Abstract

Background and purpose: The prevalence of low birth weight (LBW) in Situbondo District increased from 2.79% in 2008 to 5.85% in 2014. The highest prevalence in 2015 was found in Bungatan Subdistrict (11%). This study aims to determine risk factors of LBW in Bungatan Subdistrict, Situbondo. Methods: A case control study was conducted in Bungatan Subdistrict. A total of 60 infants born at the Bungatan Public Health Centre were included in the study. Cases were infants with LBW and controls were those with normal birth weight. All infants born with LBW were taken as cases (20 infants) while 40 controls were selected using a systematic random sampling. Data were collected from February to March 2016. Data were analysed using bivariate and multivariate technique with logistic regression test. Results: The majority of respondents were unemployed (80%), aged between 20-35 years during the pregnancy (53.33%), with parity of ≤3 (66.67%), low education level (61.67%), anemia during the pregnancy (68.33%), good nutritional status (75.00%), poor nutrition intake during the pregnancy (51.67%) and without pre-eclampsia (88.33%). Parity of >3 increased the risk of LBW (AOR=6.4; 95%CI: 1.66-24.75). Chronic energy deficiency increased the risk of LBW (AOR=5.6; 95%CI: 1.41-22.57). Conclusions: Parity of more than three and chronic energy deficiency increase the risk for LBW in Bungatan Public Health Centre, Situbondo District.

Abstract

Background and purpose: The World Health Organization (WHO) has set a target of 90% reduction in new hepatitis B infections by 2030. The WHO program includes prevention of new infections, and increases testing uptake and treatment. In order to effectively foster prevention, it is necessary to understand the risk factors for transmission. This study aims to determine the risk factors of hepatitis B infection in pregnant women. Methods: A case control study was carried out among 52 pregnant women with HBsAg (+) as cases and 104 pregnant women with HBsAg (-) as controls. The source of cases and controls was the register of pregnant women at the Pringgasela Public Health Center from January 2016 to October 2017. Cases were selected using systematic random sampling from 67 HBsAg (+) pregnant women and controls were selected in the same way from 1644 pregnant women with HBsAg (-). Cases and controls were matched by domicile. The data collected were age, age of first marriage, education, family income, parity, history of miscarriage, maternal or husband’s history of working abroad, frequency of marriage, history of injection, surgery, blood transfusion and dental care. Data collection was carried out with a pre-tested questionnaire in April-May 2018 through interviews at the home of each respondent. Multivariate analysis with logistic regression was carried out to determine the adjusted odds ratio of each risk factor. Results: Cases and controls characteristics were found to be similar in terms of age, domicile, education and income. Significant risk factors were the history of husband working abroad (AOR=4.28; 95%CI: 1.66- 11.08), age of first marriage ≤19 years (AOR=2.52; 95%CI: 1.16-5.48) and frequency of husband's marriage more than one time (AOR=3.08; 95%CI: 1.35-7.08). Conclusion: Husbands with history of working abroad, age of first marriage <20 years and frequency of husband's marriage more than one time were found to be the risk factors for hepatitis B infection in pregnant women. These factors require public health attention in order to reduce the transmission of hepatitis B.

Abstract

Background and purpose: Globally, maternal mortality rate due to labor complications remains high, with postpartum hemorrhage as the most common cause. Studies on the risk factors of postpartum hemorrhage have been widely published, however, its association with maternal anemia is inconsistent. This study aims to explore anemia as a risk factor for postpartum hemorrhage. Methods: A case control study was carried out with 69 women who experienced postpartum hemorrhage as cases and 207 women who had given birth but did not experience hemorrhage as controls. Cases and controls were taken from medical records at the dr. R. Soedjono Hospital, Selong, East Lombok District, West Nusa Tenggara. Cases were selected by systematic random sampling from 147 mothers who experienced postpartum hemorrhage during 2017. Controls were selected in the same way from 2,855 mothers who did not experience postpartum hemorrhage. Cases and controls were matched for birth weight of infants and maternal employment. Data were obtained by medical record extraction in May 2018 consisting of information on hemoglobin concentration, parity, age at pregnancy, birthing method, gestational age, birth spacing and length of labor. Data analysis was performed using logistic regression to determine the adjusted odds ratio (AOR) of anemia. Results: Characteristics of cases and controls were found to be similar in terms of infant birth weight, maternal occupation, parity, gestational age and length of delivery but statistically different by maternal age at pregnancy and birth spacing. Analysis with logistic regression showed that anemia during pregnancy with hemoglobin concentration ≤10 gr/dL was a significant risk factor for postpartum hemorrhage with AOR=16.32 (95%CI: 7.69-34.64). Delivery through caesarean section (SC) or oxytocin drip was also found to significantly reduce the risk of postpartum hemorrhage (AOR=0.03; 95%CI: 0.01-0.13). Conclusion: Anemia during pregnancy with hemoglobin concentration ≤10 gr/dL was found to be a risk factor for postpartum hemorrhage. Prevention and control of anemia in pregnant women need to be enhanced to prevent postpartum hemorrhage.

