Abstract

Introduction: Limited of government’s budget for psychiatric patients has contributed to the expenses should be paid from out of pocket of the poor patients. The purpose of this research was to describe the Government policy on health social security insurance for the poor people experiencing psychiatric problems in DKI Jakarta.Methods: Qualitative research method was used with phenomenology approach to identify and describe the themes relevant to government public policy in providing health insurance for poor people with mental health problems. An in-depth interview and focus group discussion were used to collect data from different informants: service user, healthcare provider, local government policy maker. The qualitative data was analyzed using content analysis.Results: The study revealed the following themes: from user perspective (understanding of psychiatric disorder, social security network/JPKM, right and obligation, the advantage and barrier for receiving services, expectation from care provider, social support and expected future direction); from the perspective of healthcare provider (understanding of social security network/JPKM, types of provided health services, recording and reporting, referral system, cost, and the criteria of poor people), from local government and health office perspective (understanding of JPKM for poor family, scope of work, working procedure, recording and reporting, cost, accountability, poor people criteria, the relationship between central Government policy with local government policy.Conclusion: The finding of this study recommend the need for leveling socialization of JPKM, the budget transparences, improving the healthcare providers’ knowledge on the mental health psychiatric problems and its needs.

Abstract

Introduction: Traditional markets are a financial resource for traders. Fire disasters at the traditional markets will have a bad effect in terms of generating both financial and psychological problems. There is a lack of studies about the psychological problems experienced by traditional market fire victims. The aim of this study is to identify the correlation of psychological capital (hope, self-efficacy, resilience, and optimism) with the anxiety level among the victims of market fire disasters in Central Jakarta.Methods: This study used a cross-sectional approach with a non-probability sampling method. This study involved 174 market fire victims from Central Jakarta. The independent variables were psychological capital, which includes hope, self-efficacy, resilience, and optimism. The dependent variable was anxiety level. The instruments used the Hope scale, the General Self Efficacy scale, The 14-item Resilience scale (RS-14), the Life Orientation Test-Revised (LOT-R) scale and the Generalized Anxiety Disorder scale. The data analysis used an Independent T-Test, Chi-Square, and multiple logistic regression prediction modeling.Results: The more kiosks burned, the more that the informant’s anxiety increased by about4.845 times after applying a control factor of self-efficacy and optimism with a Wald value of 23.146.Conclusion: Psychological capital (self-efficacy and optimism) have a significant correlation with anxiety in the market fire disaster victims. Good self-efficacy and optimism can reduce the level of anxiety felt.  This study highlighted that psychological capital is a part of the disaster assessment as the basis for providing disaster nursing interventions.

Abstract

Adolescents are a vulnerable group mental health problem development after frequently being exposed to disaster-related information. Some studies show that adolescent living in earthquake-prone areas experience anxiety. Adolescents’ untreated anxiety possibly causes mood disorders, eating disorders, and even anxiety disorder. Nursing intervention conducted to reduce anxiety consists of relaxation and distraction techniques. Relaxation techniques consist of deep breathing exercises and five-finger relaxation techniques. This case study aims to describe the change signs and symptoms of anxiety on adolescent in earthquake prone area that giving relaxation and distraction techniques. The subjects of this study were four high-school-adolescent-students suffering from moderate anxiety about earthquake and were measured by Hamilton Anxiety Rating Scale (HARS). The results reveal that anxiety levels of all subjects decreases with average HARS score is 11,5. It is concluded that relaxation and distraction techniques can use as one of theraphy to reduce anxiety on middle adolescent in earthquake- prone area. This case study is considered as an important reference material for practitioners who find a similar case in the pratice setting.

Abstract

Flood disasters in Indonesia increased from 2014-2018 with a total of 659 cases. Flood disasters cause considerable psychological impact, one of which is Acute Stress Disorder or ASD. The psychological impact that was first experienced by a person affected by a flood when in a crisis situation is the emergence of panic and fear and loss. Disaster victims who began to realize that they experienced loss after the panic response experienced began to decline, and this causes many disaster victims to feel unprepared to accept the conditions of loss. The unpreparedness of this individual in accepting loss causes a sad response which is commonly known as grieving. some other people the process of grieving can increase, if not treated can develop into chronic grieving, causing a stress response that is characterized by dissociative symptoms after trauma called Acute Stress Disorder. The  objective to identity the sign and symtoms of acute stress disorder in flood victims. The method this type of research is cross sectional used  to ilusstrate or describe a phenomenon that is supported by the results of a visit, population all affected by flood victimsin the vilages of Talang Putri and Talang Bubuk in Palembang.  Data collection was carried out with interview and observation techniques related to signs and symptoms of Acute Stress Disorder. Field research results indicate that traumatic experiences in which psychological conditions (fear, panic, loss of grief, stress) experienced by a person. Apart from that disositive symptoms found also a decrease in the ability to express emotions, fixated, feeling this incident was a nightmare. which can increase the potential for experiencing ASD.

