Abstract

Perdarahan merupakan salah satu kondisi yang sering dialami oleh pasien yang mengalami kecelakaan. Perdarahan yang tidak ditangani dengan segera akan menjadi keadaan darurat medis karena dapat mengakibatkan syok dan mengancam nyawa manusia. Program pengabdian ini bertujuan untuk mengetahui pengaruh pelatihan penanganan darurat perdarahan dan pertolongan pertama kecelakaan terhadap pengetahuan remaja di SMPN 2 Jember. Kegiatan dilakukan dengan program pengabdian pada sekolah dengan desain pre-experimental one group pre-test post test dan pelatihan berbasis case simulation method. Sasaran program pengabdian ini adalah 39 remaja di SMPN 2 Jember yang terdiri dari 2 laki-laki dan 37 perempuan. Data pengetahuan remaja terkait penanganan perdarahan menunjukkan nilai mean -pre-test 42,05, nilai mean post-test 70,76, dan hasil meant different bernilai positif 28,7. Kegiatan program pengabdian sekolah berpengaruh terhadap peningkatan pengetahuan remaja terkait penanganan darurat perdarahan dan pertolongan pertama kecelakaan.

Abstract

Background: Patients in intensive care units (ICUs) with tracheostomies are particularly susceptible to nosocomial infections, notably ventilator-associated pneumonia (VAP) and lower respiratory tract infections. A key contributing factor is the presence of self-care deficits, as these patients are generally unable to maintain airway hygiene or perform tracheostomy wound care independently. These deficits are compounded by healthcare worker workload, constrained ICU resources, and the absence of standardized care protocols. A comprehensive synthesis of existing evidence is therefore necessary to clarify the relationship between self-care deficits and infection risk. Purpose: To explore and analyze the association between self-care deficits and infection incidence in ICU patients with tracheostomy, with a focus on identifying key risk factors, implementation challenges, and effective infection prevention strategies. Method: A systematic literature search was conducted using databases including ProQuest, JSTOR, BMC, and Google Scholar, covering studies published between January 2015 and December 2024. Of 1,157 articles initially identified, 35 were selected for full-text review based on predefined inclusion and exclusion criteria. Quality appraisal was conducted using the Joanna Briggs Institute (JBI) framework, yielding 11 articles for final inclusion. Results: Self-care deficits substantially contribute to nosocomial infection risk in ICU patients with tracheostomies. Identified risk factors include ineffective secretion management, poor oral hygiene, and improper suctioning techniques. Inadequate education for both patients and caregivers following ICU discharge further exacerbates this risk. Additional barriers include limited medical equipment availability, insufficient training in tracheostomy wound care, and healthcare provider burden. Furthermore, there is a lack of research addressing the long-term consequences of self-care deficits and the effectiveness of patient-centered educational interventions. Conclusion: Self-care deficits significantly elevate the risk of infection among ICU patients with tracheostomies, primarily due to impaired hygiene and secretion management. Contributing factors include insufficient resources, absence of standardized protocols, and limited educational support for patients and caregivers. Although nursing interventions have demonstrated potential in mitigating these risks, their implementation remains inconsistent. Further research is warranted to assess the long-term effectiveness of targeted, patient-centered self-care education strategies.

Abstract

Natural disasters produce both immediate and prolonged psychological consequences, including stress, anxiety, and depression. These effects require comprehensive interventions combining stress management strategies with the Spiritual Emotional Freedom Technique (SEFT). This study aimed to enhance mental health cadres' capacity to support disaster resilience programs in agricultural communities, focusing on Indonesia's disaster-prone Lumajang District. The research implemented a two-phase intervention: First, 30 health cadres received training using a quantitative pre-experimental design with pre-test and post-test evaluations. Second, these trained cadres conducted home visits for 80 community members, with outcomes measured using the PTSD Checklist (PCL-5). Results demonstrated significant improvements across both cadres and community members. Cadres' stress management knowledge increased substantially from an average pre-test score of 50% (poor category) to 76.6% (good category) post-intervention. Most importantly, SEFT implementation reduced PTSD symptoms among community members from 41.25% to 11.25%. The study also revealed high participant engagement, with all 30 cadres completing training and demonstrating strong knowledge retention. These findings confirm that targeted training in SEFT and stress management can effectively enhance mental health cadres' professional capabilities. By strengthening frontline responders' skills, communities gain greater capacity to address post-disaster psychosocial challenges and build long-term resilience. The research highlights the value of integrating evidence-based psychological interventions with community health worker programs, particularly in vulnerable agricultural regions facing recurrent disasters. This cadre-centered approach offers a sustainable model for improving mental health outcomes in disaster-affected populations while supporting broader disaster preparedness initiatives. Where it will also support the achievement of SDG 3.

