Abstract

Background: There is an increase in prevalence of hypertension all over the world as well as Indonesia. Hypertension affects almost all body organs and systems. Hypertension prevention and management are influenced by knowledge and attitude towards the disease. The aim of this study was to assess knowledge and attitude towards prevention and management of hypertension.Methods: This was a descriptive study with cross–sectional approach conducted in Jatinangor sub-district in 2013. Sample sizes were determined based on proportion of the case in population and precision estimates (95% confidence intervals). One hundred twenty respondents aged 18–60 year-old were selected from population using stratified random sampling method. Hypertensions knowledge and attitude were assessed using validated questioner as the scale measurement. Frequency of knowledge level was presented as three ordinal categories; good, moderate, and poor. Attitude of hypertension was presented as frequency in two categories; relatively positive and relatively negative.Results: Out of 120 respondents administered the questioner, 42.50% had good knowledge; 41.67% had moderate knowledge; 15.83% had poor knowledge related to hypertension prevention and 21.67% had good knowledge; 19.17% had moderate knowledge; 59.17% had poor knowledge related to management of hypertension. Hypertension attitude of respondents were 60.83% relatively positive in topics of hypertension prevention and 39.17% in topics of hypertension management.Conclusions: Respondents are knowledgeable about hypertension prevention but are less knowledgeable about its management. Respondents have relatively positive attitudes towards hypertension prevention. In the other hand, respondents have more relatively negative attitudes towards its management. [AMJ.2016;3(2):222–9]DOI: 10.15850/amj.v3n2.801

Abstract

Background: Hypertension is one of the most prominent global diseases. Despite the availability of effective therapies, hypertension remains poorly controlled in Indonesia. In many cases, patient’s noncompliance may be attributable to the low patients’ knowledge, attitude, and life-style practices such as polypharmacy. Polypharmacy is defined as the administration of many drugs at in one prescription. Polypharmacy increases expenses, possible adverse reaction to a single agent, incidence of drug interactions, and decreases patient’s compliance This study aimed to identify the practice of polypharmacy in hypertension treatment in primary health centers.Methods: A cross-sectional descriptive observational study was carried out on 60 patients from two primary health centers in Jatinangor, West Java, Indonesia in October 2013. Sociodemographic profile, degree of hypertension, types of antihypertensive drugs, concomitant drugs given together with antihypertensive drugs, and treatment compliance data were collected and presented in tables and figures.Results: The incidence of hypertension was more common among male patients compared to female patients. Thirty-three patients (55%) have low compliance to their medication. Twenty-nine patients (48%) received single drug and 31 patients (52%) received more than one drugs.Conclusions: The percentage of polypharmacy practice in treating hypertension in primary health centers is 52%. The most frequently prescribed anti-hypertensive are angiotensin-converting enzyme (ACE) inhibitors and calcium-channel blockers (CCB). Most of hypertensive patients have low compliance to therapy. Abstract

  Background: Liver damage may be caused by various factors. SolanumnigrumL.fruit is known to contain flavonoid antioxidant which is responsible for its hepatoprotective effect. A study was conducted to determine the protective effect of Solanumnigrum L. fruit infusion (SNFI) on CCl4-induced hepatic cell damage in rats. Methods: A complete randomized experimental study was conducted on 25 male Wistar strain-white rats (Rattusnorvegicus) which were divided into five groups during the period of September– October 2012. Group I (negative control) was given standardized food and water; group II (positive control) was induced by carbon tetrachloride (CCl4) 10% paraffin intraperitoneally by 8 mL/kg body weight on the 8th day of the study; group III, IV, V (treated) were given Solanumnigrum L. fruit infusion (SNFI) by 22.5g/100mL, 45 g/100mL and 90g/100mL concentrations for 8 days, respectively, prior to CCl4 induction. The calculation of necrotic liver cells was performed in 48 hours after induction. Data were statistically analyzed using Kruskal-Wallis test followed by Mann-Whitney post-hoc test. Results: The percentage of necrosis liver cells in group III, IV, and V was smaller compared to  the positive control group. The protective effect of SNFI against CCl4-induced hepatotoxicity may be related to its ability to elevate the antioxidant agent in the body. There were significant differences in necrotic between Group II and group III, IV, V which were treated with SNFI. Conclusion: Further investigation is required to characterize the active ingredients and the mechanism of SNF action to confirm the hepatoprotective and antioxidant effects.Keywords: CCl4, hepatoprotective, Solanumnigrum L.DOI: 10.15850/amj.v2n1.433

