Abstract

Background: Human Immunodeficiency Virus (HIV) infection causes Acquired Immune Deficiency Syndrome (AIDS), a collection of symptoms arising from the deteriorating immunity. The number of people living with HIV worldwide continues to increase year-by-year. Those infected with HIV need antiretroviral (ARV) therapy to suppress the amount of HIV in the body. Purpose: This study aimed to investigate the determinants of adherence in people living with HIV/AIDS (PLHIV) undergoing ARV therapy in Situbondo Regency. Methods: This study employed a quantitative analytic observational approach with a cross-sectional design. Data analysis involved both descriptive and multivariate analysis. The population for this study consisted of 328 individuals, from which 212 were selected as samples, all being PLHIV on ART as of April 2022, sourced from ARV treatment services in Situbondo district. Results: The research analysis was based on predisposing factors, measured using three indicators: work, knowledge, and belief in ARVs. Supporting aspects were measured by two factors: ownership of health insurance and adherence counselling. Reinforcing factors were measured using two indicators: family support and stigma in the community. Conclusion: The study concluded that predisposing and reinforcing factors significantly influenced PLHIV adherence to antiretroviral therapy. However, supporting factors did not show a significant influence on adherence in PLHIV undergoing antiretroviral therapy in the Situbondo District.

Abstract

Therapy slow deep breathing exercise and acupressure is a complementary therapy that aims to reduce the symptoms of the disease to improve the quality of individual health. Complementary therapy is rarely done in hospitals so that the symptoms felt by patients are not reduced without medication. This therapy can reduce the symptoms of shortness of breath in patients who specifically experience impaired breathing patterns that are not effective. The study aims to determine the effectiveness of combination therapy slow deep breathing exercises and acupressure to address ineffective breathing pattern nursing problems in nursing care. This type of research is a case study using the nursing care method. The research sample was patients treated in the Gardena room at dr. Soebandi General Hospital Jember. Patients were given nursing interventions in the form of a combination of slow deep breathing exercise therapy and acupressure for 1-2 times a day for 3 days. Providing nursing interventions in the form of a combination of slow deep breathing exercise therapy and acupressure can overcome ineffective breathing pattern nursing problems. This is indicated by the improvement in the patient's condition that the shortness of breath is reduced, the frequency is improving, not using the auxiliary muscles of breathing, and the depth of breathing is improving. Patients can also carry out the therapy independently properly and correctly. A combination of slow deep breathing exercise therapy and acupressure can overcome the problem of ineffective breathing pattern nursing.

Abstract

Physical activity exercise is an essential component in comprehensive diabetes management. Non-adherence to physical activity exercises can lead to poor metabolic control, causing increased complications, morbidity, and premature death due to diabetes. One of the factors that can determine the behavior of physical activity is the psychosocial factor of people with T2DM. The study aims to assess the effect of psychosocial factors on physical activity behavior. This study was an analytical observational with a cross-sectional approach. The sample size was 138 respondents with multistage random sampling. The dependent variable was physical activity behavior, and the independent variables were psychosocial factors (knowledge, coping, distress, perceptions of family support, and perceptions of nurse support). Data analysis used multiple linear regression (α=0.05). Psychosocial factors that influenced the physical activity behavior were distress (p=0.001 < α=0.05). Meanwhile, knowledge, coping, perceived family support, and perceived nurse support did not affect physical activity behavior (p=0.684; p=0.919; p=0.235; p=0.108 > α=0.05) with Adjusted R2=0.113 (F=4.502; p=0.001 < α=0.05). Psychosocial factors that influence physical activity behavior in T2DM patients are diabetic distress. Therefore, health workers, in providing health services as much as possible, must be able to prevent and reduce diabetes distress to increase the physical activity behavior of people with T2DM.

