Abstract

ABSTRACT Self-care is fundamental in the care of clients with chronic diseases such as hypertension. The purpose of this paper is to clarify the concept of self-care for hypertensive clients. Literature review with search strategies carried out through Scopus, Ebscohost, Embase, Sciencedirect, PubMed and Google Scholar web sources published from 2012 to 2022 which were then analyzed according to Walker & Avant, S. (1995) to clarify the concept of self-care. This concept analysis shows that self-care has definition attributes as a process of physical and psychological actions carried out by individuals to maintain health, reduce risk and manage disease in an effort to maintain health, avoid risk factors that cause disease and efforts to reduce the effects of disease. Case models, Borderline and Contrary cases, antecedents, consequences, and empirical references to explain the concept of self-care. The definition of the concept of self-care identifies a number of comprehensive attributes, namely actions that include ongoing processes that include physical and psychological activities carried out by individuals in disease management. Nurses are actively involved as educators-counselors who can maximize self-care agency of hypertensive clients in the self-care process. Topics related to self-care should be explored further to advance nursing interventions in managing hypertensive clients. Keywords: Risk Factors, Physical and Psychological, Disease Management, Hypertensive Clients, Self Care.  ABSTRAK Perawatan diri merupakan hal mendasar dalam perawatan klien dengan penyakit kronis seperti hipertensi. Tujuan penulisan ini adalah untuk memperjelas konsep perawatan diri klien hipertensi. Literatur review dengan strategi pencarian yang dilakukan melalui websources Scopus, Ebscohost, Embase, Sciencedirect, PubMed dan Google Schoolaryang diterbitkan dari tahun 2012 hingga 2022 yang selanjutnya dilkaukan analisis konsep menurut walker & Avant,S. (1995) untuk memperjelas konsep self care. Analisis konsep ini menunjukkan bahwa perawatan diri memiliki atribut definisi sebagai proses tindakan fisik dan psiklogis yang dilakukan individu untuk menjaga kesehatan, mengurangi risiko dan menajemen penyakit dalam upaya mempertahankan kesehatan, menghindarkan diri dari faktor risiko yang menyebabkan penyakit dan upaya mengurangi efek dari penyakit. Model kasus, kasus Borderline dan Contrary, anteseden, konsekuensi, dan referensi empiris untuk menjelaskan konsep self care. Definisi konsep self-care mengidentifikasi sejumlah atribut yang komprehensif yaitu tindakan yang mencangkup proses berkelanjutan yang meliputi aktivitas fisik dan psikologis yang dilakukan individu dalam manajemen penyakit. Perawat terlibat aktif sebagai edukator-konselor yang dapat memaksimalkan self-care agency klien  hipertensi dalam proses perawatan diri.Topik yang terkait dengan perawatan diri harus dieksplorasi lebih lanjut untuk memajukan intervensi keperawatan dalam mengelola klien hipertensi. Kata Kunci:  Faktor Risiko, Fisik dan Psikologis, Manajemen Penyakit, Klien Hipertensi, Self Care.

Abstract

Separation due to hospitalization and the condition of infant diseases can cause stress and anxiety in parents.Parental anxiety can affect the parent-infant relationship and cause developmental problems in infant. FamilyCentered Care (FCC) is an effort to reduce parental anxiety when infants undergo treatment. This study aimedto identify the effect of FCC intervention on parents' anxiety in the perinatology ward. This study used aquasiexperimental design with one group pretest posttest design. A total of 29 parents were recruited byconsecutive sampling technique, with following inclusion criteria: parentsof infants who were undergoingtreatment in the perinatology ward, infants who were not immediately intubated when admitted to theperinatology ward, willing to become respondents, able to read and write; Meanwhile, the exclusion criteriawere parents who did not follow the intervention process until the end of the intervention, and infants who wereintubated during the intervention process. The intervention was carried out by providing FCC education andinvolving families for 3 days of baby care. Parental anxiety measurements were carried out before and after theintervention using the Parental Stressor Scale-NICU (PSS-NICU) questionnaire. The data analysis usedWilcoxon test. The results of this study showed that there was a significant difference in the mean anxiety ofparents before and after the FCC intervention (p = 0.001). FCC intervention is effective in reducing parentalanxiety. It is recommended that this intervention can be applied to the parents from the beginning of thetreatment in perinatology ward.

