Abstract

Chronic Kidney Disease (CKD) is a significant health problem with increasing prevalence, characterized by a progressive decline in kidney function resulting in fluid accumulation in the body. One common clinical manifestation is edema in the lower extremities. Ankle pump exercises and foot massage are non-pharmacological interventions that can help reduce edema by improving venous and lymphatic circulation. This study aims to analyze the implementation of nursing care in relation to the effect of ankle pump exercise and foot massage on reducing leg edema in patients with chronic kidney disease (CKD). This study used a case study design with a nursing care approach in a CKD patient with edema in the Anturium Ward of Dr. Soebandi Regional General Hospital, Jember. Ankle pump exercises were performed for 10 minutes, followed by a 15-minute foot massage, which was repeated for three consecutive days of treatment. The degree of edema was evaluated using the edema pitting scale before and after the intervention. The results showed a decrease in edema from +2 to +1 (1 mm in the left leg) and from +1 to 0 (2 mm in the right leg). There was also a decrease in ankle circumference of 20 cm and instep of 25.5 cm on the right leg, and 19 cm at the ankle and 23.7 cm at the left leg. Furthermore, muscle strength increased from 2 cm to 5 cm in the left leg and from 1 cm to 4 cm in the right leg. Ankle pump exercises and foot massage effectively reduce edema in CKD patients. This therapy can be used as an independent nursing intervention to enhance the quality of life for patients with CKD who experience edema.

Abstract

Hypertension is a common health problem among the elderly, and its management requires high self-care motivation. Family empowerment has been shown to have a significant effect on increasing self-care motivation in hypertensive patients. The purpose of this study was to analyze the effect of family empowerment on self-care motivation in hypertensive patients based on the results of a literature review. Methods: The literature study was conducted using the Sciencedirect, Pubmed, and Scopus databases, with articles published between 2018 and 2024. The keywords used included "Family Empowerment", "Self-care", "Hypertension", and "Elderly". Results: Of the seven articles identified, the majority showed that family empowerment, either through direct intervention or using a digital platform, contributed to increased self-efficacy, adherence to treatment, and better blood pressure management in hypertensive patients. In addition, family support in the form of education and motivation plays an important role in creating an environment that supports self-care. Conclusion: Family empowerment plays a major role in increasing the motivation of hypertensive patients to carry out effective and sustainable self-care. Therefore, interventions involving family empowerment can be an effective strategy in managing hypertension, especially in the elderly..

Abstract

Work stress among farmers remains an overlooked occupational health issue in Indonesia despite high vulnerability to economic and environmental pressures. This study aimed to map farmers’ stress levels and identify demographic factors influencing stress. A quantitative descriptive design was used with 140 farmers selected through purposive sampling in Jember Regency. Stress was measured using the DASS-42 stress subscale, and data were analysed descriptively and through multiple linear regression. Results showed that 62.9% of farmers experienced mild stress, 30.7% moderate stress, and 6.4% severe stress. Regression analysis demonstrated that gender (p=0.012), educational attainment (p=0.005), family role (p=0.030), number of family members (p=0.025), land size (p=0.008), and monthly income (p=0.001) significantly influenced stress levels. Female farmers, individuals with low education, heads of households, farmers with more than four family members, land ≤ 0.65 ha, and income < Rp 1,916,983 had markedly higher stress scores.  The study concludes that farmer stress is shaped by socioeconomic vulnerability and limited resources. These findings highlight the need for targeted interventions, including capacity-building programs, improved access to agricultural support, and policies that enhance economic stability to reduce work-related stress among Indonesian farmers

Abstract

Background: Improper diet arrangements in type 2 diabetes (T2D) patients can cause eating patterns to be disturbed in the form of binge eating behavior. The family can play a helping role in improving diabetes management. One of the interventions that can be given to empower families is the Family Caregiver Empowerment Model (FCEM). This study aimed to analyze the effect of FCEM on binge eating in T2D patients. Methods: This study was a randomized control group pre-test and post-test design with a total sample of 85 respondents taken by cluster random sampling, divided randomly (treatment = 41; control = 44). The independent variable is FCEM, while the dependent variable is binge eating behavior. Data was collected using the Indonesian version of the Binge Eating Scale questionnaire and analyzed used the Wilcoxon sign rank test and independent T-test. Results: There were differences in binge eating behavior in the treatment group before and after intervention (Z= -5.347; p=0.001), whereas in the control group, there was no difference in pre-test and post-test scores (Z= -0.599; p= 0.549). The study result showed that there was a significant difference in the average value between the treatment and control groups (t =5.017; p=0.001). Conclusion: FCEM intervention can reduce binge eating behavior to increase adherence to the diabetes diet program. Nurses can use FCEM intervention to empower family caregiver to help T2D patients improve and maintain eating habits to improve diabetes self-care behaviors.