Abstract

Exhausted nurses in providing nursing care can impact work stress. Many factors cause work stress. Work stress is the type of psychological stress that reduces the lacrimal gland's activity, affecting dry eye syndrome. The study aims to analyze the correlation between work stress and nurses' dry eye syndrome in Nganjuk General Hospital. The study was a descriptive correlative observational with a cross-sectional design. The sample was 84 nurses, taken by simple random sampling. Data were collected using Health and Safety Executive (HSE) and Ocular Surface Disease (OSDI) questionnaires. The study showed nurses experienced mild stress (96.43%) and moderate dry eye syndrome (36.90%). There was a positive correlation between work stress and dry eye syndrome in nurses in the weak category (r=0.356; p= 0.001< 0.05). There were no significant differences in work stress by age, gender, working period, and marital status (p=0.388; p=0.687 p=0.730; p = 0.839 > α 0.05). There was a significant difference in work stress based on education (p=0.033 < α 0,05). There were no significant differences in dry eye syndrome with age, gender, working period, and marital status (p=0.644; p=0.436; p= 0.865; p=0.072; p=0.719 > α 0,05). The study conclusion is that work stress is related to dry eyes syndrome. Apart from that, work stress is also influenced by the level of education. Therefore, interventions to reduce work stress in nurses should pay attention to education level to reduce the risk of dry eyes syndrome.

Abstract

Abstrak Perawatan kaki sangat penting untuk menurunkan risiko ulkus kaki diabetik. Perilaku perawatan kaki sebagai bagian dari pengelolaan mandiri sangat ditentukan oleh faktor psikososial. Tujuan penelitian adalah menganalisis faktor psikososial yang mempengaruhi perilaku perawatan kaki penderita diabetes mellitus tipe 2.  Penelitian ini merupakan penelitian kuantitatif dengan desain cross-sectional. Besar sampel adalah 138 responden dengan teknik multistage random sampling. Variabel penelitian adalah faktor psikososial (pengetahuan, koping, distres, dukungan keluarga, dan dukungan perawat) sebagai variabel independen dan perilaku perawatan kaki sebagai variabel dependen. Instrumen penelitian berupa kuesioner, yaitu SKILLDS, Coping Scale, DDS, HDFSS, persepsi peran perawat. dan NAFF. Hasil penelitian menunjukkan bahwa sebagian besar responden berusia 56-65 tahun (48,5%), perempuan (64,4%), pendidikan dasar (39,1%) dan penghasilan rendah (60,1%). Sebagian besar mempunyai pengetahuan, koping, persepsi dukungan keluarga, dan dukungan perawat dalam kategori sedang (45,6%; 67,3%, 53,6%, dan 63,7%), sedangkan distres dalam kategori tinggi (37,6%). Perilaku perawatan kaki dalam kategori sedang (63,0%). Hasil analisis data dengan regresi linier berganda didapatkan bahwa model fit (F=2,419; p=0,039; R2=0,049). Hasil uji partial menunjukkan bahwa hanya pengetahuan yang berpengaruh signifikan terhadap perilaku perawatan kaki (p=0,035). Upaya peningkatan perilaku perawatan kaki dalam pengelolaan mandiri dapat ditingkatkan dengan meningkatkan pengetahuan pasien dalam melakukan perilaku perawatan kaki secara mandiri. The study aimed to analyze the influence of psychosocial factors on the foot care behavior of people with T2DM.  The study was a quantitative study with a cross-sectional design. The sample size was 138 respondents with a multistage random sampling technique. The psychosocial factors (knowledge, coping, distress, family support, and nurse support) were independent variables, and foot care behavior was the dependent variable. The research instruments were questionnaires (SKILLDS, coping scale, DDS, HDFSS, nurse's role perception, and NAFF). Most respondents have the knowledge, coping, family support perceived, and nurse support perceived in the moderate category (45.6%; 67.3%, 53.6%, and 63.7%), and distress was in the high category (37.6%). In contrast, foot care behavior was in the moderate category (63.0%). The results of data analysis with multiple linear regression found that the model fit (F=2.419; p=0.039; R2= 0.049) and only knowledge significantly affected foot care behavior (p=0.035). Efforts to improve foot care behavior in self-management can be increased by increasing patient knowledge in performing foot care behavior independently. Abstract