Abstract

Background: Physiological parameters are vital indicators of neonatal health, particularly in premature and low birth weight (LBW) infants. Specific Background: Kangaroo Mother Care (KMC) enhances neonatal outcomes, yet optimal positioning—such as the recently introduced Supported Diagonal Flexion (SDF) position—requires further validation. Knowledge Gap: Limited evidence exists on whether SDF positioning offers physiological advantages over the conventional upright kangaroo position. Aims: This study aimed to determine the effect of SDF positioning on physiological stability in premature and LBW infants during KMC. Method: A randomized crossover trial was conducted involving 31 mother-infant dyads. Each infant experienced both SDF and upright kangaroo positions in two sessions, with at least one hour per session. Physiological parameters—body temperature, heart rate, and oxygen saturation—were measured at baseline and post-intervention, analyzed using General Linear Model Repeated Measures and Mann-Whitney tests. Results: No statistically significant differences were observed across parameters: temperature (p=0.370), heart rate (p=0.956), and oxygen saturation (p=0.373). Novelty: This is among the first trials comparing SDF and upright KMC positions in a crossover design. Implications: Although physiological differences were not significant, SDF positioning demonstrated safety and maintained stability, supporting its use as an alternative in KMC practices for premature and LBW infants. Highlights: Highlights the safety and stability of SDF positioning in KMC. Uses a robust crossover design for internal control. Supports clinical application despite non-significant differences. Abstract

Clinical judgment is a critical and multifaceted process essential for ICU nurses, integrating technical expertise and interpersonal skills to ensure safe, effective patient care. Clinical judgment, recognized as a core competency in intensive care nursing, requires further conceptual clarity. This study aimed to define and analyze the concept of clinical judgment in ICU nurses using Walker and Avant’s eight-step concept analysis method. A comprehensive literature search was conducted across databases including CINAHL, Medline, Scopus, ProQuest, SpringerLink, ClinicalKey Nursing, EMBASE, ScienceDirect, EBSCOhost, and Google Scholar, covering publications from 1994 to 2024. The analysis identified key attributes of clinical judgment: clinical knowledge, critical thinking, practical wisdom, data prioritization, intuition, scientific analysis, evidence use, and interaction. Antecedents included education, knowledge, experience, analytical ability, and the nursing process. Consequences of clinical judgment were accurate responses, reflective practice, safe nursing care, and improved patient safety. Decision-making was identified as the primary empirical referent. This concept analysis demonstrates that clinical judgment in ICU nurses is developed through a synthesis of critical thinking, clinical expertise, and situational experience, reinforcing its significance in the delivery of high-quality critical care.

Abstract

Background: The transition of premature infants from the intrauterine to the extrauterine environment is a complex and challenging adaptation process for infants and their mother. Empowering parents through education and support is crucial in navigating these challenges. Propose: To review parental empowerment through early kangaroo care a holistic approach to neonatal health. Method: Systematic review research according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. This literature research uses seven electronic databases, namely PubMed, Proquest, Sciencedirect, Wiley Online, Emerald insight, Sage Journals, and JSTOR. The data search began on September 30, 2024, using the keywords "parent" or "mother" or "maternal" and "infant" or "preterm" and "early" or "immediately" and "skin-to-skin" or "kangaroo-mother-care". The literature criteria used were full texts that discussed prematurity (newborns with a gestational age of <37 weeks), mothers who performed early KMC, parents who performed early KMC, the outcomes measured in this study were infant outcomes (morbidity, mortality, physiological status, weight, and length of stay) and maternal outcomes (exclusive breastfeeding, maternal mental health), and this study used a randomized control design. This study is registered with PROSPERO (CRD42025633170). Results: The search process identified 318 articles. Further screening resulted in a final sample of 7 articles. The review results showed that early KMC has benefits for both mothers and infants. Benefits of early KMC for mothers include increased exclusive breastfeeding, reduced depression, and increased maternal self-satisfaction in caring for preterm infants. Furthermore, in preterm infants, early KMC has been shown to improve physiological stability, weight gain, reduce mortality, morbidity, infections, weight gain, and reduce length of hospitalization. Conclusion: Early implementation of KMC not only provides clinical benefits for preterm infants, but also contributes to improved maternal mental health. These findings support the promotion of KMC as a standard intervention in neonatal intensive care units to optimize short- and long-term outcomes for mothers and preterm infants.

