Abstract

Introduction: Mental health has become a major global public health concern due to its increasing prevalence and its profound social, economic, and healthcare consequences. Although the volume of mental health research has expanded substantially, evidence remains fragmented across disciplines, populations, and thematic areas. This systematic review aimed to map the global landscape of mental health research by identifying emerging trends, key determinants, health policy implications, and future research directions. Research Methodology: A systematic review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines. Literature searches were performed in PubMed, Scopus, Web of Science, and Google Scholar for studies published between 2020 and 2025. After duplicate records were removed and eligibility was assessed, 100 studies met the inclusion criteria and were included in the final synthesis. Data were extracted using a standardized form and analyzed through narrative thematic synthesis. Results: From 1,450 records initially identified, 100 studies were included in the final review. The findings revealed four dominant research themes: social and economic determinants (28%), environmental factors (19%), digital technology and mental health (17%), and health policy and service delivery (12%). Socioeconomic disadvantage, housing insecurity, limited social support, and educational and occupational stress emerged as the most frequently reported risk factors. Access to green spaces and community support were consistently associated with improved psychological well-being. The review also identified a growing interest in digital mental health interventions and artificial intelligence applications, while highlighting persistent gaps in policy implementation and equitable access to mental health services. Conclusion: Mental health outcomes are shaped by interconnected social, environmental, technological, and policy-related factors. Effective responses require integrated and multisectoral strategies that address structural determinants while strengthening mental health systems. Future policies should prioritize equitable access to services, community-based interventions, and evidence-informed decision-making to improve mental health outcomes globally

Abstract

Nursing-led interventions play a critical role in strengthening primary health care (PHC) delivery; however, empirical evidence on their policy-level impact remains fragmented and under-theorized. This review synthesizes quantitative and qualitative findings to evaluate how nursing-driven strategies influence PHC policy implementation outcomes across diverse health system contexts. A systematic search across five major databases and grey literature sources identified eligible studies assessing the effectiveness, feasibility, and contextual determinants of nursing-led interventions using established methodological frameworks. Quantitative analyses demonstrated that these interventions were associated with enhanced measurable policy compliance, improved continuity of care indicators, and greater alignment with PHC implementation targets. Qualitative synthesis further highlighted perceived patient empowerment, the contextual barriers faced by nurses, and the organizational enablers that shape intervention effectiveness. This review provides an integrative synthesis linking nursing-led interventions to PHC policy adherence, offering a novel framework for evaluating community-based policy outcomes. Findings underscore the importance of context-sensitive implementation and highlight opportunities for strengthening PHC systems through nursing leadership.

Abstract

Introduction: Stunting remains a persistent public health challenge in Indonesia, reflecting structural socioeconomic inequalities and uneven policy implementation. While demographic determinants are well documented, limited evidence explains how these factors interact with health policy processes. This study aimed to explore how demographic determinants of stunting among children under five are mediated through local health policy implementation dynamics. Research Methodology: A qualitative multiple-case study design was conducted in selected Indonesian districts with high stunting prevalence. A total of 25 informants participated, including 10 policy and health system actors and 15 caregivers, recruited through purposive sampling. Data were collected using semi-structured in-depth interviews and policy document review. Thematic analysis was performed using a combination of deductive coding based on the Social Determinants of Health framework and inductive coding to capture emerging themes. Results: The findings identified four interconnected themes: structural demographic vulnerability, health service delivery gaps, policy implementation and governance challenges, and socio-cultural dynamics. Although qualitative, the results indicate consistent directional relationships supported by prior evidence, where low maternal education, low socioeconomic status, high parity, and rural residence increase the risk of stunting (OR>1; p<0.05). These effects are mediated by targeting accuracy, workforce competence, intersectoral coordination, and service responsiveness. Misalignment between demographic risk profiles and policy implementation capacity contributes to persistent stunting disparities. Conclusion: Demographic determinants of stunting are structurally embedded and operate through policy-mediated mechanisms. Effective stunting reduction requires integrating demographic risk mapping with adaptive, equity-oriented policy implementation strategies, strengthening workforce capacity, and improving intersectoral coordination.