Abstrak Salah satu indikator kualitas pelayanan di Unit Gawat Darurat adalah angka kematian. Salah satu penyebab tingginya angka mortalitas adalah perilaku waktu tanggap petugas. Penelitian ini bertujuan untuk mengetahui pengaruh pelatihan dan penghargaan terhadap perilaku waktu tanggap melalui efikasi diri. Metode penelitian yang digunakan adalah cross sectional. Sampel penelitian adalah perawat IGD rumah sakit tipe B di Jawa Timur sebanyak 102 orang dengan teknik pengambilan sampel total sampling. Variabel penelitian terdiri dari pelatihan, penghargaan, perilaku waktu tanggap, dan efikasi diri. Instrumen penelitian menggunakan kuesioner. Analisis data menggunakan analisis deskripsi, dan analisis inferensial menggunakan Structural Equation Model (SEM). Hasil penelitian menunjukkan bahwa mayoritas responden berusia 18-40 tahun dengan persentase 75%, sebagian besar responden berjenis kelamin laki-laki dengan persentase 69,6%, dan pendidikan terakhir yang paling banyak adalah perawat dengan persentase 64,7%. Hasil uji hipotesis menggunakan SmartPLS menunjukkan bahwa pelatihan berpengaruh positif terhadap perilaku waktu tanggap melalui efikasi diri dengan nilai p-values sebesar 0,011. Sementara itu, penghargaan berpengaruh positif terhadap perilaku response time melalui self-efficacy dengan nilai p-values sebesar 0,004. Upaya peningkatan efikasi diri dilakukan untuk meningkatkan perilaku waktu tanggap petugas. Kata Kunci: pelatihan, penghargaan, self efficacy, perilaku response time   Abstract One of the indicators of service quality in the Emergency Department is mortality. One of the causes of the high mortality rate is the response time behavior of officers. This study aimed to identify the effect of training and appreciation on response time behavior through self-efficacy. The research method used was cross-sectional. The research sample was emergency room nurses of type B hospitals in East Java as many as 102 people with total sampling technique. Variables of the study consist of training, awards, response time behavior, and self -efficacy. The research instrument used a questionnaire. Data analysis applied description analysis, and inferential analysis used Structural Equation Model (SEM). The results showed that the majority of respondents aged 18-40 years old with 75%, most of the respondents were male with 69.6%, and the common education was nurse with 64.7%. The results of hypothesis test using SmartPLS showed that training has a positive effect on response time behavior through self-efficacy with p-values of 0.011. Meanwhile, the award has a positive effect on response time behavior through self-efficacy with p-values of 0.004. Abstract

