Abstract

A preliminary research at PMB Bidan Wike A in June 2023 revealed the developmental outcomes of toddlers using the Pre-Screening Development Questionnaire (KPSP). Out of 189 toddlers, 77.8% exhibited typical development, 21% had questionable development, and 1.2% displayed developmental deviations. Many mothers lacked knowledge about the care and examination procedures related to the pre-screening developmental questionnaire. This community service aims to provide mothers with knowledge about child growth and development while introducing the Pre-Screening Development Questionnaire instrument. This community service utilizes counseling and socialization methods for mothers with toddlers. Participants will undergo both pretest and posttest assessments. This initiative employs the Developmental Pre-Screening Questionnaire instrument and self-evaluation notes. In the initial stage, the counselor will engage in a question-and-answer session, followed by assessing the mother's comprehension using the feedback method. Subsequently, it proceeds with discussions or brainstorming sessions. The outcomes of the community service revealed that following counseling and the socialization of KPSP to mothers of toddlers, their knowledge increased by 93%. Early and regular screening is of utmost importance to prevent developmental deviations in toddlers, allowing for prompt treatment of any deviations that may arise. Growth screening enables the assessment of children's nutritional status, while developmental screening for children under five can be conducted using the KPSP (Developmental Pre-Screening Questionnaire) method. Keywords: growth screening, nutritional status, developmental screening, KPSP method.

Abstract

According to the World Health Organization (WHO) in 2022, the use of family planning in Asia has increased. In Indonesia, the coverage of contraceptive methods within family planning programs is substantial. The role of family planning programs has a significant impact on both women's and men's reproductive health. This study aims to assess the effectiveness of video media in influencing the convenience, knowledge, and attitudes of one-month injectable contraceptive acceptors at PMB I in Sumedang Regency. The study used research (quasi experimental research) with pre and post test design with control group. This research was conducted in July - August 2024 at PMB I Sumedang Regency. The sample in this study consisted of mothers who had attended more than two follow-up visits for 1-month injectable contraceptives. The sampling technique uses total sampling, with a total of 60 respondents, divided between intervention and control groups, with pretest and posttest conducted. An Independent Sample t-test was used for data analysis. The respondents were predominantly between 31-45 years old (63.3%). In both the intervention and control groups, 50% of respondents were aged 30 years or younger. The majority of respondents had an education level of elementary, middle, or high school, and most were unemployed or homemakers. The normality test results were found to be insignificant, prompting the use of non-parametric analysis. The results showed that respondents' knowledge about 1-month family planning (p=0.000), attitudes (p=0.095), and ease of use (p=0.000) were better in the intervention group using video compared to the control group using leaflets. The conclusion of this study is that video media proved more effective in improving user convenience, knowledge, and attitudes compared to leaflets. Thus, video is a more readily accepted form of media in the community.

Abstract

The incidence of spina bifida varies between two to ten cases per thousand births in various regions worldwide. The Ministry of Health of the Republic of Indonesia has conducted sentinel surveillance. From the results of this surveillance, congenital abnormalities of the nervous system rank second among the most commonly found congenital anomalies in Indonesia, with a percentage of 22.3%. One way to reduce MMR and IMR is by providing continuous midwifery care from pregnancy to family planning through the midwifery management approach. The purpose of providing Continuity of Care is early detection of complications that can threaten life, thus worsening the condition of the mother and baby. The method used during the case study was the Continuity of Care method, which includes collecting subjective and objective data, establishing diagnoses, providing management, and documenting in SOAP format. Visits were conducted 9 times: once during pregnancy, once during childbirth, 4 times during the postpartum period, and 3 times during the newborn period. In the third trimester of pregnancy, the mother experienced discomfort, namely frequent urination and back pain, which could be addressed by providing health education about limiting drinks at night and avoiding caffeinated drinks, as well as providing mechanical body health education. There were no problems found during Mrs. G's delivery. During childbirth, the mother received loving care. During the postpartum period, no problems were found, and visits were conducted according to postpartum care standards. Spina bifida was found in the newborn. The baby received care according to the standard care for normal newborns, followed by referral. No other problems were found in the newborn. The conclusion from the assessment of pregnancy, childbirth, postpartum, and newborn care for Mrs. G was conducted according to midwifery service standards. The problems and discomfort experienced could be addressed. In this case, there was a gap where the patient underwent routine examinations with midwives and doctors, but the case of spina bifida was not detected. This may be due to the fact that spina bifida cannot be detected by midwife examinations, and during ultrasound examinations, it is not clearly visible due to the fetus's position. Midwives are expected to be able to continuously improve the quality of midwifery services and take preventive and management measures for cases of spina bifida.

