Abstract

Background and purpose: Early marriage considered as a major issue in Indonesia including West Nusa Tenggara Province with a prevalence of 23.9%. This results in the increasing number of total fertility rate (TFR) as many as 2.8 children. This is higher than the national TFR (2.36). An effective implementation of family planning program can decrease TFR. Until currently very limited study have identified factors associated to the use of contraceptive methods among early marriage couple in Aikmel Subdistrict East Lombok. Method: This is a cross-sectional quantitative study involving 84 early marriage women in Aikmel Subdistrict East Lombok. Samples were selected using convenience method and were interviewed to obtain data related to characteristic, contraceptive method history, knowledge, information from health provider and support from their husband. Data were analysed using univariate and bivariate (chi square test). Results: The majority of respondent’s first time marriage were at aged ≥17 year (64.3%). Most of the respondents have low education level (66.7%) and do not work (57.1%), have low level of knowledge (92,9%), lack of information from health provider (91.7%) and lack of support from husband (51.2%). Husband’s support was found to be the most significant factor associated to contraceptive use among early marriage women (p<0,001). Other factors including age at first marriage, education level, employment, level of knowledge and information from health providers were not statistically associated to contraceptive use. Conclusion: Support from husband is the most significant factor associated to contraceptive use among early marriage women in Aikmel Subdistrict East Lombok.

Abstract

Background and objective: Recent survey indicated that patient opinion of services at Mengwi I Community Health Centre was low and there was noticeable decline in patient visits. This study aims to investigate the issues related to the quality of health services given by the paramedics. Methods: A cross sectional study was conducted with 38 paramedics on duty at the outpatient unit. They were interviewed to obtain the data on their opinions about the quality of their service, competence, teamwork and career development opportunities. Chi square test was used for bivariate analysis and logistic regression for multivariate analysis. Results: The results of bivariate analysis indicated there was significant association between the paramedics’ quality of service with the competence, teamwork and career development (p<0,001). Multivariate analysis indicated that the paramedics’ competence (p=0,035) and development of career (p=0,042) are related to the paramedics’ quality ofservice. Conclusion: The quality of service of paramedics at Mengwi I Community Health Centre with the paramedics’ competence and development of career.

Abstract

Background and purpose: Good quality of life among elderly will reduce social and economic burden among the productive age people. This study aims to determine association between social function and quality of life among elderly in Denpasar. Methods: A cross-sectional survey was conducted among 144 elderly in Denpasar who were selected by cluster sampling and data were collected by a structured interview. Data were analyzed using logistic regression to determine the relationship between quality of life of the elderly with social activities, social interactions and family functions. Results: The study showed that the proportion of lower quality of life of the elderly in Denpasar was 64.58%. Proportion of elderly with lower quality of life was found among elderly of older age, female, low education, not working, low-income and widow/widower. Multivariate analysed showed that there was an association between quality of life of elderly with gender (AOR=6.42; 95%CI: 1.79-23.0), employment (AOR=9.81; 95%CI: 2.45-39.1), health status (AOR=8.65; 95%CI: 2.57-29.02), social activities (AOR=3.85; 95%CI: 1.22-12.1), social interactions (AOR=5.59; 95%CI: 2.01-15.5) and family functions (AOR=21.7; 95%CI: 6.09-77.7) Conclusion: Good quality of life of elderly was found associated with gender, employment, health status, social activities, social interactions and family functions.

Abstract

Background and purpose: Hypertension was a significant public health problem. This study aims to determine risk factors of hypertension. Methods: The study used a case-control design, involving 77 cases and 77 controls aged 18-65 years old, who were selected consecutively. Cases and controls were matched on age and sex. Cases were defined as patients who were diagnosed with hypertension by clinician at the primary health center (PHC), and controls were patients at PHC who were not diagnosed as hypertension. Data were collected through interview regarding sosiodemographic status and risk factors of hipertension. Direct measurement was performed for weight, height and abdominal circumference. Multivariate analysis was conducted using logistic regression. Results: Cases and controls were comparable in term of sex, age and education level. Variables that associated with hypertension were family history (AOR=9.20; 95%CI: 3.47-24.41), moderate stress (AOR=13.01; 95%CI: 3.70-45.79), severe stress (AOR=16,75; 95%CI: 3,32-84,38), less physical activity (AOR=3.53 (95%CI: 1.38-9.01), obesity (AOR=5.72; 95%CI: 2.09-15.68) and excessive salty food consumption (AOR=3.08; 95%CI: 1.17-8.09). Eating fatty foods may also indirectly cause hypertension. Income, mild stress, smoking habits, being passive smokers, coffee consumption habits, frequency of fruits and vegetables consumption were not found to be risk factors. Conclusion: Family history, moderate and severe stress, less physical activity, obesity and excessive salty food consumption were risk factors of hypertension.