Abstrak Burnout pada caregiver dapat muncul sebagai reaksi dari peran, tanggung jawab, dan kurang optimalnya caregiver keluarga dalam merawat penderita T2DM.  Burnout yang terjadi mengindikasikan kurangnya sumber daya dalam penatalaksanaan T2DM yang menyebabkan kemampuan keluarga tidak memadai. Penelitian ini bertujuan untuk menganalisis pengaruh intervensi Family Caregiver Empowerment Model (FCEM) terhadap Burnout pada Caregiver pasien Diabetes Melitus tipe 2. Penelitian ini merupakan penelitian quasi eksperimental dengan desain pretest dan posttest dengan sampel sebanyak 74 responden, secara acak menggunakan cluster sampling (perlakuan = 35, kontrol = 39). Variabel independen adalah FCEM, dan variabel dependen adalah burnout. Instrumen yang digunakan adalah kuesioner Maslach Burnout Inventory (MBI-HSS). Analisis statistik menggunakan Wilcoxon signed rank test dan Mann Whitney. Hasil penelitian menunjukkan bahwa pada kelompok intervensi dan kelompok kontrol terdapat perbedaan skor burnout yang signifikan pada saat pretest dan posttest (p=<0.001 p=0.002 < α =0.05). Terdapat perbedaan yang signifikan pada skor burnout antara kelompok intervensi dan kelompok kontrol (p=<0,001). Intervensi FCEM dapat mengurangi depresi pada penderita T2DM. Perawat dapat menggunakan intervensi FCEM untuk meningkatkan kapasitas keluarga dalam perawatan diri diabetes untuk mengurangi burnout pada caregiver pasien T2DM.    Kata kunci: burnout, pemberdayaan, pengasuh keluarga, diabetes mellitus tipe 2   Abstract Burnout in caregivers can arise as a reaction to the role, responsibility, and less-than-optimal family caregivers in caring for people with T2DM. Burnout may indicate a lack of resources in managing T2DM, which can result in inadequate family abilities. This study aims to analyze the effect of the Family Caregiver Empowerment Model (FCEM) intervention on Burnout in Caregivers of type 2 Diabetes Mellitus patients. The study was a quasi-experimental pretest and posttest design with a sample of 74 respondents, randomly using cluster sampling (treatment = 35, control = 39). The independent variable is FCEM, and the dependent variable is burnout. The instrument used was the Maslach Burnout Inventory (MBI-HSS) questionnaire. Statistical analysis used Wilcoxon signed rank test and Mann Whitney. The results showed that in both the intervention and control groups, there was a significant difference in burnout scores at the pretest and posttest (p=<0.001 p=0.002 < α =0.05). There was a significant difference in burnout scores between the intervention and control groups (p=<0,001). FCEM intervention could reduce depression in people with T2DM. Nurses can use FCEM interventions to increase family capacity in diabetes self-care and reduce burnout in caregivers of patients with T2DM. Abstract

Background: Stroke is a neurological disorder frequently associated with reduced cerebral perfusion and impaired consciousness. While conventional interventions are widely implemented, their effectiveness may be enhanced by evidence-based approaches such as Familiar Auditory Sensory Training (FAST), a non-pharmacological intervention that utilizes meaningful auditory stimuli to activate neural pathways and support consciousness recovery. Objective: This study aims to evaluate the effectiveness of FAST in addressing the nursing problem of risk for ineffective cerebral tissue perfusion in an ischemic stroke patient. Methods: A case study was conducted involving a 73-year-old male patient in a soporous state. FAST was administered over three consecutive days, three times daily, with each session lasting 10 minutes. Data were collected through direct observation and Glasgow Coma Scale (GCS) measurements before and after each session. Patient outcomes were evaluated using the Nursing Outcomes Classification (NOC), focusing on indicators including consciousness level, blood pressure, and agitation. Results: The patient’s level of consciousness improved from sopor (6) to somnolence (9). Improvements were also noted in blood pressure, heart rate, and respiratory rate, although an increase in restlessness was observed. FAST supported cerebral responsiveness and cognitive function recovery, contributing to the resolution of the nursing problem: risk for ineffective cerebral tissue perfusion. Conclusion: FAST can be utilized as a therapeutic nursing intervention to enhance hemodynamic stability in patients with decreased consciousness

