Abstract

COVID-19 memberikan dampak pada berbagai aspek khususnya kesehatan, hingga menyebabkan kematian.  Kasus terkonfirmasi positif dan kematian akibat COVID-19 cukup tinggi termasuk di Indonesia. Identifikasi faktor risiko kematian akibat penyakit ini menjadi penting untuk membuat keputusan yang tepat sehingga dapat mengendalikan dan meminimalkan kematian akibat pandemi ini. Tujuan dari tinjauan ini adalah untuk menganalisis penyebab kematian akibat COVID-19. Penelitian ini dilakukan menggunakan scoping review. Pencarian dilakukan pada database online Proquest, SCOPUS, dan Science Direct dengan 3 kata kunci berikut "faktor risiko" dan "mortalitas" dan "COVID-19" kemudian disaring berdasarkan publikasi khusus pada tahun 2020, dalam bahasa Inggris, teks lengkap, dan jurnal pilihan berdasarkan judul dan abstrak. Terdapat beberapa faktor risiko kematian pada pasien terinfeksi COVID-19 diantaranya adalah faktor karakteristik (usia dan jenis kelamin), faktor penyakit kronis (komorbiditas), faktor gizi (obesitas) dan faktor hasil laboratorium darah. Faktor yang dapat dikendalikan oleh perawat adalah pengendalian faktor IMT (Indeks Masa Tubuh) memperhatikan kebutuhan nutrisi pasien. Faktor risiko kematian pada pasien COVID-19 tersebut perlu dikaji saat awal dalam penanganan pasien COVID-19, sehingga dapat dilakukan tindakan untuk meminimalisir risiko kematian akibat COVID-19

Abstract

ABSTRACT: POST STROKE PATIENTS IN THE SUBACUTE PHASE: INFORMATION REQUIRED Introduction: Post-stroke patient care in the subacute phase is carried out independently by the patient and family at home. Post-stroke patients must have adequate knowledge to carry out self-care at home. The lack of information obtained by post-stroke patients can affect the patient's ability to carry out self-care so that his health status decreases. Therefore, nurses as educators must be able to provide information according to the needs of post-stroke patients.Purpose: This study was conducted to determine the information needs of post-stroke patients in the sub-acute phase so that the nursing care provided is in accordance with the patient's needs.Method: This study is a quantitative descriptive study conducted on post-stroke patients at the Neurology Polyclinic, one of the hospitals in Bandung. The sample technique used was consecutive sampling as many as 83 people. The inclusion criteria of post-stroke patients in this study were having full awareness and not experiencing aphasia. The instrument used in this study was based on the concept of the needs of post-stroke patients Kevitt (2009) and Moreland (2009) with a validity coefficient of 0.73 and an r of 0.75.Result: The information needs of post-stroke patients in a row include information about self-management to prevent recurrence or stroke recurrence ( = 1,59), nformation about nutrition that must be taken ( = 1,54), information about risks and side effects of treatment ( = 1,49), information about certainty of disease condition ( = 1,48), and information about stroke   ( = 1,45).Conclusion: Post-stroke patients need all information related to stroke, especially regarding self-management. Therefore, health workers are expected to provide education according to patient needs. Education for stroke patients can be done using various media and focuses on the patient. Keywords: Informations needs, Subacute, Stroke Patients         INTISARI: PASIEN PASKA STROKE PADA FASE SUBACUTE: INFORMASI YANG DIBUTUHKAN Latar Belakang: Perawatan pasien post stroke pada fase subacute dilakukan secara mandiri oleh pasien dan keluarga di rumah. Pasien paska stroke harus memiliki pengetahuan yang adekuat untuk menjalankan perawatan diri di rumah. Kurangnya informasi yang didapatkan pasien paska stroke dapat mempengaruhi kemampuan pasien menjalankan perawatan diri sehingga status kesehatannya menurun. Oleh karena itu, perawat sebagai edukator harus mampu memberikan informasi sesuai kebutuhan pasien paska stroke.  Tujuan: Penelitian ini dilakukan untuk mengetahui kebutuhan informasi pasien paska stroke pada fase subacute agar asuhan keperawatan yang diberikan sesuai dengan kebutuhan pasien.Metoda Penelitian: Penelitian ini merupakan penelitian deskriptif kuantitatif yang dilakukan kepada pasien paska stroke di Poliklinik Saraf dua Rumah Sakit di Bandung. Teknik sample yang digunakan adalah consecutive sampling sebanyak 83 orang. Kriteria inklusi pasien paska stroke dalam penelitian ini adalah memiliki kesadaran penuh dan tidak mengalami aphasia. Instrumen yang digunakan dalam penelitian ini disusun berdasarkan konsep kebutuhan pasien paska stroke khususnya bagian kebutuhan informasi yang dikembangkan oleh Kevitt (2009) dan Moreland (2009) dengan koefisien validitas 0,73 dan r 0,75.Hasil: Kebutuhan informasi pasien paska stroke secara berturut-turut diantaranya adalah informasi mengenai manajemen diri untuk mencegah kekambuhan atau stroke ulang ( = 1,59), informasi mengenai nutrisi yang harus dijalani ( = 1,54), informasi mengenai resiko dan efek samping pengobatan ( = 1,49), informasi mengenai kepastian kondisi penyakit ( = 1,48), dan informasi mengenai penyakit stroke ( = 1,45).Kesimpulan: Pasien paska stroke membutuhkan semua informasi yang berkaitan dengan stroke khususnya mengenai manajemen diri. Oleh karena itu, petugas kesehatan diharapkan dapat memberikan edukasi sesuai dengan kebutuhan pasien.  Edukasi pada pasien stroke dapat dilakukan dengan menggunakan berbagai media dan berfokus pada pasien. Kata Kunci : Kebutuhan informasi, Pasien Stroke, subacute