Abstract

Pelatihan dan peningkatan kompetensi tenaga medis memainkan peran penting dalam meningkatkan daya tanggap pusat layanan stroke, terutama dalam mempercepat penanganan dan meningkatkan kualitas perawatan. Penelitian ini bertujuan untuk meninjau secara sistematis pengaruh pelatihan dan kompetensi tenaga medis terhadap daya tanggap Stroke Center serta dampaknya pada kualitas perawatan dan keselamatan pasien  stroke.  Penelitian ini merupakan tinjauan sistematis terhadap enam artikel yang dipilih dari berbagai basis data daring terkemuka antara tahun 2020 hingga 2024, yang membahas pengaruh pelatihan dan kompetensi terhadap respons pusat layanan stroke. Seleksi artikel dilakukan secara ketat dengan menggunakan alat penilaian kritis dari lembaga internasional. Sumber data diperoleh dari pencarian di Google Scholar, ProQuest, Wiley, dan BMC untuk mengidentifikasi studi yang relevan. Artikel yang disertakan adalah yang menggunakan metode kuantitatif, kualitatif, atau campuran. Hasil kajian menunjukkan bahwa pelatihan tenaga medis berdampak positif dalam mempercepat respons waktu, meningkatkan akurasi diagnosis, dan memperbaiki koordinasi tim dalam pelayanan stroke. Meskipun terdapat temuan bahwa pelatihan tertentu dapat menambah beban kerja, manfaatnya terhadap keselamatan pasien dan efisiensi layanan lebih menonjol. Penelitian ini menyimpulkan bahwa pelatihan dan pengembangan kompetensi tenaga medis secara berkelanjutan sangat esensial untuk peningkatan respons pusat layanan stroke.

Abstract

Low family support in diabetes management can decrease the self-efficacy of people with type 2 diabetes mellitus (T2DM), impacting diabetes management and outcomes. Healthcare providers can use the family caregiver empowerment model (FCEM) intervention to empower families as caregivers. The study aimed to analyze the effect of the FCEM intervention on people with T2DM self-efficacy. The randomized control trial study was conducted with 85 respondents (intervention group = 41; control group = 44), randomly selected using cluster random sampling. The intervention group received an empowering intervention based on the FCEM model, while the control group received standard care. Sociodemographic and diabetes self-efficacy questionnaires were used to collect the data. Data were analyzed using the dependent t-test and Mann-Whitney U test. The results showed a significant difference in self-efficacy in both groups before and after the intervention (p = 0.001). There was also a significant difference between the intervention and control groups (p = 0.001). The FCEM interventions had a positive effect on the self-efficacy of the people with T2DM. Therefore, healthcare providers can use the FCEM intervention to enhance the family's role in diabetes management and increase the self-efficacy of people with T2DM, improving diabetes self-management and outcomes.

Abstract

Background: Measurement of adaptation in nursing based on Roy's Adaptation Model is critical to understanding how patients adapt to chronic medical conditions. This model examines patient adaptation in four main modes: physiological, self-concept, role-function, and interdependence. This study aims to review and evaluate adaptation measurement instruments developed based on Roy's Adaptation Model, as well as assess their effectiveness and limitations in the context of nursing patients with chronic diseases. Methods: This study used a Systematic Literature Review (SLR) approach by following the PRISMA guidelines. The literature search was conducted systematically through reputable academic databases, such as Scopus, PubMed, Google Scholar, and DOAJ. The search strategy incorporated Boolean Operators to ensure broad coverage and topic relevance. The keywords used included measurement instruments, the Roy Adaptation Model, and aspects of validity and reliability. Studies included in this review contained information related to the development or application of patient adaptation measurement instruments with the Roy Adaptation Model in a nursing context. Selected articles were published between 2015 and 2025 and met strict inclusion and exclusion criteria. Results: Twenty articles were found that met the inclusion criteria. Adaptation measurement instruments based on Roy's Adaptation Model showed good validity and reliability, but had limitations in measuring all modes of adaptation holistically. Existing instruments are mostly applied in specific care contexts, such as patients with heart failure, neurodegenerative diseases, and palliative care. Some instruments only measure one or two modes of adaptation, which limits a comprehensive picture of the patient's adaptation process. Conclusions: Although the existing instruments are valid and reliable, there is still a need to develop more holistic instruments that can integrate all four modes of adaptation in one cross-culturally validated measurement tool. Future research is recommended to expand the application of the Roy Adaptation Model in various care contexts.

