Abstract

Background: The digital native generation is the current generation of young people who are very familiar with the internet and gadget applications. This makes them vulnerable to the risk of disease transmission, both directly and indirectly. Objective: The study aimed to analyze the concept of the vulnerability of digital natives, or young adults, to HIV/AIDS. Methods: This study used the 8-step concept analysis framework developed by Walker and Avant, along with a comprehensive literature review done across three databases: google scholar, ProQuest, and ScienceDirect. The systematic literature reviewed included papers published between 2008 to 2022. Results: The concept identification findings derived from a comprehensive evaluation of 16 papers indicate that the defining features include individual characteristics, social factors, structural problems, and health programs. Conclusion: The digital generation, characterized by their convenient access to the internet and digital media, has a greater vulnerability to HIV/AIDS transmission as a result of engaging in hazardous behaviors and participating in health initiatives. The use of digital media in intervention programs has the potential to decrease healthcare expenditures and improve the efficacy of HIV/STI control measures.

Abstract

Non-Communicable diseases (NCD) atau penyakit tidak menular (PTM) merupakan penyebab utama kematian secara global. Peningkatan kejadian PTM tersebut salah satunya diebabkan oleh obesitas. Penelitian ini bertujuan untuk mengetahui faktor dominan yang berpengaruh terhadap terjadinya obesitas pada pekerja masinis kereta api DAOP 02 Bandung Jawa Barat. Desain penelitian ini menggunakan desain cross sectional. Pengambilan sampel dengan simple random sampling dengan jumlah sampel 121 pekerja masinis. Analisis bivariat menggunakan chi square menunjukkan bahwa faktor gaya hidup tidak sehat (p = 0,012) dan stress kerja (p = 0,015) merupakan faktor yang berpengaruh terhadap kejadian obesitas pekerja masinis. Setelah dianalisis menggunakan Regresi logistik, menunjukkan bahwa gaya hidup tidak sehat merupakan faktor paling dominan terhadap obesitas (p=0,029 dan OR=2,344). Pengaruh tersebut diperkuat oleh stress kerja (p=0,036; B=0,820 dan OR=0,440). Temuan penelitian ini dapat dijadikan acuan dalam menyusun kebijakan kesehatan pekerja dan memberikan asuhan keperawatan komunitas khususnya keperawatan kesehatan pada pekerja. Penelitian selanjutnya dapat melakukan penelitian eksperimen sebagai upaya menurunkan prevalensi obesitas pada pekerja.

Abstract

Background: Nurses constitute the largest proportion of healthcare human resources globally, accounting for 59% of all healthcare professionals worldwide. Poor sleep quality among nurses can lead to decreased performance, lack of concentration, physical and mental health issues, and a reduced quality of life. Purpose: To determine the impact of occupational health (job dissatisfaction and work-life balance) on poor sleep quality in nurses. Method: This study employed a quantitative cross-sectional design with purposive sampling. A total of 38 respondents participated in the study. The research instruments included Microsoft Forms, utilizing the Work-Life Balance (WLB) questionnaire and the Pittsburgh Sleep Quality Index (PSQI) to assess sleep quality. Results: A total of 92.1% of nurses experienced poor sleep quality, 57.9% WLB was unbalanced, and 76.3% were dissatisfied with their jobs. The results of the bivariate analysis using the chi-square test showed that the p value of WLB was 0.651 and job satisfaction was 0.015*. This indicates that WLB is not related to sleep quality in nurses, while job satisfaction is related to sleep quality in nurses. Conclusion: There is a significant relationship between nurses' sleep quality and job satisfaction, but no association with WLB. Suggestion: Strengthening job satisfaction among nurses should be carried out extensively through strategies such as mindfulness-based stress reduction interventions. These efforts aim to enhance job satisfaction and workplace resilience, enabling nurses to provide optimal patient care in healthcare settings.   Keywords: Job Satisfaction; Nurses; Occupational Health; Sleep Quality; Work-Life Balance.   Pendahuluan: Perawat merupakan sumber daya manusia (SDM) tenaga kesehatan yang paling banyak secara global sebesar 59% dari seluruh profesi kesehatan di dunia. Kualitas tidur yang buruk pada perawat dapat menyebabkan kinerja berkurang, tidak konsentrasi, gangguan kesehatan baik fisik, dan mental serta menurunkan kualitas hidup. Tujuan: Untuk mengetahui dampak kesehatan kerja (ketidakpuasan kerja dan keseimbangan kehidupan kerja) terhadap kualitas tidur yang buruk pada perawat. Metode: Penelitian kuantitatif desain cross-sectional dengan teknik purposive sampling. Sampel pada penelitian sebanyak 38 responden. Instrumen penelitian menggunakan Microsoft form dengan kuesioner Work-Life Balance (WLB) dan Pittsburgh Sleep Quality Index (PSQI) untuk mengukur kualitas tidur. Hasil: Sebanyak 92.1% perawat mengalami kualitas tidur yang buruk, WLB tidak seimbang 57.9%, dan tidak puas terhadap pekerjaannya sebanyak 76.3%. Hasil analisis bivariat menggunakan uji chi-square, menunjukkan bahwa p-value WLB sebesar 0.651 dan kepuasan kerja 0.015*. Hal ini menunjukkan bahwa WLB tidak berhubungan dengan kualitas tidur pada perawat, sedangkan kepuasan kerja berhubungan dengan kualitas tidur pada perawat. Simpulan: Terdapat hubungan yang signifikan antara kualitas tidur perawat dengan kepuasan kerja, tetapi tidak ada hubungan dengan WLB. Saran: Penguatan kepuasan kerja yang diberikan kepada perawat perlu dilakukan secara massif melalui strategi, seperti intervensi mindfulness-based stress reduction program. Hal ini sebagai upaya meningkatkan kepuasan kerja pada perawat dan penguatan resiliensi di tempat kerja agar dapat memberikan asuhan keperawatan yang maksimal pasien di pelayanan kesehatan.    Kata Kunci: Kepuasan Kerja; Kesehatan Kerja; Kualitas Tidur; Perawat; Work-Life Balance.