Abstract

Background: Various analgesics prescriptions for elderly are not appropriate according to the guideline and can cause the increase of side effects such as gastric problems. Puskesmas as a public health center in Indonesia has an important role in anticipating this problem. The objectives of this study was to identify the  analgesic usage in elderly patients at the public health center. Methods: This retrospective descriptive study was conducted for 3 months at Tanjungsari public health center, Sumedang, West Java, Indonesia, using total sampling. The data was taken from 417 medical records from 2013. The data taken from medical records were: sex, analgesic drug, diagnosis, and drug for gastric problem.Results: From the collected data, the most analgesics prescribed for the elderly patients was paracetamol, followed by Piroxicam, Mefenamic acid, and Ibuprofen. Not all of the elderly patients who received NSAIDs, were given gastric drug. Conclusions: The most prescribed analgesic drug given to elderly patients at the public health center is paracetamol. Abstract

AbstrakEkstrak etanol daun sendok mengandung banyak zat aktif, termasuk triterpenoid dan flavonoid yang memiliki efek anti proliferatif, anti angiogenesis dan menginduksi apoptosis sel kanker. Hipergastrinemia dapat menginduksi over-ekspresi Gen Regenerating 1, menekan produksi Caspase 3 dan perubahan histopatologi jaringan mukosa lambung. Penelitian ini menganalisis pengaruh pemberian ekstrak etanol daun sendok terhadap penghambatan over ekspresi gen Regenerating 1, peningkatan kadar Caspase 3 dan perubahan histopatologi  jaringan mukosa lambung tikus model hipergastrinemia. Penelitian ini dilakukan secara eksperimental dengan post test only with control group design. 28 tikus putih dibagi menjadi empat kelompok. Kelompok I (kontrol) diberi Omeprazol dosis 8mg/200gBB tikus/hari, Ciprofibrate dosis 12,5mg/200gBB tikus/hari dan 3 kelompok studi (Kelompok II, III dan IV diberi Omepr azol dosis 8mg/200gBB tikus/hari, Ciprofibrate dosis 12,5mg/200gBB tikus/hari dan ektrak etanol daun sendok masing-masing dosis 50mg, 100 mg dan 200mg per 200gBB tikus/hari). Semua hewan coba dikorbankan dibawah pengaruh anestesi setelah perlakuan selama 2 bulan. Sampel darah dan gaster hewan coba diambil untuk pemeriksaan kadar gastrin dan Caspase 3 dengan metode ELISA, ekspresi Gen Regenerating1 dengan RT-PCR serta histopatologi jaringan mukosa lambung. Kadar gastrin hewan coba meningkat 3-4 kali kadar gastrin normal (>169 pg/mL). Rerata ekspresi Gen Regenerating 1 terendah terjadi pada Kelompok III (1,226±0,108) Analisis statistik menunjukkan terdapat perbedaan ekspresi Gen Regenerating 1 secara bermakna antara Kelompok III dengan kontrol (nilai p= 0,016;IK 95%). Rerata kadar Caspase 3 tertinggi terdapat pada IV (0,603±0,172ng/mL). Analisis statistik menunjukkan terdapat perbedaan kadar Caspase 3 secara bermakna antara kelompok IV dan Kelompok lainnya (nilai p <0,05;IK 95%). Gambaran histopatologi mukosa lambung menunjukan atropi dan tidak terdapat perbedaan ketebalan jaringan mukosa lambung secara bermakna diantara kelompok penelitian (nilai p=0,092; IK95%). Pemberian ekstrak etanol daun sendok mencegah over-ekspresi