Abstract

ABSTRACT Ventilator-Associated Pneumonia (VAP) is a common nosocomial infection in ICUs, contributing significantly to morbidity, mortality, and healthcare costs. Implementing VAP bundles has shown effectiveness in reducing VAP incidence, but its success depends on various internal and external factors. This study employed a literature review with a quantitative approach. Data were gathered from 27 relevant articles published between 2013 and 2023, selected from an initial 231 articles obtained through structured searches in Google Scholar, PubMed, Scopus, and ScienceDirect using Boolean keywords such as “Ventilator-Associated Pneumonia” AND “VAP bundle” AND “Compliance.” Analysis revealed that internal factors, including medical staff knowledge and competency, significantly affect compliance with VAP bundles. Challenges such as insufficient continuous training and limited resources were highlighted. External factors, such as regular audits, managerial support, and a conducive work environment, were identified as critical in supporting successful VAP bundle implementation. Addressing internal challenges through structured training programs and resource allocation is essential. Simultaneously, external support mechanisms like frequent audits and strong managerial backing are necessary to ensure sustained compliance with VAP bundles. An integrative approach that encompasses intensive training, routine audits, and reinforced institutional policies is vital for the effective implementation of VAP bundles, thereby improving ICU patient care quality. Keywords: Ventilator-Associated Pneumonia, VAP Bundle, Internal Factors, External Factors, Protocol Compliance, ICU.  ABSTRAK Ventilator-Associated Pneumonia (VAP) merupakan infeksi nosokomial yang umum terjadi di ruang ICU, yang berkontribusi secara signifikan terhadap morbiditas, mortalitas, dan biaya perawatan. Implementasi bundle VAP terbukti efektif dalam menurunkan angka kejadian VAP, namun keberhasilannya bergantung pada berbagai faktor internal dan eksternal. Penelitian ini menggunakan metode tinjauan literatur dengan pendekatan kuantitatif. Data dikumpulkan dari 27 artikel relevan yang diterbitkan antara tahun 2013 hingga 2023, diseleksi dari 231 artikel awal yang diperoleh melalui pencarian terstruktur di Google Scholar, PubMed, Scopus, dan ScienceDirect dengan kata kunci Boolean seperti “Ventilator-Associated Pneumonia” AND “VAP bundle” AND “Compliance.” Analisis menunjukkan bahwa faktor internal, termasuk pengetahuan dan kompetensi staf medis, secara signifikan memengaruhi kepatuhan terhadap bundle VAP. Hambatan seperti kurangnya pelatihan berkelanjutan dan keterbatasan sumber daya menjadi tantangan utama. Faktor eksternal seperti audit rutin, dukungan manajerial, dan lingkungan kerja yang kondusif diidentifikasi sebagai elemen penting dalam mendukung keberhasilan implementasi bundle VAP. Mengatasi tantangan internal melalui program pelatihan terstruktur dan penyediaan sumber daya sangat diperlukan. Di sisi lain, mekanisme dukungan eksternal seperti audit berkala dan dukungan manajerial yang kuat juga esensial untuk memastikan kepatuhan berkelanjutan terhadap bundle VAP. Pendekatan integratif yang mencakup pelatihan intensif, audit rutin, dan penguatan kebijakan institusional sangat penting untuk implementasi bundle VAP yang efektif, sehingga meningkatkan kualitas perawatan pasien di ruang ICU. Kata Kunci: Ventilator-Associated Pneumonia, Bundle VAP, Faktor Internal, Faktor Eksternal, Kepatuhan Protokol, ICU.

Abstract

Several studies indicate a significant increase in diabetes and its complications in agricultural regions. Jember is an agricultural area with a high prevalence of diabetes and a heightened risk of Diabetic Foot Ulcer (DFU). Reducing aggressive risk factors involves a self-efficacy theory based program. This intervention aimed to evaluate the program's effectiveness on foot care behaviors, self-efficacy in self-care foot management, and the risk of DFU occurrence. The study spanned over six months, consisting of three stages: program introduction, health education delivery, and program evaluation. It employed a Pre-Experimental Design with a Group Pretest-Posttest approach. Sample selection was done using simple random sampling, resulting in a sample of 28 research participants. The statistical test used was the Wilcoxon Test. The study's results showed significance values for all three variables < 0.001, signifying the influence of the intervention. The risk of diabetic foot ulcers had a negative rank, indicating a decrease in the risk of DFU after the intervention, with an average reduction value of 13.00. Foot Self-Care Behavior showed a positive rank of 14.50, signifying an improvement in Foot Self-Care Behavior following the intervention. Foot Care Confidence displayed a positive rank, indicating an increase in Foot Care Confidence after the intervention, with an average improvement of 14.50. Consequently, nurses can implement a self-efficacy-based foot care education program to reduce the risk of diabetic ulcers by enhancing self-care behaviors and self-efficacy in independent foot care.
