Abstract

Carpal Tunnel Syndrome (CTS) is a clinical syndrome that arises due to the depression of the median nerve in the carpal tunnel (tunnel carpal) on the wrist, which causes pain, numbness, and tingling in the distribution of median nerve. CTS risk factors include obesity, monotonous wrist activity, pregnancy, genetic offspring, and rheumatic inflammation. So it is necessary to handle proper and carried out holistically with a family medicine approach to achieve the success of therapy. The purpose of this study was to implement the service of an evidence-based medicine based family doctor in patients by identifying risk factors, clinical problems, and patient management based on a framework for solving patient problems with the Patient Centered and Family Approach approach. This study is a case report. The data obtained are primary data obtained through autoanamnesis, alloanamnesis, physical examination, and home visits and secondary data obtained from the patient's medical record. Patient Ny. T 56 years old comes with complaints of thumb, index finger, and middle finger of the right hand feels numb since one month ago. Patients also often complain of his fingers feel like shock and spread to the arms, until lately the patient cannot hold or hold objects. Patients work as washerwashers who work daily to wash and make the same movements repeatedly. Clinically the patient is diagnosed with CTS. Furthermore, management, intervention and evaluation are carried out. After holistic and comprehensive management, patients experience increased knowledge about their disease, and complaints of numbness in patients are reduced.

Abstract

Diabetes Mellitus is a metabolic disorder characterized by chronic hyperglycemia. Diabetic ulcer is a state of ulcer, infection, and/or tissue damage, which is associated with neurological disorders and peripheral vascular disease in the lower extremities. The family approach in the management of diabetic ulcers and Diabetes Mellitus helps to identify factors that influence both clinically, personally and psychosocially in the family so that management will be more comprehensive. The purpose of this study was to application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient problem-solving framework using a patient-centered and family approach. This study is a case report. Primary data were obtained through anamnesis (autoanamnesis), physical examination, supporting examination and physical environment. The assessment is based on a holistic diagnosis from the beginning, process, and end of the study qualitatively and quantitatively. The patient complained of an open wound on the leg, numbness on both left, frequent urination and easy thirst. The patient has been diagnosed with DM since 2019. The patient was diagnosed with DM based on history, physical examination and supporting examinations. Knowledge of patients and families about the disease they suffer is lacking. After the intervention, the patient showed clinical improvement and there was a change in behavior. In the quantitative assessment, there was an increase in all aspects assessed, namely knowledge, diet and physical activity. Diagnosis and management of these patients is carried out in a holistic and comprehensive manner, patient center, family appropriate with regular DM treatment according to EBM is needed in order to control blood sugar by changing the patient's behavior so as to prevent other complications from arising.

Abstract

Diabetes Mellitus is a degenerative disease with a high prevalence and increasing every year. There is a possibility that there will be an increase in the number of people with Diabetes Mellitus in the future, so that comprehensive management must be carried out which is expected to prevent complications and improve the quality of life of patients. Application of evidence-based medicine-based services to patients by identifying risk factors, clinical problems, and patient management based on a patient-centered problem-solving framework with a patient-centered, family-focused, and community-oriented approach. This study is a case report. Primary data were obtained through anamnesis, physical examination and home visits to assess the physical environment. The assessment is based on a holistic diagnosis of the initial, process, and final studies qualitatively and quantitatively. After the intervention, there was an increase in the knowledge of patients and their families regarding the importance of controlling health workers, taking medication regularly, eating patterns and physical activities that are suitable for people with Diabetes Mellitus. Holistic management has been carried out using the family doctor approach, Mrs. S, 52 years old with Type 2 Diabetes Mellitus adjusted based on initial holistic diagnostics. The interventions carried out have increased patient and family knowledge regarding the importance of control to health workers, regular drug consumption, dietary patterns and physical activity as indicated by improvements in the final holistic diagnostic.