Abstract

Diabetes Mellitus (DM) is a chronic disease that requires long-term and consistent monitoring to prevent complications. However, low adherence to follow-up visits remains a significant challenge in primary healthcare settings, including at Cakranegara Public Health Center in Mataram City. Based on data from January to May 2025, only 62.6% of the total 957 DM patients returned for scheduled follow-up visits. This community service initiative aimed to implement and analyze the effectiveness of a digital innovation, SI-KEPPO (Educational Control Information System), in improving follow-up adherence among DM patients. The methods included needs assessment and the development of an Android-based SI-KEPPO application. Evaluation was conducted through observations and interviews with program officers and patients. The results demonstrated improved follow-up compliance during the 4-day trial period. Patients reported that the application made it easier to remember visit schedules and made them feel more cared for through regularly delivered educational messages. The SI-KEPPO application also enhanced family involvement in patient support and facilitated monitoring of non-compliant patients by program officers. In conclusion, the implementation of the SI-KEPPO innovation effectively increased follow-up visit adherence among DM patients, strengthened digital approaches in community health services, and is recommended for integration into the Prolanis management system at the primary healthcare level.

Abstract

Low adherence of patients with Non-Communicable Diseases (NCDs) and the elderly to scheduled follow-up visits remains a major challenge in primary healthcare services. This issue contributes to delayed detection of complications and reduced effectiveness in managing chronic conditions. This community service program aimed to evaluate the effectiveness of the TERKONTROL application—an automated WhatsApp-based reminder system designed to improve the regularity of patient follow-up visits. The program was implemented at Dasan Agung Public Health Center using a Participatory Action Research (PAR) approach, actively involving program officers, academic facilitators, and patients. The desktop-based application features patient data input, visit scheduling, a monitoring dashboard, and an automated reminder system. Implementation results showed that out of 16 patients who received reminder messages, 13 attended their scheduled visits, achieving a compliance rate of 81.3%. Evaluation revealed positive perceptions from both patients and healthcare staff regarding the application's usability and usefulness. These findings suggest that simple digital innovations such as TERKONTROL (Tertib Kontrol dan Respon Lanjut Pasien PTM & Lansia) can serve as effective tools for real-time patient monitoring, program reporting enhancement, and improved communication in primary healthcare settings.

Abstract

Happiness at work (HAW) plays a critical role in nurses’ performance and their ability to deliver compassionate care, as it affects nurses’ motivation, empathy, and caring behavior. However, culturally grounded interventions to improve nurses’ happiness, particularly within Islamic spiritual frameworks, remain underexplored. Therefore, this study examined the effect of an Islamic-based spiritual program based on local religious and cultural values on nurses’ HAW. This research employed a quasi-experimental design with a non-equivalent control group. The intervention included a structured religious talk on the meaning of life and daily listening to Quranic recitations (murottal) during work for four weeks. The study involved 64 inpatient nurses from hospitals. The participants were assigned to an intervention group (n = 32) and a control group (n = 32). Data were collected using the Shortened HAW questionnaire and analyzed using repeated-measures ANOVA and the Kruskal–Wallis test. The results showed that happiness scores increased significantly in the intervention group (mean difference = 1.53, p < 0.001; η² = 0.354). In contrast, the control group showed minimal change. These findings indicate that Islamic-based spiritual programs can effectively enhance nurses’ happiness at work and may contribute to nurse retention, strengthen caring values, and improve healthcare quality.