Gen Regenerating 1 dengan dosis 100mg/200gBB/hari, meningkatkan kadar Caspase 3 dengan dosis 200mg/200gBB/hari tetapi pada dosis penelitian yang digunakan tidak dapat menghambat perubahan histopatologi jaringan mukosa lambung tikus model hipergastrinemiaKata Kunci: Ekstrak etanol daun sendok, Over ekspresi Gen Regenerating 1, Caspase 3, Histopatologi mukosa lambung, Model HipergastrinemiaAbstractPlantago major L has a lot of active substances, including triterpenoid  and flavonoid  that show anti proliferative effect, anti angiogenesis and induce cancer cells apoptosis. Hypergastrinemia able to induce overexpression of Regenerating 1 gene, inhibit activation of caspases include Caspase 3 and cause histopathologic alteration  of gastric mucosal tissues. This study was to evaluate  the effect of Plantago major L extract.on inhibition of Regenerating 1 gene overexpression, increasing of Caspase-3 level and histopathologic alteration of gastric mucosal tissues on hypergastrinemic rat (Rattus norvegicus) model. By experimental study and post test only with control group design,  28 of rats was divided in to 4 groups. Group I  as a negative control was given Omeprazole 8mg/200gbw rat and Ciprofibrate dose 12,5 mg/200gbw rat/day and aquadest per oral. Group II was given Omeprazole 8mg/200gbw rat and Ciprofibrate dose 12,5 mg/200gbw rat/day and Plantago major L. extract 50mg/200gbw rat/day per oral. Group III was given Omeprazole 8mg/200gbw rat and Ciprofibrate dose 12,5 mg/200gbw rat/day and Plantago major L. extract 100mg/200gbw rat/day per oral. Group IV was given Omeprazole 8mg/200gbw rat and Ciprofibrate  dose 12,5 mg/200gbw  rat/day  and Plan tago major  L. extract  200mg/200gbw rat/day per oral. They were killed after 2 month intervention under anaesthetic. Blood sample and gaster organs were collected for measurement of gastrin and caspase 3 level by using ELISA method, expression of Regenerating 1 gene by RT-PCR and histopatological examination of gastric mucosal tissues. There were increasing of gastrin level of rats up to 3-4 times fold compared to normal level and  to indicate hypergastrinemic condition. The lowest average of Regenerating 1 gene expression was found on Group III (1,226±0,108). Statistic analysis show that there were significantly difference of Regenerating 1 gene expression between group III and Control (p= 0,016; 95%CI) The highest average of Caspase-3 level was found on Group IV(0,603±0,172ng/mL) and there were significantly difference of caspase-3 level between group IV and the other Groups (p< 0,05; 95%CI). There were atrophic process on gastric mucosal tissue but there were not significantly difference of gastric mucosal thickness between group  (p= 0,092; 95%CI). This study can be concluded that administration of Plantago major L extract able to inhibit overexpression of  Regenerating 1 gene by dose 100mg/200gbw rat/day, to increase Caspase 3 level by dose 200mg/200gbw rat/day but can not to inhibit histopathologic alteration of gastric mucosal tissues by using all of the doses.  Keyword: Ethanol extract of Plantago major L, Regenerating 1 gene overexpression, Caspase 3, gastric mucosal histopatology, Hypergastrinemia model

