Abstract

Background: Maternal and infant mortality rates in Aceh province are still quite high compared to other provinces in Indonesia. In addition to disease factors, the factor that contributes to the rates is socio-cultural factor. Objective: This study aims to know and understand the view of postpartum mothers, health personnel, cadres, and the chairperson of the Aceh traditional assembly about the culture related to postpartum and newborn and integrated infant management program (MTBM) in Aceh. Methods: This was a qualitative study. Data collection was conducted from October 2016 to January 2017 on seven health cadres by focus group discussion and in-depth interviews on five postpartum mothers, one health worker, and the Aceh traditional assembly chairperson. The open-code software program was used for data analysis. Results: Three themes emerged from data: 1) The 'Madeung' Culture in postpartum mothers in Aceh has a relationship with maternal health problems, 2) 'Peucicap' and 'troen tanoh' cultures in newborns in Aceh are at risk of causing complex problems in newborns, and 3) Community-based integrated management of young infants can be implemented while maintaining Acehnese culture. Conclusion: To improve access to maternal and neonatal health services with socio-cultural issues, the community-based integrated management of young infants is considered appropriate in Aceh culture.

Abstract

Background: Integrated Management of Childhood Illness (IMCI) is an integrated guideline in dealing with infants and sick children at the community health center. However, many students cannot apply this guideline because they are not being exposed. Objective: This study aims to explore the perspectives of nurse educators and clinical instructors regarding the effectiveness of teaching learning process of IMCI in diploma nursing students. Methods: This was a qualitative study with interpretive approach. There were nine informants selected using purposive sampling, which consisted of nurse educators and clinical instructors. Data collection was conducted in December 2016 - February 2017 using focus group discussions and in-depth interviews. Data were analyzed using Colaizzi process. Result: There were four themes emerged from data, namely 1) Competency of IMCI for Diploma Nursing Students, 2) Deepening of IMCI Material, 3) Learning methods of IMCI in the class and clinic should be active and structured, 4) Student confidence in the application of IMCI in clinical setting. Conclusion: The learning process of IMCI will have an impact on the improvement of knowledge, skills and attitude in the application of IMCI in the clinical setting.

Abstract

Background: Social-emotional development in preschooler children is an important component in child development, as it becomes the foundation in preparing children for confidence, empathy and intellect, building trust, and being able to use language in communication and connect with others. One of the factors that affect children's emotional social development is the quality of mother's care. Children of young mothers have risks in emotional and intellectual social problems in children. Objective: The purpose of the study was to determine the relationship of quality care of young mothers with social-emotional development in preschool children in the working area of the Health Center of Kemalang, Klaten Regency, Indonesia. Methods: This was a correlational analytic research with cross sectional design. The sample of the study was 124 young mothers with preschool children selected using consecutive sampling technique. The study was conducted from July to August 2017 at six kindergarten schools in the working area of the Health Center of Kemalang, Klaten Regency. Data were collected using parent behavior questionnaire and stage social-emotional instrument. Data were analyzed using chi square test. Results: Findings showed that 58.1% of young mothers had good quality of care, and 55.6% of preschoolers were at risk of developing emotional social development problems. There was a significant correlation between the quality of care of young mothers with social-emotional development in preschool children (p-value <0.05). Conclusion: There was a correlation between the quality of care of young mothers with social-emotional development in preschool children.

Abstract

Background: Although the WHO strategy integrated management of childhood illness (IMCI) for primary care has been implemented in over 100 countries, there is less global experience with hospital-based IMCI training. Until recently, no training had been done in Indonesia, and globally there has been limited experience of the role of IMCI in rebuilding health systems after complex emergencies. Objective: We aimed to examine the effect of hospital-based IMCI training on pediatric nurse competency and explore the perception of Indonesian doctors, nurse managers and pediatricians about IMCI training and its development in West Aceh, a region that was severely affected by the South-Asian tsunami in December 2004. Methods: This study used stepped wedge design. Training was conducted for 39 nurses’ staff, 13 midwives, 6 head nurses, 5 nurse managers, 5 doctors, 1 pediatrician, and 3 support facilities (nutritionist, pharmacist, laboratory) in Cut Nyak Dien (CND) Hospital in Meulaboh, West Aceh, Indonesia. The IMCI training was developed based on the WHO Pocketbook of Hospital Care for Children. A nurse’s competency questionnaire was used based on the guideline of assessment of the quality of child health services at the first level reference hospitals in districts / municipalities issued by the Ministry of Health in 2007. A linear mixed model was used for data analysis. Results: The hospital based IMCI training improved the competences of nurses pediatric in assessing emergency signs of the sick children, management of cough and difficulty breathing, diarrhea, fever, nutritional problems, supportive care, monitoring, discharge planning and follow-up. The assessment highlighted several problems in adaptation process of material training, training process and implementation in an environment soon after a major disaster. Conclusion: Hospital based IMCI training can be implemented in a setting after major disasters or internal conflict as part of a rebuilding process. The program requires strong management support and the emergency phase to be subsided. Other pre-requisites include the existence of standard operating procedures, adequate physical facilities and support for staff morale and well-being. Improving the quality of pediatric care requires more than just training and clinical guidelines; internal motivation and health worker support are essential.