Abstract

COVID-19 menjadi pandemi global yang memicu kecemasan terutama pada perawat kritis (IGD dan ICU). Perawat IGD berisiko memiliki kecemasan karena menjadi lini pertama penanganan, sedangkan perawat ICU merawat pasien COVID yang berada di bawah ventilasi mekanis dan memerlukan prosedur invasif seperti suction yang meningkatkan risiko penularan COVID-19. Pengetahuan menjadi dasar perawat melakukan tindakan yang aman dan tepat bagi pasien dan perawat. Tujuan penelitian ini adalah untuk menganalisis hubungan antara pengetahuan dengan tingkat kecemasan yang terjadi pada perawat kritis. Metode yang digunakan analitik korelasional dengan rancangan cross sectional. Sampel pada penelitian adalah perawat di ruang ICU dan IGD di Rumah Sakit Kabupaten Kuningan Sebanyak 103 perawat yang diambil secara total sampling. Kuesioner tentang pengetahuan COVID-19 dan Hamilton Anxiety Rating Scale menjadi instrumen dalam penelitian ini. Data dianalisis dengan somer’s d gamma. Hasil analisis univariat menunjukkan sebagian besar pengetahuan perawat tentang COVID-19 dalam kategori baik sebanyak 97 orang (94,2%) dengan catatan perlu adanya peningkatan pengetahuan tentang etiologi dan proses transmisi COVID-19, dan memiliki tingkat kecemasan pada kategori tidak ada kecemasan yaitu sebanyak 78 orang (75,7%). Hasil uji statistik didapatkan nilai ρ-value 0,026 dan nilai r = -0,665. Terdapat hubungan antara pengetahuan dengan tingkat kecemasan yang dialami oleh perawat kritis yang ada di Rumah Sakit Kabupaten Kuningan. Perawat diharapkan dapat meningkatkan pengetahuan tentang COVID-19 guna memiliki mekanisme koping adaptif selama pandemi.

