Abstract

BACKGROUND End-stage renal disease (ESRD) is a major public health concern worldwide, with peritoneal dialysis (PD) offering a key alternative to hemodialysis. Flow restriction due to catheter migration is a common complication, affecting 7–26% of patients with PD. This study aimed to reduce complications in pediatric patients by examining a novel laparoscopic Tenckhoff catheter insertion technique using an Alken telescopic metal dilator. METHODS In a prospective observational study conducted between September 2018 and October 2022, 33 pediatric patients with ESRD underwent laparoscopic Tenckhoff catheter insertion using a combination of laparoscopic and Seldinger techniques with Alken dilators. This approach involves rectus sheath tunneling using a nephrostomy puncture needle, wire insertion, and dilatation of up to 15 Fr using Alken telescopic metal dilators under laparoscopic guidance. Survival analysis was used to assess catheter survival and complication rates. RESULTS The mean (standard deviation [SD]) patient age was 11.92 (3.7) years, with a median follow-up of 21.3 months. Catheter survival was 84.8%, with 5 catheter withdrawals due to infections (4 cases) or obstruction (1 case). Early and late complications, including exit-site infection, peritonitis, and catheter obstruction, occurred annually at rates of 0.10, 0.03, and 0.02 episodes per patient, respectively. No pericatheter leakage occurred. CONCLUSIONS This novel laparoscopic Tenckhoff catheter insertion technique using an Alken telescopic metal dilator become a simple and minimally invasive method, and associated with high catheter survival and low complication rates. Therefore, further comparative studies are recommended.

Abstract

Children's ability to read, write and count (calistung) can be said to be very important in life, because the teaching and learning process or activity is based on reading ability. To support learning and to know the growth and development of children, teachers and parents should know the right method or way to teach children to read, write and count. One method or way to develop it is through educational game tools (APE). First grade students at SD Inpres Kehiran have quite low learning abilities. This can be seen from almost 60% of students still having difficulty reading, writing and counting and is indicated by the scores obtained which are still below the Minimum Completion Criteria that have been set. To help teachers and students at SD Inpres Kehiran in understanding reading, writing and counting, our Community Service Team (PKM) aims to introduce the use of simple educational game tools (APE) made from used materials, namely through letter cards, drink bottle caps, and objects around the school or around the student's residential environment. By using learning media through educational game tools, the effectiveness of the learning method is evaluated and the level of student participation is measured. The stages carried out in the implementation of this community service activity start from planning, namely by coordinating with the school and asking permission from the leader (Dean), then the implementation stage starts from preparing materials, tools and materials, training and mentoring and finally the reporting stage, namely making a final report and making publications. The methods used to support the success of this program are in the form of training, demonstrations and practice questions. The implementation of this Community Service has not fully achieved the expected targets and outputs, but this activity has made a positive contribution to students in terms of recognizing letters, spelling, reading and simple arithmetic.

Abstract

BACKGROUND Various techniques are available for hypospadias surgery, but the most effective suturing techniques and suture materials remain debated. This study aimed to evaluate the outcomes of hypospadias repair using various suturing techniques and materials. METHODS This prospective cohort study was conducted from August 2023 to August 2024 and included all patients who underwent hypospadias surgery at Cipto Mangunkusumo Hospital. We compared the outcomes of 2 suturing techniques (interrupted versus continuous) and 2 suture materials (6-0 polydioxanone [PDS] versus poliglecaprone 25) used for neourethra suturing. Follow-up assessments were conducted at 1, 2, and 4 weeks postoperatively, and subsequently every 3 to 6 months, focusing on complications. RESULTS 162 participants were included, with a mean age of 6.27 (3.99) years and a mean follow-up period of 20.39 (14.25) weeks. Complications occurred in 31 patients (19.1%), with a significant association between hypospadias type and complication occurrence (p = 0.039). Complication rates were 9.4% in distal, 12.5% in midshaft, and 26.8% in proximal cases. No significant differences were found between suturing techniques (interrupted: 19% versus continuous: 12%, p = 0.429) or suture materials (6-0 PDS: 22% versus 6-0 poliglecaprone 25: 9%, p = 0.998). Subgroup analyses by hypospadias type also showed no significant differences by suture type or technique (all p>0.05), although a trend toward significance was noted in proximal cases (p = 0.062). CONCLUSIONS The choice of suturing techniques and suture materials does not affect complication rates in hypospadias repair, allowing surgeons to decide based on personal expertise and preference.