Abstract

Background: One of the efforts in the health sector for programs to reduce infant and under-five mortality and morbidity rates is by the competence-based integrated management childhood illness (CB-IMCI) program. However, the program has been poorly implemented by health workers at the health center. Thus, the involvement of cadres is needed and their competency remains important to be improved. Objective: To quantitatively analyze cadre knowledge and competencies in the CB-IMCI program, and its relationship with the implementation of CB-IMCI, as well as to qualitatively explore and analyze the responses of cadres, health workers, and the community to the addition of cadre roles to the management of sick children. Methods: This was a mixed methods study with randomized controlled trials and qualitative method. For randomized controlled trials, 100 cadres were selected, which 50 randomly assigned in the intervention and control group. And for quantitative methods, informants included 10 cadres, 4 health workers, and 20 mothers. Paired t-test and independent t-test were employed for quantitative analysis, and triangulation was used for qualitative analysis. Results: The improvement of knowledge in the intervention group was better (I=14.42/C=4.44/p-value 0.00). The intervention group was more competent in the management of child illness (p-value 0.00). In addition, the cadres have additional competencies assisted by health workers to confidently detect cases earlier, and health center services reach a wider coverage in the community. Conclusion: The addition of cadres’ roles and competencies in the management of child illness in the community can be implemented in Banyumas District. It is suggested that the implementation of this program should be followed up in other health centers by the Health Department of Indonesia.

Abstract

Adolescent with cancer have a complex problem. Adolescents with cancer at risk for developmental phase delay caused limitations in carrying out its role as adolescents, because of the side effects of medication and intensive relationships with health care providers. Nurses are health workers who have a frequency of more interaction to the teenagers who suffer from cancer. Form of nursing care provided by nurse will shape perceptions of teenagers. Adolescent perception of cancer patients with the care received does not match the hope will establish a nursing care desired by adolescents. This study to explore perception and hope of adolescents toward quality of nursing care in Adam Malik Hospital Medan. Qualitative research methods through a phenomenological approach. Adolescent with cancer as research subjects, aged 13-19 years who had been diagnosed with cancer and was undergoing treatment protocol. The sampling technique used the maximum variance. Data were collected through semi-structured interviews with tools interview, observation, field notes, and tools as well as the record will be analyzed by the method Colaizzi The themes that reflect the perceptions and hope of adolescents to nursing care include 1) an overview of nurses’ profile, 2) the ability and skills of nurses, 3) nurses’ attitude, 4) preparedness nurses, 5) the provision of social support. Adolescents also hope hospital’s management add room facilities in order to easy to access the health service especially service care for adolescents. Adolescents with cancer have a different impression at the beginning of the meeting and after getting care by nurses. Further researcher can choose suitable location to interview and research about hope of adolescent’s cancer and nurse’s role in adolescent’s cancer.

Abstract

ABSTRAKTujuan penelitian: Untuk mengetahui pengalaman ibu dalam memberikan ASI sebelum, saat, dansetelah terjadi bencana erupsi Merapi 2010. Metode: Penelitian dengan rancangan kualitatif danpendekatan fenomenologi. Partisipan penelitian ialah 5 orang ibu menyusui yang berada di huniantetap Cangkringan. Analisis data menggunakan metode Colaizzi. Pengumpulan data dilakukan padabulan September sampai Desember 2013. Izin etik diperoleh dari Komisi Etik Fakultas KedokteranUniversitas Gadjah Mada. Hasil: Pada kondisi sebelum bencana, terdapat satu tema yang diperolehyaitu pemberian ASI lancar meskipun ada kesulitan. Pada saat terjadi bencana, diperoleh duatema yaitu kenyataan di shelter yang berbeda dengan harapan dan dukungan sosial diterima Ibudari suami dan masyarakat. Pada kondisi setelah bencana diperoleh satu tema yaitu kelancaranPemberian ASI. Kesimpulan: Pada kondisi bencana, ibu yang menyusui mengalami berbagaitantangan agar tetap dapat menyusui anaknya secara eksklusif. Ibu yang menyusui disarankandapat mengantisipasi apabila sewaktu-waktu terjadi bencana untuk mempersiapkan bekal berupapengetahuan terkait merawat payudara dengan benar serta menjaga agar ASI tetap lancar meskipundalam kondisi cemas yaitu dengan tetap memberikan ASI pada anak dan mengkonsumsi makanansehat agar nutrisi tercukupi. Bagi tenaga kesehatan diharapkan dapat lebih banyak memberikanedukasi terkait kesulitan dalam menyusui. Pemerintah diharapkan lebih memperhatikan kondisikesehatan korban bencana khususnya kesehatan ibu dan anak.Kata Kunci: ASI, bencana.WOMEN’S EXPERIENCE OF BREASTFEEDING BEFORE, DURING AND AFTER THE 2010ERUPTION OF MERAPIABSTRACTObjective: To identify women’s experience of breastfeeding before, during and after the 2010eruption of Merapi. Methods: The study employed qualitative design and phenomenologicalapproach. Participants consisted of 5 breastfeeding mothers in permanent housing of Cangkringan.Data were analyzed using Colaizzi. Data were collected from September to December 2013.Ethical clearance was obtained from the Ethics Committee of the Faculty of Medicine, Gadjah MadaUniversity. Results: In the pre-disaster, one theme was obtained that breastfeeding was smoothdespite of diffi culties. During the disaster, two themes obtained that reality in shelters was differentfrom their expectation and social support was received by mothers from their husbands and society.After the disaster, one theme was obtained that breastfeeding was smooth. Conclusion: During thedisaster, breastfeeding mothers experienced various challenges in order to able to breastfeed theirchildren exclusively. It is suggested that breastfeeding mothers could anticipate a disaster whichmight occur at any time by preparing knowledge of how to treat breast properly and to keep thebreastmilk drain well despite of anxious conditions by breastfeeding continuously to children andconsuming healthy food in order to obtain adequate nutrition. It is expected that healthcare workersprovide further education related to diffi culties in breastfeeding. It is expected that the governmentpays more attention to the health condition of disaster victims, particularly mother and child health.Keywords: Breastmilk, Disaster

