Abstract

Background: Early initiation of breastfeeding can reduce the infants mortality under the age of 28 days and can affect exclusive breastfeeding process (Roesli, 2010). In early initiation of breastfeeding and exclusive breastfeeding, there is disparities of each health center in Bantul district. Objective: The aim of this study was to identify the association between maternal education, maternal occupation, place of childbirth, birth attendant, and health provider’s support with early initiation of breastfeeding. Methods: The study was conducted from March 23 to April 13, 2014, used crossectional design. Subjects were the entire breastfeeding mothers with infants aged 6-8 months who meet the inclusion and exclusion criteria (204 samples). Sampling with consecutive tecnique. Data obtained from secondary data and questionnaires. Data Analysis with SPSS using Chi-square test. Results: In bivariate analysis, there was no statistically and clinically significant association between maternal education with early initiation of breastfeeding p=0.882; OR=0.9; 95% CI=0.44 to 2.03, maternal occupation p=0.463 : OR=1.3 : 95% CI=0.65 to 2.59 , and birth attendant p=0.211 : OR=4.8 : 95% CI=3.69 to 6.33. Other factors that have a statistically and clinically significant association with early initiation of breastfeeding is the place of childbitrh p=0.007 OR=16.4; 95% CI=1.78 to 150.96 and health provider’s supports p=0.001 : OR=4.4 : 95% CI=2.03 to 9.68. Conclusions and suggestions: There is statistically and clinically significant association between place of childbirth and health provider’s supports with early initiation of breastfeeding. Further research is expected involving the variable support of her husband/family with a prospective cohort to examine the association between early initiation of breastfeeding with exclusive breastfeeding. Abstract

ABSTRACT Child treatment in PICU is a total treatment that conducted by health workers, so the family involvement in this room is limited. Family involvement is important component in Family Centered Care. Purpose of this study is to explore The Family centered Care in PICU. This study used phenomenology qualitative and conducted with purposive sampling to 12 participants consisting of parents, nurses, physicians. Data Collected by in depth interview, observation, and study document. Study verbatim of interview was Colaizzi analyzed manually using Open Code Software. Result of themes were (1) Value of parents and child in PICU, (2) parents and nurses perception, (3) the implementation, (4) benefits, (5) challenges, (6) the expectation of Family Centered Care implementation in PICU. The restricting rules in PICU was caused family couldn’t participated for treatment child in PICU. Recommendation of this research is needed easy access for family to involve in treatment their child in PICU.

Abstract

Background: The era of globalization bring nursing education institutions in Indonesia run nursing education process varies with the quality control system education that goes well, but not optimal so that the quality of graduates is very diverse. One of the efforts to realize the improvement of the quality and equity of health workers is to improve the quality control of graduates through the competency test. Meanwhile, the utilization of information and communication technology (ICT) -based e-learning is a new paradigm in health education. The need for nurses prepares for a competency test into an opportunity for universities to take advantage of ICT-based e-learning for nurse competency test preparation. Methods: This study is action research. Stages of research begin with needs analysis through interviews, observation and focus group discussion involving three lecturers and 14 participants UKNI. The results of the needs analysis are used as a reference in designing the prototype e-learning. E-learning prototype evaluation was performed using the theory of the Technology Acceptance Model (TAM). Results: The prototype eLearning developed in accordance with the needs analysis include: the accessibility of e-learning, quality of information, human resources and management content.   The results of the evaluation of the prototype e-learning on perceived usefulness, 51.8% of respondents strongly agree and 47% agree, while the perceived ease of use, 42.9% of respondents strongly agree and 56% of respondents agreed. Conclusions: The need for the development of e-learning for UKNI preparation is the substitution, meaning that learning is done through the internet entirely. The evaluation results usefulness respondents perceived the prototype e-learning was helpful. The results of the evaluation of perceived ease of use of respondents feel the ease operate prototype e-learning.

