Abstract

Latar Belakang: Sepsis adalah masalah kesehatan global, yang ditandai adanya disfungsi organ disebabkan oleh disregulasi respon inang dalam menanggapi infeksi mikroba. Insidensi dan mortalitas pasien sepsis yang dirawat di ICU masih cukup tinggi. Diagnosis dinisangat diperlukan untuk pengobatan yang efektif dan menghindari penggunaan antibiotikyang tidak perlu. Procalcitonin adalah biomarker yang menunjukkan nilai diagnostik yang lebih baik daripada penanda proinflamasi lainnya dalam mengidentifikasi pasien dengan sepsis dan dapat digunakan dalam diagnosis infeksi bakteri. Tujuan dari penelitian ini adalah untuk menentukan sensitivitas dan spesifisitas prokalsitonin sebagai biomarker pada pasien sepsis.Metode: Desain penelitian ini adalah retrospektif dengan menggunakan data rekam medis pada pasien yang dirawat di ICU RSUP Dr. Sardjito Yogyakarta periode Januari – Desember 2018. Uji diagnostik dilakukan pada pasien sespsis dan non-sepsis yang mengalami disfungsi organ sesuai dengan kriteria skor SOFA. Cutoff point procalsitonin ditentukan menggunakan analisis receiver operating characteristic (ROC). Analisis data dilakukan untuk mengetahui sensitivitas, spesisifitas, nilai duga positif, nilai duga negatif procalcitonin pada pasien sepsis.Hasil: Pada uji diagnostik procalcitonin pada pasien sepsis dengan cut-off point 3,27 ng/ml, didapatkan hasil sensitivitas 89,0%, spesifisitas 90%, nilai duga positif 90,1% dan nilai duga negatif 88,9%. Pada analisa ROC procalcitonin terhadap sepsis, didapatkan AUC 0,941 (AUC > 0,9).Kesimpulan: Procalcitonin juga memiliki nilai diagnostik yang baik sebagai biomarker pada pasien sepsis yang dirawat di ICU RSUP Dr. Sardjito. Procalcitonin memiliki kemampuan diskriminasi sangat kuat untuk pasien sepsis.

Abstract

Perawatan multi modal yang memastikan oksigenasi optimal dan hemodinamik stabil denganjumlah cairan minimal tampaknya bisa dibenarkan. Dengan tidak adanya pemahaman lengkap tentang patogenesis edema pulmo, untuk mengurangi komplikasi ibu dan janin, manajemen terutama didasarkan pada pengobatan gejala dan tanda-tanda efek sekunder preeklamsia. Manajemen pasien ini idealnya harus multidisiplin dan ahli anestesi harusterlibat dalam perawatan pasien sejak tahap awal. Asesmen awal yang cermat oleh ahlianestesi direkomendasikan untuk semua pasien dengan preeklamsia, terutama dalam kasus yang berat. Kecuali ada kontraindikasi yang diketahui sebelumnya, seperti koagulopati, anestesi regional direkomendasikan untuk pasien preeklamsia. Mengingat risiko anestesi umum pada pasien dengan preeklamsia, anestesi spinal harus dipertimbangkan dalam kasus-kasus urgensi tanpa mengesampingkan anestesi epidural. ketika diperlukan anestesi umum, sangat penting untuk mengantisipasi dan mengobati tekanan darah yang tidak stabil. Jugapenting untuk dipersiapkan untuk manajemen jalan nafas ibu yang sulit, mengingat peningkatan risiko edema faringolaringeal.

Abstract

Opioids have become the flagship agents of modern perioperative care and pain management. The word opium comes from the Greek opion (“juice poppy”) or opium poppy (Papaver somniferum) is the source of 20 different alkaloids. Recent studies have shown that the role of opioid receptors on immune function is complicated, but they work through a variety of different mechanisms. Administration of various opioids shows different effects on the immune system: immunosuppressive, immunostimulatory, or both. Previously it was believed that most opioids suppress the immune system, but recent research suggests they may have a dual effect. However, the mechanism by which opioids and opioid receptors work in the immune system is still not clearly understood.

Abstract

A 32-year-old woman, G3P2A0, at 34 weeks of gestation came to Sardjito hospital emergency department with complaints of not being able to speak and right limb weakness since 8 hours before entering the hospital. Patient have hypertension since before pregnancy. CT-scan result obtain bleeding in the parietal lobes sinistra and basal nuclei with a bleeding volume 60 cc accompanied by midline shifting. Patient was diagnosed with superimposed severe preeklamsia, coma cum dekstra lateralization, and spontaneous intracerebral hemmorhage. Section cesarean and craniectomy hematoma evacuation then was performed. Postoperatively, patient was treated in the ICU and then transferred to the stroke unit.

