Abstract

Introduction: Stroke treatment must be done quickly and precisely because the longer the stroke is not treated immediately, the higher the severity of the stroke. One of the complications of stroke is cerebral circulation disorders which cause several symptoms including cerebral tissue hypoxia. Objective: This study aimed to analyze nursing care for stroke patients by giving elevated head positions to overcome the nursing problem risk of ineffective cerebral perfusion at RSI Surabaya A. Yani. Methods: This research design is a case study with  Mrs. M who has nursing problems at risk of ineffective cerebral perfusion. Data collection was done directly by interview, observation, and physical examination of Mrs. M and her family directly. Nursing interventions for providing elevated head positions are carried out for three days for 30 minutes daily. Results: The results showed that after implementation there was a significant increase in oxygen saturation in Mrs. M on the first day SpO2 95%, on the second day SpO2 97%, and on the third day SpO2 99%. In addition to an increase in oxygen saturation, there was an increase in the outcome criteria to be achieved, namely the level of consciousness on a scale of 4 (moderately improved), headache on a scale of 4 (moderately decreased), average blood pressure values on a scale of 4 (moderately improved), and awareness on a scale of 4 (moderately improved). Conclusions: Giving an elevated head position is effective in increasing oxygen saturation in stroke patients with nursing problems of ineffective cerebral perfusion risk. Room nurses can perform elevation head positioning to improve oxygen saturation in stroke patients with nursing problems at risk of ineffective cerebral perfusion.

Introduction. Lack of awareness of type 2 DM patients and the duration of suffering are factors that inhibit type 2 DM sufferers from carrying out self-management properly. The purpose of this study was to determine the effect of family support and long suffering with self-management in patients with Diabetes mellitus (DM) type 2. Methods. The study design used cross sectional. The population is patients with type 2 diabetes mellitus. The total sample was 167 respondents, taken according to inclusion and exclusion criteria. The sampling technique used is purposive sampling. The independent variables are family support and long suffering. The dependent variable is self-management. Data was collected using questionnaires that have been tested for validity and reliability using product moments. The data were analyzed using spearman rho analysis. Results. The results showed that there was an association of family support for self-management (p = 0.000) and no association of long suffering with self-management diet (p = 0.879). Discussion. High family support can make people with type 2 DM have good self-management. long suffering from DM does not necessarily make DM sufferers have good self-management. As long as people with type 2 DM have the awareness to carry out self-management well, complications can be prevented. For health workers, it is expected to further optimize health education and counseling in increasing spiritual intelligence in carrying out self-management of type 2 DM.

Abstract

The incidence of phlebitis will increase along with poor infusion insertion actions or not in accordance with hospital SPO. This study aims to analyze the relationship between nurse compliance in the implementation of infusion SPO with the incidence of phlebitis in the inpatient room. This study used a cross sectional approach. The sampling technique used in this study used total sampling with a sample size of 22 nurses and 22 inpatients at RSI Mabarrot MWC NU Bungah in February 2024. The instruments used in this study were observation sheets for compliance with SPO for infusion and VIP (Visual Infusion Phlebitis) screening sheets to measure the occurrence of phlebitis. The results showed that out of 22 nurses, 20 nurses were compliant with SPO for infusion with 18 patients (72.7%) not experiencing phlebitis, and 4 patients (18%) patients experiencing phlebitis. The results of data analysis showed a significance value of 0.015 <0.05, which means that there is a relationship between nurse compliance in implementing SPO infusion with the incidence of phlebitis in patients. The compliance of nurses who carry out the SPO for infusion installation will obediently reduce the incidence of phlebitis in patients. It is recommended for nurses to always improve compliance with infusion installation according to the SPO set by the hospital to minimize the occurrence of phlebitis in patients during hospitalization.

Abstract

One of the successes in managing diabetes care is compliance in taking medication. Apart from that, you must have high ability and self-confidence or what is usually called self-efficacy in carrying out good diabetes management. This study aims to determine the relationship between self-efficacy and medication adherence in diabetes sufferers. This research uses a cross sectional approach. The sampling technique used in this research used purposive sampling with a sample size of 52 people with diabetes who received outpatient treatment at RSI Mabarrot MWC NU Bungah in January-February 2024. The instrument used in this research was the Diabetes Management Self Efficacy Scale questionnaire (DMSES-UK) and Morisky Medication Adherences (MMAS-8). Data analysis used the Spearman rank correlation statistical test. The results showed that of the 52 people, 44 people had high self-efficacy, with 38 people (73.1%) having high medication adherence, and 2 (3.8%) people having moderate medication adherence. The results of data analysis show a significance value of 0.000 <0.05 with a correlation coefficient value of 0.552, meaning that there is a strong relationship between self-efficacy and adherence to taking medication in diabetes mellitus sufferers. High self-efficacy will increase adherence to taking medication in diabetes sufferers, because they have a high sense of confidence in managing diabetes care.

Abstract

Diabetes mellitus (DM) is a metabolic disease condition characterized by increased blood sugar levels or hyperglycemia. Uncontrolled diabetes mellitus can cause complications such as diabetic neuropathy (nerve damage), stroke, blindness, and diabetic ulcers (wounds that are difficult to heal). This study aims to determine the effect of hyperbarics on the quality of life of diabetic foot ulcer sufferers. This research uses a quasi-experimental design with a non-equivalent Control Group Design where an intervention group and a control group are selected and then pre- and post-tests are carried out. The sample was 27 people with diabetes. The instrument used is (WHOQOL-BREF). Data analysis used the analysis technique used was Wilcoxon and Mann Whitney. on the post test. The results showed that the average pre-test score for the quality of life of respondents was in the low category, namely the physical domain 68 people (97.1%), the psychological domain 67 people (95.7%), the social domain 54 people (77.1%), and environmental domain 62 people (88.6%). The p value obtained for the physical domain was 0.00 (p<0.05), for the psychological domain 0.012 (p<0.05), for the social domain 0.003 (p<0.05), and for the environmental domain 0.001 (p<0, 05). It can be concluded that there is a significant difference between the patient's quality of life before and after hyperbaric therapy according to the physical, psychological, social and environmental health domains, and there is an influence of hyperbaric therapy on the patient's quality of life.