Abstract

Kualitas hidup merupakan salah satu acuan keberhasilan dari upaya pengobatan termasuk kemoterapi pada pasien kanker. Pasien kanker yang menjalani kemoterapi dapat mengalami perubahan pada kualitas hidupnya. Tujuan penelitian ini adalah untuk menilai kualitas hidup pasien kanker serviks dengan menggunakan kuesioner EQ-5D sebelum dan setelah mendapatkan kemoterapi. Penelitian ini merupakan penelitian non-eksperimental bersifat analitik menurut perspektif pasien dengan teknik pengumpulan data secara prospektif. Subjek penelitian adalah pasien kanker serviks rawat inap kelas 3 yang memenuhi kriteria inklusi. Penelitian dilakukan di RSUP Dr. Hasan Sadikin Bandung. Data yang digunakan adalah data pasien dari bulan Juni sampai Desember 2015. Penelitian dilakukan dengan menggunakan kuesioner EQ-5D. Selanjutnya dilakukan perhitungan persentase permasalahan pasien, nilai indeks EQ-5D (utility) dan nilai EQ-5D VAS. Nilai utility dan EQ-5D VAS dianalisis secara statistik dengan uji Wilcoxon (α=5%) untuk melihat terjadinya perubahan kondisi yang bermakna sebelum dan setelah kemoterapi. Hasil menunjukkan bahwa setelah kemoterapi siklus pertama, persentase masalah kemampuan berjalan/bergerak dan kegiatan yang biasa dilakukan mengalami peningkatan, rasa sakit/tidak nyaman dan rasa cemas/depresi mengalami penurunan, sedangkan perawatan diri tidak mengalami perubahan. Berdasarkan hasil uji statistik, setelah kemoterapi siklus pertama, nilai utility dan EQ-5D VAS mengalami peningkatan dan menunjukkan peningkatan bermakna (nilai T hitung masing-masing 2,0 dan 4,5) pada pasien stadium I.Kata kunci: Kanker serviks, kemoterapi, kualitas hidup, kuesioner EQ-5D Quality of Life Evaluation of Cervical Cancer Inpatients Before and After Chemotherapy by Using the EQ-5D QuestionnaireAbstractThe quality of life is one of success parameters of the treatment effort including chemotherapy in cancer patients. Cancer patients undergoing chemotherapy may experience changes in their quality of life. This study aimed to assess the quality of life of patients with cervical cancer in Indonesia using the EQ‑5D questionnaire pre and post-chemotherapy. The research was an analitycal non-experimental study according to the patient’s perspective using prospective data collection techniques. The research subjects were the patients who suffered from cervical cancer who were hospitalized in class 3 and fullfilled the inclusive requirement. The research was conducted in Dr. Hasan Sadikin Bandung general hospital, from June until December 2015. The study was conducted by the EQ-5D questionnaire. Futhermore, the percentage of the patients’ problems, EQ-5D index (utility) and EQ-5D VAS were calculated. Utility and EQ-5D values was analysed statistically using Wilcoxon test (α=5%) to determine if there were significant condition changes before and after chemotherapy. The results showed that after the first cycle of chemotherapy, the percentage of patients reporting problem is related to ability to walk/move and the usual activities problem increased, pain/discomfort and anxiety/depression problem decreased, while the self‑care problem did not change. After the first cycle of chemotherapy, the utility and EQ-5D values increased and showed significant improvement (T count value=2.0 and 4.5 respectively) in patients with stage I. Abstract

Luaran humanistik seperti kualitas hidup merupakan parameter yang sangat penting untuk menilai keberhasilan kemoterapi. Penelitian ini bertujuan untuk mengevaluasi dampak penggunaan regimen cisplatin-vinkristin-bleomisin pada kualitas hidup, sehingga dapat diketahui bagaimana perbedaan kualitas hidup sebelum dan setelah kemoterapi. Penelitian ini dilakukan menggunakan rancangan cross-sectional pada pasien rawat inap di RSUP Dr. Hasan Sadikin Bandung dari bulan Juni 2015 sampai dengan Maret 2016. Data diambil secara prospektif dengan observasi lapangan. Pengukuran kualitas hidup pasien dilakukan dengan menggunakan kuesioner European Organization for Research and Treatment of Cancer Quality of Life Questionnaire C-30 (EORTC QLQ-C30). Perbedaan nilai domain sebelum dan setelah kemoterapi dievaluasi secara statistik dengan paired t-test (p<0,05). Hasil menunjukkan bahwa pada domain fungsional terdapat peningkatan dalam seluruh aspek, kecuali fungsi sosial. Pada domain gejala, terdapat kecenderungan penurunan kecuali penurunan nafsu makan (p=0,009).  Domain status kesehatan global meningkat secara bermakna setelah kemoterapi (p=0,000). Secara keseluruhan, penggunaan regimen cisplatin-vinkristin-bleomisin selama tiga siklus cenderung meningkatkan kualitas hidup pasien kanker serviks. Kata kunci: Kanker serviks, kualitas hidup, kuesioner EORTC QLQ-C30, regimen cisplatin vinkristin-bleomisin Humanistic Outcome of Cervical Cancer Patients Using Cisplatin-Vincristine-Bleomycin RegimenAbstractHumanistic outcomes such as quality of life are very important parameters for assessing the success of chemotherapy. Therefore, this study aimed to evaluate the impact of using the cisplatin-vincristin-bleomycin regimen before and after chemotherapy on quality of life. It was conducted using a cross-sectional design in inpatients at Dr. Hasan Sadikin Bandung from June 2015 to March 2016. The data were taken prospectively by field observation. The patient's quality of life was measured using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) questionnaire. Furthermore, the difference in domain values before and after chemotherapy was evaluated statistically by paired t-test (p<0.05). The results show an increase in all aspects of the functional domain, except social functions. There was a tendency to reduce the symptom domain except for decreased appetite (p=0.009). The domain of global health status increased significantly after chemotherapy (p=0.000). Overall, using the cisplatin-vincristin-bleomycin regimen for three cycles improves the quality of life of cervical cancer patients.Keywords: Cervical cancer, cisplatin-vincristine-bleomycin regimen, EORTC QLQ-C30 questionnaire, quality of life