Abstract

The patient is a 16 year old male who suffered a cervical injury due to a slipping fall. Pain was felt in the neck, followed by weakness in the limbs, but did not immediately go to the hospital. The patient was referred to Sardjito Hospital about 1 month after the incident, with examination at the previous hospital the patient had a cervical injury with tetraparesis.The patient underwent corpectomy, decompression, and stabilization surgery on the second day. Postoperatively the patient was admitted to the ICU. On the second day of treatment, extubation was carried out, but was later re-intubated because the patient had respiratory distress. The next therapy was physiotherapy, nebulizer and mucolytic, ventilator weaning with pressure support mode and progressive ventilator free breathing (PVFB), Percutaneous Dilatation Tracheostomy (PDT), nutritional therapy, analgesics and antibiotics. On the way, the patient experienced pulmonary atelectasis, sepsis, and several failures in the ventilator weaning trial, so PDT was performed. The patient then experienced an improvement in his condition which was characterized by an increase in lung capacity, extremity muscle strength, and improvement in the condition of sepsis. Until finally the patient was able to be released from the use of the ventilator on the 15th day of treatment by using a T-piece through atracheostomy tube.

Abstract

The development of the e-commerce business is an opportunity for logistics service providers as third parties who deliver goods sold by the seller to the location intended by the buyer. This study aims to analyze the behavior of fashion e-commerce customers in choosing the best vendor. Data were collected by in-depth interviews with five selected informants for vendor criteria and sub-criteria, then a survey was conducted on 100 respondents. The data were analyzed using the Analytical Hierarchy Process method to determine criteria and sub-criteria in selecting the preferred vendor. The first criterion was reliability of delivery, with the sub-criteria of timeliness, accuracy of delivery destination, and security of the condition of the goods. The second criterion was price. The next criterion was products with regular delivery, express, and same-day sub-criteria. The last criterion was shipping features with its sub-criteria shipping tracking, wide delivery area, and customer service. For the case of Indonesia in this study, the best vendor in delivering products from e-commerce to customers is obtained by Si Cepat in the first place, followed by J&T and JNE in second and third places.

Abstract

This research aims to analyze the quality of professionalism, guide services related to shipping safety, ship security which influences the smoothness of ship traffic in Indonesia's main ports. Sea ports that serve ships for loading and unloading activities, both for inter-island and international transportation, are generally required to guide. This research method uses descriptive quantitative with data collection techniques using questionnaires distributed to scouts and stakeholders at the port. The R-Square value for smooth ship traffic is 0.762, indicating that the variables of professional pilotage, pilotage services, shipping safety and ship security are able to explain around 76.2% of the variation in smooth ship traffic. This means that the professional factors of pilots, pilotage services, shipping safety and ship security have a major contribution to the smoothness of ship traffic. The R-Square value for shipping safety is 0.626, indicating that the professional guide and guiding service variables are able to explain around 62.6% of the variation in shipping safety. This shows a moderate contribution to the smoothness of ship traffic. The R-Square value of guiding services is 0.425 indicating that guiding professionals explain around 42.5% of the variation in guiding services. This indicates that although there is a fairly strong relationship, there are other factors that also influence guiding services.  The R-Square value of ship safety is 0.580, the pilot professional and pilot service variables explain around 58% of the variation in ship safety. This shows quite a significant influence, indicating that professional pilotage and pilotage services are an important factor in ship safety.

Abstract

The main problem of this research is that there was an import customs settlement process that still requires the results of a surveyor company certified by the government to be attached to the Bonded Logistics Center warehouse outside the port, increasing the waiting time.This research analyzes the import handling process, government policies, and customs compliance in handling heavy equipment spare parts.This research used a qualitative analysis approach with purposive sampling with 10 informants from various companies.The research findings are that there was still a lack of government role in increasing the effectiveness of handling the import process of heavy equipment spare parts at the heavy equipment trading company, due to weaknesses in determining import restrictions.Steel heavy equipment spare parts were invalid because heavy equipment spare parts were made in factories in the home country of the heavy equipment maker. It is difficult to transfer technology to domestic production because the price of fabrication machines requires quite a high investment and high technical specifications. The company also experienced losses in terms of transportation costs

Abstract

Importing the specific rice could only be done by BUMN (Badan Usaha Milik Negara) which in this article represented by PT DFG. The company is still lack of supervision of the customer relationship management implementation due to the absence of the special department and also lack of supervision of distribution. The study used Path Analysis with 35 distributor companies as sample. The purpose of the research is to analyze, directly and indirectly, the variables of customer relationship management and supply chain management on customers’ loyalty of imported specific rice intervened by competitive advantage variables. The results of the study show that the competitive advantage cannot act as an intervening variable and has an indirect effect on customer relationship management of the rice commodity customers’ loyalty. On the other hand, the competitive advantage can act as an intervening variable and has an indirect effect on supply chain management of the specific rice customers’ loyalty. Thus, the changes of the supply chain management implementation in PT DFG can affect customers’ loyalty indirectly through competitive advantage.