Abstract

Latar belakang : Angka Kematian Ibu (AKI) dan Angka Kematian Bayi (AKB) merupakan indikator kualitas kesehatan masyarakat di suatu negara. Di Indonesia pada tahun 2012, AKI 359 per 100.000 kelahiran hidup sedangkan AKB sebesar 32 per 1000 kelahiran hidup, atau ada lebih dari 200.000 balita Indonesia yang meninggal setiap tahunnya. Perawatan ekstra untuk bayi prematur dengan menggunakan Kangaroo Mother Care (KMC). . KMC merupakan cara yang efektif untuk memenuhi kebutuhan bayi yang paling mendasar yaitu kehangatan, air susu ibu, perlindungan dari infeksi, stimulasi, keselamatan dan kasih sayang. Metode: ini merupakan salah satu teknologi tepat guna yang sederhana, murah dan sangat dianjurkan untuk perawatan BBLR. KMC tidak hanya sekedar menggantikan peran inkubator, namun juga memberikan berbagai keuntungan yang tidak dapat diberikan incubator. Berdasarkan fakta di atas maka dapat dirumuskan masalah penelitian bagaimanakah pelaksanaan KMC oleh perawat pada ibu yang memiliki BBLR di RSUD Prof. Dr. Margono Soekarjo Purwokerto. Hasil observasi pelaksanaan KMC menemukan adanya perbedaan signifikan pada pelaksanaan KMC setelah intervensi dibandingkan dengan sebelum intervensi dengan p=0,000 (<0,05). Yang berarti hubungan yang signifikan antara pengetahuan dengan pelaksanaan tindakan perawat dalam KMC. Kata Kunci: Bayi Premature, Kangaroo Mother Care (KMC), Kematian Bayi

Abstract

Background: Excess iron levels and low hemoglobin (Hb) levels are one of the complications of Beta-Thalassemia. The implementation of education using WhatsApp is expected to increase parental knowledge of the illness and children to adhere to iron chelation therapy. Objective: To identify the effect of the implementation of education using WhatsApp through the SKEDit application compared to using booklets on parental knowledge and medication adherence in pediatric patients with thalassemia. Method: The study was quasi-experimental using pre and post-test with control group.  The total sample was 61 respondents with purposive sampling technique. Parental knowledge was measured using a Parental knowledge about Thalassemia questionnaire and the level of adherence was measured using an indirect method known as 'Pill Count'. The data were then analyzed using the Mann whitney and wilcoxon test. Results: There was a significant difference in the level of knowledge in the pretest and posttest on the intervention group (p: 0.001) and the pretest and posttest in the control group (p: 0.001). There was a significant difference in the level of adherence of the pretest and posttest in the intervention group (p: 0.028). There was no significant difference in the level of adherence of the pretest and posttest in the control group (p: 0.214). There was no difference in the level of knowledge between the two groups (p: 0.994). There was no difference in the level of adherence between the two groups (p: 0.987). Conclusion: Providing education using WhatsApp through the SKEDit application and also booklets both effectively increase the level of parental knowledge and medication adherence of pediatric patients with thalassemia.

Abstract

Background: Nursing Outcome Classification (NOC) Self-care for Toileting is an instrument for measuring the abilities of toileting self-care among children with physical disabilities. However, there has been no cultural adaptation in the Indonesian context, especially among children with physical disabilities, so that the validity and inter-rater reliability of the instrument has not been reported. Testing the content validity and interrater reliability of this instrument is necessary so that it could be used to measure the ability of toileting self-care.Purpose: This study aimed to examine the content validity and inter-rater reliability of NOC Self-care for Toileting among children with physical disabilities.Methods: This was a descriptive quantitative non-experimental research with a cross-sectional design. In the content validity test, this study employed seven experts teaching in nursing academies with a minimum degree of S2/specialist and had the experiences in basic nursing to assess the validity of NOC Self-care for Toileting. As for the inter-rater reliability test, this study involved two raters of nursing students who have passed block 2.4 lecture on “Growth and Development” and 36 children with physical disabilities at a Special School in Bantul, Yogyakarta. The validity test was analyzed using CVR, CVI, and Aiken’s V indexes, while the inter-rater reliability test was analyzed using Kappa and percent agreement.Results: The validity test showed the CVR value range of 0.71-1.00, the CVI value of 0.91, and the V coefficient range of 0.86-1.00. In the inter-rater reliability test, the Kappa value obtained was 0.958, and the percent agreement value was 97.22% for all indicators of NOC Self-care for Toileting.Conclusion: NOC Self-care for Toileting is a valid and reliable instrument for measuring the abilities of toileting self-care among children aged 6-18 with mild to moderate degrees of physical disabilities. Further research can be conducted by involving different respondents and cultures with more than two raters if possible.