Abstract

Decreased cognitive function sometimes dramatically affects the function, quality of life and integration ofcancer survivors in the community. The objective of this study was to identify the relationship of cognitivefunction to the quality of life of post-chemotherapy breast cancer survivors. This comparative descriptivestudy uses a cross-sectional design at Dharmais Cancer Hospital, Jakarta. A total of 110 samples weredetermined by purposive sampling method. The questionnaire used to identify cognitive function is theIndonesian version of the Montreal Cognitive Assessment (MoCA), known as the MoCA-Ina. Thequestionnaire used to measure quality of life is EORTC QLQ C30. A total of 69 (62.7%) respondentsexperienced a decline in cognitive function. Based on cognitive function scores, the average visuospatial /executive is 4.30 (0.80), naming 2.76 (0.45), attention 4.25 (1.16), language 2.39 (0.80), abstraction 1.63(0.52), delayed recall 2.17 (1.01), and orientation 5.88 (036). The mean global health status score was lowerin the group with decreased cognitive function compared to the normal group, which was 56.76 (13.04) and79.47 (10.05), p <0.001. Decreased cognitive function contributes to quality of life. Assessment of cognitivefunction is recommended as a routine assessment by nurses and interventions to improve the quality of lifeof survivors.

Abstract

Children are one of the most vulnerable groups to lead exposure. Exposure in children is primarily related to hand-to-mouth activities and environmental conditions. Studies on lead level in homes measurements, which are linked to blood lead levels in children, are still rare. The objective of the study was to identify lead levels on painted surfaces, consisting of doors and walls, in the homes of children aged 12-59 months who had high blood lead levels (≥ 20 μg/dl). The method was a cross- sectional study conducted in Java, Indonesia, comparing lead exposed and unexposed in areas with battery recycling activities to a demographically similar control area without such activities (Cinangneng-Bogor). The results showed among the 145 inspected homes in the exposed areas, 40 were measured on doors and 93 on walls; meanwhile among the 240 inspected homes in the unexposed areas, 35 were measured on doors and 36 on walls. In the exposed areas, the lead levels at the door ranged from 0.00 µg/cm2 – 891.36 µg/cm2 and it was not different (p = 0,671) from that in the unexposed areas, i.e. ranged from 0.00 µg/cm2 – 537.03 µg/cm2. Meanwhile, the lead levels at the walls in the exposed areas were measured ranged from 0.00 µg/cm2 – 279.21 µg/cm2 (median 0.65 µg/cm2), and it was significantly different (p <0.001) from that in the unexposed area, i.e. ranged from 0.00 µg/cm2 – 45.64 µg/cm2 (median 0.00 µg/cm2). The findings of lead exposure in the children’s homes lead to a strong recommendation to regulate the elimination of lead in the household paints, and to escalate community concern of domestic lead hazards.

Abstract

BACKGROUND: The burden of disease due to lead exposure continues to increase. Lead interferes with 25(OH)D hydroxylation and calcium transport, increasing osteoclastic activity and bone resorption. Pyridinoline crosslinks (PYD), as an indicator of bone damage, can be seen earlier compared to imaging changes. Therefore, it is necessary to determine the correlation between serum 25(OH)D levels and the urinary PYD/creatinine ratio in workers exposed to lead, since up to now, there are only limited studies related to it.METHODS: This cross-sectional study involved 104 workers exposed to lead, selected from parents whose children had blood lead levels above 10 µg/dL. Questionnaires and physical examination were performed to obtain characteristic data from subjects. Data regarding blood lead levels, serum 25(OH)D levels, urinary PYD levels, and urinary creatinine levels were also obtained from various laboratory methods.RESULTS: Most subjects (86.5%) had inadequate serum 25(OH)D. Median blood lead levels was 6.3 (1.2-35.5) µg/dL, chronic lead exposure index was 35.3 (1.2-535.8) years µg/dL, serum 25(OH)D levels was 22 (8-52) ng/mL, and urinary PYD/creatinine ratio was 5.3 (3.6-28.1)×10-6. There was a significant negative correlation between serum 25(OH)D levels and urinary PYD/creatinine ratio in workers exposed to lead. There was also a significant positive correlation between chronic lead exposure index and the urinary PYD/creatinine ratio.CONCLUSION: Since urinary PYD/creatinine ratio is correlated with serum chronic lead exposure index and serum 25(OH)D levels, it suggests that pyridinoline might be a potential biomarker to detect bone metabolism disorder due to the chronic lead exposure. Vitamin D adequacy is also an important factor in preventing bone metabolism disorder amidst chronic lead exposure.KEYWORDS: 25(OH)D, bone resorption, pyridinoline, lead, worker