Abstract

 ABSTRAK Hipertensi merupakan merupakan penyakit kronis yang dapat dikendalikan salah satunya dengan perawatan diri (self-care). Sehingga diharapkan dengan self-care akan meningkatkan derajat kesehatan dan kualitas hidup. Untuk mencapai keberhasilan dalam mengendalikan hipertensi, diperlukan keterlibatan unsur masyarakat, salah satunya kader kesehatan. Kader kesehatan dapat berperan serta dalam mendampingi dan men-support pasien hipertensi dalam self-care hipertensi. Namun, belum ada program yang secara khusus melatih kader kesehatan dalam mengkaji self-care hipertensi. Sehingga program pengabdian masyarakat yang berorientasi pada pemberdayaan kader kesehatan dalam self-care hipertensi diperlukan guna mencegah komplikasi dan meningkatkan kualitas hidup pasien hipertensi. Kegiatan ini bertujuan meningkatkan pengetahuan kader dalam mengkaji self-care pada pasien hipertensi. Metode kegiatan berupa pendidikan masyarakat yaitu pelatihan kader kesehatan dalam mengkaji self-care pasien hipertensi. Target dan sasaran pada kegiatan ini adalah kader kesehatan di wilayah kerja Puskesmas Pasundan Kota Bandung sebanyak 37 orang. Luaran kegiatan ini adalah peningkatan pengetahuan kader dalam mengkaji self-care pada pasien hipertensi. Evaluasi dilaksanakan dengan mengukur pengetahuan kader kesehatan sebelum dan setelah dilakukan pelatihan. Hasil kegiatan didapatkan bahwa terdapat perbedaan pengetahuan (α 0.000) setelah mengikuti pelatihan. Kegiatan pengabdian pada masyarakat berupa pelatihan kader kesehatan dalam mengkaji self-care hipertensi ini efektif dalam meningkatkan pengetahuan kader kesehatan mengenai hipertensi dan self-care hipertensi. Kata Kunci: Hipertensi, Kader Kesehatan, Self-Care  ABSTRACT Hypertension is a chronic disease that can be controlled by self-care. It is hoped that self-care will improve the health and quality of life. To achieve success in controlling hypertension, it is necessary to involve one element of the community (health cadre). Health cadres can participate in assisting and supporting hypertensive patients in self-care hypertension. However, there is no program that specifically trains health cadres in assessing hypertension self-care. So that community service programs oriented to empowering health cadres in hypertension self-care are needed to prevent complications and improve the quality of life for hypertensive patients. The purpose of this activity is to increase health cadre knowledge in assessing self-care in hypertensive patients. The method of activity is the training of health cadres in assessing self-care for hypertensive patients.  The target of this activity was 37 health cadres in the work area of Pasundan Health Center in Bandung City. The outcome of activities is an increase in cadre knowledge in assessing self-care in hypertensive patients. Evaluation is seen from the knowledge of health cadres before and after training. The result of the activity was that there were differences in knowledge of health cadres (α 0.000) after attending the training. Abstract

There are many health education methods, most of which do not involve the participants actively. Workbooks are educational media that encourage a person's active role and are expected to increase self-efficacy, where self-efficacy itself is a predictor of self-care management behavior. This study aimed to identify differences in hypertension prevention's self-efficacy before and after health education using a workbook. This study used a quasi-experimental method with a pre-post-test design with a control group. This study's population was teaching staff in two health faculties at one of the universities in West Java, Indonesia. The sample was determined using the convenience sampling technique and obtained 24 respondents divided into the control and intervention groups. Data collection using the instrument "self-efficacy for prevention of hypertension." The data were analyzed using the median and frequency distribution, while for the different tests, the Wilcoxon test and the Mann Whitney test were used. The median pre-test for self-efficacy was 105 and 110, while the post-test was 104.5 and 110.5 in the control and intervention groups. While the p-value of the difference test between the two groups before the intervention was 0.326 (p> 0.05), while after the intervention, the p-value was 0.002 (p <0.05). This study indicated a significant difference in the self-efficacy of hypertension prevention after being given education using a workbook in the two groups, where the self-efficacy score in the intervention group was higher than the control group. As a recommendation, workbooks can be used as an educational tool for hypertension prevention to increase self-efficacy.