Abstract

Background: Low birth weight (LBW) is a persistent public health challenge globally, including in Indonesia, particularly in remote areas like Papua. While biological risks are well documented, the influence of sociocultural factors on LBW infant care remains underexplored. There is a critical gap in understanding how cultural beliefs and family dynamics act as barriers to effective caregiving in these settings. Objective: This study aimed to explore cultural barriers to the care of LBW infants in Papua, Indonesia. Methods: A qualitative descriptive design was employed. Data were collected through face-to-face, semi-structured interviews with ten mothers of LBW infants in Jayapura, Papua, from May to July 2024. Interviews were transcribed verbatim and analyzed using inductive thematic analysis. Results: Inductive thematic analysis resulted in six themes: (1) Traditions and traditional practices during pregnancy and childrearing; (2) Perceptions of herbal remedies and traditional practices in managing infant health and growth; (3) Traditional care practices for LBW infants and their adaptation within community health efforts; (4) Challenges and obstacles for parents in caring for LBW infants; (5) Family support, roles, and hospital-based education in LBW care; and (6) Parental understanding, needs, and expectations regarding LBW care. Conclusion: Despite improvements in health information and services, ongoing cultural negotiation is needed to achieve optimal family-based LBW care. Nurses and other health workers should enhance cultural competence and employ patient-centered, culturally sensitive nursing care that integrates local traditions with evidence-based practice, aiming to improve infant health and reduce complications.

Abstract

Saat ini, perawatan metode kanguru mulai dianjurkan bagi bayi prematur karena kelahiran prematur dan bayi dengan berat badan lahir rendah merupakan salah satu penyebab kematian bayi terbesar. Tujuan penelitian ini adalah untuk mengetahui pengaruh perawatan metode kanguru terhadap respons fisiologis bayi prematur. Desain quasi experiment one group pre and post-test dilakukan di 2 rumah sakit di Jakarta. Sebanyak 16 bayi prematur yang memenuhi kriteria inklusi dipilih sebagai sampel. Hasil penelitian menunjukkan bahwa terdapat pengaruh yang bermakna dari perawatan metode kanguru terhadap respons fisiologis bayi prematur seperti peningkatan suhu tubuh ke arah suhu normal (p value = 0,000), peningkatan frekuensi denyut jantung ke arah normal (p value = 0,003), dan peningkatan saturasi oksigen ke arah normal (p value = 0,023). Oleh karena itu, metode perawatan kanguru merupakan cara yang efektif, mudah, dan murah untuk merawat bayi prematur. As premature birth and low birth weight are the main cause of neonatal mortality, kangaroo mother care is now suggested to care premature infants. The purpose of this study was to identify the effect of kangaroo mother care on physiological response of preterm infants. A quasi experiment design with one group pre and post test design was conducted in two hospitals in Jakarta. Sixteen preterm infants matching the inclusive criteria were selected as sample. The study found significant effect of kangaroo mother care intervention on physiological response of preterm infants, i.e. increasing body temperature to normal (p value = 0,000), increasing heart rate to normal (p value = 0,003), and increasing oxygen saturation (p value = 0,023). Therefore, the kangaroo mother care is therefore an effective, simple, and cheap method to care the preterm infants.