Abstract

Background and purpose: The incidence of diarrhea among underfive years children in Indonesia is considerably high. Studies exploring risk factors of diarrhea have been extensively conducted in Indonesia. However, the association between handwashing practices of the mother and quality of toilets with diarrhea among under-five years children are still inconsistent across studies. This study aims to examine the association between handwashing practices of the mother, quality of toilets and the episodes of diarrhea among under-five years children. Methods: A cross-sectional survey was conducted involving 104 under-five years children in Selagalas Village. Samples were selected using systematic random sampling. Data were collected in July 2017 by observation and face-to-face interviews using questionnaires. Bivariate analysis and logistic regression were conducted to examine the association between hand washing practices, quality of toilets and diarrhea among under-five years children. Results: We found more than half of under-five years children (64.42%) had experienced at least one episode of diarrhea over the last three months. We found a significant association between diarrhea with unhygienic toilets with an adjusted odds ratio (AOR) of 2.84 and poor handwashing practices of the mother with AOR of 2.46, however, both have a lower limit of AOR confidence interval that close to one, namely 95%CI: 1.05-5.97 and 95%CI: 1.03-5.87. Conclusion: Poor hand washing practices and unhygienic toilets are associated with the episodes of diarrhea among under-five years children, however, both have a low programmatic importance. Further study should be carried out to understand the association between handwashing practices and quality of toilets with diarrhea. Despite of the low programmatic importance, good hand washing practice should be promoted and access to toilets that meet the health standard must be enhanced.

Abstract

Background and purpose: Anaemia among pregnant women is still being a public health problem in Indonesia. This study aimed to determine association between iron folate consumption, energy and iron adequacy level with prevalence of anaemia among pregnant women. Methods: This was a cross-sectional study conducted in eight community health centers in Jember, East Java with 128 pregnant women selected using stratified random sampling from pregnant women’s cohort in each primary health center. Data were collected from interviews using semi quantitative food frequency questionnaire, chronic energy deficiency status by measuring the middle upper arm circumference (MUAC), and data of haemoglobin levels with Easy Touch GCHb (Bioptic Technology Inc., China). Data were analysed bivariate by chi-square and multivariate by logistic regression. Results: This study found that the prevalence of anaemia among pregnant women in Jember was 60.16% (95%CI: 51.76-68.56). Multivariate analysis resulted that variables which significantly reduce risk of anaemia among pregnant women were: household income (adjusted OR=0.09; 95%CI: 0.03-0.51), iron folate consumption (adjusted OR=0.16; 95%CI: 0.04-0.69), adequacy of energy (adjusted OR=0.06; 95%CI: 0.01-0.33), and iron adequacy level (adjusted OR=0.11; 95%CI: 0.01-0.90). Conclusion: Household income, intake food with high energy and iron, and also iron folate consumption significantly reduced the risk of anaemia among pregnant women.

Abstract

Background and purpose: One of the strategies adopted to improve coverage of TB patients (BTA+) in Gianyar Regency is to involve the pakraman (desa adat) TB cadres and the regular (PPTI) TB cadres with four tasks in order to help health care workers to trace and find tuberculosis suspect patients. The purpose of this study was to evaluate work performance between two types of community TB cadres in Gianyar Regency. Methods: This study was an observational research using secondary data of cadres work reports and interview to study participants to understand cadres characteristics and health workers supervision. Numbers of participants were all village TB cadres namely 29 pakraman and 88 regular TB cadres located in Gianyar Regency. Data collection was carried out using questionnaires and observation sheets. Univariate, bivariate and multivariate analysis was conducted. Results: The result shown that work performance of pakraman TB cadres was statistically better than regular TB cadres (adjusted RP=6.1; 95%CI: 3.3-11.2). Better work performance of pakraman TB cadres was found in all four tasks of cadres namely helping education (adjusted RP=7.8; 95%CI: 4.2-14.2), finding tuberculosis suspects (adjusted RP=7.4; 95%CI: 1.7-33.1), tracing of defaulters TB suspects (adjusted RP=17.1; 95%CI: 1.8-166.4) and tracing loss of follow up TB patients (adjusted RP=3.8x108; 95%CI: (1.6-8.9)x108). Conclusion: Work performance of pakraman TB cadres was better than regular TB cadres in all four tasks.