Abstract

Objectives: Therefore, this literature review aimed to identify the efficacy of non-invasive monitoring to evaluate the hemodynamics of patients with HF. Methods: This study was a literature review. Keywords include “heart failure patients” AND “non-invasive hemodynamic monitoring” AND “invasive hemodynamic monitoring” and searched extensively through Springer Link, PubMed, and Science Direct. Data analysis was assisted by a data extraction table and qualitative analysis. Result: Seven articles were included in the qualitative analysis after being selected through the title, abstract, and inclusion-exclusion criteria. The analysis revealed that non-invasive hemodynamic monitoring in heart failure patients could be carried out in the Intensive Care Unit,  non-intensive care unit, and at home. The Non-invasive Pulse Contour Analysis (NPCA), caretaker BP device, and Photoplethysmography-Based Device were used in the ICU. While in the non-ICU, Ballistocardiography (BCG) and Impedance Cardiography (ICG) were used, and Non-Invasive Telemonitoring in bioimpedance monitoring can be used in the home setting.  Conclusions: Overall, the use of technology to monitor hemodynamics in heart failure patients non-invasively had good validation and efficacy compared with invasive monitoring. Furthermore, this non-invasive technology can be used in the ICU, in Non-ICU settings, and at home (telemonitoring).

Abstract

Hemodynamic monitoring in critically ill patients is often invasive, using an arterial line. The monitoring accuracy is highly dependent on the patency of the arterial line, which is often compromised by artery blockages. So far, heparin in normal saline is believed to prevent these complications compared to normal saline alone. This study aimed to compare the effectiveness of normal saline flushing with heparin with normal saline use on arterial line patency. The method of the study was a review. Keywords were compiled using words such as critical care unit, heparinized saline, normal saline, and arterial line patency with boolean operators. Searched extensively through PubMed, Science Direct, and Google Scholar, Data were analyzed using the extraction table and presented with qualitative analysis. The initial search identified 370 articles. Seven articles were obtained after being selected through the duplication, title, abstract, and inclusion-exclusion criteria. All articles mentioned that there was no significant difference between the use of normal saline added with heparin with normal saline on arterial line patency. Based on the study, it can be concluded that the use of normal saline alone as an arterial line flush is recommended because it reduces the risk of Heparin-Induced Thrombocytopenia, allergy to heparin, bleeding and is economically more cost-saving. Abstrak: Pemantauan hemodinamik pada pasien kritis seringkali dilakukan secara invasif, menggunakan jalur arteri. Keakuratan pemantauan sangat tergantung pada patensi arterial line, yang seringkali terganggu oleh penyumbatan arteri. Sejauh ini, heparin dalam salin normal diyakini dapat mencegah komplikasi tersebut dibandingkan dengan pemberian normal salin saja. Penelitian ini bertujuan untuk membandingkan efektivitas flushing menggunakan normal saline ditambah heparin dengan penggunaan normal salin terhadap kepatenan arterial line. Penelitian ini merupakan literatur review. Kata kunci dikompilasi menggunakan critical care unit, heparinized saline, normal saline, dan arterial line patency yang dikombinasikan menggunakan boolean operator. Hasil-hasil penelitian dicari secara ekstensif melalui PubMed, Science Direct, dan Google Scholar, data dianalisis menggunakan tabel ekstraksi dan disajikan menggunakan analisis kualitatif. Pencarian awal berhasil mengidentifikasi 370 artikel. Tujuh artikel kemudian diperoleh setelah melalui seleksi menggunakan kriteria duplikasi, judul, abstrak, dan inklusi-eksklusi. Semua artikel menyebutkan bahwa tidak ada perbedaan yang signifikan antara penggunaan normal saline ditambah heparin dengan normal saline terhadap kepatenan arterial line. Berdasarkan penelitian dapat disimpulkan bahwa penggunaan flushing menggunakan normal salin saja direkomendasikan karena mengurangi risiko trombositopenia yang diinduksi heparin, alergi terhadap heparin, perdarahan dan dilihat dari biaya, lebih ekonomis.