Abstract

BACKGROUND: Amino acids are one of the essential metabolites, especially the 20 amino acids that are preserved as the building blocks of protein. Alterations in amino acid concentrations are related to disease such as inborn error of metabolism, cancer, as well as nutritional status. Hence, it is necessary to define reference values of 20 plasma-free amino acids for Indonesian toddlers and to establish a robust measurement technique using chromatography with tandem mass spectrometry (MS).METHODS: This study was a cross-sectional preliminary study to establish reference values. The sample was prepared by mixing plasma with 20% sulfosalicylic acid. Plasma-free amino acids were measured with high-performance liquid chromatography (HPLC) non-derivatization technique using column XTerra for chromatographic separation coupled with tandem MS. Amino acids reference values were taken from 101 healthy Indonesian toddlers aged 1-3 years old. Since amino acids data were not Gaussian distributed, the lower and upper of the reference value was established from the 5th percentile and the 95th percentile, respectively.RESULTS: Analysis for 20 amino acids was validated. The accuracy ranged from 90.53-105.39% and the precision ranged from 0.06-3.80%. The limit of detection range was 1-2 nmol/mL, and the limit of quantification range was 2-4 nmol/mL. The result was linear, with R2 higher than 0.998. There was no significant difference between boys and girls for all amino acids except for glycine.CONCLUSION: HPLC with tandem MS method can be used to evaluate amino acids in clinical practice. The reference values obtained are specific for aged 1-3 years old from urban areas in Indonesia. Abstract

Background: Stunting has become a priority in Indonesia's development agenda that national and sub-national teams are established with multi-sectoral involvement to tackle the problem However, coordination hurdles, particularly at the mid-level, underscore the need for understanding frontline workers' capacity, prompting a study to conduct a training needs assessment (TNA) to bolster local governments' commitment to stunting and nutrition promotion. Methods: The TNA was conducted from July to November 2021 employing online quantitative survey and series of focus group discussions (FGDs) among central and local government officers. Quantitative data were about knowledge on stunting, attitudes toward program management, performance assessment, and capacity building needs. The FGDs focused stunting reduction program management and implementation capacity. Results: The present study highlighted the personal and organizational aspects of the management of stunting reduction program in Indonesia. The findings from both aspects were used to understand the needs and gaps revealed so that a relevant and appropriate training/capacity building program can be proposed. Conclusions: Capacity building is required to enhance local governments' commitment to stunting and nutrition promotion programs, particularly focusing on middle management to improve communication and foster multi-sectoral collaboration. Such efforts necessitate a team-based training program to develop skills in coordination and interprofessional collaboration. Abstract

Background: Inadequate quantity and quality of complementary feeding contribute substantially to micronutrient deficiencies, particularly iron, zinc, and calcium. In Indonesia, intake of these micronutrients is frequently suboptimal, and their bioavailability is often compromised by dietary inhibitors, primarily phytate. Objective: This study aimed to compare the in vitro bioavailability of iron, zinc, and calcium from selected locally available animal-and plant-source foods with potential for promotion in complementary feeding among children aged 6–24 months, and to evaluate the effects of pre-processing treatments on micronutrient bioavailability and phytate reduction. Method: A laboratory-based experimental study was conducted to assess the micronutrient bioavailability and phytate content in sticky corn, mung beans, cowpeas, and moringa leaf subjected to soaking (24 h), boiling, and germination (72 h). Bioavailability was analyzed using a solubility-based in vitro digestion model. Anchovy and chicken liver served as animal-source comparators. Result: Pre-processing treatments significantly reduced phytate content in mung beans, cowpeas, and sticky corn (p < 0.001), with germination yielding the greatest reduction. However, desirable calcium bioavailability was not achieved in germinated cowpeas and sticky corn. Anchovy and chicken liver exhibited substantially higher iron, zinc, and calcium bioavailability compared with plant-source foods. Conclusion: Germination of plant-source foods enhances iron, zinc, and calcium bioavailability; however, their bioavailability remains lower than that of animal-source foods. Further studies are needed to determine the optimal animal-to-plant-source food ratios for the development of nutrient-dense complementary foods that meet iron, zinc, and calcium requirements.