Abstract

Maternal health is an important aspect of the health care system, as the quality of maternal health has a direct impact on the well-being of the mother and her unborn child. High maternal risk refers to conditions that increase the potential for complications during pregnancy, labor and postpartum. Early identification of high maternal risk is crucial to provide appropriate care and reduce the risk of complications that can harm both mother and baby. One tool that can be used to support early detection of high maternal risk is the Pre-Referral Back Sheet (PRBS). PRBS is an instrument that presents a collection of questions or certain parameters that must be filled in by health workers, especially midwives during prenatal examinations. It is designed to identify risk factors and early symptoms that may indicate potential maternal high risk. Although the LBPR has been integrated into maternal health care practice, no in-depth study has specifically evaluated its effectiveness in early detection of maternal high risk. Therefore, this study aimed to evaluate the effectiveness of the Pre-Referral Back Sheet in early detection of maternal high risk and identify the extent to which this tool can improve the quality of maternal health care. This study used an observational method with a cross-sectional approach with a sample of 450 pregnant women who attended prenatal examinations at selected health centers. Of the total 450 respondents, the majority were pregnant women of reproductive age (20-35 years) with an average age of 28 years. 60% of respondents had secondary education, and 80% lived in urban areas. The prevalence of maternal high risk in the Subang Health Office Working Area was found to be 18%, based on the criteria identified by the LBPR. Statistical analysis showed that the use of LBPR significantly improved early detection of maternal high risk. The sensitivity of LBPR reached 75%, with a specificity of 85%. These results indicate that the LBPR can identify most maternal high-risk cases with good accuracy. Based on these findings, it can be concluded that the Pre-Referral Back Sheet (LBPR) is effective in early detection of maternal high risk in the Subang Health Office Working Area. The use of LBPR can improve the quality of maternal health services and contribute positively to efforts to prevent and manage high maternal risk.

Abstract

Normal labor is a complex physical and emotional experience for a mother, with pain being a major element in this process. The interaction between labor pain and maternal anxiety levels is an aspect that requires in-depth understanding, given its influence on the psychophysical well-being of both mother and baby. Although the scientific literature has highlighted the importance of pain and anxiety management during labor, comprehensive studies that consider the psychophysiological and subjective perceptual influences on this dynamic interaction are limited. Therefore, this study aimed to explore the relationship between pain in normal labor and maternal anxiety levels through a multivariable approach. This research method uses a quantitative approach with an analytic observational research type. The research design was conducted prospectively. This research method involves maternal respondents who experience normal labor, with data collection through measuring anxiety levels using standardized questionnaires and psychophysiological analysis using objectively measured parameters. In addition, a multivariable approach was used to analyze the complexity of the interaction between the variables involved. The sample size was 100 laboring mothers, randomly selected from the population of mothers who met the inclusion criteria. The population was mothers giving birth at TPMB Midwife Delima in Bandung City. Using cluster sampling, only 40 pomegranate midwives (10 sub-districts) were taken from a total of 127 pomegranate midwives from 30 sub-districts. The results showed that the average level of pain during labor reached 6.8 with an average duration of 8 hours. The average maternal anxiety level reached 55 on a scale of 0-100. A significant correlation was found between pain and anxiety (r = 0.68, p < 0.05), while psychophysiological influences such as frequency of uterine contractions and blood pressure also played a role in the experience of pain. Social support was also shown to influence anxiety levels (p = 0.005). These findings illustrate the complexity of interactions between psychophysiological and social aspects in the context of normal labor, providing a basis for the development of more targeted intervention strategies in the management of maternal pain and anxiety. The conclusion of this study shows that there is a significant relationship between pain in normal labor and maternal anxiety levels. Psychophysiological influences, such as frequency of uterine contractions and blood pressure, as well as social support, play an important role in determining the mother's experience during labor. Therefore, increased attention to pain management and provision of adequate social support may be key to reducing maternal anxiety levels. A suggestion for health practitioners is to strengthen efforts to integrate holistic pain management strategies and enhance aspects of social support during normal labor, in the hope of improving the mother's overall emotional and physical well-being.

Abstract

The study investigates the relationship between obesity and cardiovascular disease (CVD) risk, comparing the use of body mass index (BMI) versus other biomarkers in predicting CVD risk among obese individuals. It conducts a systematic literature review following the PRISMA guidelines, searching databases for relevant articles published from 2017-2022. The review analyzes 12 eligible articles and finds that factors beyond just BMI, such as genetics, physical activity, metabolic disorders, previous heart disease history, nutrition, fat distribution, and changes in BMI, can significantly impact the prognosis of heart disease in obese individuals. Importantly, the study shows that measures of fat distribution, like waist-to-height ratio, waist circumference, log-transformed body shape index (LBSIZ), and the ratio of visceral adipose tissue (VAT) to subcutaneous adipose tissue (SAT), are superior to BMI in predicting CVD risk among those with obesity. The key takeaway is that while obesity is strongly linked to CVD risk, BMI alone often fails to predict that risk accurately. Fat distribution measures may be a more effective tool for identifying obesity status and predicting associated CVD risk compared to relying solely on BMI.