Abstract

The eruption of Mount Merapi at the end of 2010 was recorded as the biggest eruption in the last 100 years. Survivors (survivors) can experience serious long-term psychological effects that have an influence on psychological well-being in carrying out daily activities. One of the symptoms most often found in victims of natural disasters is post-traumatic stress or Post Traumatic Stress Disorder (PTSD). The writing of this article is to find out the description of PTSD in victims of the natural disaster of the eruption of Mount Merapi for a decade. This type of descriptive qualitative research, with a population of adults (20-60 years) of 220 people who experienced post-eruption of Merapi in 2010, the sample according to inclusion criteria, namely dukuh and seven residents. Data collection is done by interview and observation. The results show that at this time the population / community is still showing signs and symptoms of PTSD which is feeling sad as if re-experiencing the eruption of Merapi in 2010, still feeling trauma (easily remembering the eruption of Mount Merapi when it did not want to remember it), anxiety when Merapi experienced increased activity and hopes that the Merapi eruption in 2010 will not happen again.

Abstract

Schizophrenia is the most widely treated psychotic disorders in mental hospitals. The prevalence of the schizoaffective disorder is difficult to determine precisely because of the limited data available. Schizoaffective disorder has positive symptoms include hallucinations and negative symptoms include self-care deficits. Interventions that can be used to resolve hallucination problems and self-care deficits, among other acceptance commitment therapy that are part of behavioral therapy. This study aims to describe cases of treatment hallucinations and self-care deficits in schizoaffective patients using acceptance commitment therapy. This study is a case report using a descriptive observational design on one patient. In this study, patients were medically diagnosed schizoaffective with nursing problems, sensory perception disorders, and self-care deficits. Patients receive treatment in the form of acceptance commitment therapy for four sessions. There was a decrease in hallucinations and self-care deficit symptoms after acceptance commitment therapy was given. Acceptance commitment therapy can reduce symptoms of sensory perception disorder hallucinations and self-care deficits in schizoaffective patient.

Abstract

Dampak gagal ginjal kronis dapat menimbulkan masalah psikososial salah satunya ansietas berupa tanda dan gejala seperti sulit tidur, tekanan darah meningkt. Tujuan penelitian untuk memberikan gambaran asuhan keperawatan ansietas pada pasien gagal ginjal kronik dengan cara teknik relaksasi. Desain penelitian dengan studi kasus. Penelitian dilakukan selama 5 hari di ruang perawatan. Dimulai dari pengkajian sampai dengan evaluasi. Tindakan keperawatan ners yang diberikan yaitu relaksasi tarik napas dalam, distraksi dan hipnosis lima jari dan tindakan keperawatan ners spesialis yaitu CBT. Hasil yang didapatkan adalah hilangnya tanda dan gejala ansietas dan meningkatnya kemampuan Ny. S dalam mengatasi ansietas. Tindakan keperawatan ners dan ners spesialis dapat digunakan terhadap pasien gagal ginjal kronik yang mengalami ansietas fisik di rumah sakit umum.

Abstract

ABSTRACT The prevalence of HIV/AIDS is increasing in almost all over the world, the most incidence is in productive age. High community stigma and discrimination, has a negative impact on people living with HIV/AIDS (PLWHA) and the wider life. Objective: Identify the relationship between demographic factors (age, gender, latest education, occupation, religion, economic status, marital status), disease factors (initial diagnosis, opportunistic infections, medication adherence), and psychological factors (anxiety and depression) with meaning, spiritual experience of illness in PLHIV in East Jakarta. This study used a cross sectional study design with consecutive sampling involving 225 respondents at the Bhayangkara Hospital Level I R. Said Sukanto, Jakarta. The independent t test obtained the age factor with a p value of 0.043, meaning that there was a relationship between age and the spiritual meaning of the experience of illness. Chi square test on the factors of gender, recent education, employment, religion, economic status, marital status, early diagnosis, opportunistic infections, adherence to taking medication, anxiety and depression obtained p value> 0.05 means that there is no relationship between these factors and meaning, spiritual experience of illness. Opportunistic infection is a dominant factor related to the spiritual meaning of the experience of illness. There is a relationship between age and the spiritual meaning of the experience of illness, and opportunistic infections are the dominant factor related to the spiritual meaning of the experience of pain. Suggestion: It is necessary to do further research based on initial diagnosis or duration of HIV infection.