Abstract

Cervical cancer is the most common malignantcancer of the female reproductive organs and affectsthem for a long period of time. If someone weresuffering from cervical cancer, then the quality of lifewill decline. This have been proven by research incancer patients in Dr. Sardjito Hospital includingcervical cancer [Perwitasari et al, 2011]. Not only theeffect of cervical cancer, the choice of chemotherapyregiment will also affect the quality of life(humanistic outcome) of the patients. There are avariety of chemotherapy regimens for cervicalcancer. Cisplatin-based chemotherapy regimen isrecommended by the NCCN (NationalComprehensive Cancer Network) and the Ministry ofHealth of Republic of Indonesia. Neverthelesshumanistic outcome research of these regimens arestill lacking. This study aims to evaluate the quality oflife of cervical cancer patients in Dr. Hasan SadikinHospital Bandung were treated with cisplatin basechemoterapy using the EQ-5D questionnaire.

Abstract

BACKGROUND: Polycystic ovary syndrome (PCOS), a common reproductive endocrinologic disorder in woman, was considered to be related to sleep disturbance. This study is aimed to analyze the correlation between excess androgen markers and Athens Insomnia Scale (AIS) in PCOS.METHODS: This observational, cross-sectional study of PCOS was conducted to 31 subjects to evaluate the correlation between serum total testosterone, sex hormone binding globulin (SHBG) and free androgen index with the incidence of sleep disturbance using AIS. Sleep disturbance was present if the score ≥6. The correlation between excess androgen markers and sleep disturbance was analyzed using Pearson’s coefficient of correlation or Spearman’s rho test. Correlation coefficient more than 0.5 with p<0.05 was considered significant.RESULTS: Out of 31 PCOS subjects aged 20-40 years, 39% subjects had AIS score ≥6. Mean serum testosterone in AIS score >6 group was higher than AIS score <6 group but not statistically significant (46.68 vs. 28.49 ng/mL, p>0.05). No significant correlation was found between serum total testosterone, SHBG and free androgen index with AIS score. After adjusting for AIS score, there was the moderate positive correlation between serum total testosterone level although not statistically significant (r=0.54, p=0.07).CONCLUSION: The serum total testosterone level might influence the occurrence of sleep disturbance in PCOS.KEYWORDS: PCOS, androgen excess, testosterone, sleep disturbance, Athens Insomnia Scale

Abstract

Latar belakang. Prevalensi anak dengan stunting di kota Bandung pada 2018 mencapai 25,8%, melebihi ambang batas WHO (20%). Rendahnya kualitas gizi asupan merupakan salah satu penyebab stunting. Konsumsi makanan kudapan dengan nilai gizi rendah pada anak di kota Bandung cukup tinggi, tetapi penelitian mengenai hubungan antara konsumsi kudapan dan stunting masih terbatas.Tujuan. Mengetahui hubungan pola konsumsi kudapan dengan stunting pada anak kelas 1-2 SDN 036 Ujungberung Kota Bandung. Metode. Penelitian analitik observasional dengan pendekatan potong lintang dilakukan pada 268 responden dengan teknik consecutive sampling. Analisis bivariat digunakan untuk mengetahui hubungan frekuensi konsumsi dan usia mulai mengonsumsi kudapan dengan stunting. Hasil. Frekuensi konsumsi kudapan berhubungan dengan kejadian stunting, p=0,032 (PR 1,61 (95% CI 1,133-1,317)). Usia saat mulai mengonsumsi kudapan saja tidak berhubungan dengan kejadian stunting. Frekuensi konsumsi kudapan ≥1x per hari dan usia mulai mengonsumsi kudapan kurang dari 5 tahun berhubungan dengan kejadian stunting yang lebih tinggi, p<0,01 (PR: 2,02 (95% CI 1,272-1,435)).Kesimpulan. Terdapat hubungan bermakna antara pola konsumsi makanan kudapan dan stunting pada anak.