Abstract

Some members of Sahabat UMKM NTB face serious challenges in terms of licensing, especially because they do not yet have a valid business license in accordance with the provisions of law in Indonesia. The absence of a business license is a serious violation of Law Number 3 of 2020 concerning the Electronically Integrated Business Licensing System (SPBE), as well as other related regulations that require every business actor to have a license to carry out business activities. This condition not only causes legal problems that have the potential to result in administrative sanctions, but also has a significant impact on the sustainability of their business, such as difficulty in accessing financing and business legality. The method used in this service is through socialization and workshops aimed at members of Sahabat UMKM NTB who do not yet have a business license. This socialization activity aims to provide a deeper understanding of the importance of a business license and guide them in the process of issuing the license. As a result of this service, on October 12, the service team from Bumigora University held an OSS-RBA Application Implementation Workshop at the Mataram Mayor's Office Hall. During the workshop, several members of Sahabat UMKM NTB successfully issued their business licenses through the OSS-RBA application. In addition, the service team also provides assistance in the process of establishing a Limited Liability Company (PT) for members who need it. This activity shows a positive impact, where MSME members who do not yet have a business license can complete their permits, which in turn will strengthen the legality and sustainability of their business in the future.

Abstract

The issues faced by Indonesia in the 2019 elections are likely to reemerge in the 2024 elections. In the 2019 elections, digital media became a highly effective tool for disseminating information related to politics and governance. Unfortunately, not all information available on digital media was accurate. Social media, in particular, saw a lot of misinformation and the spread of fake news (hoaxes). This led to a negative perception of the government among the public, which consequently influenced the opinions and political participation of Indonesian society. Therefore, a transformation is needed to support the formation of a positive government image. One of the strategies is maximizing government public relations. On August 8, 2023, the Communication Department at the Faculty of Social and Political Sciences, Universitas Airlangga, held a community service activity on the topic of government public relations training in Magetan Regency to address the political year. The main target of this activity was government officials in Bulukerto Subdistrict, Magetan District, Magetan Regency. The implementation mechanism consisted of three stages. In the initial stage, the community service team conducted surveys and coordinated with local government officials to gain a detailed understanding. During this stage, the Communication Department observed the involvement and performance of government public relations to obtain an initial overview of the strategies or designs used for the training. Subsequently, it was decided that the activity would be conducted through seminars and workshops covering topics such as public speaking, digital government, corporate social responsibility, and government public relations. After the activity, an evaluation was conducted to measure its success. It is hoped that this activity can serve as a foundation for improving the function of government public relations in addressing the political year and can be a supportive factor in ensuring the success of the 2024 elections.

Abstract

Objective: This study aims to investigate the characteristics of androgen receptors (AR), estrogen receptor 1 (ER1) and estrogen receptor 2 (ER2) expression in dartos tissue of patients with congenital hypospadias, compared to normal penis. Material & Methods: We harvested 63 dartos tissue consisting of 53 congenital hypospadias that underwent urethroplasty (20 distal and 33 proximal) and 10 normal penis that underwent circumcision as controls from September 2017 to September 2018. The expressions of AR, ER1, and ER2 were measured using Quantitative Real-Time PCR (qPCR). All data were analyzed by Prism 7, and one-way ANOVA tests were used to compare gene expressions between the groups. Results: The mean age was 68.99 (± 45.5) and 65.6 (± 25.8) months in boys with and without hypospadias, respectively. The expression of mRNA AR was decreased in proximal (6.26 ± 2.30) and distal hypospadias (6.43 ± 2.22) compared to controls (9.69 ± 1.10), which were statistically significant (p=0.0001 and p<0.0001, respectively). We found a statistically significant difference of ER1 expression compared to controls (p=0.0064). The expression of ER2 was significantly increased in distal (21.03 ± 5.00) and proximal hypospadias (25.21 ± 8.06) groups compared to controls (11.80 ± 2.49) (p<0.0001). There was no statistically significant mean difference in mRNA ER1 expression (p=0.65). Conclusion: The repressed AR and elevated ER mRNA as shown in our study may suggest that defects in those receptors’ interaction and/or balance may contribute to hypospadias and penile curvature condition. Further studies are needed to evaluate any gene-related problems in hypospadias. Keywords: Hypospadias, androgen receptor, estrogen receptor, dartos tissue.