Abstract

Objektif : Remaja cenderung beresiko mengalami ansietas yang disebabkan oleh kejadian bencana alam. Penelitian ini bertujuan untuk mengidentifikasi pengaruh cognitive behaviour therapy (CBT) terhadap tingkat ansietas remaja di wilayah banjir.Metode: Desain peneliatian Quasi experimental with control group pretest-posttest dengan tekhik purposive sampling dan dilanjutkan cluster random sampling. Terdapat 73 sampel yaitu 37 intervensi dan 36 kontrol, dan tindakan keperawatan ners dan CBT sebanyak 4 kali pertemuan. Hasil: hasil menujukkan bahwa ada penurunan yang signifikan dari tingkat ansietas (p-value <0,05) pada kelompok yang dirawat dengan menggunakan intervensi keperawatan ners dan CBT, serta penurunan signifikan lebih besar daripada kelompok yang hanya diintervensi dengan keperawatan ners.Kesimpulan: CBT dapat menurunkan tingkat ansietas pada remaja di wilayah bencana banjir, sehingga dapat direkomendasikan perlunya pengembangan program kesehatan jiwa remaja berbasis keperawatan komunitas. Kata kunci: CBT, Ansietas, Banjir, Remaja

Abstract

ABSTRAKPenelitian ini membahas mengenai tingkat kecemasan dan stres yang dialami oleh mahasiswa mengikuti OSCE. Tujuan: Penelitian ini bertujuan untuk menggambarkan tingkat kecemasan dan stres pada mahasiswa yang mengikuti OSCE. Metode: Penelitian ini melibatkan 109 responden dengan kriteria inklusi yaitu mahasiswa aktif dan sudah mengikuti ujian OSCE praktikum anak. Penelitian ini menggunakan total sampling. Instrumen yang digunakan yaitu Hamilton Rating Scale Anxiety (HRS-A) dan Perceived Stress Scale (PSS). Uji reliabilitas instrumen HRS-A memiliki nilai Cronbanch’s Alpa yaitu 0,752 dan uji reliabilitas instrumen PSS memiliki nilai Cronbanch’s Alpa yaitu 0,706. Data dianalisis secara univariat. Hasil penelitian: mahasiswa yang mengalami kecemasan ringan yaitu 71 (65,14%), kecemasan sedang 19 (17,43%), kecemasan berat 17 (15,6%), dan panik 2 (1,83%. Mahasiswa yang mengalami stres ringan 1 (0,9%), stres sedang 78 (71,6%) dan stres berat 30 (27,5%). Kesimpulan: Hasil penelitian ini diharapkan bisa memberikan informasi bagi institusi pendidikan mengenai gambaran tingkat kecemasan dan stres pada mahasiswa yang mengikuti OSCE. Sebagian mahasiswa mengalami kecemasan sedang berat dan panik sehingga perlu penelitian lanjutan tentang faktor faktor yang mempengaruhi tingkat stres dan kecemasan serta strategi koping yang digunakan dalam mengatasi kondisi stres yang muncul saat melaksanakan OSCE.Kata Kunci: Kecemasan, stres, mahasiswa keperawatan dan OSCE Anxiety and Stress Level in Students Who Take Objective Structure Clinical Examination (OSCE)ABSTRACTThis study discusses the level of anxiety and stress experienced by students following the OSCE. The reliability test of the HRS-A instrument has a Cronbanch’s Alpa value of 0.752 and the reliability test of the PSS instrument has a Cronbanch’s Alpa value of 0.706. Data was analysed with univariate analysis. Results: The results showed that of 109 respondents, students experienced mild anxiety, namely 71 (65.14%), moderate anxiety 19 (17.43%) and severe anxiety 17 (15.6%) and panic 2 (1.83%). The results showed that of 109 respondents, students who experienced mild stress 1 (0.9%), moderate stress 78 (71.6%) and severe stress 30 (27.5%) Conclusion: The results of this study are expected to provide information for educational institutions regarding the description of anxiety and stress levels in students who take part in OSCE Some students experience moderate anxiety and panic, so it is necessary to further research on the factors that influence stress and anxiety levels and coping strategies used in dealing with stress conditions that arise when implementing OSCE.Keywords: anxiety, stress, nursing student and OSCE