Abstract

Background: Sometimes ago child mothering generally done by mother because they are not work. Nowdays, at the height of education of woman and demand of live the effect of monetary crisis that was made mother work to earn life. Mother with child of preschool ought to be correct how to lead time prudently, because the basic in raising child personality especially self confidence was at preschool. The problem is how mothering applied by mother to their children so that the child growing up to become strong and stabilize person, with high self confidence.Objective: The aim of this research is knowing whether there are difference mothering style between working and not working mother with the growing up child's self confidence. Methods: This research was an quantitative method by using cross-sectional design.The analysis of the data was using Chi-square. This research was doing to ward 51 couples of mother and their child which is studying in TK Purbonegaran Sagan Yogyakarta, 43 respondents is fulfilling criterion divided in two group that is working mother 17 people (39,5%) and not working mother 26 people (60.5%). Data collecting by using questionnaire consisted of respondent identity, mothering style questionnaire, and child's self confidence questionnaire.Results: The result of this research showing there was no significant difference of mothering style between working and not working mother (p > 0,05). There was difference of child's self confidence between working and also not working mother (p < 0,05). There was significant difference of child's self confidence between autoritarian, permissive, and authoritative pattern (p < 0,05). There was no difference of mothering style between working and not working mother with the growing up the child's self confidence divided in social interaction aspect, independence aspect, and also tolerance aspect (p > 0,05).Conclusion: The categorizing of child's self confidence shows that mostly child have middle category with autoritative pattern, and mostly child have middle category of child's self confidence with not working mother.Keywords: child's self confidence, mothering style, working mother, not working mother

Abstract

The quality of caregiver and children under five interaction in YogyakartaPurposeThe purpose of this study was to determine factors related to the quality of caregiver interactions with children under five in Yogyakarta. MethodsThis study used a cross sectional design. Research subjects were all caregivers who have children aged 1.5 years to 4.5 years who have received cadre assistance in the work area of Mantrijeron Puskesmas Yogyakarta. Research subjects were recruited using total sampling techniques. The instrument used was questionnaire and PICCOLO observation sheet. Data analysis was done by Chi-Square test.ResultsThere was a significant correlation between knowledge factor and quality of domain interaction. There was no significant correlation between age factor, occupation, mental status, and income with interaction quality. ConclusionCaregivers are expected to improve the quality of interaction and communication with children, have sensitivity and responsiveness in practicing specific care, such as feeding, sensitivity to sick children, and able to develop each other's cognitive language.

Abstract

Factors associated with adolescents psychosocial problems in the Code riverbank in YogyakartaPurpose: To identify factors related to adolescent psychosocial problems in the watershed of the Code River in Yogyakarta.Method: The samples are 173 adolescents aged 12-19 years who were selected by consecutive sampling technique based on predetermined inclusion and exclusion criteria. The research instruments used were questionnaire of respondent characteristics, Pediatric Symptom Checklist-Youth Report questionnaire, Child And Adolescent Social Support Scale questionnaire, and residence environment questionnaire. This study used univariate analysis, Chi square test, Fisher test and logistic regression test.Results: A total of 11.6% of adolescents in the Code riverbank area experienced psychosocial problems. Level of education, parental support and the fairness of residence are associated with adolescent psychosocial problems, with level of education as the greatest factor.Conclusion: Adolescent psychosocial problems that occur in the watershed of the Code River in Yogyakarta are related to educational level, parental support and the fairness of adolescent residence.