Abstract

Objective: This study aimed to investigate the incidence of T. vaginalis infection among pregnant women in the city of Denpasar, Bali, Indonesia and the associated factors. Methods: There were 376 pregnant women who participated in this study. Study participants were selected using cluster sampling method. These pregnant women were then interviewed and tested for T. Vaginalis infection (microscopic and culture). Results: Infection of T. vaginalis was found in 7.4% of the pregnant women. The infection is most likely to be found in pregnant women who have a lower education level, middle-class status and a history of STIs. It is also most likely if they never had any vaginal douching before if they have intercourse ≥3 times per week, use sitting toilets and use river water as their main water source. Type of toilet used, the practice of vaginal douching, and condom usage show a significant association with trichomoniasis among pregnant women (p values were 0.001, 0.002 and 0.000 respectively). Conclusion: Early detection of sexually transmitted infections (STIs), especially T. vaginalis infection, should be performed during prenatal care to avoid the negative effects on pregnancy.Keywords - pregnancy, STI, vaginal trichomoniasis

Abstract

Pregnant women are one of the groups with the risk of having anemia. Some factors that are correlated to anemia is an infectious disease, bleeding, and low iron daily intake.These factors were related to the knowledge level of pregnant women and iron supplementation compliance. This is a cross-sectional study with descriptive methods, where 34 pregnant women were chosen by consecutive sampling method. Anemia was then measured by sahli method, and hemoglobin level < 11 g/dl were categories as anemia. The results were analyzed with univariate and bivariate analysis using SPSS 23. Anemia in pregnant women at Sangkan Gunung village, Sidemen sub-district is20,6%.  Most anemia incidents were found in 20-35 years old pregnant women, with low education level, work at the office, zero parity, and in afirst trimester. Anemia was not associated with chronic energy deficiency, knowledge level and iron supplementation compliance level (P>0,05). Anemia prevalence of pregnant women was relatively low, but the preventive effort must be taken to decrease therate of anemia prevalence.Keywords: Knowledge, Compliance, Iron tablets, Pregnant women, Anemia.

Abstract

Background: Cases of COVID-19 patients are increasing every day. The psychological impact felt by nurses was anxiety, fear, depression and a high risk of contracting COVID-19. Recommendations for preventing the risk of transmission are the use of PPE and hand washing behavior. Although PPE as a means of preventing the virus transmission, there are still nurses who do not comply with using it. Purpose: The purpose of the study was to determine the adherence of nurses in using PPE and hand washing behavior. Methods: This study is an analytic cross-sectional study, with a sample of 100 people in the isolation ward of Karangasem General Hospital. Data were analyzed by chi square test by lookcing at value, odds ratio (OR) and adjusted odds ratio (AOR). Results: 55% nurses use PPE in the obedient category, 52% nurses' hand washing behavior in the obedient category. Conclusions: Most are compliant in using PPE and washing hands.