Abstract

This study aims to analyze the effect of household characteristics, Kadarzi and environmental sanitation as risk factors for stunting in a comparative study of agricultural and non-agricultural families in South Lampung district. This study uses primary data obtained from questionnaires, that are data of family characteristics, Kadarzi and environmental sanitation from 198 household samples and secondary data, that are data of stunting toddlers and agricultural and non-agricultural areas in South Lampung district in 2022. To achieve the purpose of research, the research method used is observational analytic with a cross sectional approach. The research was conducted from July 2022 to September 2022. The results showed that on the household characteristics variable, variables that had a significant effect were family occupation and family income, while variables that had no significant effect were parents’ education, nutritional knowledge, and gender of children. On the Kadarzi variable, variables that had a significant effect were the behavioral variables of Kadarzi and exclusive breastfeeding, while variables that had no significant effect were variables of weight measurement, various foods, iodized salt and nutritional supplements. On the environmental sanitation variable, the variables that have a significant effect are family latrines, sewerage and agricultural and non-agricultural zones, while variables that have no significant effect are variables of clean water sources and trash bins.

Abstract

Nutrition in pregnancy is needed for fetal growth and development. Insufficient nutrition has the risk of causing Chronic Energy Deficiency (CED). The CED rate among pregnant women in Indonesia is 17.3% and in Lampung Province it is 13.6%. Nutritional status is influenced by nutritional intake and helminthiasis infection. Apart from that, there are other factors such as social determinants of health, namely economics, education, employment and age. This research uses a cross-sectional research design. The research sample was 86 pregnant women. Data collection was carried out using a questionnaire, measurement of Upper Arm Circumference (MUAC), and examination of feces using the formol ether sedimentation method. Bivariate analysis used chi square, Fisher, and Mann Whitney. The results of the study showed that there was a significant relationship between helminthiasis (p = 0.009), education (p = 0.023), and per capita income (p < 0.001) with the nutritional status of pregnant women. Meanwhile, for other variables (knowledge about helminthiasis, preventive behavior, age, and occupation) there was no significant relationship (p > 0.05). Helminthiasis is related to the nutritional status of pregnant women, so active screening is needed for the incidence of helminthiasis in pregnant women in the working area of the Karang Anyar Community Health Center, Jati Agung District, South Lampung Regency.

Abstract

Chronic Energy Deficiency in pregnancy is a condition where pregnant women suffer from a lack of food that lasts for a long time. Pregnant women with chronic energy deficiency have poor eating behavior, which is influenced by knowledge, perception, beliefs and health education. Health education provided by health workers can increase knowledge and change eating behavior and can be done better if assisted by an expert system. The aim of this activity is to train health workers (doctors, midwives, nutritionists) to carry out early detection of chronic energy deficiency in pregnancy using an expert system, increasing knowledge, attitudes and eating behavior of pregnant women. The method that will be carried out is training using the web-based expert system modelangraini.com for health workers, health education for pregnant women, and increasing knowledge and changing eating behavior of pregnant women. This activity was carried out for 1 day and took place in Faculty of Medicine, University of Lampung on Saturday 30 September 2023 with 45 family doctors, midwives and nutritionists in Bandar Lampung City participating. The results of the evaluation of the implementation of the service showed that there was an increase in participants' understanding by 91.1% to good understanding, 8.9% of participants had a fairly good understanding, and no participants had a poor understanding of the causes, risk factors, impacts, prevention methods, and early detection of chronic energy deficiency in pregnancy with techniques that are fast, easy and can help health workers in providing education and interventions on the eating behavior of pregnant women. This training activity can improve the skills of health workers in an effort to increase knowledge, attitudes and eating behavior of pregnant women so that they can prevent, overcome and help reduce the prevalence of chronic energy deficiency in pregnant women and prevent stunting in toddlers. Abstract

