Abstract

Neonates with low birth weight (LBW) have a higher risk of growth failure. To optimize their growth to target of 15-20 g/kg/d, neonates must receive adequate nutrition. Parenteral nutrition implementation is one of the approaches to provide adequate energy and nutrition to LBW neonates who can not tolerate enteral feeding. This study aims to investigate growth velocity in LBW neonates receiving parenteral nutrition in hospital settings and observe if the target growth velocity was achieved, also describes current parenteral nutrition practices in tertiary hospital settings. A retrospective observational study was conducted in the Neonatal Intensive Care Unit (NICU), Kandou General Hospital, Manado. Sample in this study were neonates with birth weight <2500 g and received only parenteral nutrition for minimum 7 d. Daily weight measurement data was collected through medical records and parenteral nutrition regimens was collected through electronic prescriptions in the Pharmacy Department. Growth velocity was assessed using two parameters, gain weight velocity and change in weight for age Z-scores (WAZ). A total of 73 neonates were used as samples. The results demonstrated that only the extremely low birth weight (ELBW) group achieved the growth velocity recommendation during parenteral nutrition, 16.17 g/kg/d. All the neonates had a negative change of weight Z-scores indicated a declining growth rate during parenteral nutrition administration. We also found lipid emulsion initiation time was later and relatively low energy intake in parenteral nutrition compared to the recommendation. Vitamin as micronutrient was not yet added in parenteral nutrition regimens. Therefore, we suggest starting lipid emulsion from the first day of parenteral nutrition and add vitamin to parenteral nutrition regimen to optimize nutrient intake in order to improve growth velocity in LBW neonates during NICU stay.

Abstract

Family caregivers for elderly play a very important role in implementing drug adherence, especially in Asian countries. Various determinants of medication adherence included caregivers as family support. Although the relationship between family support is not always significant, understanding the factors of family caregivers on medication adherence is important, especially when improving adherence in Asian countries. The following scoping review was conducted to provide an overview of caregivers, and its relationship to medication adherence in Asian settings. PRISMA-ScR guideline and checklist was used for the search process and to guide reporting. Review selection and characterization were performed by reviewers independently. In total, 22 papers were included in the review, published from 2011 to 2021. The studies varied in terms of purpose, methodology, and detail of reporting. Family caregiver is an important determinant of medication adherence, especially for areas where extended family culture is still strong, such as in Asia. Understanding the local culture, aspects of family burden and economic, as well as the role of the health profession, especially pharmacists in health services are needed to improve adherence. Therefore, further research with quality methods is needed.

Abstract

The 2020 Indonesian Health Profile states that diarrhea is an endemic disease that has the potential to cause extraordinary events. Diarrhea is still one of the contributors to death in Indonesia, especially in children under 5 years old. This study aimed to evaluate the use of zinc and probiotic supplementation on the length of stay (LoS) of patients under 5 years old with diarrhea. It was an observational study using medical records of patients under 5 years old with diarrhea at the Academic Hospital, UGM Yogyakarta in the period of 2021 to 2022. Patient characteristics, including use and medications such as zinc supplementation and probiotics, as well as clinical outcomes including LoS were obtained from 100 medical records. Data were analyzed using chi square from Microsoft Excel 365 & Statistical Program for Social Science (SPSS) with a 95% confidence level (p<0.05). The majority of patients were male (61%) with age range 12-23 mo (47%), 3-4 times frequency of diarrhea (69%), dehydration mild-moderate (89%), and acute diarrhea (96%). The treatment of diarrhea was antidiarrheal (44.81%), fluid rehydration (32.55%), and antibiotics (22.64%). Most patients left the hospital in an improved condition and were allowed to go home (92%). There is a significant relationship between the use of zinc, probiotics, and a combination of zinc and probiotics on LoS.

Abstract

According to the 2018 Indonesian Basic Health Research data, the prevalence of hypertension in the Yogyakarta Special Region (11.0%) is higher than the national average of 8.8%. Hypertension with chronic kidney failure is currently a prevalent and increasing issue. This study aimed to investigate the pattern and rationality of antihypertensive use in hypertension patients with chronic kidney failure (CKF) participating in the National Health Insurance (Jaminan Kesehatan Nasional/JKN) program at Universitas Gadjah Mada (UGM) Academic Hospital period of 2020 to 2022. The study was conducted using quantitative descriptive non-experimental methods. Data collection was carried out retrospectively through medical records of patients diagnosed with hypertension with CKF (ICD I12.0). Samples were selected using purposive techniques and analyzed by comparing the suitability of drug selection with standard hypertension therapy with CKF based on JNC 8 and NKF-K/DOQI, including the rationality of their use. A total of 68 patients over the age of 18 were used as samples. The The majority of drug groupsresults showed that a combination of 2 drugs is more dominant (52.9%) compared to monotherapy (29.4%) and a combination of 3 drugs (17.6%). The majority of drug groups used were angiotensin receptor blockers (ARBs), especially candesartan (45%). ARB and calcium channel blockers (CCB), especially candesartan+amlodipin (50%), and ARB +CCB+diuretic, especially andesartan +amlodipine + furosemide (33%).. The rationality of drug use showed that 66.2% in the right indications, 89.7% the right drugs, 100% in the right patients and 85% the right doses. Therefore, health workers still need to make efforts to encourage rational drug use.

Abstract

Gastroenteritis (GE) is the leading cause of vomiting in children. It is the main reason children admitted into emergency departments (EDs). Vomiting can cause fluid loss therefore oral rehydration solution (ORT) is recommended as supportive care for mild to moderate dehydration. Antiemetic therapy for GE is not fully recommended by any management guidelines. However, in some studies, antiemetics including ondansetron and domperidone are used in vomiting-related GE.