Abstract

Introduction: Compliance with monitoring blood glucose levels is crucial to diabetes management. Self-monitoring behaviour of blood glucose levels can be influenced by psychosocial factors in people with T2DM, such as knowledge, coping, diabetes distress, family support, and nurse support. Objective: The study aimed to analyze psychosocial factors that influence the behaviour of self-monitoring of blood glucose levels in people with T2DM. Methods: The study was a descriptive-analytic study with a cross-sectional design. The sample was T2DM patients in the Jember Regency with a sample size of 138 respondents, taken using multistage random sampling. Data were collected using a questionnaire (SKILLDS, Coping Scale, DDS, HDFSS, Nurse's role perception, and the SMBG questionnaire) and analyzed using a multiple linear regression test. Results The modelling results show that psychosocial factors (knowledge, coping, diabetes distress, family support, and nurse support) have a simultaneous and significant influence on the behaviour of monitoring blood glucose levels (F = 9.292; p = 0.001 < α = 0.05) with adjusted R2 = 0.232. It showed that psychosocial factors could explain the variance of behaviour monitoring blood glucose levels by 23.2%. Partially, there are three influential variables, namely coping, perception of family support, and perception of nurse support (p= 0.039; p= 0.016; and p= 0.020 < α =0.05). Better coping, family support, and nurse support could improve blood glucose monitoring. Conclusion: Psychosocial factors can affect monitoring blood glucose levels in people with T2DM. The nurse can make efforts to improve the behaviour by monitoring blood glucose levels by considering the psychosocial aspects of people with T2DM through preventive promotion efforts to improve coping, family support, and nurse support to improve self-management and quality of life of people with T2DM.

Abstract

Coastal areas in Jember Regency have a high risk of getting a big impact if an earthquake and tsunami. These potential disasters force all elements to be prepared to face disasters that can arise anytime, including nurses in the Coastal Public Health Center of Jember. Self-efficacy is thought to have a relationship in improving nurse preparedness in dealing with disaster situations. The aim of this research is to analyze and describe the relationship between the self-efficacy of coastal nurses and the level of preparedness of nurses in dealing with earthquakes and tsunamis in Jember. This study was a correlational research study with a cross-sectional approach. The population of this study was 161 nurses. The sampling method used was convenience sampling, with the total number of nurses participating in this study was 144. We used two main questionnaires to collect the data: the General Self-Efficacy (GSE) and the Disaster Preparedness Evaluation Tool (DPET). We analyzed the data using Pearson Product moment with a significant level (α) =0.05. We found a significant correlation between the disaster preparedness and self-efficacy of the coastal nurse in Jember with a p-value of 0,008 (< 0.05). The degree of relationship was 0.619, which is considered significant and positive. There is a relationship between self-efficacy and nurse preparedness in dealing with disasters. The association is in a positive direction, meaning that the higher the level of self-efficacy, the higher the nurse's disaster preparedness. Abstract

Self-monitoring of blood glucose (SMBG) is important in diabetes self-management to achieve better HbA1c control and decrease complications, morbidity, and mortality in people with type 2 diabetes mellitus (T2DM). SMBG behavior can be influenced by various factors, one of which is sociodemographic factors. This study aimed to analyze the sociodemographic factors that influence the behavior of self-monitoring blood glucose in patients with T2DM. The cross-sectional study was conducted in ten health centers in Jember Regency with a sample size of 130 respondents using multistage random sampling. The independent variable is sociodemographic factors (age, gender, marital status, education level, income level, family type, and ethnicity), while the dependent variable is SMBG behavior. The instrument used was the Self-Monitoring Blood Glucose Questionnaire (SMBG-Q). Data were analyzed by multiple logistic regression. Most respondents had SMBG behavior in the good category (64.6%). Sociodemographic factors simultaneously had an influence on SMBG behavior (p = 0.001 < α = 0.05; R2 = 0.324). The sociodemographic factor that significantly affects SMBG behavior partially is ethnicity (p = 0.001 < α = 0.05). Meanwhile, age, gender, marital status, education level, income level, and family type did not have a significant partial effect (p = 0.095; p = 0.149; p = 0.083; p = 0.359; p = 0.507; p = 0.152 > α = 0.05). The sociodemographic factor that influences SMBG behavior is ethnicity. Therefore, efforts to improve SMBG behavior must pay attention to values and culture to improve the self-monitoring blood glucose behavior of people with T2DM.