Abstract

Early Postoperative pain remains a major clinical challenge that can delay recovery, increase healthcare costs, and diminish patients’ quality of life. Despite advancements in surgical techniques and analgesic protocols, many patients still experience moderate to severe pain in the immediate postoperative period. One key non-pharmacological intervention—early mobilization—has been shown to reduce pain intensity, enhance functional recovery, and minimize opioid use. However, the integration of early mobilization into routine postoperative care is frequently hindered by various systemic and patient-related barriers. These include anxiety about pain during movement, lack of staff training, and inconsistent implementation strategies across healthcare settings. As a result, the full benefits of early mobilization remain underutilized, particularly in vulnerable patient populations. This scoping review aimed to explore the clinical effectiveness, implementation challenges, and contextual factors affecting early mobilization in postoperative pain management. A systematic search of PubMed, Proquest, and ScienceDirect databases was conducted for peer-reviewed studies published between 2020 and 2025. Using the Joanna Briggs Institute (JBI) methodological framework, 35 studies out of 870 screened were included in the final analysis. The findings revealed that initiating mobilization within 48–72 hours after surgery significantly reduced pain and accelerated recovery, especially in orthopedic patients. However, patients undergoing abdominal or thoracic surgery experienced delayed mobilization due to incisional pain and mobility limitations. The review also identified key gaps in current research, particularly regarding the long-term effects of early mobilization on chronic pain, recovery sustainability, and its application in elderly or high-risk populations. Addressing these issues requires structured protocols, multidisciplinary involvement, and the adoption of digital health technologies to enhance implementation and accessibility.

Abstract

Background: The family plays a vital role in T2DM self-management, but many families need to understand their role and function as caregivers. This lack of understanding can increase the burden of disease management, contributing to the emergence of psychological problems in people with T2DM, such as fear of hypoglycemia, which in turn worsens blood glucose control. The Family Caregiver Empowerment Model (FCEM) intervention is one model that can be used to empower families as caregivers in T2DM self-management.Purpose: This study aimed to analyze the effect of the FCEM intervention on the fear of hypoglycemia in people with T2DM. Methods: The study design was quasi-experimental with a randomized control pretest-posttest design. The sample size consisted of 85 respondents (a pair of family caregivers and people with T2DM), who were randomly divided into two groups: the treatment group (41 respondents) and the control group (44 respondents). The FCEM intervention was conducted once per week for ten weeks for family caregivers, and fear of hypoglycemia was measured using the Indonesian version of the Fear of Hypoglycemia Scale questionnaire. The data were analyzed using the Wilcoxon signed rank test and independent t-test.Results: The results showed significant differences in fear of hypoglycemia between the pre-test and post-test in the treatment group (p=0.001) and the control group (p=0.001). However, there was a significant difference in the decrease of fear of hypoglycemia between the treatment and control groups (t=-7.087; p=0.001). This finding suggests that FCEM intervention can significantly reduce the fear of hypoglycemia in people with T2DM.Conclusions: The FCEM intervention can reduce the fear of hypoglycemia in people with T2DM by increasing the family caregiver’s ability and support in managing T2DM. Nurses can use the FCEM intervention in T2DM management to improve diabetes management outcomes.

Abstract

Pain is the main problem experienced by many postoperative patients. One of the therapies for patients with rectal cancer is surgery, so patients with rectal cancer also feel pain after getting surgery. > 80% of patients experience postoperative pain, but appropriate pain management only occurs in approximately 30-50%. Benson's relaxation therapy reduces pain intensity in postoperative patients without causing side effects. This study aims to analyze benson therapy to reduce pain in postoperative surgery for patients with rectum cancer. The method used in this research is a descriptive case study. The research was conducted for three days on one patient with postoperative surgery in the Seruni Room at RSD dr Soebandi Jember. Benson therapy is carried out twice a day for 10-15 minutes. This study found that Benson therapy can reduce the level of acute pain in postoperative patients from the NRS scale 6 to the NRS scale 3, respiration rate 20 x/minute, SpO2 98%, blood pressure 120/80mmHg, pulse 87x/ minute. The result of reducing the pain scale indicates that Benson therapy can be applied to treat acute pain in postoperative patients to make certain patients' conditions feel better.

Abstract

Background: Nursing problems in brain injury patients include the risk of ineffective perfusion due to reduced oxygen flow to the brain, which, if not treated immediately, will increase intracranial pressure (ICP). One way to increase ICP is with 30-degree head-up position intervention. Objective: This study aimed to analyze the application of 30-degree Head Up Position on Cerebral Perfusion in Moderate Brain Injury Patients. Method: This research is an observational study with a case study design. Primary data was obtained through anamnesis and physical assessment with a sample of a patient being treated in the Hospital by assessing the effect of the 30-degree head-up position carried out for 30 minutes for 3 consecutive days. Results: In this study, the results showed an increase in cerebral perfusion as indicated by cerebral perfusion indicators, namely blood pressure, MAP, pulse, patient breathing improved, and the patient's level of consciousness increased until the final evaluation. Conclusion: From the results of the innovation analysis in this case, providing a 30-degree head-up position in patients with a risk of ineffective cerebral perfusion is one form of effectiveness in increasing the patient's cerebral perfusion so that it can prevent an increase in intracranial pressure.
