Abstract

Venipuncture procedures undeniably increase anxiety behavior and pain while children are in the emergency department. This study aimed to analyze the effectiveness of virtual reality (VR) on anxiety behavior and pain with the Dorothy-behavior theory during the venipuncture procedures procedure in the emergency department. The research used a quasi-experimental two-group pretest-posttest design with a control group design with 42 respondents in the yellow zone of the emergency department of RSPUN Cipto Mangunkusumo. Data analysis used the Wilcoxon test, Mann-Whitney, and ordinal regression. There were significant differences and comparisons between anxiety behavior (p-value <0.001) and pain (p-value <0.001) when the venipuncture procedure was performed in the experimental group. School-age children (p-value 0.026; OR=4.958) affect pain. This study recommends that nurses in emergency departments provide VR during venipuncture procedures to address anxiety and pain behaviors. In addition, Dorothy's behavioral theory in the vein puncture procedure has become an innovation in nursing care. Provide aspects of protection based on the age, gender, and experience of the child; maintenance aspects by looking at VR effects; and aspects of stimulation as reference materials or recommendations for further research on VR to overcome anxiety and painful behavior in children during venipuncture procedures.

Abstract

Newborns, especially premature infants, are at high risk of ineffective thermoregulation due to their less complex skin structure. Impaired thermoregulation experienced by premature infants can lead to hypothermia. Premature infants who experience hypothermia can cause complications that have the potential to cause disability and death. One of the efforts to prevent hypothermia is to place premature infants in an incubator that gives moisture so that moisture regulation is needed to help regulate the baby's temperature, especially in premature infants who experience ineffective thermoregulation. Humidity regulation is required for the incubator to prevent fluid loss through evaporation. This study aims to determine the effectiveness of providing incubator humidity education on nurses' knowledge, attitudes, and skills in the Perinatology room. The research design used a pre-experimental group without control with an accidental sampling technique; namely, there was a pre-test before and a post-test after treatment. Thus, the treatment results can be more accurate because they can be compared with the situation before treatment. This study involved 37 respondents in the perinatology room in one of the state hospitals using pre-and post-test questionnaires conducted from April to May 2022. The results showed that there were significant differences before and after education on knowledge (p=0.001), attitudes (p=0.001), and skills (p=0.001). The results of this study can used as a reference in developing standard procedures for incubator humidity to prevent ineffective thermoregulation in premature infants through cognitive improvement of nurses about incubators.

Abstract

Background: Chronic diseases in children, such as chronic kidney disease (CKD), diabetes, and asthma require complex long-term management because they can affect the quality of life physically, psychologically, socially, and educationally. One of the new innovations in supporting the management of chronic diseases in children is the use of chatbots that play a role in education, health monitoring, and psychological support. Purpose: To review the literature on the effectiveness of chatbots in supporting self-care in children with chronic diseases. Method: This literature review study examined the effectiveness of chatbots in supporting self-care in children with chronic diseases. The articles used were from ProQuest, Scopus, ScienceDirect, and Google Scholar. Inclusion criteria included publications from 2014-2024 that focused on children with chronic diseases. The search keywords used were "conversational agent", "chronic disease management", and "children". The search for articles used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and obtained 6 articles for review. Results: Analysis of the selected articles suggests that chatbots have the potential to provide easily accessible educational resources, assist in regular monitoring of children's health, and provide emotional support to children in managing chronic diseases. However, there are several challenges, such as limitations in chatbot personalization and the importance of family involvement in chatbot use. Conclusion: Chatbots have great potential to support chronic disease management in children, although further development is needed on the aspects of personalization and data privacy to make their use more effective and safe for children. Suggestion: Long-term research is needed to assess the lasting impact of chatbots on children’s health outcomes, including mental development and quality of life. Developing more personalized and responsive chatbots and exploring their integration with other technologies such as wearables are also interesting areas for further exploration.

Abstract

Background: The implementation of child care education will be different from adult care education. This is because children are not miniature adults. However, the role of child care cannot be separated from the involvement of parents or families. Motivational interviewing (MI) educational interventions have been suggested to improve efficacy, knowledge, and health behavior change in pediatric patients. Psychological approaches such as Motivational Interviewing (MI) have emerged as a promising alternative to overcome these limitations, with the aim of creating more sustainable behavior change in the context of health. Purpose: To evaluate the impact of Motivational Interviewing (MI)-based education on efficacy, knowledge, and behavioral change in children. Method: A systematic literature review study using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and the PICOS approach. The keywords used were, "motivation" OR "self-efficacy" OR "knowledge" OR "behavior change" AND "pediatric" OR "pediatrics" AND "motivational interviewing" OR "directive counseling". Identified 6 articles related to the impact of motivational interviewing (MI)-based education on efficacy, knowledge, and behavioral change in children. Results: Showed that MI intervention has a positive impact on self-efficacy, increased knowledge, and behavioral change in children with various health conditions, such as obesity, asthma, dental caries, and alcohol dependence. Conclusion: Although the results showed significant improvements in various parameters, differences in the level of effectiveness between studies indicate the need for a more specific approach to the target population and certain health problems. Suggestion: Further research with a more targeted and specific focus is needed to provide more comprehensive results and support the development of future MI-based interventions.   Keywords: Children; Education; Efficacy; Knowledge; Motivational Interviewing (MI). Metode: Penelitian sistematik literature review menggunakan panduan Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) dan pendekatan PICOS. Kata kunci yang digunakan yakni, “motivation” OR “self efficacy” OR “knowledge” OR “behavior change” AND “pediatric” OR “pediatrics” AND “motivational interviewing” OR “directive counselling”.