Chronic Kidney Disease (CKD) is a condition in which kidney function progressively declines and irreversibly diminishes so that the kidneys are unable to maintain metabolic, fluid and electrolyte balance resulting in uremia and azotemia. One of the kidney replacement therapies in stage 5 of CKD  is by hemodialysis procedure. There are 5 – 8 g/dialysis amino acids losses during the hemodialysis (HD) procedured , if protein loss is not replaced with adequate dietary protein intake it will certainly affect malnutrition which can increase the risk of mortality. Inadequate protein intake is associated with protein energy wasting. However, the associated impact of protein intake on mortality remains unclear. Literature review was conducted to identify the effect of  protein  intake restriction on mortality risk in CKD patients undergoing hemodialysis. Articles were searched through NCBI (The National Center for Biotechnology Information), PubMed/Medline and Science Direct. Prisma Method was used to describe the process of selecting journals in a structured manner so that valid journals are obtained for analysis in this study.  The results showed that appropriate management of protein intake for patients undergoing haemodialysis is necessary to prevent malnutrition, high protein intake can increase phosphorus and potassium levels and risk metabolic acidosis, and extensive restriction can lead to malnutrition and death with the hazard ratio 1.33. It is recommended that protein intake in CKD patients undergoing hemodialysis be in the range of 1.0 - 1.2 g/kg/day.

Abstract

Stress is a psychological condition that can affect nutritional status through changes in eating patterns and body metabolism. This study aimed to analyze the relationship between stress levels and nutritional status among final-year medical students at the Faculty of Medicine, University of Lampung. The study was conducted from November 2024 to January 2025 using a cross-sectional approach. The population consisted of final-year medical students in the 2024 academic year. A total of 141 students were selected using a simple random sampling technique. Data analysis was performed using the gamma correlation test. Stress levels were assessed using the Medical Student Stressor Questionnaire (MSSQ), while nutritional status was measured through anthropometric assessments. The results showed that 34.8% of respondents had an overweight or obese nutritional status, and 44.7% experienced moderate stress levels. Bivariate analysis revealed a significant relationship between stress levels and nutritional status (p = 0.033, p < 0.05) with a weak positive correlation (r = 0.260). This positive correlation indicates that higher stress levels are associated with an increased risk of being overweight or obese. In conclusion, this study demonstrates a significant relationship between stress levels and nutritional status among final-year medical students. These findings highlight the importance of effective stress management to prevent the risk of excessive weight gain and its potential contribution to degenerative diseases.

Abstract

Demam tifoid merupakan penyakit infeksi yang disebabkan oleh bakteri Salmonella Typhi. Insiden Demam tifoid menunjukkan kecenderungan meningkat dari tahun ke tahun dengan rata-rata kesakitan 500/100.000 penduduk dan angka kematian antara 0,6–5%. Prevalensi Demam tifoid di Indonesia mencapai 1,7%. Studi ini bertujuan untuk menerapkan pelayanan dokter keluarga yang berbasis evidence-based medicine secara holistik dan komprehensif pada pasien dengan mengidentifikasi faktor risiko, masalah klinis, serta penatalaksanaan pasien berdasarkan kerangka penyelesaian masalah pasien dengan pendekatan patient centered, family oriented, serta community oriented. Studi ini merupakan laporan kasus. Data primer diperoleh melalui anamnesis (autoanamnesis), pemeriksaan fisik, pemeriksaan penunjang dan kunjugan rumah. Penilaian dilakukan berdasarkan diagnosis holistik awal, proses, dan akhir studi secara kuantitatif dan kualitatif. Sebelum dilakukan intervensi, pengetahuan pasien mengenai penyakit yang dideritanya, kebersihan diri, makanan dan lingkungan kurang, dan tingkat kecukupan gizi berlebih. Setelah dilakukan intervensi, didapatkan perbaikan skor pengetahuan pasien dan keluarga meningkat sebanyak 50 poin, terjadi perubahan perilaku aktivitas fisik, perubahan perilaku dalam menjaga kebersihan diri, makanan dan lingkungan, serta konsumsi makanan sesuai tingkat kecukupan gizi.
