone for population was gained from area analysis in normal distribution using comfortable level. Results: The equation found were: y (average ECAP)=21.19+5.87 the average electrodes to modiolus distance (mm)+1.31, threshold level (cu)+0.48 CI length use (months). (R square=0.60).Comfortable zone was acquired with ECAP between 84-87,5% comfortable level variation. Conclusion: The electrode to modiolus distance, duration of CI use and t level are ECAP determinant factor. The value of ECAP can be used as guidance to identify electrode distance deviation and to gain comfortable zone.Keywords: cochlear implant, ECAP, electrode location, sensoryneural hearing loss

Abstract

Intoduction: Acute and chronic exposure to toluene at high doses is known to affect all organs of the body including the spermatogenesis process. In the industrial sector, the use of toluene as a solvent is still widely used, up to 10 million tons per year. The control over health problems that may occur is carried out by applying work exposure threshold values. This research aims to explore the effect of toluene exposure at the threshold value range on spermatogenesis.Method: This research used laboratory experiment on 30 male Wistar rats which were divided into five groups of different exposure levels, namely 12.5 parts per million (ppm], 25 ppm, 50 ppm, 100 ppm, and no exposure (control). Exposure was given for 4 hours daily over 14 days through a hood with measured release in the glass cage. The toluene exposure markers observed were Malondialdehyde (MDA) in the blood tissue and testicles using the Thiobarbituric Acid Reactive Substances (TBARS) method. The effect on the spermatogenicity process was assessed by counting the spermatogonia A cells of male Wistar rats with Periodic Acid Schiff (PAS) staining and is calculated by the Abercrombie formula. Analysis of the correlation between the level of exposure and its effect on the increase in malondialdehyde, and spermatogenesis was carried out using the Spearman correlation analysis.Result: There was a moderately positive correlation between levels of toluene exposure and plasma MDA levels (r = 0.42; p = 0.025). Meanwhile, on [the issue of] the quantity of spermatogonia cells, a high level of negative correlation with exposure levels was obtained (r = -0.68; p = 0.001).Conclusion: Toluene exposure in male Wistar rats within the range of threshold values influenced the increase in plasma MDA levels and decreased the Spermatogenia A cells. However, toluene exposure did not affect the testicular MDA levels of male Wistar rats.

Abstract

Pandemi COVID-19 dan Produktifitas Pandemi COVID-19 telah berdampak merugikan masyarakat, tidak hanya kesakitan dan kematian tetapi juga pada berbagai bidang sosial dan ekonomi. Efek pandemi dengan berbagai situasi yang berhubungan seperti pembatasan kegiatan, pembatasan perdagangan, pembatasan pergerakan, dan penguncian daerah menyebabkan terbatasnya kegiatan berbagai sektor produksi. Pada bulletin Word Food Program COVID-19: Economic and Food Security Implications (4th Edition)1 merilis data tentang situasi Indonesia setelah 9 bulan mengalami masa pandemi. Dari data yang disampaikan didapatkan bahwa Product Domestic Bruto (PDB) Indonesia terus menyusut selama kuartal kedua berturut-turut karena pandemi, dengan tingkat inflasi tahunan meningkat menjadi 1,6% pada November 2020. Harapan optimis dengan gambaran secara berangsur adanya pertumbuhan positif di semua sektor ekonomi.