Abstract

Background and purpose: Optimal nutrition status will lead to health and productive adolescent. Recently, double burden of nutritional status has occurred among adolescent, including underweight and overweight. This study assessed that relationship among adolescent in urban area. Methods: This study was cross-sectional survey with a total sample of 75 female students who were randomly selected from three high schools. Data were collected by interviews using a structured questionnaire, Semi-quantitative Food Frequency Questionnaire (SQFFQ) and Adolescent Physical Activity Recall Questionnaires (APARQ). Anthropometric measurement was done for height, weight, middle upper arm and stomach circumference. Data of food consumption were analyzed using software Nutri-Survey, WHO Anthro Plus. Thecorrelation of risk factor and nutritional status were tested by linier regression. Results: This study supports the evidence of double burden in nutritional status among high school female students. Despite 18,67% of chronic energy deficiency, 8% students were overweight. Consumption of snack and fast food was also frequent, that may relate to high level of fat sufficiency among them. Variable significantly associated in all indicators of nutritional status was weight control (p<0.05). Physical activities had no significant relationship to nutritional status. Conclusion: There was double burden of nutritional status among high school female students especially overweight. Weight control had significant relationship toward nutritional status, while physical activities had no significant relationship to nutrition status.

Abstract

Traditional herbal treatment is one of treatment efforts beyond medical science which is currently quite popular among people in Denpasar. Seeking health care from traditional herbal healers is not only has a positive impact but also negative ones. Excessive promotion in various media causes people to be less rational in choosing health care services. This study aims to determine the association between knowledge, attitudes, beliefs and media information with the health care-seeking from traditional herbal healers. The study design was analytic cross-sectional. Sample was consecutively selected from the population involving 129 people who have met the inclusion criteria. Data were collected by using questionnaire. Chi-square results showed that all variables examined were significantly associated with health care-seeking from the traditional herbal healers. Results from logistic regression indicated that only belief was significantly associated with the level of patient visits to traditional herbal healers (RP=6.57; 95%CI: 1.43-8.84, p=0.006 and R2=49.5%). Denpasar City Health Office should improved guidance and control the traditional herbal healers so as to provide a sense of security to the community.

Abstract

The Public Health Center (PHC) III of South Denpasar is one of the endemic areas of Dengue Hemorrhagic Fever (DHF) in Denpasar, Bali Province. Dengue morbidity rate was high (>55 per 100,000 population), while the Percentage of Larvae Free Rate (PLFR) was low (<95%). This study aims at discovering the relationship between community's knowledge, attitude, behavior of Aedes aegypti (Ae. aegypti) larvae eradication and environmental health with the existence of Ae. aegypti larvae in the working area of PHC III of South Denpasar. A cross-sectional study was conducted with 147 households using systematic random sampling from a total of 5781 households. The respondents were the head of the family unit. The independent variables were knowledge, attitude, behavior and environmental health, while the dependent variable was the presence of Ae. aegypti mosquito larvae. The data were collected by using interview and observation in the respondent's house using instruments of questionnaire. The data were then analysed in stages covering the univariate, bivariate and multivariate analysis. Results indicated the PLFR was 87.1%. The variables related to the existence of larva were behavior (PR=17.89; 95%CI: 4.99-64.11) and environmental health (PR=7,08; 95%CI: 2.48-20.23). Multivariate analysis revealed that dominant variable was the behavior (PR=11,60, 95%CI: 2,98-45,13). Meanwhile, knowledge and attitude were not statistically associated with the existence of larvae. It can be concluded that the behavioral changes efforts that support of Ae. aegypti larvae eradication is still needed. It was recommended that the PHC needs to upscale health promotion efforts addressing the severity of DHF and prevention methods, cross-sector coordination, and involvement from healthcare providers as well as specially employed field workers in developing societies to eradicate mosquito breeding in order to increase the community's behavior of mosquito-larva eradication in order to increase the PLFR and to reduce the incidence rate of DHF.

Abstract

Background and purpose: Maluku Province is one of the regions in Indonesia with the highest prevalence of anaemia among pregnant mothers; in addition, the prevalence of malaria in pregnant women in this province is also high. The aims of this study were to determine the relationship between consumption of iron supplements and malaria infection with anaemia of pregnant mothers in Ambon City Methods: This study used a cross-sectional design. Study sample was pregnant mothers in their 3rd trimester, and a total sample of 83 respondents was selected using multistage random sampling. Primary data on iron supplement intake were collected through interviews. Secondary data on malaria infection were obtained from medical records. Data on anaemia was obtained through collected blood samples using a digital hemometer. Data were analyzed using univariate, bivariate (chi-square test) and multivariate (logistic regression) analysis. Results: Multivariate analysis indicated there was a significant relationship between anaemia with consumption of iron supplement with OR=4.570 (95%CI: 1.174-17.788), as well as malaria infection with OR=20.216 (95%CI: 4.773-85.620). Conclusion: Malaria infection and iron supplement intake was associated with anaemia of pregnant mothers in Ambon City.