Abstract

Kanker serviks merupakan penyebab utama kematian dan kecacatan, yang berdampak lebih dari 14 juta orang setiap tahun. Deteksi dini kanker serviks bertujuan untuk mengetahui perubahan prekanker, apabila tidak ditangani secara dini dapat menyebabkan kanker. Di Indonesia proporsi masyarakat yang melakukan deteksi dini kanker serviks masih rendah. Penelitian ini bertujuan untuk mengetahui bagaimana persepsi WUS dan intensitas pelaksanaan deteksi dini kanker serviks metode IVA. Penelitian ini menggunakan metode deskriptif kualitatif. Data diperoleh melalui wawancara mendalam dengan 10 WUS sebagai informan inti dan 1 orang penanggung jawab program dan 1 kepala puskesmas sebagai informan tambahan. Data dikumpulkan pada bulan September-Oktober 2019. Data dianalisis secara tematik dan disajikan dengan menggunakan pendekatan naratif. Penelitian ini menemukan bahwa gambaran persepsi WUS terhadap pemeriksaan IVA yaitu adanya rasa takut, malu dan tabu serta rasa percaya diri tidak beresiko terkena kanker serviks, hal ini disebabkan karena kurangnya intensitas dan kerja sama stake holder, tokoh agama dalam melakukan penyuluhan IVA. Meskipun semua WUS memiliki persepsi bahwa pemeriksaan IVA sangat bermanfaat untuk mencegah terjadinya kanker serviks, tetapi masih banyak wanita usia subur belum memiliki kemampuan untuk melakukan pemeriksaan. Selain itu juga faktor pendorong dalam pemeriksaan IVA adalah proses pengambilan keputusan dan dukungan suami, dimana sebagian besar suami yang dominan dalam mengambil keputusan terutama untuk kesehatan. Persepsi WUS dalam pemeriksaan IVA sebagian besar wanita enggan melakukan tes IVA karena adanya rasa takut, malu dan dan tabu, rasa percaya diri tidak berisiko pada diri individu, meskipun WUS tahu manfaat yang pemeriksaan IVA. Proses pengambilan keputusan dan dukungan suami merupakan faktor pendorong bagi WUS dalam melakukan deteksi dini kanker seviks. Serta intensitas pelaksanaan program IVA mempengaruhi persepsi WUS terhadap IVA

Abstract

Background and purpose:World Health Organization (WHO) reported the number of people with hypertension worldwide reached one billion in 2015. Hypertension screening is an effort to manage and control hypertension through hypertension management stages. This study aims to explore in-depth the implementation of hypertension screening guidelines performed by the public health centers (PHCs). Methods: This is a qualitative exploration with an exploratory case study approach by adopting the Lawrence W. Green Theory. This study was conducted in two PHCs in Tabanan District, those with the highest and the lowest target achievement. The factors examined were the implementation of guidelines and factors affecting it. The data were collected using in-depth interviews with ten informants and data were analyzed using thematic methods. Results: We found the predisposing factors of the implementation including the management of Health Operational Assistance (HOA) funds, the grouping of targets at risk of hypertension, hypertension screening guidelines, and the attitudes and beliefs of the officers and the community. The driving factors consisted of the lack of community participation. The inhibiting factors were indicated by the insufficient number of health workers and community support. Conclusion: There are several obstacles for the implementation of hypertension screening guidelines both from the health services side including budget allocation and community participation. Therefore, more thorough planning and measures to improve participation is needed.

Abstract

Background and purpose: The risk of hypertension increases in the elderly, while treatment adherence plays a crucial role in controlling blood pressure in the elderly. This study aims to explore the elderly's perception of hypertension and its treatment. Methods: This study used an exploratory qualitative design. Data were obtained through in-depth interviews about the elderly's perception of hypertension and its medication, health-seeking behavior, and cues to action in undergoing treatment for 6 elderlies with hypertension and 4 health workers in Gianyar District in July-September 2020. The data obtained were analyzed using thematic methods. Results: This study identified 2 big themes, namely the elderly's perception of hypertension and seeking behavior. The elderly's perception of hypertension is described from the perceived vulnerability from the aspect of age, lifestyle, and heredity. Hypertension is seen as a serious disease by the elderly, particularly from the complications aspect, while complaints felt are not considered serious unless they interfere with activities or have to undergo treatment. Reduced complaints and controlled blood pressure are the perceived benefits of treatment and lifestyle adjustments: long-term medication, side effects of drugs, and traditional medicine as obstacles for the elderly in undergoing treatment. The elderly seek treatment based on their trust and comfort in health workers, infrastructure, access to health services, and support from health workers and the government. Conclusion: The perception of barriers to treatment and health-seeking behavior of the elderly in seeking treatment play a significant role in determining the medicational therapy that the elderly undergoes, which can prevent complications. Family support for the elderly is essential in overcoming obstacles and increasing elderly adherence to medication.