Abstract

Stunting events in Indonesia have fallen by around 7% in the last 5 years. That declined nationally has not been followed by a decrease in the prevalence of stunting in all primary health care (PHC), one of the PHC in Gunung Kidul Regency, has been able to reduce stunting prevalence in the last 3 years. The purpose of this study was to find out how the challenges and implementation faced by PHC practitioner Gunung Kidul Regency to overcoming the problem of stunting. Qualitative descriptive with single case study design. Data collection was conducted with semi-structured interviews with 5 PHC practitioners and triangulation of resources with cadres and village government. PHC practitioners village government and cadres had the same perspective to overcoming the problem of stunting, but the community was still unfamiliar. This was a challenge for health center practitioner. In addition, the measurement of growth of children under five carried out by cadres at the posyandu was considered to be inaccurate health workers. Therefore the implementation of the 1000 HPK Movement places more emphasis on strengthening the capacity of cadres in capturing stunting toddlers and how cadres provide infant and child feeding counseling so that stunting toddlers were more quickly detected and stunting problems were more likely to be overcome. PHC practitioners do the same perception with the village government and cadres in overcoming the problem of stunting, so that the collaboration has an impact on reducing the prevalence of stunting. The challenge faced by PHC practitioners is to maintain the accuracy of the measurement of infant growth by cadres in the posyandu by increasing assistance.

Abstract

Background: Breastfeeding will provide a wide range of benefits for both mother and baby. Breastfeeding is one of the physical adaptations and processes of changing maternal roles in postpartum mothers. Not all mothers can go through this process correctly. A small amount of milk production on the first day of postpartum is one of the complaints that is often felt by mothers. The hormone prolactin is one of the factors that influence milk production. One way to stimulate the production of the hormone prolactin is by breast treatment. Breast care will affect hypopise to release the hormone progesterone, estrogen, prolactin, and oxytocin more. Objective: The purpose of this study was to identify the effect of pressure on the Jianjing GB-21 acupressure point and Oketani massage on the level of the hormone prolactin.Method: The type of this research is a quasi-experiment with the design used is a pre-post-test design with the control group. Prolactin hormone levels were examined before the intervention (on the first day) and on a ninth day. The examination was carried out using the ELISA (Enzym Linked Immunosorbent Assay) technique. Data obtained was analyzed using an independent sample T-test.Result: The study showed that there was an increase in mean values of maternal prolactin levels after oketani massage intervention and pressure on GB-21 acupressure points. The administration of a combination of interventions, namely Oketani massage and pressure at the GB-21 point, can increase the highest levels of the hormone prolactin, which is 3.41. Whereas in the control group that did not get any intervention, had the lowest levels of the hormone prolactin that is equal to 2,925Conclusion: it can be concluded that oketani massage and applying pressure at the GB-21 point can increase the levels of the hormone prolactin, respectively.

Abstract

This study aimed to conduct a psychometric evaluation of the Indonesian version of the Communities Advancing Resilience Toolkit Assessment Survey (CART-AS) instrument with validity and reliability testing. A cross-sectional study was conducted among 280 respondents. The translation was done using the Beaton method. The psychometric test process was conducted by testing the construct validity and reliability. Confirmatory factor analysis (CFA) values indicated that the model fits the previous model with the Chi-square/df=1.37 conformity indicator; comparative fit index (CFI)=0.99; goodness of fit index (GFI)=0.90; root mean square of approximation (RMSEA)=0.03; and standardized root mean square residual (SRMR)=0.03. In the CFA measurement, the convergent validity value or the loading factor value of each indicator was also found in the range of 0.55-0.99. The average variance extracted (AVE) values on the five constructs of the CART-AS instrument show values in the range of 0.55-0.98. The results of the reliability test showed the Cronbach alpha value of 0.94. The Indonesian version of the CART-AS instrument is a valid and reliable instrument to measure the resilience of disaster-affected communities in Indonesian culture.

Abstract

Patients, especially those who use health insurance have felt that the quality of primary care services tends to be less satisfying in terms of both public services and dental care services. Based on the results of the annual report at the Kahuripan Health Center, the number of patient visits to BPG decreased from 2016 to 2017 by 802 people. This study aimed to determine the quality of dental care services and the satisfaction of patients visiting BPG Kahuripan Health Center. This case study used a qualitative method and an explanatory research design. The sample in this study consisted of 40 patients as the respondents responding to the questionnaires about service quality and patient satisfaction, and 12 people were interviewed. The results showed that all dimensions of the quality of dental care services at the Kahuripan Health Center were categorized as very good i.e. real dimensions (82.71%), reliability (83.21%), responsiveness (87.12%), assurance (84.93%), and empathy (83.54%). As for patient satisfaction, three stages of patient satisfaction were categorized as very satisfied, namely the anamnesis and diagnosis stage (83.02%), action (83%), and completion of action (82.62%). The stages categorized as satisfying were the registration and waiting room stage (78.87%), and the stage of preparation of treatment (72.50%). The result of interview showed that the average respondents stated that the quality of dental care services was very good. All the five dimensions of the quality of dental care services were categorized as very good. Patients who visited BPG expressed satisfaction with the five stages, namely they were very satisfied with the three stages and satisfied with the other two stages.