Abstract

Penyakit kardiovaskular yang menjadi penyebab utama kematian nomor satu di dunia adalah hipertensi. Hipertensi yang tidak terkontrol akan menyebabkan terjadinya komplikasi sehingga salah satu upaya untuk mencegahnya adalah dengan pengendalian hipertensi. Upaya yang dapat dilakukan yaitu dengan perawatan diri (self-care) hipertensi sehingga kualitas hidup dan derajat kesehatan pasien akan meningkat. Dalam pelaksanaannya, pengendalian hipertensi ini memerlukan keterlibatan unsur masyarakat, salah satunya kader. Agar kader mampu berperan serta dalam mendampingi dan mendukung pasien dalam self-care hipertensi, diperlukan adanya suatu kemampuan dan motivasi yang kuat untuk melaksanakannya. Penelitian ini bertujuan untuk mengidentifikasi motivasi kader dalam mengkaji self-care pasien hipertensi. Desain penelitian ini adalah deskriptif kuantitatif. Sampel pada penelitian ini adalah 37 orang  kader kesehatan yang didapatkan dengan menggunakan teknik purposive sampling dengan kriteria inklusi aktif sebagai kader. Pengumpulan data dilakukan menggunakan kuisioner motivasi. Analisis data menggunakan distribusi frekuensi. Hasil penelitian menunjukkan bahwa motivasi kader dalam mengkaji self-care pasien hipertensi hampir setengahnya berada pada kategori tinggi dan sebagian besar berada pada kategori rendah. Dari hasil penelitian, dapat disimpulkan bahwa sebagian besari dari kader memiliki motivasi yang rendah dalam mengkaji self-care pasien hipertensi. Sehingga perlu adanya pelatihan khusus dari pihak puskesmas mengenai self-care pada pasien hipertensi. Kata Kunci: Hipertensi, kader kesehatan, motivasi, self-care Abstract Motivation of health care in assessing self-care in hypertension patients. There needs to be strong motivation and ability  from cadres to be able to participate in helping and supporting patients in self-care. This study aimed to identify health cadre motivation in studying self-care for hypertensive patients. It used a descriptive quantitative approach. Participant consisted of 37 health cadre, acquired through purposuve sampling with inclusion criteria is active as cadre. Motivation was measured using a questionnaire. Data were analyzed using frequency distribution. The results showed that cadre motivation in studying self-care for hypertensive patients wa in the high category (48.6%) and in the low category (51.4%). Can be concluded that most of cadres have low motivation in studying self-care for hypertensive patients. There needs to be training from health center regarding self-care in hypertensive patients. Keywords: Health Cadres, Hypertension, Motivation, Self-Care

Abstract

Children and adolescents with cancer can experience unpleasant experiences, such as pain, anxiety, fatigue, sleep disturbances, balance disorders, and even mental, behavioral, and emotional disturbances. These negative experiences lead to decrease in quality of life. A recent research indicates that complement therapies can reduce the terrible effects of cancer. So this literature study aimed to identify the use of complementary therapies in children and adolescents with cancer. This study was a literature review that begins with a search for articles on digital databases, Science Direct and Google Scholar, using the keyword "complementary therapy AND cancer AND adolescents". The inclusion criteria in this study were articles published in 2010-2020, full text, research results, in English, and contained about the use of complementary therapies in children and adolescents (0-18 years) with cancer. Total of 12 articles were analyzed in this literature study. The results of this study showed there are four groups of complementary therapies for children and adolescents with cancer. Complementary and alternative medicine in general, cutaneous stimulation (massage therapy and acupressure), art therapy (group art therapy and creative art therapy), as well as body and mind therapy (eurythmy therapy and yoga), have both physiological and psychological effects. These four therapeutic groups can elicit physiological and psychological responses in children and adolescents with cancer. Patients can feel a reduction in the discomfort. Based on the results of this literature review, nurses and patients' families are expected to provide complementary therapies in the care of children and adolescents with cancer.

Abstract

Burns is a global public health problem, and it is estimated that burns cause 180,000 deaths every year. There are many known wound care techniques, especially in the world of health, which are now constantly developing better by technological developments, consumer demands, and the development of related diseases. Negative Pressure Wound Therapy (NPWT) is a non-invasive therapy that promotes granulation and wound closure. This study aimed to prove the effectiveness of NPWT against burns in children. This study used a literature study approach with a descriptive review type. Based on the search results, seven articles were obtained using keywords, and papers were received from science direct, PubMed, and google scholar. The seven articles were published between 2014-2020, with the research method of each article being a randomized controlled trial and a retrospective study. Based on the results of several research articles, it was found that NPWT (Negative Pressure Wound Therapy) has been proven to be effective in treating burns in children.