Abstract

Nutrisi yang adekuat diperlukan untuk proses penyembuhan, pertumbuhan dan perkembangan bayi berat lahir rendah, namun belum semua perawat mampu memfasilitasi pemenuhan kebutuhan nutrisi yang adekuat. Penelitian ini menganalisis penerapan model kenyamanan Kolcaba dalam memenuhi kebutuhan nutrisi pada bayi berat lahir rendah. Desain yang digunakan adalah studi kasus, terdapat lima kasus yang dibahas. Diagnosis keperawatan yang dirumuskan adalah: Ketidakefektifan pola menyusu pada bayi, ketidakseimbangan nutrisi: kurang dari kebutuhan tubuh, ketidakcukupan ASI, kekurangan volume cairan, risiko infeksi, ketidakefektifan termoregulasi: Hipotermia, pola nafas tidak efektif. Intervensi utama yang diterapkan adalah perawatan metode kanguru dan terapi musik. Hasil akhir penerapan model kenyamanan Kolcaba menunjukkan bahwa perawatan metode kanguru dan terapi musik dapat memfasilitasi pemenuhan kebutuhan nutrisi pada bayi berat lahir rendah ditunjukkan dengan adanya peningkatan berat badan per hari dan meningkatnya produksi ASI. Asuhan berpusat pada keluarga seperti perawatan metode kanguru penting untuk memfasilitasi kenyamanan dan pemenuhan kebutuhan nutrisi pada bayi berat lahir rendah.

Abstract

Pentingnya bayi prematur mencapai perilaku tidur tenang dan terjaga tenang agar pengeluaran resting energy expenditure (REE) dapat diminimalkan sehingga energi yang dimiliki dapat digunakan untuk proses metabolisme guna mencapai pertumbuhan dan perkembangan optimal. Pendekatan Transcultural Nursing melalui intervensi keperawatan yang berbasis kultural adalah pemberian stimulasi biological maternal sounds (BMS). BMS merupakan stimulasi audiosensori dengan memperdengarkan suara rekaman dan denyut jantung ibu pada bayi prematur dengan menggunakan media audio untuk mengalihkan kebisingan yang muncul dalam lingkungan perawatan dan meminimalkan komplikasi iatrogenic. Penelitian ini bertujuan untuk menggambarkan penerapan teori Transcultural Nursing melalui intervensi BMS dalam memenuhi kebutuhan perilaku tidur-terjaga bayi premature dimana menciptakan kondisi lingkungan bayi sama seperti bayi saat berada didalam lingkungan rahim Ibu yaitu memperdengarkan rekaman suara Ibu pada frekuensi 500-1000 Hz atau sama dengan 20-50 dB. Penelitian ini menggunakan metode studi kasus dengan mengelola lima kasus bayi premature. Hasil penelitian ini adalah lima bayi dengan rerata usia koreksi lebih dari 30 minggu berespon positif setelah diberikan BMS, dengan menunjukkan rerata pencapaian status tidur tenang selama 35 menit dalam waktu 90 menit pengamatan. Hal ini menunjukkan bahwa pada usia diatas 30 minggu dalam proses perkembangan sistem auditori dan sistem neurobehaviour adalah terbentuknya hubungan antara lobus temporal dengan korteks auditori sehingga dapat mendengar dan berespon terhadap suara Ibunya, mulai membeda-bedakan suara yang ada disekitarnya, serta memulai perkembangan untuk fungsi bahasa dan bicara.

Abstract

Several disinfection agents have been proven to have controversial issues with neonates. These agents continue to be used to disinfect prior to the insertion of intravenous catheters, both peripheral and central venous catheters in neonatal care rooms. Objective to identify the effectiveness of disinfection agents used prior to intravenous catheter placement among neonates and the possible adverse events. A systematic review was conducted. Databases including Pubmed, Science Direct, Proquest, Ebscohost, Clinical Key Nursing, and Wiley were searched from inception to October 2024. The eligibility criteria were neonates who needed intravenous catheter placement (peripheral or central), with experimental research design, and articles written in English. The quality of studies was assessed depending on the study design. Our study is registered on PROSPERO number CRD42024618924. 1119 articles were found. After the screening processes, we finally included 7 articles. Disinfection agents frequently used in neonatal settings in the included studies were chlorhexidine (with varying concentrations), aqueous and alcohol-based, and povidone-iodine 10%. The disinfectant used prior to the insertion of the intravenous catheter was effective in reducing pathogen colonization on the skin and preventing bloodstream infection. Even so, there were some concerns regarding the adverse effects of the disinfectant agent used. The disinfection prior to invasive procedures in neonatal care is a debated topic. Despite this, inadequate disinfection poses more life-threatening due to the risk of bloodstream infection. Key factors to consider when choosing an appropriate topical agent disinfectant prior to intravenous catheter placement in neonates include gestational age, birth weight, and skin condition.