Abstract

ABSTRACT Self-care is fundamental in the care of clients with chronic diseases such as hypertension. The purpose of this paper is to clarify the concept of self-care for hypertensive clients. Literature review with search strategies carried out through Scopus, Ebscohost, Embase, Sciencedirect, PubMed and Google Scholar web sources published from 2012 to 2022 which were then analyzed according to Walker & Avant, S. (1995) to clarify the concept of self-care. This concept analysis shows that self-care has definition attributes as a process of physical and psychological actions carried out by individuals to maintain health, reduce risk and manage disease in an effort to maintain health, avoid risk factors that cause disease and efforts to reduce the effects of disease. Case models, Borderline and Contrary cases, antecedents, consequences, and empirical references to explain the concept of self-care. The definition of the concept of self-care identifies a number of comprehensive attributes, namely actions that include ongoing processes that include physical and psychological activities carried out by individuals in disease management. Nurses are actively involved as educators-counselors who can maximize self-care agency of hypertensive clients in the self-care process. Topics related to self-care should be explored further to advance nursing interventions in managing hypertensive clients. Keywords: Risk Factors, Physical and Psychological, Disease Management, Hypertensive Clients, Self Care.  ABSTRAK Perawatan diri merupakan hal mendasar dalam perawatan klien dengan penyakit kronis seperti hipertensi. Abstract

Separation due to hospitalization and the condition of infant diseases can cause stress and anxiety in parents.Parental anxiety can affect the parent-infant relationship and cause developmental problems in infant. FamilyCentered Care (FCC) is an effort to reduce parental anxiety when infants undergo treatment. This study aimedto identify the effect of FCC intervention on parents' anxiety in the perinatology ward. This study used aquasiexperimental design with one group pretest posttest design. A total of 29 parents were recruited byconsecutive sampling technique, with following inclusion criteria: parentsof infants who were undergoingtreatment in the perinatology ward, infants who were not immediately intubated when admitted to theperinatology ward, willing to become respondents, able to read and write; Meanwhile, the exclusion criteriawere parents who did not follow the intervention process until the end of the intervention, and infants who wereintubated during the intervention process. The intervention was carried out by providing FCC education andinvolving families for 3 days of baby care. Parental anxiety measurements were carried out before and after theintervention using the Parental Stressor Scale-NICU (PSS-NICU) questionnaire. The data analysis usedWilcoxon test. The results of this study showed that there was a significant difference in the mean anxiety ofparents before and after the FCC intervention (p = 0.001). FCC intervention is effective in reducing parentalanxiety. It is recommended that this intervention can be applied to the parents from the beginning of thetreatment in perinatology ward.

Abstract

Background: Physiological parameters are vital indicators of neonatal health, particularly in premature and low birth weight (LBW) infants. Specific Background: Kangaroo Mother Care (KMC) enhances neonatal outcomes, yet optimal positioning—such as the recently introduced Supported Diagonal Flexion (SDF) position—requires further validation. Knowledge Gap: Limited evidence exists on whether SDF positioning offers physiological advantages over the conventional upright kangaroo position. Aims: This study aimed to determine the effect of SDF positioning on physiological stability in premature and LBW infants during KMC. Method: A randomized crossover trial was conducted involving 31 mother-infant dyads. Each infant experienced both SDF and upright kangaroo positions in two sessions, with at least one hour per session. Physiological parameters—body temperature, heart rate, and oxygen saturation—were measured at baseline and post-intervention, analyzed using General Linear Model Repeated Measures and Mann-Whitney tests. Results: No statistically significant differences were observed across parameters: temperature (p=0.370), heart rate (p=0.956), and oxygen saturation (p=0.373). Novelty: This is among the first trials comparing SDF and upright KMC positions in a crossover design. Implications: Although physiological differences were not significant, SDF positioning demonstrated safety and maintained stability, supporting its use as an alternative in KMC practices for premature and LBW infants. Highlights: Highlights the safety and stability of SDF positioning in KMC. Uses a robust crossover design for internal control. Supports clinical application despite non-significant differences. Keywords: Kangaroo Mother Care, Supported Diagonal Flexion, Premature Infants, Physiological Stability, Randomized Crossover Trial
